Convergence Factors Influencing Clinical Reasoning Competency of Nursing Students (간호대학생의 임상적 추론 역량에 미치는 융합적 영향요인)

  • In this study, an attempt was made to investigate the clinical reasoning of nursing students and to identify the factors that influence it. This study was held to grasp the learning ability related to core nursing competency for 297 3rd and 4rd grade nursing students. Results indicated that there was a significant difference when comparing clinical competence by grade(p=0.001). Also variables effecting clinical reasoning competence were Problem solving ability, Clinical Decision making ability, Clinical practice performance, and grade of nursing students were found to be significant (p,.001). Outcome of this study can be used as the basis for the development of various curriculums to enhance the clinical reasoning competence, in the future, research for this education development is expected to be needed.

Converged Influencing Factors on the Clinical Reasoning Competency of Senior Grade Nursing Students (졸업학년 간호대학생의 임상추론역량에 미치는 융합적 영향요인)

  • This study is a descriptive research study for investigating the factors influencing clinical reasoning competency of senior grade nursing students. This study was targeted at 160 senior grade nursing students in M city and G city and data was collected from April 15, 2019 to May 15, 2019. The collected data was analyzed using the SPSS/WIN 21.0 program, and t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis were carried out. There was a significant correlation between clinical reasoning competency, critical thinking disposition, metacognition, and empathy. The factors influencing the clinical reasoning competency included metacognition(${\beta}=.48$, p<.001), critical thinking disposition(${\beta}=22$, p=.021), and empathy(${\beta}=-.19$, p=.012). These variables explained 35.0% of the clinical reasoning competency. Based on the results of this study, a demonstration study for developing a convergence education program including metacognition, critical thinking disposition, and clinical reasoning competency and verifying its effect is necessary.

  • This study attempted to identify the relationship between nursing competency, clinical reasoning competence, and empathy ability according to the center of enneagram power for nursing students. The subjects of the study were 218 students enrolled in the department of nursing at two universities located in one region, data collection was conducted from 16 October to 27 October 2023. Data analysis was performed using SPSS/WIN version 26.0 program, descriptive statistics, and difference verification were analyzed by t-test, ANOVA, pearson's correlation coefficient, Results, The enneagram personality type of the subjects of this study was the most common type 9. And in the enneagram center of power, the instinct-centered type had the highest nursing competence, the thought-centered type had the highest clinical reasoning competence, and the emotion-centered type had the highest empathy ability. In addition, nursing competence and clinical reasoning competence showed a significant positive correlation, and clinical reasoning competence and empathy ability were also found to be positively correlated. Therefore, it is important to continue to develop and apply individualized competency building programs that reflect personality type tests to nursing students. In addition, the higher the empathy ability, the higher the clinical reasoning competence, so it is thought that it is necessary to develop a standardized curriculum that can improve nursing competence and clinical reasoning competence and verify its effectiveness.

  • Simulation education is a method that allows learners to learn repeatedly in realistic simulated situations. It solves the limitations of clinical practice that lacks direct nursing performance and is useful for acquiring and developing the knowledge, nursing skills, communication skills, learning self-efficacy, and clinical reasoning required for nursing students. The purpose of this study was identify the effects of High fidelity simulation-based education on communication ability, learning self-efficacy and clinical reasoning of nursing students. The subjects were 84 nursing students who were enrolled in the senior who had received simulation training using dysphagia patient care scenario. Data were analyzed using SPSS/WIN 22.0. As a result, There was a statistically significant difference in the communication ability, learning self-efficacy, clinical reasoning. And the mean score of simulation effectiveness is 2.64 points (a perfect score of three). Therefore, In order to improve the communication ability, learning self-efficacy and clinical reasoning of nursing students, it is necessary to develop and apply realistic scenarios that reflect various clinical situations.

Validity and Reliability of a Korean Version of Nurse Clinical Reasoning Competence Scale (한국어판 간호사 임상적 추론 역량 척도의 타당도와 신뢰도)

  • This study is a methodological research study that tests the validity and reliability of the NCRC (Nurse Clinical Reasoning Competence scale), an instrument developed by Liou and his colleagues as the basic data for enhancing the clinical reasoning competence of nurses, by translating it into Korean and checking the similarity of the sentence structure and meaning (between the two versions?). This study verified its validity and reliability by examining 166 nurses working in four tertiary hospitals located in Seoul and Busan. An analysis of the content validity by experts showed that all of the items have a content validity higher than CVI 0.8. From the exploratory and confirmatory factor analysis, it was found that the instrument includes a total of 15 items consisting of one factor. In addition, the correlation with the Korean version of the Nurse Clinical Reasoning Competence scale is confirmed to test the concurrent validity, by using a measurement tool of nurses' critical thinking dispositions and clinical decision-making abilities (correlation coefficient =.55-.64(p<.001) and Cronbach's ${\alpha}=.93$). Thus, the Korean version of the NCRC may be a useful instrument for evaluating the clinical reasoning competence of Korean nurses and providing the basic data for assessing their clinical reasoning competence and developing their promotion strategies.

Relationship Between Clinical Reasoning Competence, Critical Thinking Propensity and Confidence in Performing Core Basic Nursing Skills of Nursing Students (간호대학생의 임상추론역량, 비판적 사고 성향과 핵심기본간호술 수행자신감의 관계)

  • This study try to confirm the relationship between clinical reasoning competence, critical thinking propensity, and confidence in performing core basic nursing skills among nursing students. The subjects of the study were 157 third and fourth year nursing students at D University in Daejeon, and the survey was conducted through a self-questionnaire from Nov. 2 to Nov. 15, 2022. The collected materials were analyzed using the SPSS/WIN26.0 program. The results showed the correlation between clinical reasoning competence and critical thinking propensity (r=.417, p<.001), clinical reasoning competence and core basic nursing performance confidence (r=.659, p<.001), critical thinking propensity and core basic nursing performance confidence (r=.303, p<.001). Therefore, the results of this study can be used as a basis for developing various curriculums to increase the confidence of nursing college students in performing core basic nursing techniques.

  • This study presented specific alternatives for the use of digital storytelling elements as college education and provided basic data for future Serious games to be more popularized and used as reference materials in various academic circles by considering the criteria for the quality evaluation of game contents, which had been pointed out as limitations of functional games. In particular, the factors to be considered were mainly in developing functional games to learn the clinical reasoning process.

Influence of Self-reflection and Insight, and Academic Self-efficacy on Clinical Reasoning Competence among Nursing Students (간호대학생의 자기성찰 및 통찰력, 학업적 자기효능감이 임상추론역량에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the influence of nursing students' self-reflection and insight, and academic selfefficacy on their clinical reasoning competence. Methods: Data were collected from 147 third- and fourth-year nursing students who had more than 6 months of clinical training experience. Clinical reasoning competence, self-reflection and insight, and academic self-efficacy were measured using self-reported questionnaires. Data analysis was conducted with IBM SPSS 25.0 using one-way ANOVA, independent t-test, Pearson correlation coefficient, and multiple regression analysis. Results: Clinical reasoning competence was positively correlated with self-reflection and insight (r=.24, p=.003) and academic self-efficacy (r=.30, p<.001). Academic self-efficacy (β=.23, p=.011), dissatisfaction with major (β=-.17, p=.034), and 5~8 case studies (β=-.39, p=.027) were identified as factors influencing clinical reasoning competence (adjusted R2=.15). Conclusion: To improve the clinical reasoning competence of nursing students, it is necessary to develop the academic self-efficacy of complex cases and provide critical inquiries, debriefing tailored to clinical reasoning, and sufficient reflection during clinical and simulation practice.

Analysis and Application of Nursing Management Practicum Case Simulation for Developing Performance-Centered Education (성과중심 교육과정 개발을 위한 간호관리실습 사례시뮬레이션 적용 및 내용 분석)

  • The purpose of this study was to develop a nursing management case simulation (NMCS) framework based on the five components of nursing management process and to apply it to clinical nursing practice of nursing college students. The subjects of this study were NMCS reports submitted by the 4th grade 105 nursing students of an university. The research tool is a simulation framework for nursing management practice. It reflects the brainstorming and debriefing process used in the previous simulation exercise based on the five elements of planning, organization, human resource management, directing and control of the nursing management process respectively. As a result of the study, 32 nursing management cases were found to have 79.6% correct rate, 11.6% concept error rate, and 5.6% classification error rate in the first brainstorming and debriefing process for the five components of nursing management process. On the other hand, in the second brainstorming and debriefing process, 94.6% correct rate, 0.0% concept error rate, and 4.4% classification error rate. Based on these results, the NMCS framework developed in this study can be applied to the nursing management theory and practice course of nursing college students as well as simulation based job training and maintenance educations for clinical nurses. Therefore, we propose follow-up studies in various clinical nursing settings and a longitudinal cohort study to investigate the effect of nursing management job skills of nursing college students after graduation.

Evaluating Course of Pediatric Dentistry on Correlation Analysis between Dental Students' Achievements of Subjects and OSCE (임상술기능력평가와 교과목간의 학업성취도 분석을 통한 소아치과학의 교육과정 평가)

  • This study aimed to evaluate the courses of pediatric dentistry based on correlation analysis between scores of objective structured clinical examination (OSCE) and related subjects for 79 fourth-year students. The score of theory were related to preclinical (r = 0.449, p = 0.000) and clerkship (r = 0.437, p = 0.000) each, but the scores of clerkship were not related to OSCE. To make the students skillful for clerkship, more professor's firsthand teaching on treating patients and adequate numbers of clinical professors are required. Patients who come to the university dental hospital prefer to be treated by professors rather than students. In these circumstances, educational conditions should be arranged by ensuring the number of professors for teaching students to improve their clinical competence through direct instruction and feedback to students. In addition, pragmatic improvement plans, which allow continuous education and evaluation about basic techniques to be examined in the clinical practice course, should be compromised with the more concrete evaluation of the curriculum in order to evaluate theoretical knowledge and technical trainings to be well exercised and deepened in the practical clinical field.
