A Comparative the Assessment of Oral Health Plans in Schools for Disabled Youth in Different Regions (지역별 장애인 학교 구강보건사업 평가 비교)

  • The purpose of this study was to examine the oral health plans of 142 special schools for disabled youth in different regions across the nation based on the self-evaluation of their school nurses in an attempt to help improve school oral health plans and provide useful information on the planning of oral health plans for schools for disabled youth, as the oral health plans of special schools for disabled youth depended on local characteristics. As a result of make a synthetic evaluation, the oral health plans of special schools located in Gyeongsang Province ranked first, and those of special schools in Seoul scored low due to the smallest number of items included. And it's recommended to select separate objectives of oral health education geared toward physically disadvantaged children, to extend school fluoride mouth rinse and pit & fissure sealing projects, and to enhance the management of school dental clinics.

A Cross-Sectional Study on Job Training Course of the Dental Hygienists at the Public Health (sub)Centers (보건(지)소 치과위생사의 구강보건직무교육실태에 관한 단면적 조사연구)

  • About 380 dental hygienists who were working in public health centers in 2004 joined this study with questionnaires about the evaluation of job training programs regarding oral health care. The findings are as follows. 1. As for the general characteristics of subjects including location, age and career, the dental hygienists in Public Health Subcenters outnumbered the dental hygienists in Public Health Centers, as the former accounted for 61.1 and the latter 35.3 percent. And the dental hygienists in their 36~40s made up the largest age group. The group in their 11~15 years career revealed 48.4, over 16 years career 35.7, under 10 years career group 15.9 percent, respectively. 2. The portion of certification in the respondents was 28.9 percent, as compared to the 71.1percent of non-certification. The major reasons of non-certification were not-applying(44.6%), participation rejected (29.3%). 3. Freqently requested curriculums were school based oral health program(4.42, the highest by Linkert 5 point scale), followed by oral health education(4.41), public oral health services for the toddler and for the elderly(4.04), for the disabled(3.92), oral health planning evaluation(3.85) and oral health survey& investigation(3.69). The gap between Dental hygienists at Public Health Center and Public Health Subcenter was statistically significant different(p < 0.001) in oral health survey& investigation and oral health planning evaluation.

  • 정부의 건강증진사업은 국민의 건강수명 연장과 삶의 질 향상에 궁극적인 목적이 있다. 이는 건강 지지적인 환경을 조성하고 질병을 예방${\cdot}$관리하며 평생 건강서비스를 제공하고 건강한 생활양식을 형성하도록 함으로써 달성될 수 있는 것이다. 건강증진사업을 위한 기반으로 연구와 개발 강화, 법과 규칙의 조정, 기금과 인력과 같은 자원 확보는 필수적이다. 정부의 건강증진전략은 첫째 자원할당의 우선순위를 확보하는 것으로 건강증진, 질병 및 손상의 예방, 재활관련 사업에 더욱 많은 자원을 할당하는 것이며, 둘째로 국가차원의 건강증진 프로그램을 개발하여 실행하는 것이다. 포괄적인 생애주기별 건강증진 프로그램(건강증진종합계획)과 모아보건, 학교보건, 산업보건, 그리고 만성질환예방프로그램 등이 포함된 포괄적인 프로그램을 개발하여 실행하는 것이다. 셋째로, 건강증진기금을 공공보건을 위한 하부기반과 건강증진 프로그램에 투자함으로써 사업의 하부기반을 정비하는 것이다. 건강증진사업을 위한 하부기반으로는 정책과 위임사항을 들 수 있는데, 국민건강증진법과 국민건강증진종합계획, 2005년 6월에 서울시 의회를 통과한 건강도시추진위원회를 들 수 있다. 정부의 건강증진사업의 법적 기반은 1995년에 제정된 국민건강증진법이다. 국민건강증진법은 국민들이 건강에 대한 가치를 인식하고 책임의식을 함양하며, 올바른 건강지식을 갖고, 국민들이 건강한 생활양식을 실천할 수 있는 환경 조성을 목적으로 하고 있다. 건강증진사업은 대중을 위한 보건교육과 건강상담, 영양관리, 구강보건관리, 질병의 조기발견과 치료를 위한 건강검진, 지역사회 건강문제에 관한 조사와 연구, 담배소비 감소와 건강증진부담금 부과를 통한 국민건강상태의 향상을 위한 사업이다. 훈련과정으로는 보건복지부에서 2005년부터 수행하고 있는 시와 지역을 위한 현장관리 프로그램과 서울시가 수행예정인 지역수준의 지도자 훈련과정이 있다. 건강증진사업의 원활한 진행을 위한 기금조성은 건강증진기금이 담배세로부터 조성되고 있으며 2004년 12월 31일 현재 담배 한 갑 당 500원으로 인상되어 부과되고 있다. 기금의 관리와 운용은 보건복지부가 담당하며, 기금은 건강한 생활양식형성에 대한 지원활동, 국민을 위한 보건교육과 교육자료 개발, 건강증진과 만성질환에 대한 연구, 질병의 조기발견을 위한 건강검진, 구강보건관리활동에 사용되고 있다. 향후 건강증진사업 투자계획은 1단계 (98-02년)에는 사업기반조성기, 2단계(03-06년) 보건소사업발전기, 3단계(07-11년)통합사업정착기로 구성되고 2단계의 인프라구축에 사용될 투자 비율은 30%에서 3단계에 15%로 감소될 예정이며, 사업실행 영역은 50%에서 65%로 확대될 계획이다. 2005년 건강증진사업의 중점목표는 건강증진사업의 지방 분산화, 건강증진사업의 근거마련, 사회적 형평성의 달성에 있다. 건강증진사업의 지방분산화를 위해서는 중앙에 관리센터가 설치되어 기획과 평가, 연구와 개발, 현장관리 훈련을 담당하게 되고, 지역관리센터에서는 자치적인 보건소 중심 건강증진사업의 수행과 평가를 진행하게 된다. 건강증진사업의 근거마련을 위해서 효과가 입증된 사업에 우선순위를 두며, 기획과 평가위원회를 설치하고, 건강증진사업의 평가결과를 환류 할 수 있는 체계를 마련하는 것이다. 또한 건강증진포럼을 구성하며 현 건강조사 체계를 수정한다. 한편 형평성 제고의 측면에서는 저소득층, 노인, 장애인들과 같은 취약계층의 건강상태 향상을 위한 중앙정부와 지방자치단체의 역할을 강화하고, 지역간${\cdot}$사회적 집단 간의 건강증진사업관련 형평성을 제고한다.

A Case of Xerostomia that Occurred during Behcet's Disease Drug Treatment of a Person with Physical Disability (지체 장애인의 베체트병 약물치료 중 발생한 구강 건조증 치험 1례)

  • Objectives : To report a case of drug-induced xerostomia treated with Integrative Korean Medicine Visiting Care Service of a Public Health Center. Methods : Home visit treatment and telemedicine were provided by public health doctor(Korean medicine doctor) for person with physical disability who were diagnosed with Behcet's Disease. Palmul-tang(PMT) soft extract was provided steadily 3 times a day for 6 months, and acupuncture, electro-acupuncture, and electro-moxibustion were provided irregularly 6 times for 6 months. Yin-deficiency questionnaire(YDQ), oral health impact profile-14(OHIP-14), oral pain visual analogue scale(VAS), and xerostomia inventory(XI) were used 3 times(baseline, intermediate and termination) to evaluate the changes. Results : YDQ decreased from 620 to 375, OHIP-14 decreased from 34 to 6, VAS decreased from 70 to 40, and XI decreased from 44 to 34. Four scales all gradually decreased. Conclusions : Integrative Korean Medicine Treatments in health insurance including PMT soft extract through home visit treatment can improve drug-induced xerostomia and Quality of life(QoL) of person with disabilities.

Evaluation of oral health service program for disabled persons in Ulsan, Korea (울산광역시 장애인 구강진료사업의 성과 분석 : 초중고 학생을 중심으로)

  • Objectives : The purpose of this study was to evaluate the oral health status of the disabled people in Ulsan. Methods : Two dentists taken a calibration training for national oral health survey examined the Oral health status on 110 disabled people in Ulsan, in 2009. The surveyed disabled people, 6 to 19 years old, have been supplied with the oral health care services by dentists, oral hygienists and volunteers at dental clinics of schools and a dental clinic supported from Ulsan Metropolitan City Nam-Gu Health Center since 1997. The obtained data from these surveys were analyzed with the SPSS statistical package. Results : Among subjects aged 12-14 years, DMF rate was 38.2%; Active D rate, 17.6%; DMFT index, 1.15; DT rate, 33.33%; MT rate, 0.00%; FT rate, 66.67%. Oral heath status of disabled people in Ulsan were evaluated to be a similar level to the Ulsan non-disabled citizens from 2010 Korean National Survey. Conclusions : The oral health care programs for disabled people using voluntary dentists, oral hygienists and other workers in Ulsan are evaluated to be effective for the disabled people.

A Study on Oral Health Projects for the Disabled in public health center (보건소의 장애인 구강보건사업에 관한 연구)

  • Oral health projects that cater to the disabled should be more prevailing in order to ensure the maintenance and successful promotion of the oral health of disabled people. 70 public dental clinics that conducted oral health projects geared toward the disabled were examined to get a precise grip on their oral health projects. The findings of the study were as follows: 1. 31 out of 70 public dental clinics investigated(44.3%) were equipped with two or more dental hygienists who were professional human resources in charge of the oral health projects for the disabled. As for the age and disability type of the beneficiaries of the oral health projects, adolescents(74.3%) and people with mental retardation(87.1%) benefited most from the oral health projects. Concerning the most common implementation frequency of the projects, the projects were carried out once to three times a week(62.9%). 2. The most dominant oral disease treatment provided to disabled people was amalgam treatment and resin treatment(68.6%), which were the early dental caries treatment. The most common preventive treatment that was offered to improve their oral health was oral prophylaxis(82.9%). As for reform measures for the oral health projects, education of personnels in charge of the projects and their specialization(58.6%) were most emphasized. 3. Regarding factors related to the preventive oral health projects for the disabled, the implementation of oral prophylaxis and toothbrushing education was linked to the age of the beneficiaries. More oral prophylaxis was offered to teens, and more toothbrushing education was provided to preschoolers and adolescents. The age of the beneficiaries and the number of dental hygienists responsible for the projects had something to do with the application of fluorides. 4. Concerning the relationship of the preventive oral health projects for the disabled to the number of dental hygienists, one of the personnels in charge of the projects, the application of fluorides( 54.4%) and pit & fissure sealing(56.8%) were more prevalent when there were two or more dental hygienists. There was a statistically significant disparity in that regard(p<0.05). The above-mentioned findings illustrated that in order to boost the oral health of the disabled, dental hygienists who are responsible for the oral health projects for the disabled should put ceaseless efforts into fostering their professional knowledge and ability and offering quality service to disabled patients. Every public dental clinic should be equipped with plenty of professional personnels to enlarge the scope of treatment and ensure the efficiency of treatment and the preventive projects.

Case report : Comparative Study Before and After Oral Health Education Program for People with Developmental Disabilities (증례보고 : 발달장애인 구강보건교육사업 전후 비교)

  • A total of nine oral health educators for 17 persons with developmental disabilities conducted oral health education once a week for four weeks. The oral health educators provided oral health knowledge education and individual toothbrushing teachings. The developmentally disabled people in their 20s and 30s conducted oral health education without a guardian. Ten people who completed all the four pieces of training had no significant improvement in the toothbrushing method and the recommended number of toothbrushing sessions a day, as well as a lack of growth in oral health knowledge. However, 58.8% of the individuals expressed satisfaction with the oral health education program, whereas, 52.9% expressed the desire for a re-education. Although the ratio of teachers to students was 1:2, and the oral health education conducted four times, it was insufficient to promote a successful oral health behavior or knowledge for people with developmental disabilities. Therefore, as a suggestion, oral health education for people with developmental disabilities ought to proceed with their guardians to promote the success of the training.
