Effects of anaerobic sealing agents on preload maintenance of screw-retained implant prosthesis with surface of screws (임플란트 보철물 나사의 전하중 유지에 나사 표면에 따른 혐기성 나사 고정제의 효과)

  • Purpose: The purpose of this study was to evaluate the preload maintenance of the retaining screw when using anaerobic sealing agents in implant fixture and abutment components. Specifically, the study examines the effects of anaerobic sealing agents on different types of screws. Materials and methods: External hexagon implants made of titanium and anti-rotational abutments were used. Titanium abutment screws and ebony abutment screws from the same manufacturer were used. The experiment was divided into four groups (n = 10 in each group). In the control group, no sealing agent was used at the implant fixture and abutment screw interface. All abutment screws were tightened according to the manufacturer's recommended torque (30 N.cm). After 24 hours, the removal torque (detorque) of each screw was measured using a digital torque gauge device. The data were analyzed by two-way ANOVA test according to normality distribution satisfaction. Results: Looking at the results for each group, titanium screws and no treatment showed detorque values of 20.3 ± 1.6 N.cm. titanium screws and applied anaerobic sealing agent showed detorque values of 32.4 ± 6.7 N.cm. Ebony screws and no treatment showed detorque values of 20.2 ± 1.5 N.cm. ebony screws and applied anaerobic sealing agent showed detorque values of 30.4 ± 4.5 N.cm. Conclusion: The detorque value was higher in the case of using anaerobic sealing agents in both the titanium screw and ebony screw groups. But there was no difference between the two screws.

Radiographic evaluation of computer aided design/computer aided manufacturing (CAD/CAM) customized abutment of implant (CAD/CAM으로 제작된 임플란트 맞춤형 지대주의 방사선학적 평가)

  • Purpose: In this study, the retrospective radiographic study is executed to evaluate amount of bone loss of various conditions in patients using customized abutment for 4 years of follow-up. Materials and methods: The subjects of this study were implant fixed dental prosthesis using CAD/CAM customized abutments. CAD/CAM customized abutment and fixed dental prosthesis were manufactured by the Prosthodontics Department of Chosun University Dental Hospital from August 1, 2011 to July 31, 2012. Radiological assessments were performed on the patients who were treated by the fixed prosthodontics. After each treatment, a retrospective study was performed for a total of 4 years at 3 months, 6 months, 1 year, 2 years, 3 years, and 4 years. Results: As a result of the study, the customized abutment using CAD/CAM showed less bone loss than the results of existing research. There was no statistically significant differences at alveolar bone loss between splinting group and non-splinting group (respectively 0.27 mm, 0.5 mm). Also, there were statistically significant differences at alveolar bone loss in mx. anterior, mx. posterior, mn. anterior and mn. posterior part (respectively 1.37 mm, 0.39 mm, 0.00 mm, 0.30 mm). Conclusion: The customized abutment using CAD/CAM showed less bone loss than the results of existing research, there were statistically significant differences at alveolar bone loss in implant positions.

Mechanical strength of Zirconia Abutment in Implant Restoration (지르코니아 임플란트 지대주의 기계적 강도에 관한 연구)

  • Purpose: As the esthetic demands of dental implant patients are increased, the demands of zirconia as implant abutment material are also increased. It has non-metalic color, good biocompatibility, high strength and high toughness. Even thought the advatage of zirconia abutment, there are a few studies about mechanical properties of zirconia abutment. This study evaluated the mecanical strength with compressive bending strength and endurance limit of implant-zirconia abutment assembly. Materials and Methods: Static and cyclic loading of implant-Zirconia abutment assembly were simulated under worst case condition according to ISO. Test groups were implants of external butt joint with straight regular diameter and angled regular diameter zirconia abutment, implant of external butt joint with narrow straight diameter zirconia abutment and implant of internal conical joint with straight narrow diameter zirconia abutment. All test group were evaluated the mecanical strength with compressive bending strength and endurance limit. After fatique testing, fracture surface were examined by SEM. Results: The compressive bending strengths exceed 927N. Regular diameter zirconia abutment were stronger than narrow diameter zirconia abutment(P<.05). The endurance limits ranged from 503N to 868N. Conclusion: Within the limitation of this study, zirconia implant abutment exceeded the estabilished values for maximum incisal biting forces reported in the literature.

Effect of Implant Types and Bone Resorption on the Fatigue Life and Fracture Characteristics of Dental Implants (임플란트 형태와 골흡수가 임플란트 피로 수명 및 파절 특성에 미치는 효과에 관한 연구)

  • To investigate the effect of implant types and bone resorption on the fracture characteristics. 4 types of Osstem$^{(R)}$Implant were chosen and classified into external parallel, internal parallel, external taper, internal taper groups. Finite elements analysis was conducted with ANSYS Multi Physics software. Fatigue fracture test was performed by connecting the mold to the dynamic load fatigue testing machine with maximum load of 600N and minimum load of 60N. The entire fatigue test was performed with frequency of 14Hz and fractured specimens were observed with Hitachi S-3000 H scanning electron microscope. The results were as follows: 1. In the fatigue test of 2 mm exposed implants group, Tapered type and external connected type had higher fatigue life. 2. In the fatigue test of 4 mm exposed implants group, Parallel type and external connected types had higher fatigue life. 3. The fracture patterns of all 4 mm exposed implant system appeared transversely near the dead space of the fixture. With a exposing level of 2 mm, all internally connected implant systems were fractured transversely at the platform of fixture facing the abutment. but externally connected ones were fractured at the fillet of abutment body and hexa of fixture or near the dead space of the fixture. 4. Many fatigue striations were observed near the crack initiation and propagation sites. The cleavage with facet or dimple fractures appeared at the final fracture sites. 5. Effective stress of buccal site with compressive stress is higher than that of lingual site with tensile stress, and effective stress acting on the fixture is higher than that of the abutment screw. Also, maximum effective stress acting on the parallel type fixtures is higher. It is careful to use the internal type implant system in posterior area.

Improvement of peri-implant complications through customized prosthesis restoration allowing soft tissue space: a case report (연조직 공간을 허용하는 맞춤형 보철 수복을 통한 임플란트 주위 합병증의 개선 증례)

  • The concept of 'Implant Supracrestal Complex (ISC)' was introduced as a way to determine the impact of implant prosthetic design form on both short-term clinical outcomes and the long-term prognosis of bone and soft tissues around the implant. Implant-prosthesis-abutment complex design forms, such as implant-abutment design, junction, and location, can have important implications for the stable and healthy long-term maintenance of the tissues around the implant. In this case, a customized concave abutment and a prosthesis with an emergence angle of about 30 degrees were restored to a patient suffering from chronic soft tissue inflammation and pain after restoration of an implant prosthesis. It was confirmed that the new prosthesis improved complications by allowing sufficient bone and soft tissue space, achieved appropriate soft tissue sealing, and maintained the long-term stability of the implant.

Three-dimensional finite element analysis for influence of marginal bone resorption on stress distribution in internal conical joint type implant fixture (변연골 흡수가 내측연결 임플란트 매식체의 응력분포에 미치는 영향)

  • Purpose: The change of the marginal bone around dental implants have significance not only for the functional maintenance but also for the esthetic success of the implant. The purpose of this study was to investigate the load transfer of internal conical joint type implant according to marginal bone resorption by using the three-dimensional finite element analysis model. Materials and methods: The internal conical joint type system was selected as an experimental model. Finite element models of bone/implant/prosthesis complex were constructed. A load of 300 N was applied vertically beside 3 mm of implant axis. Results: The pattern of stress distribution according to marginal bone resorption was similar. The maximum equivalent stress of implant was increase according to marginal bone resorption and the largest maximum equivalent stress was shown at model of 1 mm marginal bone resorption. Although marginal bone loss more than 1mm was occurred increasing of stress, the width of the stress increase was decreasing. Conclusion: According to these results, the exposure of thin neck portion of internal conical joint type implant is most important factor in stress increasing.

Removal of fractured implant screws: case report (파절된 임플란트 나사의 제거: 증례보고)

  • Screw loosening and screw fracture of abutment is one of most frequent mechanical complications in implant restoration. Fractured fragments in implant restoration like abutment and screw should be completely removed and the procedure needs minimal damage to the fixture of implant. In some cases, it could fail to remove fractured fragments and cause a lot of damage to the fixture of implant. These situations could render implant unusable at the worst. This article describes three different situations and simple techniques for successful removal of fractured fragments without damage of implants. The procedures used are described in this clinical report.

Customized abutment and screw-type implant prostheses after cementation based on the digital intra-oral impression technique (구강 내 디지털 인상채득을 통한 맞춤형 지대주와 시멘트 합착 후 나사형 임플란트 보철 수복 증례)

  • Fixed dental prostheses such as inlay, onlay, crown, and bridge fabricated by CAD/CAM technique combined with digital impressions is getting popular due to the recent rapid progress of digital impression taking system. For the scope of implant prosthesis, however, digital intra-oral scan hasn't been actively utilized for the fabrication of superstructures. In this case report, 6 cases of titanium-milled custom abutment based on the iTero intra-oral scan data were introduced, five of them were restored with screw-type prosthesis after cementation (SCRP) and the clinical results were satisfactory on restoring the function and esthetics.
