Full mouth rehabilitation of mandibular edentulous patient using implant hybrid prosthesis (하악 무치악 환자에서 임플란트 하이브리드 보철물을 이용한 전악 수복 증례)

  • Implant prosthodontics is beneficial for edentulous patients in enhancing the support, retention, stability, phonation and so on. Various types of prosthesis supported by implant, including implant retained- or supported- overdenture for the removable type and ceramo-metal and fixed prostheses with processed acrylic teeth for the fixed type, are frequently used. Treatment planning for the prosthesis with implant must be made after considering individual characteristics such as form of residual ridge, soft tissue, interocclusal relationship, economic status. Fixed prosthesis with processed acrylic teeth (also known as 'implant hybrid prosthesis' or 'bone anchored bridge') has the advantages of both removable and fixed prosthesis such as proper soft tissue profile, esthetic outcome, increased masticatory efficiency and psychological stability. The 73-years-old female patient came to the department of prosthodontics, Dental hospital of Yonsei University. She was diagnosed with Kennedy class I partial edentulism in the maxilla and complete edentulism in the mandible. This article reports a satisfactory clinical and esthetic outcome of full mouth rehabilitation using removable partial denture in the maxilla and implant hybrid prosthesis in the mandible.

  • Although gag reflex is an essential function and a useful physiological defense mechanism, it can become an obstacle in a dental treatment. In this case, a questionnaire was designed and used in consultations to objectify the factors associated with gag reflex. Based on the questionnaire, various treatment protocols were planned, such as behavioral control, which is a systematic desensitization that adapts by placing dental instruments in the oral cavity, and habit control to eliminate vomiting habits after drinking. Except for the placement of the implant fixture under general anesthesia, all restoration procedures were performed in the outpatient clinic in the same manner as the conventional implant fixed prosthetic restoration. The systematic desensitization and behavior technique were repeated until the patient was able to endure discomfort and relieve gag reflex in dental treatment. The patient was successfully received the implant-fixed prosthetic restoration in the partial edentulous region.

  • Purpose. The purpose of this study is to help increase the success rate by analyzing the types and characteristics of implant prosthesis and the survival rate. Materials and methods. Among implants placed between 2011 and 2020 at Sanbon Dental Hospital, College of Dentistry, Wonkwang University, a case restored by a prosthetic surgeon was investigated for the characteristics and correlation of failure. The causes of failure were classified as failure of osseointegration, peri-implantitis, fixture fracture, abutment fracture, screw fracture, screw loosening, prosthesis fracture, and loss of prosthesis retention. Prosthetic method, cantilever presence, placement location, etc. were analyzed for their correlation with implant failure. Results analysis was derived through Chi-square test and Kaplan-Meier survival analysis using SPSS ver 25.0 (IBM, Chicago, IL, USA). Results. A total of 2587 implants were placed, of which 1141 implants were restored with Single Crown and 1446 implants with Fixed Partial Denture, and the cumulative survival rate was 88.1%. The success rate of SC was 86.2% (984) and the success rate of FPD was 89.6% (1295), showing statistically significant differences, among which factors that had significant differences were abutment fracture, screw fracture, and screw loosening (P < .05). Conclusion. As a result of the 10-year follow-up, more failures occurred due to biomechanical factors than biological factors. Further studies on the success of implants will be needed in the future.

Treatment with upper complete denture and lower implant-fixed restorations on an elderly patient presenting fully edentulous maxilla and bilateral posterior edentulous mandible: a case report (상악 완전무치악 및 하악 양측 구치부 결손 고령환자에 대한 상악 가철성, 하악 고정성 보철치료: 증례보고)

  • Stable posterior occlusal support is crucial for adequate masticatory function and facial aesthetics. In elderly patients over the age of 65, masticatory ability has a significant impact on nutritional intake and overall health. This case report presents a prosthetic treatment of an elderly patient with edentulous maxilla and bilateral posterior edentulous mandible. The upper jaw was restored with a complete denture to establish an ideal occlusal plane, and the three-dimensional positions of the mandibular implants were determined accordingly. The implants were placed through computer-guided implant surgery and were immediately loaded with fixed provisional restorations. The implant-fixed zirconia final restorations were inserted in the lower posterior region, and the occlusal surface of posterior artificial teeth in the upper denture was substituted with cobalt-chrome alloy to resist occlusal wear. The patient's posterior occlusal support and masticatory function were promptly restored through this treatment process, and long-term stability against wear was also obtained.

  • Crossed occlusion can be treated either by overdenture and telescopic denture or by placing an implant at the edentulous area to reestablish the support on the occlusion. If alveolar bony support is sufficient and an the environment where an implant is inserted is favorable to restoring the masticatory and aesthetic function of a patient, the implant-supported fixed prosthesis can provide more definitive occlusal support and more aid for other oral functions. In this case report, a patient with a severe residual alveolar bone resorption following the extraction of teeth and who had a crossed occlusion was treated with sinus bone graft and alveolar bone augmentation in order to place the implants at prosthetically position. The definitive restoration was made to reflect the patient's occlusal and aesthetic function using the CAD/CAM double scanning method. Finally, the treatment had the masticatory and aesthetic function adequately restored, which is reported here.

  • Proper management of provisional prosthesis is key to success in prosthodontics. Provisional restoration on maxillary anterior missing area frequently come across a incident of falling off especially in patients with long span pontics and oval arch shape. This is because maxillary anterior teeth are more exposed to horizontal force than the posterior teeth and additional anterior cantilever effect will negatively affect to the retention of provisional prosthesis. Beside that maxillary anterior provisional prosthesis should provide proper incisal guidance during the mandibular functional movements. However occlusal contacts on the prosthesis in maximum intercuspal position are located on opposite side of fulcrum line of prosthesis which will cause removing force against the provisional prosthesis. This case report present that provisional implant prevent pre-described harmful effect on maxillary anterior fixed provisional prosthesis and provide comfort and satisfactory result during post-extraction healing period.

  • Traditional options for posterior edentulous treatment include removable partial dentures and implant fixed prostheses. Recently, the concept of implant assisted removable partial denture, in which two treatments are fused, has been introduced in consideration of systemic health and patient's needs, costs, residual alveolar bone status and so on. Implant assisted removable partial denture has the advantage of increasing the retention and stability of the denture and improving its esthetics in cases of large bone defects or biomechanical disadvantages. In addition, it is possible to strategically place the implants in a site where the alveolar bone is relatively sufficient, thereby overcome the limit of the conventional removable partial denture design as well as reducing the burden on a wide range of implant surgery. Cost reduction is also expected. In this case, the patient was treated by placing the implant in both premolar sites of the mandible and fabricating the distal extension removable partial denture with the implant fixed prosthesis as an abutment. After delivering the definitive prosthesis, the patient showed satisfaction with the masticatory function and esthetics. and has been regularly followed-up for more than one year. The following 20-months follow-up case report describes the design of an implant-assisted-removable partial denture (IARPD) in which two cementretained implant crowns used to provide support and stability.

Using implants for worn dentition with the altered vertical dimension of occlusion based on shortened dental arch concept: a case report (과도한 마모를 보이는 환자에서, 임플란트를 활용해 수직 고경을 증가시키며 shortened dental arch 개념을 적용한 보철 수복 증례)

  • Excessive occlusal wear can result in unacceptable damage to the residual teeth structure, anterior guidance and masticatory function. An 86-year-old man presented with worn dentition and anterior deep bite due to loss of the posterior support. Two implants covered by Korean national health insurance, fixed partial denture and direct composite resin were used to restore the dentition, instead of removable prosthesis. The occlusal vertical dimension was increased by 2 mm in the premolar area and shortened dental arch concept was applied. This treatment was a less expensive alternative to full-mouth rehabilitation with positive esthetic and functional outcomes after 7 months of follow-up.

  • Often there are edentulous patients with severe vertical resorption of the alveolar ridge so that implant placement in the mandibular posterior is impossible. In this case, implants can be placed in the anterior portion of the mandible and the patient can be treated with an implant assisted overdenture, but the inherent discomfort of the removable prosthesis can not be overcome. However, if implants are appropriately placed between the mental foramen of both sides and a fixed hybrid prosthesis is fabricated to the patient, the patient will be satisfied without the inconvenience of the removable prosthesis. However, the treatment plan should be well designed to prevent disadvantages and expected complications of fixed hybrid prosthesis. Based on my clinical experience, I will explain treatment plan and protocol in detail.

Fabrication of definitive complete-arch implant-supported fixed prosthesis in upper and lower completely edentulous patient using temporary prosthesis scan: a case report (상하악 완전 무치악 환자에서 임시 보철물 스캔을 이용한 일체형 임플란트 지지 고정성 보철물 수복 증례)

  • When fabricating definitive implant-supported fixed prostheses in upper and lower completely edentulous patients, it is crucial to get information about the vertical dimension, jaw relationship, implants, abutments and gingival contour. In this case, temporary prostheses were used to take the information. The temporary prosthesis was scanned outside of the mouth so that it increased the efficiency of scanning it. During the scan of the prosthesis, a scan zig which connected both posterior parts of the prosthesis was used to minimize the scan errors. We report this case because we obtained satisfactory functional and esthetic results by using the digital technology.
