A CASE OF CONDUCT DISORDER CONFINED TO FAMILY CONTEXT (가정에 국한된 품행장애 1례)

  • Conduct disorder has been subclassified by the symptom characteristics of socialization, aggression, or age of onset. In ICD-10, ‘Conduct disorder confined to family context’ has been developed as a distinct subtype. Which delineates the conduct problems which take place only in the family and related situation. These authors experienced a case which presents a child who shows aggressive and violent behavior to his parent and brother, severe tamper tantrum and destructiveness only in family. By through history taking, observation and treatment after admission, we could find that insecure and ambivalent attachment between parent and child due to chronic neglect and abuse, and inconsistent parental behavior contribute to many behavioral and emotional problems of this child. We reviewed the relationship between conduct disorder and family pathology and treatment strategy for conduct disorder with family problems.

Study on the Perceived Stress Degree and Content of Mother of High-Risk Infants in Neonatal Intensive Care Unit (신생아 집중실에 입원한 고위험 신생아 어머니의 스트레스 정도 및 내용에 관한 연구)

  • This study was conducted to understand the degree and contents of stress which the mothers of high-risk infants can be experienced from the hospitalization of ICU for their new borns, and thus to offer the basic program to he nursing intervention program for these. Subjects were the 171 mother of hospitalized newborn in NICU of 1 University Hospital in Busan from June, 20, 2001 to September, 15, 2001, who agreed to take part in this study. The instruments used in this study were Parental Stressor Scale:NICU(PSS:NICU) developed by Miles et al. The questionnaire has 4 dimensions and 45 items ; sight and sounds of NICU(5 items), babies' appearance and behavior nursing intervention(19 items), parental role alteration and relationship with their baby(10 items), health team communication(11 items). The data was analysed as average, frequency, Standard Deviation, t-test, ANOVA, Pearson correlation coefficient by use of SPSS/PC+. The results of this study are as follows ; 1. The total perceived stress level score of mothers of high-risk infants was slightly high(3.44±0.71). The highest scored dimension was 'appearance and behavior of the baby'(4.06±0.80), and next were 'relationship with their baby and parental role change'(3.55±0.98), 'sight and sounds of NICU'(3.22±1.01), 'communication with health team'(2.93±0.91). 2. The total perceived stress level score was significantly correlated with birth weight (F=2.35, p<.05). 3. In sight and sounds of NICU, the perceived stress level score was significantly correlated with nursing in the incubator(t=2.28, p<.05) and birth weight(t=2.26, p<.05). In summary, information about physical environment of NICU, birth weight and nursing in the incubator must be included in nursing intervention program of mother's of high-risk infants in reducing the patents stress level. And, it is suggested that there need to find the coping mechanism of mother of high-risk infants.

The Research on the Breast Feeding and its Related Factors of Premature Infant (신생아 중환자실에 입원한 미숙아의 모유수유 실태와 관련요인 분석)

  • Purpose: The purpose of this research is to provide basic informations for the encouragements of premature infants' breast feeding. Method: From August 10 to October 9, 2002, we have carried out a statical research which surveyed 148 mothers of premature infants registered in NICU. The sample had generated cluster-randomly from 25 General Hospitals, all over the Korea peninsula and being surveyed with 74 questionnaires. Result: Mean hospitalized day of premature infants was 27.9 days. The mean total feeding period was 19.1 days and continuous breast feeding period 12.4days. They were interested in breast feeding education-they answered that they would join the breast feeding education if they were given the chance 87.8%. There was significant relation between babies fed only breast milk and the body weight of birth(p<.05). The reasons why mother gave the baby her breast milk include 'for her baby's health' and 'people said breast milk is good for babies' with a portion of 99%. The main reason why mothers could not execute breast feeding was 'the deficit of breast milk volume' 50.0% and other reason were 'because of start to support more nutritions '18.2%, 'difficulty to carry out the breast milk to hospital'13.6%. The reasen why mothers could not try breast feeding at first were 'deficit of breast milk volume'37.0%, 'not to be prepared for breast feeding because of unexpected delivery'32.6%. Conclusion: We need a program to inform importance and excellent of the breast feeding and a plan to increase the premature infants' breast feeding through the importation of fortifier.

  • Aspiration of tooth can occur not only during dental treatment, but also due to factors like trauma or physiologic exfoliation of primary tooth. If this occurs, complications such as fever, cough, dyspnea, pain, and bronchitis can be appeared. 9 years-old girl with Lennox-Gastau syndrome visited the dental clinic for regular checkups. Calcified tooth-like material was observed in the chest PA x-ray, and maxillary left primary first molar was not observed in the patient's oral cavity. She had a history of hospitalization for pneumonia two months ago. Because it was considered that the tooth was likely to be aspirated and caused pneumonia, the extraction of remaining primary teeth with mobility was performed. It is necessary to reduce the risk of aspiration in patient with disabilities by performing active treatment such as removal of primary tooth with mobility or ill-fitting restorations.

Effects of Maternal Empowerment Program on Stress, Anxiety, Depression and Parenting Confidence in Mothers of Preterm Infants in the Neonatal Intensive Care Unit (신생아집중치료실 입원 미숙아 어머니를 위한 임파워먼트 프로그램이 스트레스, 불안, 우울 및 양육자신감에 미치는 효과)

  • Purpose: This study aimed to evaluate the effects of an empowerment program on maternal stress, anxiety, depression and parenting confidence. Methods: A total of 44 mothers of preterm infants were assigned into an experimental or a control group (n=22 each). The experimental group received the usual nursing care and 7 sessions of an empowerment program. The control group only received the usual care. The program was implemented from June to December, 2016 in the neonatal intensive care unit of K university-affiliated hospital in Daegu, Korea. The outcome variables measured were parental stress (PSS: NICU), anxiety (STAI), depression (CES-D) and parenting confidence. Data were analyzed using t-test or repeated measures ANOVA. Results: Scores for both parental stress (t=3.07 p=.004) and depression (F=3.76, p=.26) were significantly lower in the experimental group than in the control group. However, there were no significant differences in anxiety between the groups (F=0.79, p=.505). Parenting confidence scores (F=9.05, p=.001) were significantly higher in the experimental group than in the control group. Conclusion: A maternal empowerment program can be an effective means of reducing parental stress and depression as well as enhancing parenting confidence, for mothers of preterm infants.

Study on the Perceived Stress of Father of High-Risk Infants in Neonatal Intensive Care Unit(NICU) (신생아 중환자실에 입원한 고위험 신생아 아버지의 스트레스 정도에 관한 연구)

  • Purpose: To investigate the degree and sources of stress which fathers of infants in NICU experience. Method: Eighty-five fathers of NICU infants at 5 university hospitals in Seoul volunteered to complete the Parental Stressor Scale for NICU (PSS: NICU) from September 1, to October 15, 2002. Result: Total perceived stress was 3.50±0.76. The highest score were in Parental Role Alteration (3.76±0.83) and Appearance and Behavior (3.65±1.04). The total perceived stress score (PSS), correlated significantly with method (t=3.50, p=.01), and route (F=5.30, p=.00) of feeding. Light & Sound correlated significantly with birth weight (F=3.39, p=.02), medical diagnosis(F=2.30, p=.03), plan of operation(t=2.50, p=.01), operation (t=3.80, p=.02), method (F=4.90, p=.003), and route (F=4.70, p=.00) of feeding. Infant Appearance and Behavior correlated significantly with birth weight (F=5.12, p=.01), plan of operation (t=2.50, p=.01), method (F=3.50, p=.01), and route (F=7.80, p=.00) of feeding. Parental Role Alteration correlated significantly with care of incubator (t=-.68, p=.03), method of feeding (F=2.80, p=.04) and education level of father (F=3.00, p=.05). Conclusion: Father with NICU infants were concerned mostly with parental role alteration and infant appearance. Nursing intervention should include fathers of NICU infants.

Factors Affecting Performance of Hand Hygiene by Parents of Hospitalized Infants (입원 영유아 부모의 손 위생 수행 영향요인)

  • Purpose: Purpose of this study was to identify factors affecting the performance of hand hygiene by parents of hospitalized infants. Methods: The study was conducted with 209 parents of hospitalized infants from one hospital in Changwon. Data were collected from June through August, 2014 and analyzed using SPSS Win 18.0 program. Results: There were positive correlations between performance of hand hygiene and perceptions of sanitary formula bottle (r=.586, p<.001), attitude to hand hygiene (r=.438, p<.001), and knowledge of hand hygiene (r=.405, p<.001) and between parent efficacy and perceptions of sanitary formula bottle (r=.572, p<.001). Awareness of perceptions of sanitary formula bottle (${\beta}$=.308, p<.001), parent efficacy (${\beta}$=.281, p<.001) and hand hygiene attitude (${\beta}$=.207, p<.001) were significant predictors of hand hygiene performance and explained 46.1% of the variance. Conclusion: To increase infection control medical institutions should provide education on hand hygiene and training for parents of hospitalized infants. In order to enhance the performance of hand hygiene, it is necessary to develop and apply nursing programs for hospitalized infants that include hand hygiene attitude, maintenance of the sanitary formula bottle and parent efficacy.

Case Study of an Intensive Mode Nursing Capstone Design Courses during COVID-19 (코로나19 상황에서 집중이수제로 운영된 간호캡스톤디자인 수업 사례)

  • This study introduces a case consisting of nursing capstone design class delivered in an intensive mode during the COVID-19 pandemic and analyzes the outcome of student capstone designs. The nursing capstone design class was held four hours a week for eight weeks from August 25 to October 27, 2020, and 18 students completed the course. Results of capstone design projects included a storybook designed to educate hospitalized children for their disease, detachable nets for safe wheelchair transfer, teaching aids for pregnant women with gestational diabetes, and adhesive alarm bells for bed rails. Students showed high satisfaction with the intensive mode of delivery in the class and positively evaluated the inclusion of team-based projects, and the average score of overall satisfaction with the course was 4.8±0.41. There is a lack of research on intensive capstone design courses in the nursing field. This study explores measures to help develop flexible cirriculums and enhance learner efficacy in a real-life context. Future research is anticipated to expand on capstone design in nursing.

Obstetric and Neonatal Outcomes of the Teenage Pregnancy (청소년 임신에 따른 임산부와 신생아의 주산기 문제점)

  • Purpose:In recent years, Korea has showed a steady increase in the frequency of teenage birth, while the overall birth rate has declined. As the teenage birth is known as a high risk pregnancy itself, we examined perinatal complications of teenage mothers and whose neonates in aspects of medical problem, and social status and support. Methods:We examined the perinatal characteristics of teenage mothers and whose babies, who were hospitalized at Korea University Ansan Hospital from January 2004 to July 2009 using medical records retrospectively. Twenty-seven teenage mothers and their 28 babies were enrolled in this study. Results:Teenage mothers were all unmarried and showed high rates of preterm labor, maternal anemia, and unexpected delivery. Among them, 11 (40.7%) were from families that were separated. Eleven mothers (40.7%) did not have any antenatal care. There were high rates of prematurity and low birth weight (60.7% and 64.3%, respectively). The complication included: respiratory distress syndrome, patent ductus arteriosus and necrotizing enterocolitis. Fourteen babies (51.9%) were not going to be brought up by their biological parents. Conclusion:Teenage pregnancy had high rates of preterm labor and associated complications, often caused by the lack of proper antenatal care. Babies from unmarried mothers were likely to be adopted and this could be a social burden. Therefore, to reduce unplanned teenage pregnancy and births, sex education and social supports should be provided to all teenagers.

MUSIC THERAPY FOR ADOLESCENTS WITH CONDUCT DISORDER (품행장애 청소년의 음악치료 사례연구)

  • The short-term music therapy was performed for adolescents with conduct disorder admitted to Seoul National Mental Hospital for 3 months from Jun to September, 1998. This case study focused mainly on two female patients who participated regularly in the group music therapy. The music therapy process was divided into three phases;beginning, opening up, and closing. This music therapy session consisted of three parts;hello song as beginning, various musical activities, and sound & movement activity as closing. Free musical improvisation, song discussion, musical monodrama, and sound & movement were the mainly applied techniques. Free improvisation was used to enhance, motivate, identify and contain the adolescents' feelings and ideas. Song discussion was used to convey their thoughts and to support each other. Musical monodrama was used to make them have insights into interpersonal relationships. Sound & movement was used to enhance spontaneity. It made them explore their body and voice as an expressive medium. Throughout three months period of music therapy, patient A's communication skill, socialization, and behavior areas were assessed with improvement. She could use music as a symbolic form and was able to share her feelings about herself and her family. Patient B's self-expression and cognitive areas were assessed with improvement. She became more spontaneous and could verbalize her emotions during the group session. Music as a non-verbal and therefore often a non-threatening medium wherein so much can be expressed provided two female patients an atmosphere where a sense of trust may be regained.
