  • The aim of this study was to evaluate the changes in dentinal permeability after application of dentin desensitizer on exposed dentin immediately after ultrasonic scaling to teeth with non-carious cervical lesions. Thirty caries-free extracted molars were fixed to slide glasses after horizontally being sectioned at 5 mm below the cemento- enamel junction (CEJ). The prepared specimen was connected to a fluid flow measuring device (nano-Flow), and a V-shaped cavity was formed at the CEJ to imitate the non-carious cervical lesion. After no fluid leakage was confirmed in the connected system with specimen, tooth surface was treated ultrasonic cleaning with piezoelectric ultrasonic scalers until dentinal tubules were exposed. And 6 different desensitizers were applied on exposed dentin. Real-time measurements of dentinal fluid flow were performed during ultrasonic scaling and application of dentin desensitizer. To evaluate the occlusion of exposed dentinal tubules, tooth surface was examined by SEM. Following results were observed. After ultrasonic scaling, more dentinal tubules were exposed on the tooth with non-carious cervical lesions compared to tooth without lesions. The rate of fluid flow measured with nano-Flow system had correlation with the degree of dentin occlusion observed with SEM after application of desensitizers on exposed dentin. Desensitizers with glutaraldehyde and HEMA did not decrease the rate of fluid flow and did not show dentin occlusion. Desensitizers with oxalate showed the limited effects on the rate of fluid flow and dentinal tubule occlusion. Desensitizer with resin monomer showed the significant effect on the rate of fluid flow and dentin occlusion.

  • The purpose of this study was to evaluate the desensitizing effect of potassium oxalate(Group I), sodium fluoride (Group II), and control group (Group III). The 120 teeth of 26 patients who had been complained dentinal hypersensitivity were divided into three groups by applicating agent. The observation was done before and immediately after treatment. The data were statistically analyzed and the results were as followed. 1. Potassium oxalate showed the best desensitizing effect to the stimuli, followed by sodium fluoride, control group, and there was a significant difference (p<0.05) in desensitizing effect among the groups. 2. Potassium oxalate showed the best desensitizing effect to the stimuli, followed by sodium fluride, control group on both cervical abrasion and gingival recession, and there was a significant difference (p<0.05) in desensitizing effect among the groups on both cervical abrasion and gingival recession. 3. There was no significant difference (p<0.05) in effect of the desensitization between cervical abrasion and gingival recession. 4. The scratch and air blast I were more effective in desensitiziation than other stimuli with significant difference (p<0.05). In view of the results mentioned above, it can be conceived that potassium oxalate is more effective than sodium fluoride on the reduction of dentinal hypersensitivity.

  • The purpose of this study is for evaluating the effect of MS Coat desensitizing agent in clinical situation. In this study total 60 teeth of 30 patients who is feeling hypersensitivity after periodontal surgery were treated with MS Coat desensitizing agent, and 20 teeth with saline solution for placebo effects and evaluated. All teeth were cleaned with rubber cup and pumice, after then the antibacterial agent was applied for 20 seconds and simply isolated using cotton roll Desensitizing agent was applied with a specific instrument in the manufacturers package by abrading motion for 10 seconds and re-done 8 times. Tactile stimulus with sharp explorer, air stimulus with syringe of dental unit, $7^{\circ}C$ cold water stimulus using micropipette applicator was done to evaluate hypersensitivity before apply test agent, immediate after application, 1 week after, and 3 month after application. The degree of hypersensitivity was recorded using 100mm Visual Analogue Scale and compared between group and evaluation times. From this clinical study sensitivity to the stimulus was significantly induced in both groups and MS Coat made a significant reduction in VAS score than placebo group did(p<.01). The results of this study could demonstrate that MS Coat desensitizing agent can use effectively to heat dentinal hypersensitivity.

  • This study compared and analyzed the occluding effects of fluoride compounds and desensitizers, which are commonly used in dental clinics, on dentinal tubules. This study also evaluated the persistence of the active ingredients over time by performing toothbrushing with an electric toothbrush. Thirty-five molar teeth, which had been extracted within the past 3 months from healthy people without tooth decays, amalgam fillings, or dental crowns, were divided into 4 pieces each. Of these, 135 teeth pieces were used as study specimens. These specimens were divided into a control group, an untreated group, and 5 experimental groups (acidulated fluoride gel, fluoride varnish, Gluma, Super Seal, and SE-Bond). The specimens were then subjected to toothbrushing equivalent to 1 week (140 times), 2 weeks (280 times), and 4 weeks (560 times), and the occluding effects on dentinal tubules in 3 regions of each specimen were examined under a scanning electron microscope. The fluoride varnish treated group showed the highest degree of dentinal tubule occlusion effects during the first, second, and fourth weeks of toothbrushing, with the SE-Bond treated group showing the second highest degree and the Gluma treated group showing the lowest degree. After 4 weeks of toothbrushing, the Gluma treated group and the Super Seal treated group showed the lowest degrees of dentinal tubule occlusion effects. In summary, the fluoride varnish treated group and the SE-Bond treated group displayed higher occlusion effects even after 4 weeks of treatment than did the other experimental groups. Therefore, it is the authors' belief that fluoride varnish and SE-Bond are effective for treating dentinal hyperesthesia.

  • Although it is not a pathological symptom, Dentinal Hypersensitivity (DH) describes pain felt by patients whose tooth roots are exposed outside of the gums and are therefore sensitive to external stimuli. DH is caused by tooth brushing or gum diseases and treatment to reduce the sensitivity can include use of materials having stimulation activity for DH or a resin material applied periodontally. This study examined the hypersensitivity treatment effects of a four-week treatment with a toothpaste containing hydroxyapatite and tricalcium phosphate (Hap-TCP toothpaste). The Hap-TCP toothpaste was made by mixing a commercially available fluorine-containing toothpaste with 10% (W/W) hydroxyapatite and 19% (W/W) tricalcium phosphate (both 99% purity based on XRD analysis). The tooth hypersensitivity treatment effect was surveyed by scoring VRS values, and showed no significant initial difference compared with the control. However, after 1 week of use, the pain reduction value was 8% in the treatment group compared to the control group. This value increased to 30% and 60% after 2 and 4 weeks, respectively. Hypersensitivity to cold stimulation, which was used as a VAS value, showed no initial significant differences compared with the control, but was significantly decreased after 1, 2, and 4 weeks in the experimental group, with more than a 3-fold difference after 4 weeks. These findings confirmed that remineralization can alleviate DH as hydroxyapatite fills dentinal tubules and calcium, phosphorus, and tricalcium phosphate ion equilibrium is established.

  • The purpose of this clinical trial was to study the effectiveness of 5% potassium nitrate dentifrice as a daily home treatment of dentinal hypersensitivity. Thirty subjects with dentinal hypersensitivity were included in a 4-week, double-blind, parallel, comparative study. Stimuli used included mechanical, cold water and compressed air blasts. A subjective assessment of the degree of hypersensitivity for each stimulus was recorded. The results indicated that 5% potassium nitrate dentifrice, in comparision with a conventional fluoride dentifrice, reduced dentinal hypersensitivity to a significant degree. The therapeutic response to potassium nitrate was apparent within 2 weeks and increased continuously for the length of the study period. We conclude that 5% potassium nitrate dentifrice is an effective agent for the daily home treatment of dentinal hypersensitivity.

  • Amelogenesis Imperfecta (AI) is a genetic disorder which retards the development of enamel and it can be classified into three types: hypoplastic, hypomaturation, hypocalcified type. This can occur both in deciduous and permanent dentition. A 8 year 8 month old patient with a chief complaints of delayed eruption on upper anteriors, calculus deposit on lower anteriors and anterior openbite visited the clinic. Anteriors had thin layer of enamel and were very narrow. Especially lower anteriors had rough surface and were in bad shape. Teeth were very hypersensitive to thermal changes. Upper and lower first molars showed severe attrition on the occlusal surface. Radiographs also verified hypoplastic enamel in the whole dentition including the teeth in the tooth bud. The patient was diagnosed as hypoplastic AI, and is being treated at the pediathc and prosthodontic department of the Kyunghee dental university hospital. To improve the function, esthetics, hypersensitivity of the AI patients, restorations on the posteriors and the anteriors with oral hygiene instruction are necessary, Constant follow-up check is needed until full growth and after full growth, cooperative care with the other department is needed.
