The Gastric-Bile Juice Reflux in Patients from Hepatobiliary Scan with Subtotal Gastrectomy (위 부분절제술 환자의 간담도 스캔에서 위-담즙역류)

  • Purpose: The hepatobiliary scan is the examination which is the possibility of knowing the function of liver, gall bladder and closing of the biliary tract. Also, after subtotal gastrectomy, the increased gastric-bile reflux is known as the primary reason of reflux gastritis. In this study, according to changing the acquisition method, we tried to prove the reflux time and reflux index in patients who underwent subtotal gastrectomy. Materials and Methods: From Oct 2008 to Jan 2009, 72 patients with subtotal gastrectomy who took the hepatobiliary scan (man: 52, woman: 20, age range: 31-77, mean age: $60.5{\pm}7$) in our department. We used the radiopharmaceutical $^{99m}Tc$-mebrofenin 185 MBq/0.5 cc. After 5 minutes, we acquired 300,000 counts anterior image on supine, and then we acquired right lateral and $45^{\circ}$ LAO position by using the time setting method. We acquired 30 min, 60 min, 90 min, 120 min and fatty meal by the same method. We painted the ROI of liver, GB and CBD on 30 min anterior image and LAO image in patients had occurred the bile juice reflux. And then we painted the ROI of stomach on others image. We calculated the reflux index from those values. Results: According to this study, we found out 40 patients (55.6%) who had occurred the gastric-bile juice reflux (1 person from 30 min, 7 persons from 60 min, 4 persons from 90 min, 28 persons from after fatty meal). Hourly, the bile reflux highest level is 6 persons from 60 min, 2 persons from 90 min, 32 persons from fatty meal among those people. The reflux index of anterior is 0.85-23.36% (mean 6.53%). The reflux index of LAO is 1-29.13% (mean 8.89%). By visual assessment, we can distinguish that there were 26 of the 40 patients (65%) had occurred the reflux on LAO image more than anterior image. Conclusion: We find out that the patients with subtotal gastrectomy had occurred gastric-bile juice reflux more than 50% from hepatobiliary scan. And after eating fatty meal, we can know that it's the most possible to occur the gastric-bile juice reflux. When it happened, we have to change the acquisition method to reduce the overlap between colon and stomach. This study will be more valuable in diagnosis.

A Study on Projection Angles for an Optimal Image of PNS Water's View on Children (유.소아 부비강 Water's 영상의 이상적 구현을 위한 촬영기준각도 변화에 관한 연구)

  • This study is to calculate the proper angle for the optimal image of PNS Water's view on children, comparing and analyzing the PNS Water's projection angles between children and adults at every age. This study randomly selected 50 patients who visited the Medical Center from January to May in 2005, and examined the incidence path of central ray, taking a PNS Water's and skull trans-Lat. view in Water's filming position while attaching a lead ball mark on the Orbit, EAM, and acanthion of the patients's skull. And then, we calculated the incidence angles(Angle A) of the line connected from OML and the petrous ridge to the inferior margin of maxilla on general(random) patients's skull image, following the incidence path of central ray. Finally, we analyzed two pieces of the graphs at ages, developing out the patients' ideal images at PNS Water's filming position taken by a digital camera, and calculating the angle(Angle B) between OML and IP(Image Plate). The angle between OML and IP is about $43^{\circ} in 4-years-old children, which is higher than $37^{\circ}, as age increases the angle decreases, it goes to $37^{\circ} around 30 years of age. That is similar result to maxillary growth period. We can get better quality of Water's image for children when taking the PNS Water's view if we change the projection angles, considering maxillary growth for patients in every age stage.

Objective Analysis of the Set-up Error and Tumor Movement in Lung Cancer Patients using Electronic Portal Imaging Device (폐암 환자에서 Electronic Portal Imaging Device를 이용한 자세 오차 및 종양 이동 거리의 객관적 측정)

  • Purpose : The aim of this study is to investigate the random and systematic errors and tumor movement using electronic portal imaging device in lung cancer patients for the adequate margin in the treatment planning of 3-dimensional conformal therapy. Material and Methods : The electronic portal imaging device is matrix ion chamber type(Portal Vision, Varian). Ten patients of lung cancer treated with chest irradiation were selected for this study. Patients were treated in the supine position without immobilization device. All treatments were delivered by an 10 MV linear accelerator that had the portal imaging system mounted to its ganrty. AP or PA field Portal images were only analyzed. Radiation therapy field included the tumor, mediastinum and supraclavicular lymph nodes. A total of 103 portal images were analyzed for set-up deviation and 10 multiple images were analyzed for tumor movement because of respiration and cardiac motion. Result : The average values of setup displacements in the x, y direction was 1.41 mm, 1 78 mm, respectively. The standard deviation of systematic component was 4.63 mm, 4.11 mm along the x, y axis, respectively while the random component was 4.17 mm in the x direction and 3.31 mm in the y direction. The average displacement from respiratory movement was 12.2 mm with a standard deviation of 4.03 mm. Conclusion : The overall set-up displacement includes both random and systematic component and respiratory movement. About 10 mm, 25 mm margins along x, y axis which considered the set-up displacement and tumor movement were required for initial 3-dimensional conformal treatment planning in the lung cancer patients and portal images should be made and analyzed during first week of treatment, individually.

The Position for Measuring BMD of the Distal Radius and The Study of the Correlation Between the Distal Radius and Lumba (원위 요골 골밀도의 측정 자세 및 요추 골밀도의 상관관계에 관한 연구)

  • The aim of this study was to evaluate the change of bone mineral density according to distal radius rotation and the correlations of the lowest BMD measured by DXA at the lumba versus distal radius. The eleven males were projected distal radius by DR X-ray and the measurement of BMD by DXA of the appropriate position of the forearm were performed on 21 males. The healthy 11 and 21 volunteers without any history of operations, anomalies, or trauma were enrolled. The experiment was performed by two methods. First, The DR X-ray was measured distal radius of 11 males in pronation and supination with three, six and nine degrees, including a neutral position. The ROI was measured by the m-view program on the PACS monitor. Second, The DXA was measured distal radius of 21 males in pronation and supination with five and ten degrees, including a neutral position to evaluate the changes of BMD according to the rotation. A correlation of the BMD in the distal radius with BMD that lumbar spine was performed, along with analysis of the data by SPSS 12.0v. The mean rotation angle of the distal radius about eleven males DR X-ray measured $7^{\circ}$ of pronation (82%, n = 9), $6^{\circ}$ of supination and $0^{\circ}$ of neutral of (9%, n = 1), The total average rotation angle in 11 male was $5.1^{\circ}$ of pronation. The rotation angle of the distal radius about twenty one males on DXA measured $7.2^{\circ}$ of pronation (43%, n = 9), $7^{\circ}$ of supination (24%, n = 5), and $0^{\circ}$ of neutral (33%, n = 7), The total average rotation angle in 21 people was $4.1^{\circ}$ of pronation. The correlation of the analysis of lumba and distal radius were r = 3.0, p = 0.18. consequently, The correlation was not significance. Because BMD of lumba was not coverd for BMD of the distal radius, with a neutral position, Pronation is needed for BMD in the distal radius with the rotation angle measuring at the lowest BMD. the rotation angle about five degrees of pronation of the distal radius is recommended.

Comparative analysis of status of safety accidents and importance-performance analysis (IPA) about precautions of safety accidents by employment type of industry foodservices in Jeonbuk area (전북지역 산업체급식소 조리종사자의 고용형태에 따른 안전사고 실태 및 안전사고 예방관리에 대한 중요도와 수행도 분석)

  • Purpose: The purpose of the study was to evaluate the status of safety accidents and importance-performance analysis (IPA) between regular and non-regular employees in industry foodservices. Methods: The participants were regular employees (n = 119) and non-regular employees (n = 163) in industry foodservices in the Jeonbuk area. Demographic characteristics, status of safety accidents, safety education, and importance and performance status were assessed using a self-administered questionnaire. Results: Approximately 66.4% of regular employees and 53.4% of non-regular employees experienced safety accidents (p < 0.05). Types of safety accidents of regular and non-regular employees were mostly burns, and causes were mostly from their own negligence. Approximately 98.3% of regular employees and 95.1% of non-regular employees experienced safety education. Approximately 88.9% of regular employees and 96.8% of non-regular employees received safety education from dietitians. Approximately 41.9% of regular employees and 50.0% of non-regular employees had difficulty applying the contents of safety education due to lack of time during work. As a result of IPA, regular and non-regular employees were aware of the importance of the following and performed them well: 'Clean the floor of the work place', 'Arrange in the work area', 'Wear safety shoes', 'Check for heater cord', and 'Safety cooking when using oil'. On the other hand, they were not aware of the importance of the following and performed them insufficiently: 'Check for the MSDS', 'Aware of chemical signs', 'Wear protection gloves etc.', 'Do stretching exercise', and 'Using ancillary tools'. Conclusion: Therefore, it is necessary to improve the consciousness of dietitians for effective application of safety education contents, development of contents, especially MSDS, and related things.

USABILITY EVALUATION OF PLANNING MRI ACQUISITION WHEN CT/MRI FUSION OF COMPUTERIZED TREATMENT PLAN (전산화 치료계획의 CT/MRI 영상 융합 시 PLANNING MRI영상 획득의 유용성 평가)

  • Purpose : By taking advantage of each imaging modality, the use of fused CT/MRI image has increased in prostate cancer radiation therapy. However, fusion uncertainty may cause partial target miss or normal organ overdose. In order to complement such limitation, our hospital acquired MRI image (Planning MRI) by setting up patients with the same fixing tool and posture as CT simulation. This study aims to evaluate the usefulness of the Planning MRI through comparing and analyzing the diagnostic MRI image and Planning MRI image. Materials and Methods : This study targeted 10 patients who had been diagnosed with prostate cancer and prescribed nonhormone and definitive RT 70 Gy/28 fx from August 2011 to July 2013. Each patient had both CT and MRI simulations. The MRI images were acquired within one half hour after the CT simulation. The acquired CT/MRI images were fused primarily based on bony structure matching. This study measured the volume of prostate in the images of Planning MRI and diagnostic MRI. The diameters at the craniocaudal, anteroposterior and left-to-right directions from the center of prostate were measured in order to compare changes in the shape of prostate. Results : As a result of comparing the volume of prostate in the images of Planning MRI and diagnostic MRI, they were found to be $25.01cm^3$(range $15.84-34.75cm^3$) and $25.05cm^3$(range $15.28-35.88cm^3$) on average respectively. The diagnostic MRI had an increase of 0.12 % as compared with the Planning MRI. On the planning MRI, there was an increase in the volume by $7.46cm^3$(29 %) at the transition zone directions, and there was a decrease in the volume by $8.52cm^3$(34 %) in the peripheral zone direction. As a result of measuring the diameters at the craniocaudal, anteroposterior and left-to-right directions in the prostate, the Planning MRI was found to have on average 3.82cm, 2.38cm and 4.59cm respectively and the diagnostic MRI was found to have on average 3.37cm, 2.76cm and 4.51cm respectively. All three prostate diameters changed and the change was significant in the Planning MRI. On average, the anteroposterior prostate diameter decrease by 0.38cm(13 %). The mean right-to-left and craniocaudal diameter increased by 0.08cm(1.6 %) and 0.45cm(13 %), respectively. Conclusion : Based on the results of this study, it was found that the total volumes of prostate in the Planning MRI and the diagnostic MRI were not significantly different. However, there was a change in the shape and partial volume of prostate due to the insertion of prostate balloon tube to the rectum. Thus, if the Planning MRI images were used when conducting the fusion of CT/MRI images, it would be possible to include the target in the CTV without a loss as much as the increased volume in the transition zone. Also, it would be possible to reduce the radiation dose delivered to the rectum through separating more clearly the reduction of peripheral zone volume. Therefore, the author of this study believes that acquisition of Planning MRI image should be made to ensure target delineation and localization accuracy.

3D Stereoscopic Augmented Reality with a Monocular Camera (단안카메라 기반 삼차원 입체영상 증강현실)

  • This paper introduces an effective method for generating 3D stereoscopic images that gives immersive 3D experiences to viewers using mobile-based binocular HMDs. Most of previous AR systems with monocular cameras have a common limitation that the same real-world images are provided to the viewer's eyes without parallax. In this paper, based on the assumption that viewers focus on the marker in the scenario of marker based AR, we recovery the binocular disparity about a camera image and a virtual object using the pose information of the marker. The basic idea is to generate the binocular disparity for real-world images and a virtual object, where the images are placed on the 2D plane in 3D defined by the pose information of the marker. For non-marker areas in the images, we apply blur effects to reduce the visual discomfort by decreasing their sharpness. Our user studies show that the proposed method for 3D stereoscopic image provides high depth feeling to viewers compared to the previous binocular AR systems. The results show that our system provides high depth feelings, high sense of reality, and visual comfort, compared to the previous binocular AR systems.

Current State of the Development of Traditional Korean Gardens, and Problems Aspects, in Overseas Countries (한국전통정원의 해외 조성 현황 및 문제점 양상)

  • This study is a basic study to develop standards and foundations for the establishment of traditional Korean gardens and aims to understand the current status of their components and expression methods and identify problems by investigating Korean gardens developed overseas. Nine sites were selected for field surveys and monitoring assessments. The results suggest: Overall, there is a lack of popular generality and temporal characteristics among these gardens, as they are mere reproductions of historical elements. There have also been errors of traditional and experimental interpretations. In terms of design aspects, traditional gardens are primarily compilations of landscape elements and certain ornamental features. In terms of landscape, they tend to be insufficient in parlaying appropriate spatial scales and experiential hierarchies; they also lack considerations of the context of neighbouring landscapes. In terms of guidance and information delivery, there is a worldwide lack, in general, of recognition of Korean gardens, given the broad variety of names attached to them; therefore, name standardization is recommended. In terms of development, management, and use, it is essential that designers suggest plant types, as well as alternatives, that match the characteristics of a given space; a receptive attitude vis-$\grave{a}$-vis the characteristics of their use is required.

Analysis of the recommended science books' suitability for elementary school students (초등학생 과학 추천도서의 적합성 분석)

  • In this study, science recommended books for elementary school students were analyzed. For the study, the screening tool with a number of criteria was produced. In order to develope a screening tool, we examined several previous studies and references. After that, through discussion and exchange of opinions, the preliminary screening tool was made. In order to ensure the reliability and validity, the pilot test was conducted. Then, detailing questions and modifications have been made by exchange of ideas. And nine science recommended books were analyzed. The results are as follows. First, as a result of examining recommended books, exploration and the nature of science were received a low rating. Second, description of the scientific knowledge was not sufficient. This discussion can be obtained through the following. First, when selecting recommended books, care should be taken in exploration and the nature of science. Second, a wide range of science trade books needs to study more.

  • Change detection, one of the main applications of multi-temporal satellite images, is an indicator that directly reflects changes in human activity. Change detection can be divided into pixel-based change detection and object-based change detection. Although pixel-based change detection is traditional method which is mostly used because of its simple algorithms and relatively easy quantitative analysis, applying this method in VHR (Very High Resolution) images cause misdetection or noise. Because of this, pixel-based change detection is less utilized in VHR images. In addition, the sensor of acquisition or geographical characteristics bring registration noise even if co-registration is conducted. Registration noise is a barrier that reduces accuracy when extracting spatial information for utilizing VHR images. In this study object-based change detection of VHR images was performed considering registration noise. In this case, object-based change detection results were derived considering various pixel-based change detection methods, and the major voting technique was applied in the process with segmentation image. The final object-based change detection result applied by the proposed method was compared its performance with other results through reference data.
