The evaluation of properties for radiation therapy techniques with flattening filter-free beam and usefulness of time and economy to a patient with the radiation therapy (Flattening filter-free beam을 이용한 방사선 치료 기법의 특성 및 환자의 시간적.경제적 유용성 평가)

  • Purpose : The aim of this study was to appraise properties for radiation therapy techniques and effectiveness of time and economy to a patient in the case of applying flattening filter-free (3F) and flattening filter (2F) beam to the radiation therapy. Materials and Methods : Alderson rando phantom was scanned for computed tomography image. Treatment plans for intensity modulated radiation therapy (IMRT), volumetric modulated arc therapy (VMAT) and stereotactic body radiation therapy (SBRT) with 3F and 2F beam were designed for prostate cancer. To evaluate the differences between the 3F and 2F beam, total monitor units (MUs), beam on time (BOT) and gantry rotation time (GRT) were used and measured with $TrueBeam^{TM}$ STx and Surveillance And Measurement (SAM) 940 detector was used for photoneutron emitted by using 3F and 2F. To assess temporal and economical aspect for a patient, total treatment periods and medical fees were estimated. Results : In using 3F beam, total MUs in IMRT plan increased the highest up to 34.0% and in the test of BOT, GRT and photoneutron, the values in SBRT plan decreased the lowest 39.8, 38.6 and 48.1%, respectively. In the temporal and economical aspect, there were no differences between 3F and 2F beam in all of plans and the results showed that 10 days and 169,560 won was lowest in SBRT plan. Conclusion : According as the results, total MUs increased by using 3F beam than 2F beam but BOT, GRT and photoneutron decreased. From above the results, using 3F beam can decrease intra-fraction setup error and risk of radiation-induced secondary malignancy. But, using 3F beam did not make the benefits of temporal and economical aspect for a patient with the radiation therapy.

Evaluation of VMAT Dose Accuracy According to Couch Rotation in Stereotactic Radiation Surgery of Metastatic Brain Cancer (전 이성 뇌 암의 정위 방사선수술에서 Couch 회전에 따른 VMAT의 선량 정확성 평가)

  • Purpose: To purpose of this study is to find the correlation of the Set-up error according to the couch rotation and suggest additional margin setting for the GTV. Target and Method: Each scenario treatment plan was created by making the frequency of non-coplanar beams different among all beams. The set-up error value was measured by using the Exact System and the dose accuracy was evaluated by creating a re-treatment plan. Results: When the couch was rotated by 30°, 45°, 60°, and 90°, the mean of the X-axis values was measured to be 0.29 mm, 0.26 mm, 0.51 mm, and 0.08 mm, respectively. The mean of the Y-axis values was measured to be 0.75 mm, 0.5mm, 0.35 mm, and 0.29 mm, respectively. The mean of the Z-axis values was measured to be 0.5 mm, 0.28 mm, 0.22 mm, and 0.1 mm, respectively. There were dose reductions of 0.1%, 3.1%, 1.9% in D99 for 1-NC VMAT, 2-NC VMAT, and 3-NC VMAT, respectively. Conclusion: When treating with 50% or more of non-coplanar beams among total beams, image verification is required. And it is considered to make the treatment plan by adding a 1.5 mm margin to the GTV.

An investigation Of IntraFraction Motion Correction For Lung Stereotactic Body Radiation Therapy By Using IntraFraction Cone Beam Computed Tomography (폐암 환자의 정위적 체부 방사선 치료 시 IntraFraction CBCT를 이용한 치료 중 자세 오차 교정에 대한 고찰)

  • Purpose: The purpose is to correct for position errors caused by long treatment times. By correcting the target motion that can occur during lung SBRT using IntraFraction CBCT. Methods and materials: We analyzed retrospectively the IFM data of 14 patients with two treatment arc in the treatment plan for lung cancer with stereotactic radiotherapy. An IntraFraction Motion was applied to the Arccheck phantom to acquire the Gamma index data. Results : IntraFraction Motion during the first treatment arc is in the left-right(LR), superiorinferior(SI), anterior-posterior(AP) directions were $0.16{\pm}0.05cm$, 0.72 cm(max error), $0.2{\pm}0.14cm$, 1.26 cm, $0.24{\pm}0.08cm$, 0.82 cm and rotational directions was $0.84{\pm}0.23^{\circ}$, $2.8^{\circ}$(pitch), $0.72{\pm}0.23^{\circ}$, $2.5^{\circ}$(yaw), $0.7{\pm}0.19^{\circ}$, $2^{\circ}$(roll). IntraFraction Motion during the second treatment arc is in the LR, SI, AP directions were $0.1{\pm}0.04cm$, 0.37 cm, $0.14{\pm}0.17cm$, 2 cm, $0.12{\pm}0.04cm$, 0.5 cm and rotational directions was $0.45{\pm}0.12^{\circ}$, $1.3^{\circ}$, $0.37{\pm}0.1^{\circ}$, $1^{\circ}$, $0.35{\pm}0.1^{\circ}$, $1.2^{\circ}$. Gamma index pass rates were $82.64{\pm}10.51%$, 48.4 %. Conclusions : In this study, we examined the validity of IntraFraction Motion correction in lung SBRT and the efficiency of IntraFraction CBCT. Due to the nature of SBRT treatment, IFM may increase due to the increased treatment time. It is believed that the increase in IFM with the increase in treatment time can be improved with the use of FFF Beam and additional position correction using CBCT during treatment.

  • Fractionated stereotactic radiosurguy (FSRS) requires precise and reproducible patient set up. For these reasons non-invasive mask fixation methods have been used in Linac based FSRS. In this study, we measured and assessed the isocenter reproducibility using a commercial head mask fixation system based on thermoplastic materials. For the verification and the measurement of isocenter deviation a special acrylic brain phantom was designed. The designed phantom has 22 vertical rods and each rod has different lengths. At the end of the 8 rods, the monochromic film is attached and irradiated due to planned target position. Deviations of isocenter were measured separately for each direction. The mean deviation showed 0.4 mm in longitudinal direction, 0.1 mm in the lateral direction, 0.1 mm in the anterior-posterior direction of the treatment couch. The data demonstrates the high accuracy and reproducibility. This study reinforces previous literature published.

Search of Characteristic for Dose Distribution Presented by Multi­isocentric Stereotactic Radiosurgical Plan Using Linear Accelerator (선형 가속기를 이용한 정위적 방사선 수술시 병소내 선량분포의 특성조사)

  • The goal of a radiation treatment plan is to deliver a homogeneous dose to a target with minimal irradiation of the adjacent normal tissues. Dose uniformity is especially important for stereotactic radiosurgery using a linear accelerator. The dose uniformity and high dose delivery of a single spherical dose distribution exceed 70%. This also results with a similar stereotactic radiosurgical plan using a Gamma Knife. The dose distribution produced in a stereotactic radiosurgical plan using a Gamma Knife and Linear accelerator is spherical, and the application of the sphere packing arrangement in a real radiosurgical plan requires much time and skill. In this study, we found a characteristic of dose distribution with transformation of beam parameters that must be considered in a radiosurgical plan for effective radiosurgery. First, we assumed a cylinder type tumor model and a cube type tumor model. Secondly, the results of the tumor models were compared and analyzed with dose profiles and DVH_(Dose Volume Histogram) representative dose distribution. We found the optimal composition of beam parameters_(i.e. collimator size, number of isocenter, gap of isocenters etc.), which allowed the tumor models to be involved in the isodose curve at a high level. In conclusion, the characteristics found in this study are helpful for improving the effectiveness and speed of a radiosurgical plan for stereotactic radiosurgery.

Evaluation of usefulness for Stereotactic Partial Breast Irradiation(S-PBI) by using Surface Fiducial Marker (표면위치표지자를 적용한 정위적 부분유방방사선치료의 유용성 평가)

  • Purpose: The goal of this study is to evaluate usefulness of noninvasive method instead of previous inserting Fiducial Marker Method when performing Stereotactic Partial Breast Irradiation in CyberKnife. Material and methods: For consistency of Imaging Center, we evaluated both oblique images at angle 45 and 315 acquired from 2D Simulator and CyberKnife quantitatively through dice similarity coefficient. Also, location reproducibility of Surface Fiducial Marker was analyzed from 2D Simulator, treatment plans and CyberKinfe images by using 8 Fiducial Markers made of gold attached to ATOM Phantom based on our institution's protocols. Results: The results of the estimated consistency were 0.87 and 0.9 at the oblique angle 45 and 315, respectively. For location consistency of Surface Fiducial Markers, values of horizontal vertical direction of left breast were Superior/Inferior 0.3 mm, Left/Right -0.3 mm, Anterior/Posterior 0.4 mm, and the values of rotational direction were Roll 0.3 °, Pitch 0.2 °, Yaw 0.4 °. The values of horizontal vertical direction of right breast were Superior/Inferior -0.1 mm, Left/Right -0.1 mm, Anterior/Posterior -0.1 mm, and the values of rotational direction were Roll 0.2°, Pitch 0.1°, Yaw 0.1°. Conclusions: We expect that the protocols used by Surface Fiducial Markers when performing Stereotactic Partial Breast Irradiation in CyberKnife will provide protection from pain and cut expenses for treatment and reduce treatment errors and make treatment more accurate by suggesting treatment protocols based on high consistency of Imaging Center and reproducibility of Fiducial Markers.

Linear Accelerator Radiosurgery for Trigeminal Neuralgia: Case Report (선형가속기를 이용한 삼차신경통의 정위적 방사선수술: 증례보고)

  • Trigeminal neuralgia is defined as an episodic electrical shock-like sensation in a dermatomal distribution of the trigeminal nerve. When medications fail to control pain, various procedures are used to attempt to control refractory pain. Of available procedures, stereotactic radiosurgery is the least invasive procedure and has been demonstrated to produce significant pain relief with minimal side effects. Recently, linear accelerators were introduced as a tool for radiosurgery of trigeminal neuralgia beneath the already accepted gamma unit. Author have experienced one case with trigeminal neuralgia treated with linear accelerator. The patient was treated with 85 Gy by means of 5 mm collimator directed to trigeminal nerve root entry zone. The patient obtained pain free without medication at 20 days after the procedure and remain pain free at 6 months after the procedure. He didn't experience facial numbness or other side effects.

Evaluation on Usefulness of Stereotactic Radio Surgery using $Fraxion^{(R)}$ System ($Fraxion^{(R)}$ System을 이용한 뇌 정위적 방사선 수술 유용성 평가)

  • Purpose : We evaluated the usefulness of $Fraxion^{(R)}$ system and s-thermoplastic mask by analyzing setup error when stereotactic radiousurgery (SRS) was treated for brain metastasis. Materials and Methods : 6 patients who received definite diagnosis as brain metastasis between May 2014 and October 2014 were selected. 3 patients were immobilized s-thermoplastic mask and mouthpiece (group1), while $Fraxion^{(R)}$ system was used for the other 3 patients (group2). Cone Beam Computerized Tomography (CBCT) scan was acquired to register planning CT scan. The registration offset was compared for each group. We compared and reported the errors using maximum, minimum, mean, and standard deviation of registration offsets. Furthermore, We used the same method as patient specific quality assurance to verify absorbed dose of PTV. Results : The setup error which is registration offset was reduced 83% in x, 40% in y, and 92% in z-direction when $Fraxion^{(R)}$ system was used compared to the case of using s-thermoplastic mask and mouthpiece. In addition, using $Fraxion^{(R)}$ system showed improved results in rotational components, pitch (rotation along x-axis), roll (y), and yaw (z) which were reduced 64, 88, and 87% respectively compared to the case of using s-thermoplastic mask and mouthpiece. In dosimetry results, when s-thermoplastic mask and mouthpiece used, absorbed dose was reduce 83% compared to before and after registration. However, using $Fraxion^{(R)}$ system showed only 1.9%. All percentage were calculated with respect to average value. Conclusion : Using $Fraxion^{(R)}$ system including mouthpiece, Fraxion frame, frontpiece, and thermoplastic mask, showed better repeatability and precision compared to using s-thermoplastic mask and mouthpiece, which is consequently considered as more improved immobilization system.
