C-분염(分染)에 의한 사람의 염색체 다형성에 관한 연구

  • Lymphocyte chromosome preparations obtained by the micromethod (Arakaki and Sparkes, 1963) from 234 our patients (165 females and 69 males) were analysed by C-, NOR-and GC-bandings for chromosome heteromorphisms. The centromeric regions of chromosomes 1,9,16 and the long arm of the Y chromosomes were tested for C heteromorphism. Minor variations found in this study such as inv(9), prominant short arms and large satellites of acrocentrics were also examined by appropriate banding techniques. Of the 234 probands, a total of 125 different C-variants were detected, and the average frequency of the variants per individual was estimated to be 0.53. The observed variations were as follows : 99 qh variants, 5 pericentric inversions of chromosome 9, and 21 satellite and/or short arm variants.

Prevalence and Its Correlates of Night Eating Syndrome in Schizophrenic Outpatients (조현병 외래 환자에서 야간식이증후군의 유병률과 관련요인)

  • Objectives : The aim of study was to examine the prevalence of night eating syndrome(NES) and its correlates in schizophrenic outpatients. Methods : The 14 items of self-reported night eating questionnaire(NEQ) was administered to 201 schizophrenic patients in psychiatric outpatient clinic. We examined demographic and clinical characteristics, body mass index(BMI), subjective measures of mood, sleep, binge eating, and weight-related quality of life using Beck's Depression Inventory(BDI), Pittsburgh Sleep Quality Index(PSQI), Binge Eating Scale(BES) and Korean version of Obesity-Related Quality of Life Scale(KOQoL), respectively. Results : The prevalence of night eaters in schizophrenic outpatients was 10.4%(21 of 201). Comparisons between NES group and non-NES group revealed no significant differences in sociodemographic characteristics, clinical status and BMI. Compared to non-NES, patients with NES reported significantly greater depressed mood and sleep disturbance, more binge eating pattern, and decreased weight-related quality of life. While 'morning anorexia' and 'delayed morning meal'(2 of 5 NES core components in NEQ) were not differed between groups, 'nocturnal ingestions', 'evening hyperphagia', and 'mood/sleep' were more impaired in NES group. Conclusions : These findings are the first to describe the prevalence and its correlates of night eaters in schizophrenic outpatients. These results suggest that NES has negative mental health implications, although it was not associated with obesity. Further study to generalize these results is required.

Association between Regular Interdental Cleaning and Periodontal Condition among Pregnant Women (임부의 정기적인 인접면 세균막 관리와 치주건강상태간의 연관성)

  • The aim of this cross-sectional study was to investigate the relationship between regular interdental cleaning and periodontal condition. A total of 319 pregnant women were recruited at 21 to 24 weeks of gestation. Information on demographics, health status and health behaviors including regulary use of interdental cleaning aids, that can influence periodontal condition was collected. Full mouth periodontal probing was performed. Periodontitis was defined as CAL of 4.0 mm or greater on 2 or more sites not on the same tooth. Gingivitis was defined as 15 percent and more sites showed bleeding on probing. Gingival crevice fluid sample was collected for bacterial analysis. We studied a total of 319 subjects, comprised of 116 subjects who use interdental cleaning device regularly and 203 subjects who didn't. Subjects who do not use interdental cleaning regularly had 2.3 times higher risk of gingivitis and 2.7 times of periodontitis, 2.3 times more amount of P. gingivalis. There was a significant relationship between regular interdental cleaning and periodontal condition in Korea.

  • Coronaviruses were originally discovered as enzootic infections that limited to their natural animal hosts, but some strains have since crossed the animal-human species barrier and progressed to establish zoonotic diseases. Accordingly, cross-species barrier jumps resulted in the appearance of SARS-CoV, MERS-CoV, and SARS-CoV-2 that manifest as virulent human viruses. Coronaviruses contain four main structural proteins: spike, membrane, envelope, and nucleocapsid protein. The replication cycle is as follows: cell entry, genome translation, replication, assembly, and release. They were not considered highly pathogenic to humans until the outbreaks of SARS-CoV in 2002 in Guangdong province, China. The consequent outbreak of SARS in 2002 led to an epidemic with 8,422 cases, and a reported worldwide mortality rate of 11%. MERS-CoVs is highly related to camel CoVs. In 2019, a cluster of patients infected with 2019-nCoV was identified in an outbreak in Wuhan, China, and soon spread worldwide. 2019-nCoV is transmitted through the respiratory tract and then induced pneumonia. Molecular diagnosis based on upper respiratory region swabs is used for confirmation of this virus. This review examines the structure and genomic makeup of the viruses as well as the life cycle, diagnosis, and potential therapy.

Overexpression of MicroRNA-31 as a Promising Biomarker for Prognosis and Metastasis in Human Colorectal Cancer (MicroRNA-31 과발현을 이용한 대장암의 예후예측 및 전이예측 바이오마커 발굴)

  • Colorectal cancer (CRC) is the third most common cancer and a leading cause of cancer-related death worldwide. Although several diagnostic and therapeutic tools have been available, CRC remains difficult to complete cure because of insufficient understanding of the molecular mechanisms underlying this disease progression. MicroRNAs (miRNAs) are small non-coding RNA molecules that strongly regulate gene expression via transcriptional and translational control mechanisms. Many crucial cellular pathways are frequently disrupted in cancer development process due to dysregulation of several miRNAs. Mir-31 functions as an oncogene that modulate expression of multiple cancer related genes. Thus, we aimed to demonstrate clinical relevance of miR-31 in human CRC. Quantitative RT-PCR analysis of miR-31 expression was performed in 175 CRC tissues and 16 normal colonic mucosa (NM). Next, we investigated clinical significances of miR-31 expression in various clinicopathologic features in CRC patients cohort. Mir-31 was significantly up-regulated in CRC tissues compared to NM. In CRC tissues, miR-31 expression level was significantly elevated in a stage-dependent manner, which was associated with poor survival in patients with CRC. High miR-31 levels in CRC tissues significantly correlated with poor prognosis (hazard ratio [HR]=2.4) as well as distant metastasis (odds ratio [OR]=2.3). In conclusion, we identified clinical significance of miR-31 expression in CRC. High miR-31 expression may be clinically able to use as a biomarker for CRC prognosis and predicting metastasis.

Relationships between Nailfold Plexus Visibility and Clinical Variables in Adult Schizophrenics (성인 정신분열병 환자에서 손톱주름 총 시도(叢 視度) (Nailfold Plexus Visibility)와 임상 변인과의 관계)

  • Objectives : There have been several evidences that the central nervous system defect is one of the etiologic factors in schizophrenia and high nailfold plexus visibility can reflect these defects indirectly. These are particularly related to the negative symptoms of schizophrenia. In this study, we examined the relationship between nailfold plexus visibility and various clinical variables in schizophrenia. Methods : Forty patients(20 males, 20 females) satisfying the DSM-lV criteria for schizophrenia and forty normal controls(20 males, 20 females) were measured for Plexus Visualization Score (PVS) by using capillary microscopic examination. We used Positive and Negative Syndrome Scale(PANSS), Ulmann-Giovannoni Process-Reactive Questi-onnaire(PRQ), Phillips Premorbid Adjustment Scale(PAS), Continuous Performance Test, and Backward Masking for psychopathology and clinical variables. Results : There was no significant relationship between schizophrenic subjects and normal controls in PVS. PVS was correlated with PANSS positively except negative symptom subscore. PVS was correlated with PRQ score negatively, and with PAS score positively. Conclusions : This study shows high PVS are associated with more severe psychotic symptoms and with clinical variables, such as disease process and premorbid adjustment, in some schizophrenics.

Clinical Significance of VEGF-C and COX-2 Expression in Gastric Carcinoma with Submucosal Invasion (점막하 침윤 조기위암 환자에서 VEGF-C와 COX-2 발현의 임상적 의의)

  • Purpose: Lymph node metastasis is an important factor in determining prognosis and therapeutic options for early gastric cancer (EGC) patients. Vascular endothelial growth factor (VEGF)-C and D are known as lymphangiogenic factors, and cyclooxygenase (COX)-2 is thought to play a role in lymph node metastasis in gastric carcinoma. This study was designed to determine whether the expression of VEGF-C, VEGF-D, and COX-2 is associated with clinicopathologic factors, especially lymph node metastasis in EGCs invading the submucosa. Materials and Methods: Tissue samples were obtained from 85 Patients undergoing standard gastrectomy with lymph node dissection between 1991 and 2007 in the Department of Surgery of Daejeon St. Mary's Hospital in Daejeon, Korea. All patients were diagnosed with gastric cancers and submucosal invasion. We examined the expression of VEGF-C, VEGF-D, and COX-2 using immunohistochemical methods. Results: Of the 85 patients, 16 (18.8%) had lymph node metastasis. VEGF-C, VEGF-D, and COX-2 were positively expressed in 34.1% (29/85), 22.3% (19/85), and 37.6% (32/85) of the patients. VEGF-C and COX-2 expression was significantly correlated with lymph node metastasis (P<0.05). A positive correlation existed between VEGF-C and COX-2 expression (P< 0.001). Conclusion: VEGF-C and COX-2 expression is associated with lymph node metastasis in gastric cancer with submucosal invasion. VEGF-C and COX-2 may thus be predictive markers for lymph node metastasis in EGC patients with submucosal invasion.

CNVDAT: A Copy Number Variation Detection and Analysis Tool for Next-generation Sequencing Data (CNVDAT : 차세대 시퀀싱 데이터를 위한 유전체 단위 반복 변이 검출 및 분석 도구)

  • Copy number variations(CNVs) are a recently recognized class of human structural variations and are associated with a variety of human diseases, including cancer. To find important cancer genes, researchers identify novel CNVs in patients with a particular cancer and analyze large amounts of genomic and clinical data. We present a tool called CNVDAT which is able to detect CNVs from NGS data and systematically analyze the genomic and clinical data associated with variations. CNVDAT consists of two modules, CNV Detection Engine and Sequence Analyser. CNV Detection Engine extracts CNVs by using the multi-resolution system of scale-space filtering, enabling the detection of the types and the exact locations of CNVs of all sizes even when the coverage level of read data is low. Sequence Analyser is a user-friendly program to view and compare variation regions between tumor and matched normal samples. It also provides a complete analysis function of refGene and OMIM data and makes it possible to discover CNV-gene-phenotype relationships. The Characteristics of Terminally Ill Cancer Patients in Hospice and Palliative Care according to Family Composition (가족 구성에 따른 호스피스 완화의료 말기암환자의 특성)

  • Purpose: The purpose of this study was to investigate the family composition of terminally ill cancer patients admitted to the hospice unit and how it affects their hospice care. Methods: We retrospectively analyzed the medical records of terminal cancer patients who died in one hospice unit between January 2009 and March 2014. The demographic and clinical characteristics of the patients were examined, and any different made by their marital status was evaluated. We calculated the time interval between cancer diagnosis and hospice admission and the survival period from hospice admission to death and analyzed their association with family composition. Results: When divided by the median time of 13 months between diagnosis and admission, Group B (>13 months) had a significantly higher proportion of patients living with their spouses; (P<0.01). The main decision maker was a spouse (52.9%) in Group B; (P=0.04). Conclusion: Among the characteristics of the family composition, the presence of spouse was an important factor associated with admission to a hospice unit. Clinicians need to be aware of the impact of marital status on end-of-life care. This study indicates that it is helpful to understand family composition of terminallyill cancer patients for an effective palliative and hospice care.

  • 정보화 시대를 거치면서 모든 산업분야에서 대량의 데이터가 생성되고 관리되고 있다. 최근에는 비즈니스 환경의 변화로 인하여 의사결정을 지원할 수 있는 고급 정보에 대한 필요성이 대두되었으며 IT 기술의 발전과 더불어 데이터마이닝에 대한 많은 연구가 활발히 이루어졌다. 데이터마이닝은 금융, 정부, 제조, 유통 등 다양한 분야에서 활용되고 있다. 한편 의료데이터는 다른 산업분야의 데이터와 구별되는 특징이 있는데, 데이터의 이질성과 복잡성, 부정확성과 오류가능성, 불완전성과 윤리 및 법적인 문제, 개인정보보호, 특징 선택의 제한, 모델의 투명성과 설명력에 대한 높은 요구도 등이 그것이다. 이와 같은 이유로 의료데이터에 대한 접근은 제한적일 수 밖에 없다. 그럼에도 병원 전산화를 통해 발생하는 의료데이터의 양은 기하급수적으로 증가하고 있으며, 임상정보를 포함하는 의료데이터는 데이터 자체로도 가치가 매우 크다. 이에 본 논문은 국내 제 3차 의료기관의 2년간 내원환자에 대한 진단데이터를 사용하여 데이터마이닝의 연관법칙을 이용, 상병간의 관계를 연구하고자 하였다. 이를 통해 잠재고객에게는 객관화된 의료지표를 제공하고, 의료기관은 예측 가능한 정보를 종합의료시스템에 활용하여 고객만족도를 높이는 효과를 볼 수 있을 것으로 사료된다.
