Arthroscopic Posterior Cruciate Ligament Reconstruction without Tourniquet (지혈대 없이 시행한 관절경하 후방십자인대 재건술)

  • Purpose: To report the results of arthroscope assisted posterior cruciate ligament reconstruction without tourniquet, which has not yet been reported in documents within the country. Materials and Methods: Out of the 75 cases of arthroscope assisted posterior cruciate ligament reconstruction on patients with posterior cruciate ligament damage from January, 1998 to May,2003 in this hospital, study was done on 49 cases of patients with isolated posterior cruciate ligament damage, excluding 16 cases of patients who were applied tourniquet in extraarticular operation due to combined damage such as in lateral collateral ligament and posterolateral collateral ligament, and 10 cases where the postoperative observation period was less than 12 months. There were 13 cases where reconstruction was done using autogenous bone-patella tendon-bone, 19 cases using allogenous bone-patella tendon-bone, and 17 cases using allogenous achilles tendon. For assessment, the 49 cases were compared using Lysholm knee scoring scale. Results: The average duration of operation was 105 minutes. There were no severe complications such as swelling, bleeding, infection etc . Assessment results attained by Lysholm knee scoring scale was 74(fair) for cases where autogenous bone-patella ten-don-bone was used,75(fair) for cases where allogenous achilles tendon was used, and 76(fair) for cases where allogenous bone-patella tendon-bone was used. Conclusion: There are no difficulties to perform the arthroscopic assisted posterior cruciate ligament reconstruction. Reconstruction without tourniquet can be thought to prevent complications that could follow when using it.

The Satisfaction Survey of Users and Patients on the Developed Disposable Tourniquet (개발한 일회용 지혈대의 사용자 및 환자 만족도 조사)

  • The 18~20G needle is used to computer tomography (CT) contrast examination. Therefore, a patient has to apply a self-administering hemostasis (conventional method: CM) and often experience bleeding in the course. Thus, we developed the new disposable transparent tourniquet (TT) for reducing. This study was to compare the usefulness between the proposed transparent tourniquet and the existing hemostatic methods. A Satisfaction survey was conducted by 50 patients and 25nurses. The survey contained the satisfaction of the convenience, safety, sanitation, and wearing sensation of transparent tourniquet. We employed face-to-face interview on 5 points likert scales. And Chi-square, paired T-test were used for the statistics verification. As for the patients, the satisfaction levels were measured for each category with the gender, age. Patients evaluation, overall satisfaction high average sore used TT and there were statistical significance by paired T-test(p<0.05). The following is the average satisfaction level for each category: $4.4{\pm}0.53$ in; $4.28{\pm}0.57$ in safety; $4.52{\pm}0.54$ in sanitation; $4.16{\pm}0.54$ in wearing sensation. So the overall satisfaction level is measured at $4.34{\pm}0.51$. As for the nurses, CT work experience and the current satisfaction with tourniquet were counted as variables. The satisfaction level for each category is: $3.8{\pm}0.7$ in; $3.6{\pm}0.68$ in safety; $3.4{\pm}0.5$ in sanitation; $3.9{\pm}0.49$ in hemostasis. The overall satisfaction level is $3.8{\pm}0.3$. Patients' satisfaction levels were very high with little difference among variables. Nurses' satisfaction levels were different with the TT depending on their work experience but their overall satisfaction was high. This TT will be a starting point to minimizing patient's inconvenience and more studies are necessary to enhance their satisfaction.

Changes in Temperature during Arthroscopic Knee Surgery (관절경 수술시 슬관절내 온도변화)

  • Purpose : To document the change in the temperature of the knee joint at the initiation and conclusion of an arthroscopic procedure and correlate this temperature change with other intra-operative variables. Material and Methods : Temperature measurements were performed in 40 consecutive patients(42 cases) that underwent arthroscopic surgery. Temperature measurements were taken at the initiation of the procedure, before and after inflation of the tourniquet. The last measurement was recorded at the end of the surgical procedure. Results : The mean knee joint temperature evaluated before inflation of the torniquet was $35.1{\pm}1.0$, at the end of surgery, $24.6{\pm}1.5^{\circ}C$. The mean temperature change observed from the beginning to the end of the procedure was $10.5^{\circ}C$. The student t test showed a statistically significant difference of the initial joint temperature(p<0.01) between the patient with no and+1 effusion / between the patient with no and +2 effusion. The temperature at the end of the procedure was found to be statistically low correlated (p<0.01) with the lower temperature of the irrigant and the lengthening of the arthroscopic procedure. Conclusion : Consideration should be given to maintaining the saline irrigant to more physiologic temperature to protect the articular cartilage from any possible temperature induced damages.

Povidone-Iodine related Chemical Burn under the Tourniquet (A Case Report) (지혈대 내측의 포비돈-요오드 용액에 의한 화학 화상 (1예 보고))

  • In the extremity surgery, pneumatic tourniquet and povidone-iodine solution are commonly used to provide an aseptic, bloodless field, and their complication rate has remained low. However, chemical burn under tourniquet has been rarely reported. Patients sustained burn injuries over the dependent, weight-bearing regions such as posterior neck, back, buttocks and posterior thighs. This rare adverse complication occurred in a 22-year-old man who underwent modified Brostrom operation with arthroscopic os trigonum excision. 10% povidone-iodine was used as topical antiseptic, and full thickness burn occurred underneath the area of tourniquet application. Main causes of povidone-iodine related chemical burn are considered maceration, irritation of the skin, long term use of the tourniquet and pressure. To reduce the complications like chemical burn, awareness of the risk and the possible pathogenesis as well as the preventive measures is important in surgical practice.

A Tunable Digital Tourniquet Using Nelaton Catheter (고무 도뇨관을 이용한 간단한 수지 지혈대)

  • Purpose: Surgery on digit requires a clear, bloodless field and it can be achieved by using a tourniquet. Several type of tourniquet have been used including Penrose drains and sterile glove with hemostat. We present a simple digital tourniquet using nelaton catheter, which is easily available, inexpensive, easy-to-apply, and effective for digital surgery. Methods: We made a small incision on the opposite side of an opening at the tip of nelaton catheter. Then the other tip of nelaton catheter is passed through the incision to form a ring. Nelaton catheter is placed aournd the base of the injured finger, making a tight circle to desired pressure. Results: A simple tourniquet using nelaton catheter effectively achieves a bloodless field and allows fine control of the pressure for digital surgery. Conclusion: We present a sismple digital tourniquet using nelaton catheter, which is easily available, inexpensive, tunable, and available for more than one finger.

  • 여호와의 증인을 부모로 둔 환아는 생후 4개월, 5.6kg이었다. 심초음파상 완전 대혈관 전위와 심실 중격 결손, 심방 중격 결손, 동맥관 개존중 및 양측상대 정맥이 관찰되었다. 수술전 혈색소 값은 14.9 g/dl이었다. 수혈없이 심실 중격 결손 교정과 대혈관 치환술을 시행하였으며, 별문제 없이 수술 후 16일에 환아는 퇴원하였다. 퇴원 당시 혈색소 값은 12.8 kg/dl 였다. 복잡 심기형을 가진 영아에서 수술전 eryrhropoietin의 투여, 수술중 철저한 지혈 및 초여과법등의 방법으로 수혈 없이 완전 교정술이 가능하였기에 보고하는 바이다.

  • Experimental studies were conducted to investigate the hemostatic effect of the water extracts of Kwibitang. For this purpose, the effects of the extracts on the bleeding time in mouse tail and prothrombin time in vitro were estimated. Furthermore, its activity on the isolated uterine muscle in rats were investigated. The results obtained were as following; The bleeding time was not shortened, but the plasma prothrombin time in vitro test was significantly shortened. The uterotonic action produced by the extract was not inhibited by pretreatment of diltiazem at the doses of $10^{-5}g/ml$.

  • In an attempt to investigate the effect of Beekeumsan on the hemostatic action and isolated uterine muscle, Beekeumsan was administered orally and the bleeding time in mouse tail, prothrombin time in vitro were estimated. Its activity on the isolated uterine muscle in rats were investigated. The following results were obtained; The bleeding time was not shortened, but the plasma prothrombin time in vitro was significantly shortened. The uterotonic action produced by the Beekeumsan was not inhibited by pretreatment of atropine, but was slightly inhibited by cyproheptadine and completely inhibited by pretreatment of diltiazem.

Aortocaval Fistula - A case report - (대동맥-대정맥루 -치험 1예-)

  • Aortocaval fistula is a rare complication of abdominal aortic aneurysm, involving less than $1\%$ of all abdominal aortic aneurysms. A 64-years old man with a long history of hypertension and abdominal aortic aneurysm had chest pain, dyspnea, epigastric discomfort and palpable abdominal pulsating mass. Physical examination revealed hypo­tension with a systolic blood pressure of 70 mmHg, a large pulsatile mass and a systolic abdominal bruit. Laboratory data revealed a hemoglobin values of 11.0 g/dL, blood urea nitrogen (BUN) value of 5 mg/dL, and creatine value of $2.5 mg\%$. Abdominal Angio CT showed a 10cm infrarenal abdominal aortic aneurysm with dilatation of the IVC and aortocaval fistula from the aortic aneurysm, which was confirmed at emergency surgery. When the aneurysm was opened and the thrombus was removed, a 1 cm communication was identified between the aorta and IVC. This was controlled with Foley catheters ballooning, and the fistula was closed by continuous suture placed outside the aneurysm. A bifurcated aorto-iliac graft was used to restore arterial continuity. The patient was discharged home after uncomplicated postoperative course.
