Adolescent Lumbar Disc Herniation Misdiagnosed As Knee Injury (슬관절 손상으로 오인된 청소년 요추 추간판 탈출증)

  • Lumbar disc herniation in the adolescent is rare. The clinical features of adolescent lumbar disc herniation are typically different from those of adults. Many adolescent with that disease may be misdiagnosed as minor trauma, muscle sprain, tight hamstrings or scoliosis. Published series reiterate that adolescents, as opposed to adults, do not respond well to conservative treatment. In adolescent lumbar disc herniation with hamstring tightness, the neurologic defects are improved shortly after, discectomy, but the hamstring tightness continue and remain even after 1 year. Earlier discectomy and adjunct postsurgical conservative measures can provide a greater opportunity for correction and stabilization of scoliotic posture. We here report on a case of adolescent lumbar disc herniation that was misdiagnosed by knee injury during 2 years. After posterior partial discectomy, the hamstring tightness and scoliosis was disappeared without complication.

  • Cornelia de Lange syndrome is a disorder of unknown etiology resulting in an syndrome characterized by specific dysmorphic features. Therefore this syndrome is diagnosed only by clinical features and other examinations for diagnostic aim are not effective. There are general growth retardation, mental retardation, hypertrichosis, confluent eye brows, low hair line, broad nasal bridge, anteverted nose tip, malformed limbs, webbing of toes, heart defect, gastroesophageal reflux disease, ear and ocular problems. Features associated oral structures are micrognathia, delayed eruption of teeth, cleft lip, cleft plate, thin upper lip and downturned angles of mouth. These are cases about two children who visited Department of Pediatric Dentistry of Chonbuk National University because of dental caries with Cornelia do Lange syndrome.

  • The most important point in the QoS management system to ensure the quality of the IoT system design goal is quality measurement system and the quality evaluation system. This research study is a matrix model for the IoT based on key quality measures by diagnosis system structure and function. Developing for the quality metrics measured Internet of Things environment will provide the foundation for the Internet of Things quality measurement/analysis. IoT matrix system for quality evaluation is a method to describe the functional requirements and the quality requirements in a single unified table for quality estimation performed. Comprehensive functional requirements and quality requirements by assessing the association can improve the reliability and usability evaluation. When applying the proposed method IoT quality can be improved while reducing the QoS signaling, the processing, the basis for more efficient quality assurances as a whole.

  • In recent decades, dental erosion has received a considerable amount of attention with a steady increase of the prevalence. However, the awareness of this matter in our society, including dentistry is still lacking. Dental erosion is an irreversible loss of dental hard tissue leading to a complex condition. To prevent detrimental effects, it is important to detect conditions as early as possible and inhibit progression. In children and adolescents, early intervention is more important than in adults. As dental erosion is a multifactorial condition, dentists need to fully understand the phenomena and do systematic oral examination with thorough history taking for early diagnosis. When the main etiological factor is detected, it is necessary to make an effort to eliminate it and stop progression of the lesions with preventive measures. Restorative treatment should not be started unless substance loss reaches a certain level. Once the decision is made for treatment, care must be taken to choose the least invasive method.

  • Tetrahydrobiopterin ($BH_4$) deficiency is caused by mutations in genes encoding enzymes involved in the synthesis and regeneration of $BH_4$. The condition is usually accompanied by hyperphenylalaninemia (HPA) and deficiency of neurotransmitter precursors L-dopa and 5-hydroxytryptophan. $BH_4$ deficiency is much rarer than classical phenylketonuria. Dihydropteridine reductase (DHPR) deficiency, an autosomal recessive genetic disorder, is a cause of malignant hyperphenylalaninemia due to $BH_4$ deficiency. When left untreated, DHPR deficiency leads to neurologic deterioration at the age of 4 or 5 months, including psychomotor retardation, tonicity disorders, drowsiness, irritability, abnormal movements, hyperthermia, hypersalivation, and difficulty swallowing. Treatment of DHPR deficiency should be initiated as early as possible with $BH_4$ supplementation and replacement of the neurotransmitter precursors L-dopa and 5-hydroxytryptophan. We report the first case of DHPR deficiency in Korea, a child diagnosed at 9 years of age by genetic testing.

  • Intramural hematoma of the aorta (IMH) is the precursor or a variant of a classic aortic dissection where hemorrhage occurs within the aorta wall in the absence of an initial intimal tear. IMH has a high rate of mortality and morbidity. The optimal therapy for IMH is uncertain, yet the involvement of the ascending aorta is usually considered as an indication for surgery due to the associated risk of rupture or cardiac tamponade. We report here on a case of a 71-year-old man who presented with syncope. Because of misdiagnosis, he underwent computed tomography (CT) after 5 hrs from arriving to the ER. Computed tomography of the aorta revealed intramural hematoma of the ascending aorta with cardiac tamponade. He also had vascular complications such as acute renal failure and visceral ischemia. We performed emergency graft replacement of the total arch and ascending aorta. He was discharged without complication on postoperative day 14.

An Efficient On-the-fly Repairing System of Order Violation Errors for Health Management of Airborne Software (항공기 소프트웨어의 건전성 관리를 위해서 순서 위배 오류를 자율 수리하는 효율적인 시스템)

  • Health management system of airborne software repairs runtime errors to provide safety and to reduce cost of maintenance. It is critical to on-the-fly repair order violation errors, because it is difficult to identify them at the development phase. Previous work, called Repairing Atomicity Violations (Repairing-AV) diagnoses order violations for each access event by comparing execution order of accesses. As a result, Repairing-AV has time overhead that is proportional to the number of access events to shared variable. This paper presents a tool called On-the-fly Repairing System (ORS) that can repair order violations of object methods containing access events. The ORS diagnoses order violations by using correct order of object methods, and treats them by stalling its thread where the error is about to occur. Experimentation with five synthetic programs shows that ORS is more efficient than Repairing-AV when the number of access events is greater than sixty.

A Case of Traumatic Pancreatic Transection with Main Duct Disruption and Pleural Effusion in a Child (소아에서 늑막 삼출액을 동반한 외상성 췌장 절단 및 췌장 주관 손상 1예)

  • An 8-year-old boy presented with abdominal pain and poor oral intake for two months. Serum amylase and lipase levels were elevated. CT of the abdomen and chest X-ray showed two pseudocysts at the pancreatic uncinate process, pancreatitis with a parenchymal defect, a large amount of ascites, and a right pleural effusion. MR cholangiography and endoscopic retrograde cholangiopanreaticography revealed a pancreatic duct disruption. The patient was successfully treated with a chest tube placement and percutaneous drainage. After surgery, his general condition improved; the serum level of amylase normalized and the pleural effusion resolved. Pancreatic injuries are rare in pediatric blunt trauma; however, diagnostic difficulty is common with isolated blunt trauma. Therefore, a high index of suspicion should follow such an injury. We report the case of an 8-year-old boy with pancreas transection, ductal disruption, ascites, and pleural effusion who was successfully treated.

  • Tuberculosis still ranks as one of the three most important infectious diseases in the world in terms of morbidity and mortality. In Korea, an increased incidence of tuberculosis has been observed in recent years. With the resurge of tuberculosis, extrapulmonary tuberculosis has increased, too. From this point of view, tuberculous arthritis affecting knee is rare in all forms of tuberculosis, but we can consider tuberculous arthritis in patients with osteomyelitis. We report the case of a 14-month-old male child who presented with fever and swelling on the left knee joint. Histologic examination of knee joint fluid showed consistent with tuberculosis. Ziehl-Neelsen stain of joint fluid was positive for acid-fast bacilli(AFB). The outcome was favorable after treatment with anti-tuberculosis medication. Because clinical signs and symptoms of musculoskeletal tuberculosis in children is more indolent, we can misdiagnose or delay diagnosis. The diagnosis of tuberculous arthritis can be elusive, necessitating a high index of suspicion.

  • 감염병소와 진단을 위해 여러 방사성성핵종 표지 사람비특이항체들이 임상이용되었으나, $^{123}I,\;^{99m}Tc,\;^{111}In$등 표지 핵종과 표지방법에 따른 체내동태의 차이에 대한 자료가 필요하며, 감염병소의 진단에 표준적으로 이용되어지던 $^{111}In$-oxine표지백혈구와 비교평가도 요구된다. 저자들은 $10^9$개의 포도상구균을 좌측 대퇴부에 주사하여 농양을 유발한 백서에서 $^{123}In$ 표지, iminothiolane을 이용한 $^{99m}Tc$ 표지, DTPA이용 $^{111}In$ 표지 사람비특이항체와 $^{111}In$-oxine 표지 백혈구의 체내동태 및 농양섭취율을 비교하였다. $^{123}In$-IgG는 갑상선 및 위의 방사능이 높아 체내 탈요드반응이 빠름이 시사되었으며, $^{99m}Tc$-iminothiolane IgG는 신장방사능이 높아 신장으로 IgG 또는 대사물이 배설됨을 알 수 있었다. $^{111}In$-oxine표지 백혈구는 간 및 비장의 방사능이 높았고, 혈액방사능 제거율이 가장 빨랐다. 주사 24시간 후의 농양섭취율은 $^{111}In$-DTPA IgG가 가장 높았고, 농양 대 혈액 방사능비는 $^{111}In$-oxine표지 백혈구가 가장 높았으며, $^{111}In$-DTPA IgG와 $^{99m}Tc$-iminothiolane IgG가 다음으로 비슷하였다. $^{111}In$-oxine표지 백혈구보다는 방사성핵종표지 IgG가 간편하게 이용될 수 있으며, $^{111}In$$^{99m}Tc$$^{123}In$보다 지연영상의 촬영에 유리함을 알 수 있었다.
