  • This study investigated the effect of spouse attitude of psychiatric patients on relapse. The research was based on the concept that spouses' attitudes toward mental illness influences relapse, mediated by marital relationship and social adaptation. Data were collected from 102 of the psychiatrically disordered who were married, and analyzed through Structural Equation Modelling to validate causal paths and mediator effects of the variables. The marital relationship was shown to act as a mediator between spouse attitude and social adaptation, while social adaptation showed a complete mediation effect between marital relationship and relapse. Thus, spouse attitude towards mental illness was shown to have a statistically meaningful effect on relapse indirectly and through sequential mediation effect of marital relationship and social adaptation. This study proposes the facilitation of counseling and training programs to improve the marital relationships of psychiatric patients. The emphasis is on the educational program for the spouses to improve their knowledge of mental illness.

  • Borderline personality disorder in adolescents present with suicide attempts or gestures, and they are characterized by the same patterns of splitting, devaluation, manipulation, need-gratifying object relations, impulsivity, and ego deficits that are finds in borderline adults, Symptomatolgy are depression, anxiety, identity crisis, and occasionally antisocial behavior in borderline personality disorder in adolescents. These findings should be differentiated to normal stormy adolescents. Theses borderline personality disorder in adolescents are known about the instability of object relations, labile affect, splitting, and psychotic episode in severe stress. According to thses finding, schizophrenia, mood disorder, schizoptypal personality disorder, paranoid personality disorder could be differentiated. Etiology is complex including psychodynamic, genetic, familial factor. Treatments are individual psychotherapy, group therapy, and pharmacotherapy. The continuity or discontinuity of borderline states from childhoon to adult life is controversy.

  • Introduction:Research on attachment in view point of the developmental perspective which began in the 1940s progressed during several decades. Many investigators focused mother-child separation in early attachment studies, and moved to the relationship with childhood psychopathology. Recently attachment theory and research are moving forward along the intergenerational transmission of attachment patterns, and adolescents and adult mental disorders in the developmental perspectives. Methods:We surveyed the research papers through Medline search, attachment-related monographs, and review or original papers published in Korean journal. Results:Developmental attachment researches have demonstrated convincingly that insecure attachment in infancy is associated with attachment disorder; several childhood psychopatholgy, such as institutional care and adoption, aggression and behavioral problems, childhood anxiety disorders and depressive disorders, gender identity disorder and feeding disorder, and child abuse and maltreatment; peer relationship and social competency, and parental behaviors. Recently the methodological advances including the Adult Attachment Interview that systematically assesses the adults' recollections of the earlier parent-child relationship they experienced could move beyond attachment researcher's initial concern with infancy to consider attachment processes throughout the life span. We could find that the quality of attachment was associated with several mental disorders in adolescents and adults significantly. Conclusion:Attachment theory would have focused on more specific parent-child relationship than general parental behavior. Recent attachment theory underscores its evolutionary origins to promote development of infant and contribute to human survival in psychobiological bases. Advances in attachment research could unite interests in evolutional biology and developmental psychology in understanding early parent-child relationship, and apply to clinical issues concerning mental health throughout the life span.

  • Objectives : Recent neuroimaging studies focus on dysfunctions in connectivity between cognitive circuits and emotional circuits: anterior cingulate cortex that connects dorsolateral orbitofrontal cortex and prefrontal cortex to limbic system. Previous studies on pediatric depression using DTI have reported decreased neural connectivity in several brain regions, including the amygdala, anterior cingulate cortex, superior longitudinal fasciculus. We compared the neural connectivity of psychotropic drug naïve adolescent patients with a first onset of major depressive episode with healthy controls using DTI. Methods : Adolescent psychotropic drug naïve patients(n=26, 10 men, 16 women; age range, 13-18 years) who visited the Korea University Guro Hospital and were diagnosed with first onset major depressive disorder were registered. Healthy controls(n=27, 5 males, 22 females; age range, 12-17 years) were recruited. Psychiatric interviews, complete psychometrics including IQ and HAM-D, MRI including diffusion weighted image acquisition were conducted prior to antidepressant administration to the patients. Fractional anisotropy(FA), radial, mean, and axial diffusivity were estimated using DTI. FMRIB Software Library-Tract Based Spatial Statistics was used for statistical analysis. Results : We did not observe any significant difference in whole brain analysis. However, ROI analysis on right superior longitudinal fasciculus resulted in 3 clusters with significant decrease of FA in patients group. Conclusions : The patients with adolescent major depressive disorder showed statistically significant FA decrease in the DTI-based structure compared with healthy control. Therefore we suppose DTI can be used as a bio-marker in psychotropic drug-naïve adolescent patients with first onset major depressive disorder.

  • This study aimed to investigate the differences of perceived stigma, psychological empowerment, job satisfaction according to the working hours by latent means analysis(LMA). To get the results, a survey was conducted, targeting employed people with mental illness. Key outcomes of the analysis were as follows. First, it was verified that perceived stigma and job satisfaction, psychological empowerment had statistically meaningful direct relationships. Second, it was analyzed that psychological empowerment had a partial mediating effect in the relationship between perceived stigma and job satisfaction. Third, according to LMA, full-time group showed higher latent mean values on the perceived stigma and lower latent mean values on the job satisfaction. However, part-time group showed lower latent mean values on the psychological empowerment. Based in these results, comprehensive interventions for activating job satisfaction were proposed.

  • Objectives: Although sleep paralysis (SP) has been known as one of the symptoms of narcolepsy, recently it has become recognized as occurring frequently in the general population. However, the prevalence of SP and its associated factors in patients with major psychiatric disorders remain unknown. This study investigated the prevalence of SP and a variety of associated experiences in those patients. Methods: The subjects were 160 psychiatric patients and 143 age- and sex-matched controls. The Korean version SP questionnaire as well as the Sleep-Wake Schedule, Epworth Sleepiness Scale and Insomnia Severity Index were administered to all the subjects. The patients were referred from Dong-A University Hospital and consisted of 74 diagnosed as schizophrenia (47.7%), 26 as bipolar disorder (16.8%) and 55 as major depression (35.5%). Results: Nearly 42% of the patient group and 39% of the control group had experienced at least one episode of SP in their lifetime, with no significant difference between the groups. However, the patient group had experienced SP more frequently than the control group. Among all subjects, no gender difference in SP incidence was found. The peak age of onset was in the range of 16-25 years for both groups. Over eight tenths of both groups reported hallucinations and over seven tenths of both groups experienced fear accompanying SP. Conclusion: This study shows that there is no difference in the lifetime prevalence of SP between psychiatric patients and the general population, whereas frequency of SP experience is higher in psychiatric patients. Terrifying hallucinations and fearful feelings frequently accompany SP in both groups.

  • This study examines the extent to which community cohesion moderates the effects of neighborhood disorder on community integration among persons with mental illnesses(MI) who are living in the community. Stress process model, social disorganization theory, and neighborhood disorder model were considered with regard to the moderating effect of community cohesion. 692 persons with MI who utilized Community Mental Health Enhancement Centers were surveyed and analyzed, employing moderated regression model. Major findings showed that neighborhood disorder increased social integration. Community cohesion moderated the relationships of neighborhood disorder to three dimensions of community integration: physical, social and psychological integration. Implications for community intervention beyond the individual and service-level interventions were addressed in order to accomplish community integration for persons with MI.

  • The purpose of this study is to investigate the effect of gardening programs using vertical gardens on helplessness, depression, and self-esteem in patients with chronic mental disorders. This study was applied to 21 patients with chronic mental disorder who were admitted to the rehabilitation hospital of E Hospital in Seoul from June to October 2018. The group was treated as an experimental group (N = 21) 5 times a week. For 20 weeks, each session lasted 30 minutes. Patients in the experimental group programmed 4 sessions of general occupational therapy, which combined treatments to improve cognition and training, and 1 session of a horticultural program using a vertical garden. Helplessness, depression, and self-esteem were evaluated for pre and post evaluation of the horticultural program. After completing the program, the average score of helplessness in the experimental group decreased by 0.91 points from 47.38 to 46.47, and there was a statistically significant change (p=0.043). The mean score of depression in the experimental group was 9.23 before and after 9.08, which was statistically significant (p = 0.035). The average score of self-esteem in the experimental group increased by 3.14 points from 27.99 to 31.13 at a statistically significant level (p = 0.001). This result suggests that a gardening program using vertical gardens has an effect on helplessness, depression, and self-esteem in patients with chronic mental disorders. And this means that long-term plans and effective programs for recovery from chronic mental disorders to the community are needed. With a horticultural work program using a vertical garden, the helplessness of the mentally impaired in a closed treatment space is restored, and such treatment space needs to be increased.
