Factors Related between Clinical Health Management and Laparoscopy-Aided Gastrectomy Patients (복강경 위절제환자와 임상적 건강관리와의 관련된 요인)

  • This study was conducted to identify the factors related between clinical health management and laparoscopy-aided gastrectomy patients. Data were surveyed using interview and questionnaires by 201 patients who visited the surgical department of a general hospital which was located in Metropolitan area from January 16 to February 16, 2012. As a result of study, first, the degree of performativity was significantly positively correlated with efficiency(r=0.24, p<.01). Secondly, the explanatory power of the model in clinical health management was to 42.7% by selected variables. Thirdly, multiple regression analysis results showed that efficiency was investigated the most influential and significant factor to affect the degree of health management practices with 0.25(p=.02). In conclusion, in order to improve the health management level of laparoscopy-aided gastrectomy patients, it needs to develop and apply a comprehensive education program including the personal characteristics of performativity, self-achievement, and perceived barriers.

Cytomegalovirus-induced Childhood Menetrier's Disease with Peripheral Eosinophilia (호산구 증가증을 동반한 거대세포바이러스 감염 소아 Menetrier병 1례)

  • Herein, the case of a 5-year-old boy with cytomegalovirus-induced Menetrier's disease, with peripheral eosinophilia, presenting with abdominal pain and vomiting, followed by generalized edema, is reported. The initial laboratory findings, hypoalbuminemia and peripheral eosinophilia were noted, with no evidence of renal, hepatic, cardiac or allergic diseases. Gastrofiberscopy was performed under the suspicion of eosinophilic gastroenteritis with protein losing gastropathy. The gastrofiberscopy showed the characteristic features of giant hypertrophy of the gastric rugae, with large quantities of adherent gelatinous material on the gastric fundus and body. The histological findings revealed foveolar hyperplasia, compatible with Menetrier's disease, with massive eosinophilic infiltrations. The presence of cytomegalovirus infection was identified by serology and confirmed by urine PCR. His symptoms, gastrofiberscopic findings and peripheral eosinophilia resolved spontaneously, and he has remained well for 10 months.

  • 신장내과를 전문 과목으로 선택하여 대학병원에서 10여년 가까이 환자를 진료하면서 아침에 회진을 할 때면 저는 전공의 들에게 "어제 입원한 환자는 당뇨병성 신증 환자인가"라고 먼저 묻는 경우가 많습니다. 그 이유는 당뇨병으로 합병된 신장병, 혹은 당뇨병환자에서 혈관질환과 감염 질환이 동반될 경우 치료도 어렵고 임상 경과가 악성 경과를 밟으며 예후가 좋지 않기 때문입니다. 또한 당뇨병으로 인한 말기 신부전증이 최근 무섭게 증가되고 있기 때문이기도 합니다. 따라서 당뇨병성 신증의 조기 발견과 적절한 치료는 아무리 강조해도 지나침이 없습니다. 저는 여기서 최근 가장 가슴이 아팠던 사례를 간단히 소개하고 당뇨병성 신증의 원인, 예방과 치료를 서술하고자 합니다.

Transepidermal Water Loss, Stratum Corneum Hydration and Transmission Electron Microscope Observation on Acetone Damaged Canine Skin Barrier Model (아세톤에 의해 손상된 개 피부장벽 모델에서의 경표피수분소실도 및 각질층보습도 평가와 전자현미경적 관찰)

  • The purpose of this study is to establish experimental canine skin barrier disruption model in dog. The study was designed to investigate the predictive properties of acetone damage using as effect parameters transepidermal water loss (TEWL) and stratum corneum (SC) hydration. To compare the structures of SC intercellular lipids in normal and acetone damaged skin, TEM observations were performed. Six clinically normal, male Beagles without dermatological problems were chosen for this study. Acetone damage was performed at 48hrs after clipping. Efficacy measurements for TEWL and skin hydration were performed before ($t_1$) and after ($t_2$) damage in a temperature- and humidity-controlled room ($2{0\sim}22^{\circ}C$, 50-60%). TEWL and SC hydration values were decreased in the acetone damaged model compared with non damaged skin. In TEM observation of acetone damaged sample, the intercellular lipid lamellae exhibit abnormal and incomplete structure compared with those of normal skin. It seems that acetone damage would be one of canine skin epidermal barrier disruption model for the study of canine atopic dermatitis (AD) as well as dry skin in veterinary dermatology research.

Minimally Invasive Arthroscopic-Assisted Reduction with TightRope® for Coxofemoral Luxation in a Korean Water Deer (Hydropotes inermis argyopus) (고라니에서 최소침습적 관절경과 TightRope®의 이용한 엉덩관절탈구 교정 증례)

  • A Korean water deer was rescued with non-weight-bearing lameness of the right hind limb. Clinical and radiographic examination revealed a craniodorsal coxofemoral luxation. The cause of the right coxofemoral luxation (CL) was unknown. We performed minimally invasive arthroscopic-assisted reduction with toggle pin fixation (mini TightRope$^{(R)}$). Craniodorsal CL in the water deer was reduced and stabilized successfully. At 60 days after admission, the water deer was successfully released back into the wild. This minimally invasive surgical technique can be used to correct coxofemoral luxation in wild animals.

The Influence of Skin surface temperature on Transdermal Absorption of Active Substances and Changes in Skin Condition (피부표면온도가 유효물질 경피 흡수 및 피부 상태변화에 미치는 영향)

  • This study was conducted to investigate the effects of skin surface temperature on transdermal absorption of active substances and changes in skin condition. A transdermal absorption test was conducted using an emulsion containing 10% heat-resistant niacinamide, and the same emulsion was used in a clinical trial to test the thermal effects. As a result, absorption was found to be 2 times greater at 10 minutes and 3 times greater at 15 minutes after application at $42^{\circ}C$ than that of compared to normal skin temperature. During the clinical evaluation on skin, no clinical subjects showed any specific adverse reactions, while moisture and oiliness showed statistically significant effects at higher temperatures. Based on these results, it was confirmed that thermal warmer improves the absorption of active substances and had a positive effect on skin condition. It is deemed that this study will provide a preliminary data to support the development of various beauty devices using heat.
