The Difference of Predictor of Bone Mineral Density in Pre and Postmenopausal Women (폐경에 따른 골밀도 예측인자의 차이)

  • We studied the relationship between prediction parameters and bone mineral density for pre-and-post menopausal women. We measured BMI%Fat by BIA, blood pressure and lipid profiles for 483 adult women who are in NPO state. SBP, TC, TG, LDL have significant statistical value in the postmenopause women group and postmenopause woman. The value of postmenopause women of these parameters are lower than premenopause woman. BMD has the most strongest relationship with LBM. The BMD and LDL level of postmenopause women have statistically negative relationship. The results show that for the premenopause cases, weight, BF, and HDL level were the major factors which affect the BMD. For postmenopause cases, however, weight, age, and LDL level turned out to be the most significant factors.

Gender Difference in Associations between Serum Cholesterol Levels and Depression Symptoms in Healthy General Population (건강한 성인에서 성별에 따른 혈중 콜레스테롤 농도와 우울증상의 연관성 : 후향적 의무기록 조사)

  • Objectives : The purpose of this study was to determine the association between serum lipid profiles and depression according to gender difference. Methods : This retrospective cohort study included 27,452 subjects(15044 men and 12408 women) who underwent health examination. The duration was from January 2013 to December 2013. We estimate the correlation between serum lipid profile and Beck Depression Inventory(BDI) scores. We compare the effect size using beta coefficient. Results : In men, serum Triglyceride level was correlated positively with BDI scores(r=0.020, p<0.01). Serum LDL-C and HDL-C were negatively correlated with BDI scores(r=-0.015, p<0.01 ; r=-0.016, p<0.05). In women, Triglyceride level was also correlated positively with BDI scores(r=0.020, p<0.01), Serum HDL-C were negatively correlated with BDI scores(r=-0.019, p<0.01). There was no statistical significance between Serum LDL-C and Beck Depression Inventory(BDI) score. Conclusions : Both men and women had more depressive symptoms when they had low serum HDL-C level or high serum Triglyceride level. The depression symptoms were more severe when serum LDL-C level was low only in men.

  • Hyperlipidemia, the status of blood with high level of low-density lipoprotein cholesterol (LDL-C), is known as a main cause of coronary artery diseases such as myocardiac infarction or brain infarct. Statin is the representative prescription to hyperlipidemia and the effects of it depend on the patient's individual conditions such as health-caring habits or adherence to medication. The main effect of statin is reducing LDL-C, which should reach the target range based on National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III) guideline. In this research, the reduction of LDL-C and attainment to patient's target range are considered effects of statin. The association between factors - individual conditions and adherence to medication of patients - and the effects of statin is analyzed with National Health Insurance Service-National Sample Cohort (NHIS-NSC).

Efficacy of Acipimox in Comparison with Fenofibrate for Hypertriglyceridemia (고중성지방혈증에서 fenofibrate에 대한 acipimox의 효과 비교)

  • 목적: 이상지방혈증 환자의 치료는 우선적으로 저밀도지단백을 감소시키고, 저밀도지단백이 목표수치에 도달한 이후에도 혈중 중성지방이 높을 경우 nicotinic acid 또는 fibrate를 사용하도록 권장되고 있다. 본 연구는 이상지방혈증이 있는 환자에서 acipimox의 효과를 fenofibrate와 비교하여 분석하고자 시행되었다. 방법: 본 연구는 서울에 있는 한 3차 대학병원의 환자를 대상으로 후향적으로 의무기록을 분석하여 시행되었다. 혈중 중성지방 농도가 200 mg/dL 이상으로써 acipimox 또는 fenofibrate를 신규처방 받은 환자를 대상으로 각각의 약물이 지단백에 미치는 영향을 36주간 추적하여 비교분석 하였다. 결과: Acipimox를 투여 받은 환자 41명, fenofibrate를 투여 받은 환자 62명이 모집되었으며, 각각의 약물을 복용한 환자군의 기본적인 인구학적인 특성은 유의하게 상이하지 않았다. 3개월 간의 약물투여 후 두 약물군 환자 모두에서 총콜레스테롤(p < 0.05) 및 저밀도지단백(p < 0.001)이 약물투여 전과 비교하였을 때 유의하게 감소하였고, 고밀도지단백은 모든 환자에서 유의하게 증가하였다(p < 0.05). 한편 중성지방 감소율은 acipimox군이 fenofibrate군에서보다 더 크게 나타났다(p < 0.05). 약물유해반응의 빈도는 두 약물군 간에 유의한 차이가 없었다. 결론: 총콜레스테롤, 저밀도지단백 콜레스테롤 등을 감소시키거나 고밀도지단백 콜레스테롤을 증가시키는 효과는 acipimox와 fenofibrate가 유의하게 다르지 않았으며, 중성지방을 감소시키는 효과는 acipimox가 fenofibrate보다 우월하였다.

Effect of Catechins on Serum Lipids in Obese Women (카테킨 음용이 비만여성의 혈중 지질농도에 미치는 영향)

  • This study was developed to test the effectiveness catechin beverage drinking as exercise supplementary food for obese women. The effects of catekin were determined by weight, total cholesterol, triglycerides, High density lipoprotein cholesterol, high density lipoprotein cholesterol. Twenty-seven obese women were participated in this study. Eight women were tea catechin drinking group, Nine women were exercise group, and ten women were tea catekin drinking-combined with exercise group. After catechin drinking or exercise conducted, blood lipid factor were analyzed. body weight and triglycerides low density lipoprotein cholesterol levels were significantly decreased in tea catekin drinking combined with exercise group (p<0.001) and exercise group (p<0.001). total cholesterol were significantly decreased in three groups (p<0.05). It is thought that together catechin drinking and exercise can maximize weight loss of obese woman and reducing of blood lipid factor.

Analysis of the Relationship between Abdominal Ultrasound Based Kidney Stones, Obesity and Metabolic Syndrome (복부 초음파 검사 기반 신장결석과 비만 및 대사증후군 관련성 분석)

  • Kidney stones are a common disease with an annual prevalence of about 30,000 people in Korea, and are deeply related to an increase in chronic kidney disease, high blood pressure, coronary artery disease, metabolic syndrome, type 2 diabetes, hyperlipidemia, and abdominal obesity. Therefore, in this study, 135 examinees who visited a general hospital in Dalseong-gun from May 2019 to June 2020 for a medical examination were examined. The relationship between kidney stones and factors related to obesity and metabolic syndrome were found in abdominal ultrasound. I tried to find out the relevance. As a result of the study, the risk of kidney stones in the abnormal group was increased by 4.255 times compared to the normal group in total cholesterol factor, and the risk of kidney stones in the abnormal group was increased by 2.072 times compared to the normal group in the low-density lipoprotein cholesterol factor. Total cholesterol factor and low-density lipoprotein cholesterol It was found that the factor affects the prevalence of kidney stones and metabolic syndrome. Since the risk of kidney stones is related to total cholesterol and low-density lipoprotein cholesterol factors, active attention should be paid to preventive purposes through health check-ups.

Relationship between HsCRP and Pulse Transit Time (HsCRP와 맥파전달시간에 대한 연구)

  • The purpose of this study is to evaluate the relationship between high sensitive C-reactive protein (hsCRP) and pulse transit time (PPT). Apparently healthy 233 subjects had been enrolled in the health promotion center of the Pusan National University Hospital from Jan. 29 to Feb. 26, 2004. They had no previous history of diabetes, hypertension and hyperlipidemia. Subjects were categorized according to tertiles of hsCRP level [Group 1: first tertile $(0.01\;{\sim}\;0.02\;mg/dl)$, Group 2: second tertile $(0.03\;{\sim}\;0.05\;mg/dl)$, Group 3: third tertile $(0.06\;{\sim}\;0.12\;mg/dl)$, and Group 4: Fourth tertile $(0.13\;{\sim}\;16.8\;mg/dl)$]. PTT body mass index (BMI), total cholesterol (T-C), LDL-cholesterol(LDL-C), blood sugar (BS), systolic blood pressure (sBP) and diastolic blood pressure (dBP) were significantly different among hsCRP groups (p<0.05). HsCRP is positively related with BMI, tryglyceride (TG), LDL, sBP and dBP (p<0.05), and negatively related with PTT and HDL-cholesterol (HDL-C) (p<0.05). PTT is significantly negatively related with hsCRP, T-C, TG, LDL-C, BS, dBP and sBP (p<0.05). The hsCRP and PTT were related before controlling BMI, T-C, LDL-C, sBP, and dBP, but not related after conkolling. The relationship between hsCRP and PTT depends on cardiovascular disease risk factors.

  • We obtained medical test data of 150 male from National Health Insurance Corporation, that included physique, blood chemistry, and lifestyle. ${\chi}^2-test$ and logistic regression, and multiple regression were done to verify how CAD risks and/or lifestyle are different between obese and non-obese person. SPSS(version 18) was used for statistical procedures and p-value of .05 was regarded as good through. ${\chi}^2-test$ showed the frequency of hypertension, htperglycemia, hyperlipidemia were higher in obese person than in non-obese This result told us that lifestyle modification could be a key to prevent cardiovascular disease by controlling and managing body composition, especially abdominal obesity.

Relationship of Body Fat Percent with Serum Lipid Level and Blood Pressure in Adults (Impedance Fat Meter로 측정한 체지방 비율과 혈청 지질치 및 혈압과의 관련성)

  • This study was conducted to clarify the relationship of body fat percent with serum lipid level and blood pressure in adults. The study subjects were 472 men and 189 women who visited Multiphasic Health Screening Center of Yeungnam University Hospital in Taegu from May 20 to September 30, 1994. The relationship of serum lipid and blood pressure with BMI, Katsura index, atherogenic index, which calculated from the health screening data and body fat percent measured by impedance fat meter(model SIF-819) were analyzed. Three groups were classified as Group I (men : body fat Percent $\geq$ 20, women : body fat percent $\geq$ 25), Group II (men : 15 $\leq$ body fat percent< 20, women : 20 $\leq$ body fat percent< 25), Group III(men : body fat percent < 15, women body fat percent <20). In this study, Group I accounted for 3.2% in men, 3.7% in women. Weight was significantly different among three groups in both sexes(p<0.01) and height was not significantly different among three groups. In men, serum total cholesterol, triglyceride, high density lipoprotein, low density lipoprotein, atherogenic index were significantly different(p<0.01). In women, serum total cholesterol and low density lipoprotein were significantly different(p<0.05) but there was no differences in triglyceride and high density lipoprotein among three groups. BMI and Katsura index were significantly different among three groups in both sexes(p<0.01). In men, body fat percent was positively correlated with weight, BMI, Katsura index, total cholesterol, triglyceride, low density lipoprotein, atherogenic index and systolic and diastolic blood pressures, and negatively correlated with high density lipoprotein. In women, body fat percent was positively correlated with age, height, weight, BMI, Katsura index, total cholesterol, triglyceride, low density lipoprotein and atherogenic index, and negatively correlated with high density lipoprotein. But there was no significant correlation between body fat percent and blood pressure in women. In multiple regression analysis for total cholesterol, fat percent, age and BMI were significant independent variables in men$(p<0.05,\;R^2=0.1286)$, and body fat percent and age in women$(p<0.05,\;R^2=0.3399)$. In case of LDL/HDL ratio, only BMI was a significant independent variable in menu$(p<0.01,\;R^2=0.0954)$, and body fat percent, age and BMI in women$(p<0.05,\;R^2=0.3164)$. In multiple regression analysis, age, low density lipoprotein and total cholesterol were significant independent variables on systolic blood pressure in men$(p<0.05,\;R^2=0.1297)$, age and total cholesterol in women$(p<0.05,\;R^2=0.1705)$. On diastolic blood pressure, only age was a significantly independent variable in men$(p<0.01,\;R^2=0.0972)$ and women$(p<0.01,\;R^2=0.1218)$. From the result of this study, it could concluded that body fat percent was significantly associated with other obesity indices and serum lipid, but had no significant association with blood pressure. To establish the relationship of body fat percent with blood pressure, further study which consider other variables that may have an effect on blood pressure should be performed.

Effects of Dietary Cholesterol on Male Reproductive Tracts by Regulating PCSK9 Gene (콜레스테롤 식이가 Pcsk9 유전자 조절을 통해 남성 생식기관에 미치는 영향)

  • Proprotein convertase subtilisin/kexin type 9 (PCSK9), is a protein mainly secreted by a liver. The PCSK9 plays an important role in low density lipoprotein (LDL) metabolism acting as a repressor of LDL receptor through transportation of the LDLR to the lysosome for degradation. Thus, the PCSK9 inhibitor suppresses PCSK9-regulated degradation of the LDL receptor as a LDL-lowering medicine. However, little is known about the role of PCSK9 in the reproductive system. Therefore, in the present study, we investigated Pcsk9 expression in male reproductive tracts including penises, prostates and testes using rats in response to their diets between a normal diet and a high-fat diet with cholesterol. Based on our previous study, the high-fat diet elevates concentration of total cholesterol and LDL in serum whereas it reduces the concentration of plasma high density lipoprotein (HDL). In addition, it dramatically affects to morphological changes of the male reproductive organs. Consistent with these results, the expression of Pcsk9 was substantially decreased in the penile tissues (P < 0.001) from rats fed a high fat diet as compared to a normal diet. Moreover, it slightly reduced in the prostate and testes (P < 0.05) of rats in response to a high fat diet. Localization of Pcsk9 was predominantly detected in urethral epithelium of penises, cylinder-shaped cells of prostate glands, and spermatogonia, spermatocytes and spermatid of testes of rats. Collectively, results of current study provide invaluable insights into the Pcsk9 gene with respect to its tissue- and cell-specific expression by a high fat diet with cholesterol.
