  • 일반적으로 오존(O$_3$)은 친전자성 및 친핵성반응 때문에 탄소-탄소 이중결합을 가진 분자들을 떼어놓을 수 있는 매우 강한 산화능력을 갖고 있는 것으로서 살균, 맛과 냄새의 조절, 색도 제거 등에 이용되어왔다. 그러나, 오존은 대기 중에 미반응상태로 잔류하는 경우에는 광화학 스모그를 발생시키는 주된 요인이 되고 있으며, 강력한 산화력을 지니고 있기 때문에 0.1-l ppm의 범위에서 인체에 노출되면 두통, 목 건조증, 점막손상과 같은 호흡기 질환 등의 원인이 될 뿐만 아니라 악취를 유발하여 불쾌감을 야기한다. (중략)

  • 후두 및 기관협착증은 선천적으로 발생하기도 하지만 대부분이 후천적으로 발생한다. 후천적인 원인으로는 외부로부터의 외상에 의하기도 하지만 대개가 기관내 삽관 후유증으로 발생하는 것으로 알려져 있다. 기관내 삽관에 의한 기도 점막의 손상은 대부분 가역적이기는 하지만 영구적으로 협착을 초래하는 경우, 그 치료에 있어서 많은 노력이 필요할 뿐만 아니라 치료 자체가 매우 까다롭고 실패 할 수도 있다. 따라서 후두 및 기관협착증의 원인을 정확히 이해하고 예방하는 것이 매우 중요하다. 이러한 관점에서 후두 및 기관협착증의 선천적, 후천적 원인들에 대해서 검토하여 보았다. 특히 기도 내 삽관에 따른 협착증이 가장 많은 빈도를 차지하는 만큼, 이의 병태생리와 발생에 관여하는 여러 인자들에 대해서 중점적으로 문헌 고찰하였다.

노출평가를 위한 TLV 근거 - PHOSPHORUS OXYCHLORIDE (옥시 염화인)

  • 눈, 피부 및 점막 자극의 가능성을 최소화하기 위해 옥시 염화인(phosphorus oxychloride)의 직업적 노출기준 TLV-TWA를 0.1ppm(0.63 mg/㎥)으로 권고하였다. 옥시 염화인 독성의 징후 및 증상으로는 두통, 현기증, 식욕 부진, 메스꺼움, 흉통 및 호흡 곤란이 있고 심한 급성 또는 반복 만성 노출에 의해서는 기관지 폐렴, 폐부종 그리고 신장 손상을 유발한다. 옥시 염화인은 물 또는 습한 공기에서 분해되어 염화수소와 인산을 형성한다. 피부, 감작제(SEN), 발암성 표기와 TLV-STEL을 권고하기에 충분한 자료가 없다.

The Effects of Leukocyte Depleted Priming Solution on the Gastric Mucosal $Co_{2}$ Partial Pressure and Serum IL-8 Level during Cardiopulmonary Bypass in Korean Mongrel Dogs (한국산 잡견에서 백혈구 제거 충진액이 체외순환 중 위점막 이산화탄소 농도와 IL-8 수준에 미치는 영향)

  • Background: Cardiopulmonary bypass is an essential process to maintain circulation for saving life during the cardiac surgery, But it is a process in which systemic inflammation was evoked inevitably because of the exposure of blood to foreign surface. The injuries to distal organs during the cardiopulmonary bypass were resulted from systemic inflammation and the disturbances of micro-circulations in the organs. We designed this study to research the effects of leukocyte depletion from pump-oxygenator priming solution on the systemic inflammation, and the micro-circulation of gastric mucosa that is suggested by the gastric mucosal $CO_{2}$ partial pressure and acidity. Material and Method: The dogs were divided into three groups according to the different pump-oxygenator priming solutions; non-hemic crystalloid solution; leukocyte-depleted homologous blood; and non leukocyte-depleted homo-logous blood. Each priming solution group contained five dogs. In all three groups, 2 hours of cardiopulmonary bypass, and 4 consecutive hours of general anesthesia was maintained on the mechanical ventilation. Each dog was evaluated for the gastric mucosal pH, $CO_{2}$ partial Pressure, arterial pH, $CO_{2}$ partial pressure, the exhaled air $CO_{2}$ partial pressure and the level of IL-8 on before the cardiopulmonary bypass, 1 hour after the cardiopulmonary bypass, 2 hours after the cardiopulmonary bypass, 2 hours after the restoration of normal circulation, and 4 hours after the restoration of normal circulation after the cardiopulmonary bypass. The levels of IL-8 were measured with ELISA (enzyme linked immunosorbent assay) technique. Result: 1. There were significant differences of gastric mucosal $CO_{2}$ partial pressure between the leukocyte-depleted homologous blood group and other two groups(vs non leukocyte-depleted homologous blood group; P=0.02, vs non-hemic crystalloid solution group; P=0,01). 2. The gastric mucosal pH of leukocyte-depleted homologous blood group was significantly different from non leukocyte-depleted homologous blood group (p=0.01). 3. The levels of IL-8, which examine the systemic inflammation, showed signi- ficantly better results in leukocyte-depleted homologous blood group and non-hemic crystalloid solution group than non leukocyte-depleted homologous blood group (p=0.01, 0.01). Conclusion: Based upon these results, we concluded that the leukocyte depletion from the pump-oxygenator priming solution has a beneficial effects in reducing systemic inflammation and the preserving of gastric mucosal micro-circulation.

Pathophysiology of the Gastroesophageal Reflux Disease (위식도역류의 병태)

  • 이상에서 고찰한 위식도역류의 병태를 간략히 요약하면 다음과 같다. 위액에는 염산, 펩신, 담즙산과 췌효소등을 포함하고 있어 역류된 위액은 상부 기관식도관에 자극을 주거나 손상을 줄 수 있으며 개개인의 점막 상피의 저항도와 적절한 타액의 분비는 위액에 의한 손상의 정도를 결정하는 중요한 요소로 알려져 있다. 위식도역류로 인한 많은 증상이나 소견은 두가지의 기전으로 나타나서 첫째, 역류된 위액이 직접 조직에 영향을 주거나 둘째, 간접적으로 식도 하부에 분포된 미주신경에 의한 연관자극에 의해 일어나는데 위식도역류의 증상 중 두부, 경부, 심장 및 폐장의 증상은 이들 장기에 미주신경의 일부가 같이 분포하여 일어나는 간접적인 기전이다. 하부식도괄약근(lower esophageal sphincter)의 기능은 역류를 막는 가장 중요한 요소로 알려져 있어 간헐적으로 하부식도 괄약근의 압력이 떨어지면 역류가 일어나게 된다. 정상적인 식도의 연동운동은 식도에 역류된 위내용물을 위로 제거되는데 중요하며 위식도역류 환자에서 흔히 연동운동이 저하되거나 비정상적인 연동운동을 관찰할 수 있다. 또한 위내용물이 소장으로 비워지는 시간이 연장되어 위내용물이 위내에 축적되는 것도 위식도역류의 원인의 하나로 생각된다. 이러한 위식도역류의 병태를 연구하는 것은 이 질환의 이해에 도움을 줄 것으로 생각된다.

A Clinical Observation on Children with Corrosive Esophagitis (소아 부식성 식도염의 임상적 고찰)

  • Purpose: This study was undertaken to evaluate the clinical features and complication such as esophageal stricture in children with corrosive esophagitis. Methods: We retrospectively reviewed medical records of 31 children who accidentally ingested corrosive materials and visited to emergency room of Chonnam National University Hospital from Jan. 1992 to Dec. 1999. Twenty-one children were examined by upper gastrointestinal (UGI) endoscopy to evaluate location and severity of caustic injury. Results: 1) Among 31 patients, there were 20 males and 11 females and the ratio of male to female was 2:1. Average age at diagnosis was 2.3 years (12 months to 9.8 years). Twenty-seven (87.1%) patients were accidentally ingested vinegar. 2) Initial presenting symptoms were dysphagia (54.8%), vomiting (48.3%), chemical burn on lips and skin (45.2%), excessive salivation (45.2%), coughing and respiratory grunting (32.3%) and aspiration pneumonia (9.8%). 3) UGI endoscopic examination showed caustic injury in 17 children: grade I in 8, grade II in 7 and grade III in 2. The region of caustic injury was proximal esophagus in 5, distal esophagus in 3, entire esophagus in 9 and stomach in 6. 4) Corrosive esophageal strictures developed in 6 children (19.4%) and gastric outlet stricture in 1 (3.2%). All of them showed grade II or III caustic injury on endoscopic examination. Conclusion: The development of esophageal stricture was related to the severity of the caustic injury. Early UGI endoscopic examination in caustic ingestion seems to be useful for prediction of development of caustic stricture.

  • This experiment was performed to evaluate the morphological responses of the gastric epithelial cells of the mouse, inoculated with Ehrlich carcinoma cells in the inguinal area, following administration of 5-fluorouracil, mitomycin C or Acriflavine-Guanosine compound (AG60). In this study, each mouse was inoculated with $1{\times}10^7$ Ehrlich carcinoma cells subcutaneously in the inguinal area. From next day after inoculations, 0.2 mL of saline, 5-fluorouracil (30 mg/kg), mitomycin C ($400{\mu}g/kg$) or AG60 (30 mg/kg) were injected to the animals every other day, respectively. Each animals were sacrificed after 7th injection and tissue were taken from the gastric mucosa. Thereafter, the ultrathin sections were stained with uranyl acetate and lead citrate. In the 5-fluorouracil-, mitomycin C- or AG60-treated mice, myelin figures and multivesicular bodies within the gastric mucous epithelial cells were observed more frequently than those of the normal control. In the 5-fluorouracil-treated mice, membrane structures containing a few mucous granules in the luminal space were observed. Indeed, bulging cytoplasmic process containing mucous granules protruding into the gastric lumen were observed in the mitomycin Ctreated mice. Therefore, this study suggested that AG60 as compared with 5-fluorourail and mitomycin C may effective medicine without damage to the secretion ability of gastric mucous epithelial cells.

Intramural Bronchogenic Cyst of the Esophagus -A case report- (식도 근육층에 함몰된 기관지성 낭종 -1예 보고-)

  • Bronchogenic cysts are anomalous cystic lesions of foregut and usually located in the lung or mediastinum. Generally intramuscular cysts of the esophagus are considered as enterogenous foregut malformations. We report a young adult with an intramural bronchogenic cyst causing dysphagia and heartburn. It was located in the muscular layer of the esophagus and was removed without any damage to the mucosa. Histopathologic findings revealed that it was a bronchogenic cyst.

Protective Effects on Gastric Lesion of Ursolic acid (Ursolic acid의 위 손상에 대한 방어 효과)

  • This study is an experiment for gastric protective effects of ursolic acid. In order to identify the effects of ursolic acid on gastrointestinal disorder, acute and chronic gastritis were also observed using HCl ethanol and indomethacin-induced gastric lesion models, respectively. As for gastric acid, it was also identified through proton pump ($H^+/K^+-ATPase$) inhibiting activity. In regards to protective factor for gastric damage, prostaglandin $E_2$ ($PGE_2$) was quantitatively analyzed. Antibacterial activity experiment was done on Helicobacter pylori (H.pylori), which is known to be the causing factor of chronic gastritis, gastric ulcer and gastric cancer. By making use of AGS cell, it was confirmed that ursolic acid was involved in apoptosis of gastric cancer cell through 4',6-diamidino-2-phenylindol (DAPI) staining and flow cytometry analysis. As a result, ursolic acid reduced gastric lesions caused by HCl ethanol and indomethacin. Ursolic acid inhibited acid secretion by inhibiting proton pump ($H^+/K^+-ATPase$), which is the gastric acid secreting enzyme involved at the final phase of gastric acid secretion. And ursolic acid was identified with gastric mucosa protection effects by increasing the concentration of $PGE_2$, a protective factor of gastric mucosa preservation. The antibacterial activity on H. pylori, which is aggressive factor in gastrointestinal disorder, ursolic acid showed inhibitory effects on H. pylori colonization. In the DAPI nuclear staining, unlike the control group, shape of the nucleus has deformed, and has been observed either shrinked cell or chromatin condensation phenomenon. In the Flow cytometry assay, confirmed the growth rate of apoptosis in a concentration-dependent manner.

Radioprotective Effects of Dandelion(Taraxacum officinale) (민들레 추출물의 방사선 방호효과)

  • Dandelion(Taraxacum officinale), an oriental herbal medicine, has been shown to favorably affect choleretic, antioxidative and protection of gastric mucosa. The protective effects of common dandelion leaf and root extract were investigated by irradiation in Sprague-Dawley rats. Male SD rats, 6weeks, were orally injected with dandelion extract(100mg/kg) for 10days. Immediately after final injection, rats were whole body irradiated with 10Gy used irradiation facility(Elekta Linac, Sweden). At 24h-15days after irradiation, complete blood counted test and apoptosis in jejunal crypt cell. Stimulated recovery by the extract was observed in platelet but was not showed in the erythrocyte and leucocyte. The jejunal crypt cells were protected significantly(p<0.05) and the radiation-induced apoptosis was reduced(p<0.05). The survival rate were carried for 15days, the survival ratio was 15% and 85% for the control and experimental group. Observations intestinal inhibition of cell death, intestinal mucosa and tissue decreased systemic inflammation and vacuole. Base on these data, we propose that dandelion may be a useful radioprotector, especially since it is a relatively nontoxic natural product.
