Knowledge and Education Needs Related to Multidrug Resistant Organisms Infection Control among Small and Medium Size Hospital (중소병원 간호사의 다제내성균 감염관리 지식 및 교육요구도)

  • This study was conducted to aid in the development of an infection control educational program for nurses, by assessing the knowledge and educational demand of nurses for MDRO infection control. Totally, 115 nurses participated in the study. Data were collected from November 15-30, 2019, using structured questionnaires. Descriptive statistics, t-test, and ANOVA were applied for analyzing the data. Experience of caring for MDRO patients was reported by 86.1% nurses, whereas 67.8% nurses had received training on MDROs. The average score for knowledge on MDROs was 25.51 out of 27 points, with minimal correct answers given for the query on level of disinfection for medical equipment used by patients, criteria for the preemptive precaution, patient management in the cohort, and timing for removing personal protective equipment. The educational demand was highest for assignment to the precaution, criteria for screening examination, and management of outbreak. Also, educational needs differed with respect to the general educational characteristics and position of the individual. We propose the need to differentiate the educational status according to the career when developing the MDROs program, and the necessity to execute education of MDROs for new nurses and career nurses.

  • u-Health 관련 보건 비용의 지속적인 증가와 건강 및 사전 예방에 대한 관심이 증가함에 따라 온라인을 통한 상담, 정보제공, 동영상 서비스 및 e-commerce등 건강 관련 서비스 시장이 확대가 되고 있다. 특히 노령인구의 급증에 따른 국내 고령층에 대한 실버산업이 대두되고 있다. u-Health 의료산업은 원무행정 분야에 대한 초기 정보화 단계에 있으며, 대학/종합병원들의 IT예산은 급속히 증가하고 있으나, 중소형 병/의원/약국의 경우 IT 투자예산 확보에 어려움이 있다. 이를 대처하기 위해 u-Health와 Wellness를 통합 함으로서 BT, NT 및 IT 관련 기술을 활용하여 u-Fitwellness 시스템을 구축하여 Ubiquitous 네트워크를 통해 고령자를 위한 건강과 의료관련 서비스, 제품, 정보를 제공하고 개인의 삶의질을 향상시킴으로써 홈 네트워킹 기반 u-Healthcare Total Solution을 통한 융합형 서비스를 제안하고자 한다.

Influencing Factors of Near Miss Experience on Medication in Small and Medium-Sized Hospital Nurses (중소병원 간호사의 투약 근접오류경험 영향요인)

  • The study was descriptive survey research for establishment of patient safety culture in small and medium-sized hospitals as providing baseline data of educational program regarding safe medication and prevention of near miss on medication, checking influencing factors of nurses near miss experience on medication in small and medium-sized hospital. The collected data was analyzed by SPSS/WIN 20.0 program to obtain mean, frequency, x2-test, independent t-test, one-way ANOVA, logistic regression. The influencing factors of near miss experience on medication was working department and patient safety culture among general characteristic. The nurses who were working in general ward had lesser chance to experience near miss rather than nurses working in special department (Odds ratio:2.23, 95%, Confidence Interval: 1.07~4.67, p=.032). The 1 point higher in patient safety culture, the lesser chance to experience in near miss (Odds ratio: 2.24, 95% Confidence Interval: 1.02~4.95, p=.045). To sum up the result of this study, nurses working in special department had higher chance to experience near miss rather than nurses working in general wards. The higher patient safety culture awareness was the lower near miss was experienced. Thus, miss surveillance system for improvement of nurses' patient safety culture awareness should be developed. Moreover, educational program for medication considering nurses' career and department' character should be requested with simulation training considering and theory education.

Development and Evaluation of the Reader for Blood Typing using Column Agglutination Method (원주응집법을 이용한 혈액형 검사 판독기의 개발 및 평가)

  • Accurate blood typing tests are essential for safe blood transfusion. Recently many automated test equipments have been introduced to reduce errors and increase the efficiency of the test. However, those equipments being high in price, it is difficult to introduce automated test equipment for every hospital. In this study, we developed a reader for blood typing using column agglutination test. In the process, the results, read out by the image processing, are stored and reaffirmed. To evaluate the reader, 148 samples for ABO and RhD blood typing tests and 154 samples for unexpected antibody test were used. The positive and negative intensity of the reading and the reading of the reaction were 100% in agreement with the result of traditional manual method. If additional verification is completed, this reader can be efficiently and economically used in small-and medium-sized hospitals.

The Mediating Effect of Depression on the relationship between Emotional Labor and Somatic Symptoms and Insomnia in Non-Medical Hospital Workers (비의료직 병원 종사자들의 감정노동과 신체증상 및 불면증 간의 관계에서 우울의 매개효과)

  • The purpose of this study is to prevent risk of emotional labor and health through verifying the mediating effects of depression on the relationship between emotional labor and somatic symptoms and insomnia in non-medical hospital workers. For this study, a sample of 533 tertiary hospital's non-medical workers completed the questionnaires of the Korean Emotional Labor Scale, PHQ-15, ISI, PHQ-9. The data was analyzed using the SPSS 25.0 program. The results can be summarized as follows. 1. Among the emotional labor sub-factors, emotional dissonance had a significant effect on depression, somatic symptoms and insomnia. Other sub-factors had a relatively small effect or had no effect. 2. The results showed the partial mediating effects of depression on the relationship between emotional labor and somatic symptoms. 3. The results showed the full mediating effects of depression on the relationship between emotional labor and insomnia. On the basis of the results, we suggest the necessity of dealing with emotional dissonance and depression in non-medical hospital workers' emotional labor and health issues.

  • Recently, U-Healthcare is receiving attentions as a research for reducing the manpower, time in treatment, and etc. Although fundamental technologies, such as sensing, measuring, and etc. are sufficiently investigated. However, Technologies of IT/Medical convergence, which graft IT technologies to medical area, are still in germ. For this, we present a novel healthcare system, which can be applied to the middle sized medical environment, such as private hospital, home, or etc., by means of pre-verified technologies and the expert system. There exist IT element technologies are sufficiently developed in the fields, such as network, database or etc. due to the remarkable developments in IT technologies, and the healthcare is a mission-critical environment. Therefore, it is important not only to investigate novel approaches but also to utilize verified technologies for the U-Healthcare solution. Presented solution provisions automated medical services based on expert system by utilizing the measured data, such as body fat, blood pressure, blood glucose, and etc., in order to provide convenient treatment environment to doctors and nurses. In addition, since people, who do not have medical knowledge, can self-diagnose themselves, it is expected to cut medical costs in various areas. Especially, since each devices communicate with each other through standardized Bluetooth technology, Presented healthcare system is an extensible solution which can easily accept various medical devices. As a result of this, we can safely say that the self measurement and diagnosis services in U-Healthcare are now enhanced by reducing medical cost through our healthcare system.

A Comparative Study on the Clinical Features and Complications of Snake Bite Patients in Urban and Rural Areas (도시와 농촌에서 발생한 독사 교상 환자의 임상적 양상과 합병증 비교 연구)

  • Purpose: The purpose of this study is to compare the clinical features and complications of snake bite patients in urban and rural areas. Methods: A retrospective study was conducted on patients over 18 years of age who were hospitalized for snake bites from January 2013 to December 2019. Patients were categorized into urban and rural groups according to their respective locations at the time of the snake bite and the clinical characteristics and complications of the two patient groups were researched and compared. Results: Of the 77 snake bite patients, 44 patients were categorized into the rural group (57.1%). The rura1 group showed significant differences in old age (p=0.011), delayed hospital visits (p=0.010), far hospital distance (p<0.001), high local effect score (p<0.001), high traditional snake-bite severity grading scale (p=0.008) and use of large amounts of antivenins (p=0.026). There was a significantly higher incidence of acute kidney injury (p=0.030), rhabdomyolysis (p=0.026), and coagulopathy (p=0.033) in the rural group as well as a longer hospitalization period (p<0.011). Conclusion: Snake bites that occurred in rural areas resulted in patients with more complications compared to urban areas due to farther distances from hospitals, causing a delay in antivenom treatment.

Analysis of Change Transitions in Regional Types in Emergency Department Patient Flows of in Jeonlado (2014-2018) (전라지역 응급실 환자의 유출입 분석 및 지역유형 변화 추이)

  • This study analyzed the inflow and outflow patterns of emergency department patients, to identify changes in regional types in cities, counties, and districts in Jeonlado, Korea. Data of areas in Jeonlado for 2014 to 2018 were extracted from the National Emergency Department Information System. The extracted data includes the patients' and emergency medical institution addresses, which were used to calculate the relevance index (RI) and commitment index (CI). The calculated indices were classified into regional types by applying cluster analysis. A non-parametric method, Kruskal-Wallis test, was employed to examine the differences between years for RI and CI by regional types. The results of cluster analysis using the relevance and commitment indices revealed three regional types. Regions in cluster 1 were classified as outflow type, in cluster 2 as inflow type, and in cluster 3 as self-sufficient. RI and CI were calculated for each cluster or regional type. There were no significant differences between years in cluster 2 (inflow type) and cluster 3 (self-sufficient type). In cluster 1 (outflow type), there were no significant differences in CI between the years; however, there were significant differences in RI between 2014 and 2017, and 2014 and 2018. It is difficult to see that the emergency medical environment has improved due to the increased concentration of emergency medical care.

The Effects of Misutkaru Water Drink on Relieving Constipation and Physiological Measurements: A Preliminary Study for the Development of Convergent Misutkaru Water Drink Intake Program in Patients with Hemodialysis (혈액 투석 환자의 변비 완화 및 생리적 지표 향상을 위한 융합형 미숫가루 음용 섭취 프로그램 개발 예비 조사)

  • The purpose of this study aimed to develop the misutkaru water drink intake program that was possible to compare the effects of misutkaru drink on relieving constipation and physiological measurementson relieving constipation of the patients with hemodialysis. This study was non-equivalent control group pretest-posttest design with 42 patients in a general hospital in S city, Korea. During the experiment, the participants had to drink misutkaru water in the morning and past the 7 days after intake as constipation assessment scale, potassium and phosphorus were measured. The data were examine using t-test, Fisher's exact test, paired t-test. After misutkaru drinking intervention, constipation (z=-2.66, p=.002) decreased significantly improvement. This suggests that misutkaru water drinking intervention improved constipation in patients with hemodialysis.
