중풍 화열 변증과 음허 변증을 구분하는 특정 지표 산출에 대한 연구 (Study of the Specific Indicators Calculation of Fire-Heat Pattern Identification and Yin Deficiency Pattern Identification in Stroke)

  • This study was done to calculate the specific indicators of Fire-Heat Pattern Identification and Yin Deficiency Pattern Identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. The subjects were 764 hospitalized patients with stroke, and a list of registry was made for each of them. Frequency and Correlations among Fire-Heat and Yin Deficiency indicators were studied. Fire-Heat pattern group included 175 patients out of 764, Yin Deficiency pattern group included 103 patients out of 764. Among the indicators of Fire-Heat pattern, those, in order of highest frequency, were 'heat vexation and aversion to heat', 'reddened complexion', 'thirst'. Among the indicators of Yin Deficiency pattern, those, in order of highest frequency, were 'dry mouth', 'red tongue', 'fine and rapid pulse'. Among the Fire-Heat pattern indicators, 'yellow fur' and 'thirst', 'heat vexation in the chest' and 'vexation and insomnia' showed most significant correlation. Among the Yin Deficiency pattern indicators, 'aphtha or tongue sore' and 'heat in the palms and soles', 'red tongue' and 'dry fur' showed most significant correlation. 'Reddened complexion', 'dry eyes', 'surging and parid pulse', 'heat vexation and aversion to heat' are classify between Fire-Heat pattern and Yin Deficiency pattern. 'Surging and parid pulse', 'Reddened complexion' are mostly representative of Fire-Heat pattern and 'fine and rapid pulse' are mostly representative of Yin Deficiency pattern.

한국형 중풍 변증 표준시안의 습담 변증 지표에 대한 연구 (Study of the Indicators of Dampness-Phlegm Pattern Identification Based on Tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke)

  • Objectives : This study was done to investigate clinical frequency and correlation among the indicators of dampness-phlegm pattern identification settled by tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. Methods : The subjects were 147 hospitalized patients with stroke, and a list of registry was made for each of them. Among the five types of pattern identification, fire-heat, dampness-phlegm, blood-stasis and deficiency of Qi and of Yin, those that have shown a high frequency in dampness-phlegm type were categorized as the dampness-phlegm pattern group. Frequency of dampness-phlegm indicators was compared with those from the non-dampness-phlegm pattern group. Correlations among dampness-phlegm indicators were also studied. Results : 1. Dampness-phlegm pattern group included 26 patients out of 147. 2. Among the indicators of dampness-phlegm pattern. those, in order of highest frequency, were 'tiredness or sluggishness', 'white Coated tongue' and 'sputum'. 3. In comparing dampness-phlegm pattern group with non-dampness-phlegml group, the indicators such as 'lightheadedness', 'nigrescence', 'sputum', 'dermatic dysaesthesia' showed significantly high frequency. 4. Among the indicators, 'sputum' and 'tiredness or sluggishness', 'sputum' and 'yellow coated tongue', and 'white coated tongue' and 'yellow coated tongue' showed significant correlation. 5. In investigation of the correlation of scale in symptoms, various results such as positive correlation and negative correlation were obtained. Conclusion : In this study, more sensitive indicators of dampness-phlegm pattern identification were found. Based on these results, it is suggested that a more practical Korean Standard Differentiation of the Symptoms and Signs of Stroke would be established through continuous clinical studies by giving weight on each specific type of pattern identification.

한국형 중풍 변증 표준안 - II와 한열허실 변증지표의 연관성 연구 (Study on the Relationship between Korean Standard of Pattern Identification (II) and Pattern Identification of Cold-Heat and Deficiency-Excess)

  • Previously standardization study for identifying 5 types of pattern identification of stroke patients has been performed and the Korean standard of pattern identification (II) was developed. In the present study we investigated the interactions between total indices designated by the Korean standard of pattern identification(KSPI II) and indices for PI of Cold-Heat and Deficiency-Excess. Indicators for Cold-Heat and Deficiency-Excess are isolated from 58 indices through the survey of oriental medicine doctors and their relationship with KSPI-II indices was analyzed by corresponding analysis method using data of 1581 stroke patients. Means and standard deviations indicated that 2 Cold indices, 14 Heat indices, 12 Deficiency indices, and 5 Excess indices were included for Cold-Heat and Deficiency-Excess pattern identification. The results of corresponding analysis shows the relationship of 57 indices and 4 types of pattern identification (excluding 1 index and 1 pattern among 58 indices and 5 patterns) using the cross-tabulation which was obtained from the clinical data. Most of Cold and Heat index were divided to dimension 1(inertia 51.9%) obtained from the result of corresponding analysis. Deficiency and Excess index were partially associated with dimension 2(inertia 31.7%). These data suggest that pattern identification of Cold-Heat plays an role in the standardization of pattern identification in stroke, although further studies are required by various trials such as analysis of surveys and clinical data.

중풍환자의 변증에 따른 맥상의 분포 (Study on the Pulse Diagnosis for Pattern Identifications in Stroke Patients)

  • This study is aim to evaluate pulse diagnosis as indicators for classification of the pattern identifications in stroke patients. To get the clinical information, we recruited the onset 1 month stroke patients through the multicenter network which consists of 13 oriental hospitals. The clinical informations about three pairs of pulse wave form and levels of their significancy based on the case report form (CRF) were collected and their distribution in each pattern identification were analyzed. The results are as follows Fire-Heat group shows high portions of floating pulse, rapid pulse and solid pulse. Qi Defficiency group has a greater portion of deep pulse, slow pulse, deficient pulse. The well-defined character of Phlegm-Retained Fluid, Yin Defficiency, Static Blood groups cannot be explained by pulse wave form. These results show a rough relationship between the pulse diagnosis and pattern identifications of stroke therefore, further studies are required to determine the pulse diagnosis as significant indicators of stroke pattern identification.

  • Objectives: The purpose of this study was to investigate distribution of pattern identification by age in stroke patients. Methods: From 1 April, 2007 to 29 September, 2008, 903 patients within a month after onset of stroke were included. Stroke patients were interviewed by oriental medicine doctors who used standard operation procedures for this study. A questionnaire was completed by a question-and-answer form between patients and doctors after explanation of details to patients and patients' agreement given. Results: Distributions of pattern identification in stroke patients differed by age groups. Dampness-Phlegm pattern was more common in the younger group aged under 80 years, while Yin-Deficiency pattern was more common in the older group aged over 80 years. Conclusions: In this study, we found a character of distribution of pattern identification by age in stroke patients within one month after stroke onset. We have concluded that these differences should be considered in the management and treatment of stroke patients.

중풍초기환자의 설상(舌象) 분포와 변증의 유용성에 관한 임상고찰 (The Characteristics of Tongue Inspection and Relationship between Tongue Inspection and Differenitiation of Syndrome)

  • To assess the usefulness of tongue inspection for evaluating the Pattern identification in oriental medicine, we observed stroke patient's tongue and tongue coat and compared it with Pattern identification. The test group was composed of 85 acute stroke stage patients(within 72 hours of onset). Subjects were randomly selected from stroke patients admitted in the KyungHee University, Hospital of Oriental Medicine from December 1 1998 to June 30 1999. We took pictures of patient's tongue and tongue coat within 72hours from onset and checked Pattern identification at the same time. Tongues colored pale rose or red greatly outnumbered other colors. Tongue shape tended to be prickly or fissured, and tongue condition tended to be unflexible or deviated. Regarding tongue coat color, there were great amounts of yellow or clark yellow tongue coats, which were moist, thick or greasy in substance. The red tongue was significantly related to Fire-heat and deficiency of Yin syndrome, while faint white tongue to Damp syndrome(P=0.006). In terms of tongue coat, thin coat was related to Wind and Fire-heat syndromes, thick coat to Damp and Blood stasis syndrome, respectively (P=0.002). In conclusion, we thought that tongue inspection could be a useful Oriental medicine diagnosis in stroke.

Human Paraoxonase 1(PON1)의 유전자 다형성에 따른 중풍환자의 습담 변증과의 상관성 연구 (Genetic Association of SNPs Located at PON1 Gene with Dampness and Phlegm Pattern Identification among Korean Stroke Patients)

  • Objective : In the present study, we investigated genetic distribution of eight single nucleotide polymorphisms of PON1 between Dampness and Phlegm and non Dampness and Phlegm pattern identification(PI) among Korean stroke patients. Materials and Methods : One hundred forty stroke subject without Dampness and Phlegm and fifty eight stroke subjects with Dampness and Phlegm were participated in this study. After informed consents, eight single nucleotide polymorphisms(SNPs) in PON1 of each subjects were identified by DNA sequencing and primer extension method and statistical analysis was performed to determine the significant difference between Dampness and Phlegm and non Dampness and Phlegm groups. Results : Among anthropometric characteristics and blood parameters, waist circumference and total cholesterol were significantly higher in Dampness and Phlegm. Among 8 SNPs of PON1, frequency of M allele and subjects with M allele in L55M SNP were significantly higher in Dampness and Phlegm group (p=0.0032 and p=0.0053, respectively) but subjects with T allele in C-2033T SNP were lower in Dampness and Phlegm group(p=0.0302). Effect of L55M and C-2033T on Dampness and Phlegm were 3.07% and 1.75%, respectively. Conclusion : Our results suggest that L55M SNP in exon and C-2033T in promoter region of PON1 maybe affect to Dampness and Phlegm pattern identification. However, further study should be carried out to find out the detailed mechanism how L55M and C-2033T can affect Dampness and Phlegm stroke patients.

한의 중풍변증표준안-III에 대한 보고 (Report on the Korean Standard Pattern Identifications for Stroke-III)

  • Objectives : The purpose of this study was to develop the Korean standard pattern identifications for stroke-III (KSPIS-III). KSPIS-III includes 4 major pattern identifications (PIs) and clinical indicators for each. Methods : To extract the indicators for 4 major PIs, we analyzed 1548 clinical data from 15 traditional Korean medicine hospitals. Patients got acute stroke within 30 days from onset. Two physicians independently checked 65 indicators and performed pattern diagnosis. If the PI were diagnosed the same, PI would be confirmed. First we built an assumption model that set up the relationship among pattern identifications. Second, we extracted the indicators for fire-heat pattern and qi deficiency pattern by comparison between excessive and deficiency group, heat and non-heat group. By comparing yin deficiency pattern and 3 other patterns respectively, we extracted the indicators for yin deficiency pattern. Dampness-phlegm pattern indicators were extracted by the same method. Results : After cross tabulation with 65 indicators on the basis of our assumption model, we finally extracted 19 indicators for fire-heat pattern, 11 for qi deficiency pattern, 7 for yin deficiency pattern, and 7 for dampness-phlegm pattern. Conclusions : KSPIS-III was more improved than KSPIS-II because it was based on more clinical data. Further study to establish the PI diagnostic model would be required for practical use in the clinical field.

중풍의 변증 진단을 위한 판별모형 (The discrimination model for the pattern identification diagnosis of the stroke)

  • The purpose of this study was to diagnosis that what patterns identification using the statistical method. Discriminant analysis using the medical specialist and resident pattern identification agree case in stroke patients within 1 month of onset. The agreement rate of dificiency of Gi(75%), heat-transformation(74%), dampphlegm syndrome(69%), deficiency of Eum(51%) and syndrome of blood stagnation(43%) are respectively 0.75, 0.74, 0.69, 0.51 and 0.43 in medical specialist and using linear discriminant function pattern identification are same. The study of inspection, pulse feeling and palpitation will be continued to evaluate concordance rate. Discrimination model will be make to get higher Accuracy and prediction, it means becomes the help in pattern identification diagnosis objectivity and scientific.

중풍환자의 기허변증지표에 관한 연구 (Study of The Indicators of Gi Deficiency Pattern Identification In Stroke Patients)

  • Background and Purpose The purpose of this study was to confirm that what symptoms are adequated indicator in the Gi-Deficiency patients. Methods In the time period July. 2005 to Sep. 2006, 136 patients with a first-ever stroke admitted in the department of Internal Medicine of Daejeon University Oriental Medical Hospital in Daejeon city, Wonkwang Oriental Medical Hospital in Iksan, JeonJu city were included. Patients were hospitalized within 3 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Gi-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Results Gi deficiency group included 23 case, Non Gi deficiency group 47 case out of 136 patients. Fatigue, weakness purse, somnolence, low voice, difficulty of uprise, pale face, pale tongue were higher among Gi deficiency group. Gi deficiency and Non Gi deficiency patients do not significantly differ in white coating tongue, light-red tongue, poor appetite, frequent sweating, teeth printed tongue. Conclusions This study was insufficiency because sample size very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the stroke.
