An investigation of reosseointegration according to time course after mechanical loosening of the osseointegrated implant fixtures (표면처리 임플란트 고정체의 의원성 동요 후 시간 경과에 따른 재골유착에 관한 연구)

  • Purpose: The purpose of this study was to investigate the reosseointegration periods when the rough surface implants, which had complete bone-implant ankylosis, suddenly losed the osseointegration. Materials and Methods: The implants with RBM surface treatment were inserted into both tibias of 23 rabbits. Two implants were submerged into each side. After six weeks, the primary removal torque was measured by Digital torque gauge, and then the implants were replaced and submerged to estimate the level of reosseointegration. After assigned healing periods for each group, the removal torque was measured again. BIC (Bone-Implant contact, %) ratio was measured through histomorphometric analysis.Paired t-test was processed by SPSS 14.0. One-way ANOVA and Tukey's post-hoc test was processed to analyze statistically significant differences among the groups. Results: In comparison with the primary removal torque, the secondary removal torque was increased after 11 days and significantly increased from 2 weeks. In fluorochrome labeling, the origin of mineralization was observed after 7 days, which showed as fluorescent bands around the bone-implant interfaces. After 11 days, the bone formation was apparent, and it is increased continuously with the passage of the time. Conclusion: In 11 days after the implant replacement, the secondary removal torque was almost as same as the primary value, and was significantly higher from 2 weeks. The mineralized shapes were observed in 7 days after the implant replacement, and then the bone formation appeared visibly in 11 days.

  • Esthetics is important in restoring maxillary anterior area. Alveolar bone resorption and loss of interdental papilla may be minimized by immediate implantation. Previous studies showed successful results with the immediate implantation in healthy extraction socket, while many of these studies objected the immediate implantation into extraction sites with periapical lesions. Recent studies, however, reported successful results of the immediate implantation into extraction sites with periapical lesions with careful debridement of extraction sockets and general medication of antibiotics prior to implantation. A 73-year-old female visited the department of Prosthodontics in ${\bigcirc}{\bigcirc}$ University Dental Hospital with the chief complaint of fallen post-core and crown on left maxillary incisor. Although the incisor was with vertical root fracture and periapical lesion, the immediate implantation following the extraction of tooth was planned. Thorough socket debridement, irrigation with chlorhexidine, and tetracycline soaking were followed by immediate implantation. The general medication of antibiotics (Moxicle Tab.$^{(R)}$, 375 mg) was prescribed before and after the surgery. Immediate provisional restoration was delivered two days after the surgery, and the definitive metal-ceramic restoration was placed about six months later after reproducing the emergence profile from the provisional restoration. This case presents satisfying result esthetically and functionally upto two years after the placement of prosthesis with the harmonious gingival line and no loss of marginal bone.

  • Peri-implantitis is the most common reason for a late failure and can occur even after years of successful osseointegration. The role of microbial plaque accumulation in the development of peri-implantitis has been well documented. On the other hand, the ideal method of implant surface decontamination to re-establish the health of peri-implant tissue remains to be determined. Removal of bacterial deposits is essential in the treatment of peri-implant infections, and various therapeutic approaches have been described in the literature, including mechanical debridement, disinfection with chemotherapeutic agents, and laser therapy. Recently, there has been a plenitude of scientific data regarding the use of laser irradiation to achieve titanium surface decontamination. Thus, research is focusing on lasers' potential use in the treatment of peri-implantitis. The aim of this literature review is to analyze and evaluate the efficacy of laser therapy for the treatment of peri-implantitis.

Comparison of removal torque of dual-acid etched and single-acid etched implants in rabbit tibias (단일, 이중 산처리 임플란트의 회전제거력 비교)

  • Purpose: Chemically strong-acids (HF and $HCl/H_2SO_4$) dual etching implant surfaces have higher strengths of osseointegration than machined implant surfaces. However, the dual acid treatment deteriorates the physical properties of the titanium by weakening the fatigue resistance of the implant and causing microcracks. The removal torque comparison between the dual-acid etched (hydrochloric acid, sulfuric acid, HS) and single-acid etched implants (hydrochloric acid, H) could reveal the efficiency of implant surface acid treatment. Materials and methods: Nine $3.75{\times}4mm$ dual-acid etched SLA implants and nine single-acid etched SLA implants were inserted into New Zealand rabbit tibias. After 10 days, removal torque, roughness, and wetting angle were measured. Results: Mean removal torque values were as follows: Mean removal torque were 9.94 Ncm for HS group and 9.96 Ncm for H group (P=.995). Mean surface roughness value were $0.93{\mu}m$ for HS group and $0.84{\mu}m$ for H group (P=.170). Root mean square roughness (RSq) values were $1.21{\mu}m$ for HS group and $1.08{\mu}m$ for H group (P=.294), and mean wetting angle values were $99^{\circ}$ for HS group and $98^{\circ}$ for H group (P=.829). Statistical analysis showed no significant difference between the removal torques, roughness, or wetting angles of the two groups. Conclusion: In this experiment, we found no significant difference in removal torque, roughness, or wetting angle between dual-acid etched and single-acid etched implants.

Burnishing effect on marginal misfit of implant-supported screw-and-cement retained prostheses: A case report (임플란트 지지 나사-시멘트 유지형 보철물에서 보철물-지대주 제거 후 변연부 연마의 효과: 증례보고)

  • When the marginal fit of fixed dental prosthesis decreases, biological and technical complications, such as plaque accumulation, periodontal disease, hypersensitivity, components fracture, cement loss, can happen. The complications affect the long-term success and survival of prostheses. This case report describes a clinical procedure to minimize the marginal gap of implant-supported screw-and-cement retained prosthesis by removing prosthesis-abutment complex and burnishing the interface area. The marginal gap was measured before and after the burnishing using a stereomicroscope and compared. This technique improves the marginal fit, thereby contributing the longevity of the prosthesis.

Early loading on a maxillary posterior single implant with deepened threads: a case report (깊은 나사선을 갖는 임플란트를 이용한 상악 구치부 조기 하중: 증례보고)

  • This case report shows an early loading at the maxillary posterior area with the wide diameter implant which has a deepened threads after removal of failed implant. Implant Stability Quotient (ISQ) value has represented favorable result for one year. This clinical report describes the potential of early loading on a maxillary posterior single implant with deepened threads.

  • Cement-retained implant prosthesis has several advantages in the esthetic and occlusal aspects. However, the difficulty of the retrievability and the possibility of peri-implantitis induced by the cement excess would be a threatening factor to the implant prognosis. Peri-implantitis resulting from the remaining cement could occur later on to the patients with periodontitis history. Retention can be controlled by selecting the right cement type. Retention of the cement was the strongest in the resin cement, followed by resin modified glass ionomer cement, poIycarboxylate cement, zinc phosphate cement and glass ionomer cement. Retention of the provisional cement weakened after thermocycling. Other factors such as the abutment number, abutment alignment, height and taper of the abutment can also affect the total retention. To the success of the cement-retained prosthesis, it's important to select the right cement for the clinical purpose. The prosthesis should be fabricated in accordance with the biomechanical requirements. The prosthesis should be cemented with the techniques to reduce the excess cement as much as possible. In addition, the excess cement should be identified using the radiography and carefully removed.

  • In case of implant-tooth connected prosthesis, a natural tooth tends to intrude. There are several mechanisms that explain an intrusion phenomenon. So it is reco mmended not to connect an implant with a natural tooth. A 68-year-old female had upper left $2^{nd}$ premolar and $2^{nd}$ molar extracted and underwent implant surgery on the missing area. We made an implant prosthesis and treated upper left $1^{st}$ molar with a gold crown. 2.5 year later, the patient complained about loose proximal contact and food impaction between upper left $1^{st}$ molar and $2^{nd}$ molar. Mesial side of upper left $2^{nd}$ molar implant prosthesis was soldered so that proximal contact became tight again. But after 7 months, about 2 mm intrusion of upper left $1^{st}$ molar occurred, and the patient felt periodontally originated pain on intruded upper left $1^{st}$ molar. After the gold crown on upper left $1^{st}$ molar was removed, extrusion occurred and pain was relived.
