Quality Assurance of CORVUS Planning System for Intensity Modulated Radiation Therapy (CORVUS Planning System을 사용한 세기조절방사선치료의 정도관리에 관한 연구)

  • The intensity modulated radiation therapy (IMRT) is believed to be on of the best treatment techniques for the goal of radiation therapy: to irradiate fatal dose to tumor region while minimizing dose to critical organs. It is essential to have comprehensive quality assurance program to assure the precision and the accuracy of the treatment due to the characteristic of the IMRT. The quality assurance technique for the Corvus treatment planning system was developed and its effectiveness was tested with the treatment planning of H&N region. Acrylic phantom, film and ionization chamber were used for this study, the discrepancy between the treatment planning and the film measurements showed 0.03 cm and 0.28 cm for the 90% of isodose line in each directions. Dose measurements showed 1% and 1.2% differences for ionization chamber and TLD, respectively. This concluded that the system can be used for clinic.

Geometrical QC/QA for Medical Linear Accelerator using Electronic Portal Imaging Devices(EPID) (전자포탈영상장치 (EPID)를 이용한 선형가속기의 기하학적인 QC/QA)

  • We have designed the software for geometrical QC/QA for medical linear accelerator using electronic portal imaging devices (EPID). The radiation-light field congruence, the collimator rotation axis, and the gantry rotation axis could be estimated with this software. Precision of the system is within 1mm. The collimator and the gantry rotation axis could be measured by superpositioning the images from 4 different collimator (or gantry) angles. The EPID system and the analysis software which was developed in this study make it possible that the quantitative and the objective geometrical QC/QA of the linear accelerator.

Reference Dosimetry and Calibration of Glass Dosimeters for Cs-137 Gamma-rays (연구용 세슘-137 조사기에 대한 흡수선량 측정과 유리선량계 교정에 관한 연구)

  • In this research, the glass dosimeter was calibrated to measure the standard absorbed dose of the Cs-137 irradiator and absorbed dose in a biological sample. Absorbed dose in water for Cs-137 gamma ray was determined by the IAEA TRS-277 protocol. The PTW-TM30013 ion chamber and the PTW-TM41023 water phantom were utilized for measuring absorbed dose and the value was compared with the reading from DoseAce GD-302M glass dosimeter from Asahi Techno Glass Corporation for its calibration. The uncertainty of measurement ($1{\sigma}$) of the calibrated glass dosimeter was 2.7% and this result would be applied to improve the accuracy in measurement of absorbed dose in a biological sample.

  • A muiltileaf collimator (MLC) is used as a replacement for conventional blocks. The MLC, however may not be appropriate for a fine field shaping. For the fine field shaping, conventional block can be added under the MLC. But it may significantly affect on the dosimetric characteristics such as surface dose of skin, buildup region and percent depth doses. We performed the study to evaluate the surface dose and the maximum depth dose using MLC conjunction with conventional blocks for various field sizes and energies. We confirmed the surface dose was increased by using the additional conventional block under the MLC ranging from 10 to 35.6% according to various field sizes and radiation beam energies. The surface dose was effectively reduced by application of 2 or 3 m thickness of lead plate as electron filter.

Quality Assessment and Comparison of Several Radioimmunoassay Kits and Chemiluminescence Immunoassay Methods for Evaluating Serum Estradiol (혈중 Estradiol 농도 측정을 위한 여러 방사면역측정 검사키트 및 화학면역발광 검사법의 성능평가 및 상호비교)

  • Purpose Serum estradiol ($E_2$) measurement is requested for diagnosing menstrual cycles, ovulation induction, infertility, and menopause. $E_2$ is measured using several methods and kits including radioimmunoassay (RIA) and chemiluminescece immunoassay (CLIA). The purpose of this study was to evaluate quality of these methods and to compare them with each other. Materials and Methods Seven radioimmunoassay kits and two CLIA methods were included in the analysis. Using standard samples and patient samples, intra-assay precision, inter-assay precision, correlation between other methods, sensitivity, and recovery rate were evaluated. Results For all tested kits and methods, coefficients of variance (CVs) of intra-assay precision test were 10.9~13.6% in low-level samples and less than 10% in medium and high-level samples. CVs of inter-assay precision test were 10.8~12.3% in low-level samples and less than 10% in medium and high-level samples with all tested kits and methods. Recovery rates were $92.7{\pm}12.4%$ for SIEMENS, $101.4{\pm}18.4%$ for DIAsource, $95.1{\pm}11.5%$ for AMP, $108.4{\pm}18.5%$ for BECKMAN COULTER, $104.2{\pm}13.5%$ for BECKMAN COULTER Ultra Sensitive, $101.3{\pm}11.6%$ for CIS Bio, and $93.1{\pm}13.2%$ for MP kits. Sensitivity was 7.5, 6.2, 5.7, 6.2, 5.3, 4.5, and 5.5 pg/mL for SIEMENS, DIAsource, AMP, BECKMAN COULTER, BECKMAN COULTER Ultra Sensitive, CIS Bio, and MP kits, respectively. The measurement by MP kit was slightly higher than those by other kits in low-level samples, and the measurement by E170 was slightly higher than those of other kits in medium and high-level samples. In the measurement of standard sample for external quality control, SIEMENS kit produced relatively lower values whereas E170, Architect, and MP kits produced relatively higher values compared with other kits. Conclusion All tested kits for $E_2$ measurement have satisfactory performance for clinical use. However, correlation between kits should be considered when test kits are to be changed, because some pairs of kits do not have correlations with each other.

The Results of the Survey about Present Situation of Quality Assurance for Radiotherapy Machine of Korea (한국의 방사선치료 기기 품질관리 현황에 관한 설문 결과)

  • As radiation therapy is one of three major cancer treatment methods, many cancer patients get radiation therapy. Because of the invisible and scattering characteristics of radiation, it is impossible to identify the quality and the amount of radiation and secondary cancer could be induced by scattered radiation. Because of advanced technique of radiation therapy and the reasons mentioned above, quality assurance of radiotherapy machine should be performed completely. International organizations such as International Atomic Energy Agency (IAEA), American Association of Physicists in Medicine (AAPM) suggest report of quality assurance to recommend united method of radiotherapy machine quality assurance. Domestic society of medical physics, however, is too small to make such a report, domestic hospitals selectively choose some of contents in global suggestions. As there are no suggestions for domestic hospitals and global suggestions are being updated, we did a survey about quality assurance for radiotherapy machine. The questionnaire is composed of possession of radiotherapy machine, items performed for quality assurance and manpower, etc. 37 of 72 hospitals answered to survey. These results could be used for making domestic standard quality assurance procedure.

A Study on Smart Phone Real-Time Motion Analysis System using Acceleration and Gyro Sensors (가속도센서와 자이로센서를 이용한 스마트폰 실시간 모션 분석 시스템에 관한 연구)

  • 본 논문에서는 가속도센서와 자이로센서를 통해 측정된 값을 무선 통신을 이용하여 스마트폰으로 전달하여 실시간으로 모션을 분석하는 스마트폰 실시간 모션 분석 알고리즘을 제안한다. 3축 가속도 센서의 실시간 모션 분석과 중력가속도를 사용한 모션 분석의 경우에는 장소나 높이 또는 주변의 자력에 따라 정확한 값을 획득하여 분석하기 어려운 점이 있다. 이에 본 논문에서는 가속도 센서와 자이로 센서를 통하여 보다 정밀한 모션 분석을 하였으며, 이를 이용하여 모션을 실시간으로 분석하여 활용하면 스포츠와 의학 등 다양한 분야에서 활용할 수 있을 것이다.

  • 폐암은 전 세계적으로 사망률이 가장 높은 암 질환으로, 조기 발견 및 신속한 치료를 위해서는 흉부 X-ray 영상 내 악성 결절을 놓치지 않는 것이 중요하다. 그러나 흉부 X-ray 영상은 정밀도의 한계로 진단 결과에 대한 신뢰도가 낮아, 이를 보조하는 도구의 개발이 요구된다. 기존의 폐암 진단 보조 도구는 학습 기반의 기법으로, 진단 결과에 대한 설명성(explainability)이 없다는 위험성을 갖는다. 이에 본 논문에서는 통계 분석에 기반한 결절의 석회화 여부 진단 기법을 제안한다. 제안하는 기법은 결절과 해부학적 구조물의 밝기 차 분포로부터 석회화 여부를 판단하며, 그 결과 민감도 65.22%, 특이도 88.48%, 정확도 83.41%의 성능을 보였다.

Clinical Characteristics of Psychiatric Patients with Military Issues Using MMPI-2-RF (군복무 적합성 평가를 위해 정신건강의학과에 내원한 환자군의 MMPI-2-RF 프로파일)

  • Objectives : The purpose of this study was to examine psychological characteristics of patients who visited psychiatric clinic for medical certificate for military service using MMPI-2-RF. We compared target group with general psychiatric patients with same age range. Methods : A total of 165 male patients for medical certificate and 154 general psychiatric patients were collected from the Department of Psychiatry of three university hospitals. There were significant differences of age and educational level between two groups. We used independent t-test, ANCOVA, ${\chi}^2$ test to examine differences between two groups. Results : Medical certificate group scored higher on the validity scales, the Higher-Order(H-O) scales, the Restructured Clinical(RC) Scales, the Specific Problems(SP) Scales, and the Personality Psychopathology Five (PSY-5) Scales. Especially, EID, RC7, HLP, SFD, SAV, SHY, DSF, and INTR-r showed significant differences between two groups on all three statistical tests. Conclusions : The present study showed that psychiatric patients who received psychological evaluation for military service have significantly higher emotional distress, helplessness, lower self-confidence, and lower quality of interpersonal relationships. And the difference of validity scales between two groups could be related with psychological burden of compulsory military service in the study sample which causes elevation on scales of infrequent responses. MMPI-2-RF would be helpful instrument to assess these emotional and psychological characteristics.

Simplification of One Step Immunotech AFP Test (AFP 검사방법 단축에 관한 고찰)

  • Purpose: The Immunotech AFP is a widely used test in 14 hospitals(RIA QC establishments: 46) as a liver tumor marker. Normally, the test takes 45 minutes including complicated 2 step method. We evaluated the possibility of 1 step method and suitable test time. Materials and Methods: Samples of 31 patients in SNUBH were used as subject. We evaluated the sensitivity, intra and inter assay precision, recovery rate, linearity between 1 step and 2 step method (45 min. and 60 min.). Results: Both of 45 minutes and 60 minutes test showed 0.1 ng/mL sensitivity. In 45 minutes test, the intra assay coefficients of variation were 3.05%, 3.43%, 1.68%. Also, inter assay coefficients of variation were 3.91%, 2.38%, 0.82%. In 60 minutes test, the intra assay were 5.00%, 3.69%, 1.97%, inter assay were 3.14%, 3.71%, 1.85%.The correlation coefficient of each 2 step and 1 step (45 min.), 2 step and 1 step(60 min.), 1 step (45 min.) and 1 step (60 min.) were y=0.9293x+2.7356 ($R^{2}=0.9999$), y=0.9193x+4.1002 ($R^{2}=0.9993$), y=0.9894x+1.3805 ($R^{2}=0.9997$). Conclusion: Compared 1step method to 2 step method, both method showed very reasonable precision, recovery rate and correlation coefficient. Also, 1 step method(45 min.) of correlation coefficient ($R^2$) was 0.999. We suggest that 1 step method test can reduce the test time and is useful for AFP test.
