  • 노인장기 요양보험제도가 시행된 지 2년이 흘렀다. 그동안 가족이 책임졌던 노인에 대한 장기요양을 국가와 사회가 부담함으로써 만성질환 및 노인성 질환으로 어려움을 겪는 노인과 가족들의 부담이 줄어들었다. 하지만 적지 않은 문제점도 드러나고 있다. 외국의 사례를 통해 우리나라 노인장기 요양보험제도의 현재와 미래를 그려보고자 한다.

The relations of the Long-term care insurance awareness and the aged parent-supporting (장기요양보험제도 안식과 노부모 돌봄과의 관계)

  • The purpose of this research was to examine the whether to recognition of the long-term care insurance have affected on aged parent-supporting. The research method was used self-report questionnaire to 331 persons who was supported their parents. And I used the Frequency, simple linear regression as an analysis methods. The result demonstrated that Educational background, the emotional intimacy of the subjects, and the recognition degree of the long-term care service have significant influence on their physically parent supporting consciousness. The researchers suggest the need of family support system based on the result for decrease the parent-supporting burden in middle age generation.

A Study of Home-visit Care Agencies' Perceptions and Attitudes toward Home and Community Long-term care Agency Quality Assessment (HCLA-QA) Program (방문요양기관의 재가장기요양기관평가에 대한 인식 및 태도)

  • National Health Insurance Corporation initiated Home and Community Long-term care Agency Quality Assessment(HCLA-QA) program in 2010. This study aims to explore long-term care agencies' perceptions and attitudes toward HCLA-QA program, focused on the home-visit care agencies. Total 473 agencies completed the questionnaires(response rate 13.6%), using the e-mail and fax survey methods with high-capacity web-fax server of NHIC. On the results of this study, we found the agencies' different perceptions and attitudes on HCLA-QA program according to their quality ratings as QA results. Compared to the agencies with low quality ratings, the agencies with high quality ratings tend to have more time for preparation, better understandings about HCLA-QA program, more positive perceptions that quality indicators generally are valid, and more affirmative attitudes that the notifications of QA results are helpful for agencies. Our findings can be very useful for establishing strategies for improving the HCLA-QA program. The policy makers, the insurer, and the agencies need to consider the opinions of the agencies with high quality ratings as well as the ones with low quality ratings.

The Difference-in-Difference Model Analysis about the Effects of Long-Term Care Insurance on Family Relationships' Change (노인장기요양보험제도가 가족관계의 변화에 미치는 영향에 대한 이중차이모델 분석)

  • The purpose of this study is to examine the effects of long-term care insurance(macro-system) on family relationship(micro-system)s' change based on ecosystems theory. Data come from the second(2007) and the fourth(2009) Korean Welfare Panel Study(KWPS). Experimental group is the beneficiary using long-term care insurance and the member of household in their households, and control group is the non-beneficiary not using long-term care insurance and the member of household in their households. The main findings of difference-in-difference model analysis are as follows. First, the ecosystems theory is a theory examining the correlation between long-term care insurance and family relationship. Second, the effects of long-term care insurance is not income effect but independent effect. This result shows that the meaning of family has faded away and family relationship has been weaken and that long-term care insurance has not complete characteristics as a social insurance. Thus, system reforms of long-term care insurance is highly needed for system comprehensiveness, coverage, adequacy, and service accessibility and is changed into family-friendly social policy.

A Study on the Dental Hygienist's Awareness on the Elderly Long-Term Care Insurance System (치과위생사의 노인장기요양보험제도에 대한 인식도 연구)

  • This research targeted 253 dental hygienists who are working in parts of the South Gyeongnam Province to identify their Degree of Recognition on the Elderly Long-term Care Insurance System executed on July 1,2008 according to their awareness. The following conclusions were obtained. 1. Experience in managing elderly patients' oral cavity and specialized education on the elderly patients while studying dental hygiene (department) manifested statically significant difference with the appropriate age of the Long-term care worker(p<0.01, p<0.05). Moreover, there was significant difference in the level of understanding on the Elderly Long-term Care Insurance System depending on the experience of volunteering and on whether they got specialized education on the elderly patients while working(p<0.01). 2. There was significant difference in the awareness of the Elderly Long-term Care Insurance System following interest in the health of the elderly patients' oral cavity(p<0.05, p<0.01, p<0.001).

Convergence study of long-term care insurance system for elderly of awareness of middle-aged people and demand for dental services in the system (한국 중년층의 노인장기요양보험제도에 대한 인식과 구강보건진료에 대한 요구의 융합 연구)

  • The purpose of this study was to awareness of dental care services in the elderly long-term care insurance system and retirement preparation of middle-aged people. The survey respondents had a 56.1% saving for old age life, And showed a tendency to they don't like depend on one's family(41.8%), elderly care was intended to use facility services and nursing care. 36.7% of respondents answered "I know a little" about the system. Also, the need for dental care services within the system was high (98.5%), If the service is provided, 92.3% of the respondents said that they would participate, otherwise unrecognized rate of dental care services in long-term care insurance was 85.2%. Therefore, it is necessary to expand the awareness of the part of dental care service in the system. The results of this study can be used as basic data for the improvement of the system of dental care services in long-term care insurance for elderly.

The Impact of the Korean Long-Term Care Insurance System on its Family Caregivers: Focusing on Family Caregiving Arrangement (노인장기요양보험제도에서 재가노인돌봄의 변화에 대한 연구)

  • The purpose of this study was to investigate the changes that family caregivers have experienced since using the Korean Long-Term Care Insurance(LTCI) system. In-depth interviews were conducted to determine how the services offered within the LTCI program had affected family caregiving and what changes they had incurred. Results from the qualitative content analysis show that the LTCI program significantly reduced the caregiving burden among family caregivers although burdens that family caregiver perceived varied greatly depending on the types of service that the family selected, and assigned family caregivers different identities and diverse roles(i.e., service user, family caregiver, certified care provider) depending on the service they use. The phenomenon of 'certified family care provider', which was not an intention of LTCI, demonstrates the practical need of elderly persons who require both care and the comfort of family and economic status of the family. Despite the positive impact of the LTCI policy on the family caregivers' burden and family relationship, the current LTCI system should be modified in order to better meet the needs of beneficiaries and their family caregivers.

  • The purpose of this study is to explore the enlightenment of the Korean long-term care insurance policy on Qingdao's long-term care insurance policy through policy comparison. China and Korea are very similar in terms of cultural background, living habits, and population structure. Therefore, the successful experience of Korean long-term care insurance has great implications for Qingdao even China to build a long-term care insurance system. Through the literature review, this article compares the long-term care insurance policy implemented by Qingdao City and Korea in Gilbert & Terrell's social welfare policy analysis framework. With the comparison this article discusses about the existing problems of the current pilot policy system in Qingdao, such as lack of legislation support and financial independence, assessment standards are not detailed, and human resources are insufficient. The author raises five suggestions to improve Qingdao's long-term care policy as the conclusion of this paper: legislation support, detailed assessment standard, expand categories of benefits, enrich delivery network, optimize financing sources.

  • The major purpose of this study is to evaluate the impact of the long-term care insurance program. In order to estimate the impact of policy accurately, certain bias which might hamper the validity of this study has been removed by Propensity Score Matching(PSM) and Double Difference(DD) method from the semi-experimental design. To study the effects of long-term care insurance on the elderly and their family members as social outcome variable sand the quality of life of their family and satisfaction in family relationships, the third and fourth waves of Korea Welfare Panel are used to match experimental and comparative groups by the propensity score matching. Then, DD method, using the panel fixed effects model, is applied to estimate the differences of those groups'treatment effects before and after the policy implementation. As a result, it was found that the Quality of life on the elderly and their family members is statistically meaningless, while the satisfaction in family relationships has much increased after the policy implementation. The result has a limitation in that this evaluation is performed at the point when the long-term insurance program has not been ripened enough. However, there is an important implication on the significance of realizing the main goal of the long-term care insurance to improve the quality of life of the elderly and their family members and as for the potentiality of further system improvements.

  • This study aims to investigate causes of the emergence of so-called 'family-care workers' in the Long-term Care Insurance system in Korea. The LTCI system introduced in 2008 financially support the utilization of formal care services for the eligible elderly with care needs by paying for services of their care workers. Interestingly, 38.4 percent of payments for the in-home services were claimed by family members registered as qualified long-term care workers in 2012. We interviewed ten family care workers in depth and analyzed the needs of the aged and their families to explain the emergence of family care workers. The emergence of family-care workers is an inevitable result of choice by family members who face a dual burden of living and caring; be the additional choice following discharge the duty to support the elderly; be the alternative choice to fulfill unaccepted needs for services. These results suggest the needs for a comprehensive public provision of both income and social service support for the aged and an introduction of financial support for family care complementing the formal care support in the LTCI in Korea.
