Premenstrual Dysphoric Disorder : A Clinical Review (월경전 불쾌기분 장애에 대한 임상적 고찰)

  • Premenstural dysphoric disorder(PMDD) imposing 4-5% of women is possibly caused by an enhanced responsiveness to the changes of sex steroid hormones and the decrease of serotonin, melatonin and GABA. The common clinical features between PMDD and depression, seasonal affective disorder, panic disorder and anorexia nervosa suggest a relatedness between PMDD and each of them. The diagnostic criteria of DSM-IV-Tr for PMDD requires psychological symptoms, that commonly include irritability, anger, depression, mood swing, affect lability, tension, anxiety, fatigue and food craving. As of today, the best pharmacological treatment for PMDD is the selective serotonin reuptake inhibiter, and leuprolide, danazol, estradiol, spironolactone and bromocriptine are possible alternatives. Nonpharmacological treatments for patients with mild to moderate symptom severity are diet, exercise, light therapy, psychotherapy and keeping a diary.

Assessment and Treatment of Somatization (신체화의 평가 및 치료)

  • Somatization is regarded as a process rather than a diagnostic entity. It should be emphasized to identify psychopathology rather than to make a choice regarding diagnosis in assessing somatizing patients. Psychiatrists should be aware of the psychosocial cues underlying the patients' physical symptoms. Special skills and strategies are required by nonpsychiatric physicians to facilitate the patients' acceptance of psychiatric treatment. The goal of treatment for somatization is management but not cure. The approach should be flexible, depending on the patients' responses and need. The difficulty in diagnosing and treating somatization is likely to be related to abnormal illness behavior such as the patients' denial of their psychosocial problems and resistance to psychiatric approach. In conclusion, biopsychosocial approach is needed to treat these patients effectively. Psychiatrists should also teach other physicians the interview skill that they could identify these patients as early as possible and facilitate their acceptance of psychiatric treatment.

Past, Present, and Future of Psychosomatic Medicine in the Field of Korean Medical Education (한국 의학 교육에서 정신신체의학의 과거와 현재 그리고 미래)

  • There were several documents that might reflect the great concern on the education of psychosomatic medicine in medical school from the 1960s. But the hour of class and proportion of psychosomatic medicine have been quite small among the total lecture time of psychiatry. Notwithstanding the importance of biopsychosocial perspective in practice and research there have been no agreement on the goal and content of teaching psychosomatic medicine in the medical school curriculum. Consultation-liaison psychiatric activity in the hospital were currently under-developed and educational content and process were not systematic. We should have established the goal of psychosomatic education in the medical school that includes making doctor who could not only cure disease but also care the ill patients. And we should develop the curriculum that covers essential area of psychosomatic medicine and checking system to monitor the process of education. With the continuance of psychosomatic perspectives from medical school education to clinical subspecialty we can make progress in this field.

The Biomedical Medicalization of Depression in Korea (우울증의 '생의학적 의료화' 형성 과정)

  • This paper investigated the biomedical medicalization of "depression"which is growing fast in Korea in terms of the treatment mechanism. Depression has been regarded as a mental disease that occurs with mixing various causations because what was the cause of this disease was not clarified until a recent date. Thus, as depression treatment, the medicine and the psycho-socio therapy have been used. However, from 1990s, as the brain science was introduced in the western society, and the high-tech diagnostic equipment of the brain disease and new drugs for the mental disease were developed, depression was rapidly redefined as 'the brain nerve system illness'that is easy to be taken and is able to obtain the permanent relief with the regular care. Under the influence of the redefinition of depression and the new treatment of it, recently, 8% of depression patients per year emerge as the aggressive cure subjects in the Korean psychiatric circle. However, according to the Korean psychiatric circle's unofficial calculation, it is estimated that only 10% of depression patients are receiving the accurate treatment but over 80% of the patients are not. If so, what does this estimation mean? Based on this question, this paper critically investigated the biomedical medicalization of depression.

Considerations about Evolutionary Ecological Study of Psychiatry (정신의학의 진화생태학적 연구 시 고려사항)

  • Evolutionary research on mental disorders is relatively difficult compared to other medical studies. It is because the cause of mental disorder is unclear relative to other medical diseases, various proximate causations are involved. And it is difficult to distinguish cause and effect and to carry out experimental research. Despite these methodological difficulties, it is possible to establish an evolutionary hypothesis on mental disorders based on constructive reductionism, and to demonstrate actual data on the model based on this hypothesis. In this paper, I will discuss some conceptual definitions needed for applying ecological approaches to evolutionary psychiatric research. We will first discuss the appropriate level of explanations and the scope of the study subjects, then discuss the conceptual definition of behaviour and function, dysfunction and the appropriate level of selection.

  • We propose to designs and to implements a robot analyzing the risk of occurrence of mental disorder of single-person households' workers through the facial expression-detecting system. Due to complex social factors, the number and proportion of single-person households continues to increase. In addition, contrast to the household of many family members, the prevalence of mental disorder among single-person household varies greatly. Since most patients with mental can not detect the disease on their own, counseling and treatment with doctors are often ignored. In this study, we design and implement a robot analyzing the risk of mental disorder of single-person households workers by constructing a system with Q.bo One, a social robot created by Thecorpora. Q.bo One is consisted of Arduino, ar raspberry pie, and other sensors designed to detect and respond to sensors in the direction users want to implement. Based on the DSM-5 provided by the American Psychiatric Association, the risk of mental disorder occurrence was specified based on mental disorder. Q.bo One analyzed the facial expressions of the subjects for a week or two to evaluate depressive disorder, anxiety disorder. If the mental disorder occurrence risk is high, Q.bo One is designd to inform the subject to counsel and have medical treatment with a specialist.

  • 충청북도 청주시 상당구 주중동에 위치한 청주성모병원은 (구)리라병원이 문을 닫게 되어 충북지역 주민의 건강유지, 질병치료에 큰 지장을 초래하게 됨에 따라 천주교 청주교구에서 재정적인 어려움을 무릅쓰고 가톨릭정신에 입각하여 지역주민의 의료에 일익을 담당하고자 병원을 인수하였다. 1998년 3월 25일 122병상 21개 전문진료 과목으로 개원한 이래 지역주민의 신뢰와 호응에 힘입어 현재 의료진 119(전공의 포함)명, 간호사 307명을 포함하여 총 831명이 근무하고 있으며 498병상에 21개 전문 진료과목을 갖추고 충북 지역 의료를 선도하는 지역 거점병원으로 자리매김하게 되었다.

Study on Comarison of Homeopathy with Sa-sang constitutional Medicine in Basic Principles from the literature (동종요법의 기본원리와 사상체질의학의 비교연구)

  • Homoeopathy was established in 1796 by German physician Samuel Hahnemann(1755~1843). This method is an alternative form of therapy involving treatment by natural remedies. The basic principles of homeopathic medicine, "Similia similibus curantur", "experimenta in hominesano", "doses minimae" and "unitas remedii", are founded upon the idea of illness as a disorder of the internal equilibrium at the physical, mental and psychic levels. The three distinguishing characteristics of homeopathy are that remedies are prescribed on the totality of a person's symptoms, that the remedy likely to cure a person is a dilution of that remedies are prepared using microdoses of substances which are diluted and then vigorously shaken. This paper describes the basic principled of homeopathy and compared with the Sa-sang constitutional Medicine from the literature. 1. Homeopathy is the holistic medicine that derived from the competition of vitalism and mechanism, and it is the one of natural medicine that absorbed the influence of asian scholarship and theoretical background of oriental medicine. 2. Homeopathic remedy typologies and Sa-sang constitutional Medicine are same in the mind-body correlativity and in-born typologies. 3. In homeopathy, constitutional types are distinguished the variable types with variety of symptoms by the remedy picture, but comparably in Sa-sang constitutional medicine, it is determined only by the constitutional symptoms.
