  • The frontal sinus is a functionally important structure. It serves as a cushioning buffer to protect the brain in cases of facial trauma. Fractures of the frontal sinus can lead to aesthetic problems by causing a visible depression in the center of the forehead. The goals of frontal sinus fracture treatment have been to protect intracranial structures and to prevent early and late complications, even with invasive methods. Recently, however, the goals have shifted to preservation of nasofrontal outflow tract function through close observation and the utilization of endoscopic procedures. Excellent cosmetic results can be achieved through minimally invasive surgery. This shift in goals took place due to the ease of diagnosis and treatment of early and late complications. Therefore, patients with a frontal sinus fracture should be followed up continuously to ensure that complications are detected promptly. Herein, the authors describe the methods and current trends of frontal sinus fracture treatment.

A Case Report of an Immediate Frontal Sinus Reconstruction Using an Outer Table Calvarial Bone Graft in an En Bloc Manner (전두동 골절 환자에서 머리덮개뼈의 바깥판을 한판으로 이용한 즉시 전두동 재건술 1례)

  • Background: In a frontal sinus reconstruction, the literature primarily recommends a surgical approach for definite treatment with the exception of for anterior wall fractures with no dislocation. Many studies have assessed a range of methods for the reduction of frontal sinus fractures. This paper presents a case, in whom the anterior wall of the frontal sinus was reconstructed using an outer table calvarial bone graft in an en bloc manner. Patient and methods: A 36-year-old male visited the emergency room with a heavy injury to the forehead. He was diagnosed with fractures of the anterior and posterior wall of the frontal sinus. The neurosurgeon removed the fractured area and repaired the meninges. Afterwards, cranialization was performed and the opening of the nasofrontal duct was obstructed. After fixing the removed bone to its original location, all fragments of fractured anterior wall were purged out and the anterior wall reconstructed using an en bloc calvarial bone graft. Results: In the post-operative 8 months period, there were no complications, the round contour of the forehead was expressed well and the patient was satisfied with the result. Conclusion: There are many methods for reconstructing the anterior wall of the frontal sinus. On the other hand, in cases of large fracture sites with many fractured bone fragments, en bloc harvesting of the outer table calvarial bone could be a better choice than making use of only plates and screws because this method shows a good results in terms of aesthetics with a low complication rate.

  • A 4-year old male Pitbull terrier dog was presented with a history of nasal discharge of left side and dyspnea for 3 days. This dog had a bitewound on the head due to dog-fighting two weeks ago. There was no abnomality except mild anemic sign on blood examination. Frontal projection of radiographs was taken. This was diagnosed as frontal sinusitis. After skin incision on frontal head, a silicone tube was inserted into the affected frontal sinus. Abscess of the frontal sinus was drainaged and irrigated with saline solution containing trypsin and cephazolin. This patient who also treated with enrofloxacin was recovered completely.

  • Foreign bodies in the paranasal sinuses are not commonly observed. Recently, we have observed an interesting case of metalic foreign body (a fragment of bullet) which remained in the posterior part of the left ethmoid sinus for about 30 years, and report here now with a review of literature.

  • 부비동 점액낭종은 주로 전두동과 사골동에 발생하며 점차 커져서 인접 기관에 영향을 미치기도 한다. 저자들은 1985년 1월부터 1991년 3월까지 충남대학교병원 이비인후과에서 부비동 점액낭종으로 수술받았던 15예를 대상으로 임상분석을 실시하여 다음과 같은 결과를 얻었다. 성별은 남자 6예 여자 9예였고, 연령은 17세에서 79세까지였으며 50대에서 5예로 가장 많았다. 병변부위로는 사골동에 국한된 경우가 7예로 가장 많았고, 전두동과 사골동을 동시에 침범한 경우가 4예, 전두동에 국한된 경우가 2예, 사골동과 접형동을 침범한 경우가 1례였다. 안증상은 11예에서 있었으며 그중 안구돌출이 7예로 가장 많았고, 시력감퇴 6예, 안구운동장애 4예 등이었다. 5예에서 두개저 파괴로 인한 경막의 노출이 있었다. 치료는 사골동 비외수술법이 7예로 가장 많았고, Lynch operation 4예, 골성형 전두동수술 2예, Lateral rhinotomy를 통한 배액 1예, Midfacial degloving approach를 통한 배액이 1예였다.

A Simplified Treatment Algorithm of Frontal Sinus Fractures: A Paradigm Shift? (단순화된 전두동 골절 치료 알고리즘: 패러다임의 전환인가?)

  • Purpose: The management of frontal sinus fractures and frontal sinus outflow tract (=FSOT) injuries is controversial. Traditional treatments include frontal sinus obliteration with a variety of material grafts, which all employ complicated algorithms for treatment planning. A simplified algorithm aiming for frontal sinus ventilation, however, can be achieved through the advanced technology of image guided endoscopic surgery and instruments. The authors explain this simplified treatment algorithm of frontal sinus fractures. Methods: The authors reviewed articles on the management of frontal sinus fractures. Results: Complicated treatment planning algorithms of frontal sinus fractures aiming for obliteration were replaced with a simplified algorithm aiming for ventilation of the frontal sinus. Conclusion: The authors suggest that a simplified algorithm can replace the complicated algorithms in selective cases.

Developmental Study of Korean Frontal Sinus by using 3D CT Scan (3차원 컴퓨터단층촬영 영상을 이용한 전두동 발달의 연구)

  • Purpose: Although there are many reports on the development of paranasal sinuses, few studies were performed with respect to the frontal sinus with 3D CT, especially in Korean children. In this study, we evaluated the normal development of frontal sinus in Korean children with 3D CT imaging. Methods: A total of 301 patients under the age of 20 were reviewed retrospectively. Maximal AP (anteroposterior) length, height and width were measured with the aid of computer device. The volume of frontal sinus was also evaluated. Results: At the age of more than 4, the pneumatization of frontal sinus was detected. Frontal sinus developed continuously and was observed in almost every children at the age of more than 17. The AP length, height, width and volume of frontal sinus were enlarged continuously until the age of 20. And the maximal growth of frontal sinus was observed at puberty. The width and volume of frontal sinus showed high correlation coefficient (r=0.8) relative to height or AP distance. Conclusion: The results of this study may be helpful in the comprehension of normal development of frontal sinus in Korean children.

  • Successful middle ear surgery requires the availability of al safe, effective bonding material. Side effect caused by synthetic materials have led to the use of biologic adhesive. However, they carry the risk of transmission of infectious diseases if they are prepared from pooled human blood. The adhesive strength of ammonium sulfate fibrin adhesive produce an adhesive strength that is half that of the homologous commercial product. It is, however, good enough for use in several otolaryngological operations, tympanoplasty, facial nerve repair, reconstruction of ossicles, reconstruction of posterior wall of ear canal and obliteration of frontal sinus and mastoid antrum using bone dust.

  • Osteoma is the most common benign tumor originating in the paranasal sinuses and the frontal sinus is the most frequent site of osteoma of paranasal sinuses. Osteoma may occur at any age but the most frequent incidence is the third decade and those are histologically classified as compact, cancellous and mixed type. Recently, the authors have seen a patient with exophthalmos and downward displacement of eyeball due to a huge osteoma in the left frontal sinus which was removed by osteoplastic operation of the frontal sinus and the frontal sinus defect was implanted with adipose tissue from the patient own left abdomen. We report this case with review of the literature.
