Impedance-based Long-term Structural Health Monitoring for Jacket-type Tidal Current Power Plant Structure in Temperature and Load Changes (온도 및 하중 영향을 고려한 임피던스 기반 조류발전용 재킷 구조물의 장기 건전성 모니터링)

  • Jacket-type offshore structures are always exposed to severe environmental conditions such as salt, high speed of current, wave, and wind compared with other onshore structures. In spite of the importance of maintaining the structural integrity for offshore structure, there are few cases to apply structural health monitoring (SHM) system in practice. The impedance-based SHM is a kind of local SHM techniques and to date, numerous techniques and algorithms have been proposed for local SHM of real-scale structures. However, it still requires a significant challenge for practical applications to compensate unknown environmental effects and to extract only damage features from impedance signals. In this study, the impedance-based SHM was carried out on a 1/20-scaled model of an Uldolmok current power plant structure under changes in temperature and transverse loadings. Principal component analysis (PCA) was applied using conventional damage index to eliminate principal components sensitive to environmental change. It was found that the proposed PCA-base approach is an effective tool for long-term SHM under significant environmental changes.

Is it necessary to distinguish semantic memory from episodic memory\ulcorner (의미기억과 일화기억의 구분은 필요한가)

  • The distinction between short-term store (STS) and long-term store (LTS) has been made in the perspective of information processing. Memory system theorists have argued that memory could be conceived as multiple memory systems beyond the concept of a single LTS. Popular memory system models are Schacter & Tulving (994)'s multiple memory systems and Squire (987)'s the taxonomy of long-term memory. Those m models agree that amnesic patients have intact STS but impaired LTS and have preserved implicit memory. However. there is a debate about the nature of the long-term memory impairment. One model considers amnesic deficit as a selective episodic memory impairment. whereas the other sees the deficits as both episodic and semantic memory impairment. At present, it remains unclear that episodic memory should be distinguished from semantic memory in terms of retrieval operation. The distinction between declarative memory and nondeclarative memory would be the alternative way to reflect explicit memory and implicit memory. The research focused on the function of frontal lobe might give clues to the debate about the nature of LTS.

  • The neck laceration is one of the most dangerous & emergent condition which is encounted by the otolaryngologist in emergency room. For the purpose of rapid & adequate management, we analyzed the clinical datum of 28 patients who visited our emergency room & were managed by the otolaryngologist from Jan.1987 to Mar.1993. The incidence was the highest in drunken males of third & forth decades who arrived at emergency room in night, especially AM 2:00. The most common cause & injured site were stabbing by broken glasses & Lt. submandibular area. Most patients were recovered by primary suture without any complications.

  • Trauma to the head and neck region can have serious consequences for vital organs such as the brain, and injuries to blood vessels can cause permanent neurological damage or even death. Thus, prompt treatment of head and neck vessels is crucial. Although the level of evidence is moderate, an increasing amount of research indicates that endovascular treatments can be a viable alternative to traditional surgery or medical management. Embolization or reconstructive endovascular procedures can significantly improve patient outcomes. This article provides an overview of various endovascular options available for specific clinical scenarios, along with examples of cases in which they were employed.

  • Chronic lateral ankle instability (CLAI) is a major complication of acute ankle sprain that can cause discomfort in both daily and sports activities and may lead to degenerative changes in the ankle joint as the long-term sequelae. The precise cause of CLAI remains debatable and is most probably multifactorial. However, identifying chronic lateral ankle ligament injury is critical in determining the primary etiology and formulating an appropriate treatment plan. This review discusses the features and effectiveness of various imaging techniques in diagnosing chronic lateral ankle ligament injury.

Damage Estimation of Structures by Second Order Modal Perturbation (2차 모우드 섭동법에 의한 구조물의 손상도 추정)

  • Most civil engineering structures such as bridges, power plants, and offshore platforms are apt to suffer structural damages over their service lives caused by adverse loadings, such as earthquakes, wind and wave forces. Accumulation of structural damages over a long period of time might cause catastrophic structural failure. Therefore, a methodology for monitoring the structural integrity is essential for assuring the safety of the existing structures. A method for the damage assessment of structures by the second order inverse modal perturbation technique is presented in this paper. Perturbation equation consists of a matrix equation involving matrices of structural changes(stiffness and mass matrix changes) and matrices of modal property changes(natural frequency and mode shape changes). The damages of a structure are represented as changes in the stiffness matrix. In this study, a second order perturbation equation is formulated for the damage assessment of structures, and solved by an iterative procedure. The effectiveness of the proposed method has been investigated through a series of example analysis. The estimated results for the structural damage indicated that the present method yields resonable estimates for the structural changes.

  • Injury of posterolateral corner is unusual, but it can cause disability due to severe instability and cartilage damage. However, the anatomical structures, diagnosis and treatment have not defined clearly yet. Posterolateral corner injury is regarded as the one of main factor to the results of failure in cruciate ligament reconstcution if it was undiagnosed and untreated. Diagnosis of postetolateral corner injury is consists of physical exam, radiographic finding, MRI, and arthroscopic findings. The treatment method of of postetolateral corner injury depends on the time and severity of injury. Anatomical reconstruction of posterolateral corner shows the better clinical outcome than non anatomical reconstructions, but the clinical results of long term follow up is still needed. Therefore, the aim of this article is to review the recent literatures and to organize diagnosis and treatment of posterolateral corner injury.

Analysis of Long-Term Performance of Geogrids by Considering Interaction among Reduction Factors (감소계수 상호영향을 고려한 지오그리드의 장기성능 해석)

  • Total reduction factor that is used when calculating allowable tensile strength of geogrids is made by multiplying the installation damage reduction factor ($RF_{ID}$), chemical degradation reduction factor ($RF_D$), and creep reduction factor ($RF_{CR}$) etc. In case of a model estimating allowable tensile strength considering reduction factor over the short-term tensile strength of geogrids, it has a limit of not considering interaction force between reduction factors. Junction strength comes to be reduced by installation damages or chemical degradation in the same way as tensile strength. Single junction test method cannot properly test damaged samples and shows large deviations as it does not consider scale effect. Besides, regarding calculating shear strength, no reasonable study on reduction factors was conducted yet. Therefore, in this study, reduction factors that may affect the long-term performance of geogrids were revaluated considering various conditions and accurate long-term allowable tensile strength was calculated considering interrelation between reduction factors. Creep results after installation damage and chemical resistance test showed lower value than calculated value according to GRI GG-4. After the installation damage test and the chemical resistance test, the reduction factor of junction strength was less than that of tensile strength. Shear strength before and after installation damage showed no change or increase.

Acute Respiratory Distress Syndrome in Respiratory Intensive Care Unit (호흡기계 중환자실에서 치료 관리된 급성호흡곤란증후군의 임상특성)

  • Background : Patients with established ARDS have a mortality rate that exceeds 50 percent despite of intensive care including artificial ventilation modality, Mortality has been associated with sepsis and organ failure preceding or following ARDS ; APACHE II score ; old age and predisposing factors. Revised ventilator strategy over last 10 years especially at ARDS appeared to improve the mortality of it. We retrospectively investigated 40 ARDS patients of respiratory-care unit to examine how these factors influence outcome. Methods : A retrospective investigation of 40 ARDS patients in respiratory-care unit with ventilator management over 46 months was performed. We investigated the clinical characteristics such as a risk factor, cause of death and mortality, and also parameters such as APACHE II score, number of organ dysfunction, and hypoxia score (HS, $PaO_2/FIO_2$) at day 1, 3, 7 of severe acute lung injury, and simultaneously the PEEP level and tidal volume. Results : Clinical conditions associated with ARDS were sepsis 50%, pneumonia 30%, aspiration pneumonia 20%, and mortality rate based on the etiology of ARDS was sepsis 50%, pneumonia 67%(p<0.01 vs sepsis), aspiration pneumonia 38%. Overall mortality rate was 60%. In 28 day-nonsurvivors, leading cause of death was severe sepsis(42.9%) followed by MOF(28.6%), respiratory failure(19.1 %), and others(9.5%). There were no differences in variables of age, sex, APACHE II score, HS, and numbers of organ dysfunction at day 1 of ARDS between 28-days survivor and nonsurvivors. In view of categorized variables of age(>70), APACHE II score(>26), HS(<150) at day 1 of ARDS, there were significant differences between 28-days survivor and nonsurvivors(p<0.05). After day 1 of ARDS, the survivors have improved their APACHE II score, HS, numbers of organ dysfunction over the first 3d to 7d, but nonsurvivors did not improve over a seven-day course. There were significant differences in APACHE II score and numbers of organ dysfunction of day 3, 7 of ARDS, and HS of day 7 of ARDS between survivors and nonsurvivors(p<0.05). Fatality rate of ARDS has been declined from 68% to less than 40% between 1995 and 1998. There were no differences in APACHE II score, HS, numbers of organ dysfunction, old age at presentation of ARDS. In last years, mean PEEP level was significantly higher and mean tidal volume was significantly lower than previous years during seven days of ARDS(p<0.01). Conclusions : Improvement of HS, APACHE II score, organ dysfunction over the first 3d to 7d is associated with increased survival Decline in ARDS fatality rates between 1995 and 1998 seems that this trend must be attributed to improved supportive therapy including at least high PEEP instead of conventional-least PEEP approach in ventilator management of acute respiratory distress syndrome.

Expression of Intercellular Adhesion Molecule- 1 after Ischemia Reperfusion Injury of the Canine Lung (폐장의 허혈-재관류 손상과 세포간부착물질-1 의 발현)

  • Background: Predicting the important role of intercellular adhesion molecule-1 expression on the acute ischemia-reperfusion injury, we set out to demonstrate it by assessing the degree of expression of ICAM-1 after warm ischemia-reperfusion in canine unilateral lung ischemia model. Material and Method: Left unilateral lung ischemia was induced by clamping the left hilum for 100 minutes in seven adult mongrel dogs. After reperfusion, various hemodynamic pararmeters and blood gases were analyzed for 4 hours, while intermittently clamping the right hilum in order to allow observation of the injured Ieft lung function. The pulmonary venous blood was collected serially to measure TNF- and cICAM-1 level. After 4 hours of reperfusion, the lung tissue was biopsied to assess cICAM-1 expression, and to measure tissue malondialdehyde(MDA) and ATP level. Result: The parameters including arterial oxygen partial pressure, pulmonary vascular resistance and tissue MDA and ATP level suggested severe lung damage. Serum TNF-$\alpha$ level was 8.76$\pm$2.37 ng/ml at 60 minutes after reperfusion and decreased thereafter. The cICAM-1 level showed no change after the reperfusion during the experiment. The tissue cICAM-1 expression was confirmed in 5 dogs. Conclusion: The increase of TNF-$\alpha$ Ievel and expression of tissue ICAM-1 were demonstrated after ischemia reperfusion injury in canine lung model.
