Attentional Dysfunction in Major Psychiatric Disorders (주요 정신장애에서의 주의력 장애)

  • Attentional dysfunction is considered as one of the core deficits in schizophrenic process. The findings, pathophysiological mechanisms, and their clinical implications of clinical and experimental neurocognitive tests for the attentional impairment in schizophrenics are reviewed. The influences of psychopathology, antipsychotic treatment, and chronic institutionalization are also included in the review. In contrast, there are only a few evidences that attentional dysfunction would be a core deficit of depressive, manic, and anxiety disorders. Some recent findings of attentional impairment in these disorders are reviewed.

  • It is important to find risk factors associated with mental disorder. Also the hazard ratio that represent the relationship of risk factors with illness is main interest in medicine. Thus we used odds ratio to explore the relationship between mental disorder and risk factors. On this paper, when we applied Bayesian network to delirium of mental disorder, we selected major risk factors and calculated odds ratio. Especially we identified odds ratio of single risk factors and multiple risk factors.

  • Borderline personality disorder in adolescents present with suicide attempts or gestures, and they are characterized by the same patterns of splitting, devaluation, manipulation, need-gratifying object relations, impulsivity, and ego deficits that are finds in borderline adults, Symptomatolgy are depression, anxiety, identity crisis, and occasionally antisocial behavior in borderline personality disorder in adolescents. These findings should be differentiated to normal stormy adolescents. Theses borderline personality disorder in adolescents are known about the instability of object relations, labile affect, splitting, and psychotic episode in severe stress. According to thses finding, schizophrenia, mood disorder, schizoptypal personality disorder, paranoid personality disorder could be differentiated. Etiology is complex including psychodynamic, genetic, familial factor. Treatments are individual psychotherapy, group therapy, and pharmacotherapy. The continuity or discontinuity of borderline states from childhoon to adult life is controversy.

  • The author treated a case with combined male erectile disorder and hypoactive sexual desire disorder. Treatment consisted of concurrent psychotherapy, sex therapy, and self-hypnosis lasting 9 months. I suggest that psychotherapy is essential for the treatment of sexual desire disorder especially complicated with deep-seated psychological conflicts, in addition, it is necessary to apply various strategies flexibly in according to the patients' need. I have experienced that self-hypnosis is tremendously useful in the enhancing sexual fantasy and for the arousal of psychological pleasure as well as the maintenance of erection in the phase of sexual excitement.

Current Situation of Psychiatry in North Korean : From the Viewpoint of North Korean Medical Doctors (북한 의사들이 바라보는 북한의 정신의학 현황)

  • Objective : Psychiatry in North Korea is believed to seem very different from psychiatry in South Korea. However, there is nearly no information regarding psychiatry in North Korea until now. Our study aimed to get information about North Korean psychiatry. Methods : Three North Korean defectors in South Korea, whose clinical experience as medical doctors in North Korea was over 10 years, were recruited. They underwent the semi-structured interview, content of which included the clinical experience with psychiatric patients, the details of psychiatry, the treatment of psychiatric patients, the stigma of mental illness, and the suicide, in North Korea. Results : In North Korea, psychiatric department was called as 49th(pronounced as Sahsip-gu-ho in Korean). Only patients with vivid psychotic symptoms came to psychiatric department. Non-psychotic depression or anxiety disorders usually were not dealt in psychiatry. The etiology of mental illness seemed to be confined to biological factors including genetic predisposition. Psychosocial or psychodynamic factors as etiology of mental illness appeared to be ignored. Psychiatry was apparently separated from political or ideological issues. The mainstay of psychiatric treatment is the inpatient admission and out-of-date therapy such as insulin coma therapy. Stigma over mental illness was common in North Korea. Suicide is considered as a betrayal to his/her nation, and has been reported to be very rare. Conclusion : The situation of psychiatry in North Korea is largely different from that of South Korea. Although some aspects of North Korean psychiatry are similar to psychiatry in former socialist countries, North Korean psychiatry is considered to have also its unique characteristics.

Considerations about Evolutionary Ecological Study of Psychiatry (정신의학의 진화생태학적 연구 시 고려사항)

  • Evolutionary research on mental disorders is relatively difficult compared to other medical studies. It is because the cause of mental disorder is unclear relative to other medical diseases, various proximate causations are involved. And it is difficult to distinguish cause and effect and to carry out experimental research. Despite these methodological difficulties, it is possible to establish an evolutionary hypothesis on mental disorders based on constructive reductionism, and to demonstrate actual data on the model based on this hypothesis. In this paper, I will discuss some conceptual definitions needed for applying ecological approaches to evolutionary psychiatric research. We will first discuss the appropriate level of explanations and the scope of the study subjects, then discuss the conceptual definition of behaviour and function, dysfunction and the appropriate level of selection.

  • 한국에 있는 치과병원의 전공의 및 대학원생과 치과 대학생의 측두하악장애와 만성 동통에 관한 이해도를 알아보기 위하여 서울대, 연세대, 경희대, 이화여대, 단국대, 원광대, 강남 성심 병원 전공의 및 대학원생 217명과 서울대, 연세대 치과 대학 3,4학년 학생 258명을 대상으로 설문조사를 실시하였다. 병태 생리, 정신 생리, 만성 동통, 및 정신과적 장애에 관한 전공의 및 대학원생과 치과 대학생의 응답과 미국 측두하악장애 및 만성동통 전문가의 응답을 비교하였는데, 정신 생리학적 및 정신과적 장애 항목에 대한 전공의 및 대학원생들과 치과 대학생들의 개념과 지식은 미국 측두하악장애 및 만성 동통 전문가들의 개념 및 지식과 유사하였으나, 병태 생리와 만성 동통 항목에서는 미국 측두하악장애 및 만성 동통 전문가들의 개념 및 지식에 미치지 못하였다.

Correlates of Self-Stigma, Self-Esteem, and Quality of Life Among Persons with Mental Disabilities (정신장애인의 자기 낙인과 삶의 질, 자존감과의 관계 연구)

  • The purpose of the this study was to investigate the association of self-stigma, quality of life, and self-esteem and analyze the mediating role of self-esteem in the relationship between self-stigma and quality of life among people with mental disabilities. For this purpose, the data were collected from 254 people with mental disabilities who utilize the community mental health centers located in D and K regions. Findings of the study indicated that there were statistically significant relationships among self-stigma, quality of life and self-esteem. In addition, self-esteem had partial mediating effects on the relationships between self-stigma and the quality of life domains such as physical, psychological, and environmental areas, while self-esteem was a full mediator in the relationships between self-stigma and social relationship in the quality of life. Based on the findings of, possible practice implications for improving the quality of life among people with mental disabilities by reducing self-stigma along with empowering their self-esteem and recommendation for further study were provided.
