Health-related Quality of Life in Korean Patients with Rheumatoid Arthritis: Association with Pain, Disease Activity, Disability in Activities of Daily Living and Depression (류마티스 관절염 환자의 건강관련 삶의 질 영향요인 분석: 통증, 질병활성도, 일상생활 수행능력 장애 및 우울과의 관련성)

  • Purpose: The purpose of this predictive study was to identify factors affecting health related quality of life (HRQoL) in patients with rheumatoid arthritis (RA). Methods: The participants in this study were 131 patients with RA who were recruited from the outpatient clinic of a university hospital in Seoul. Disease activity in rheumatoid arthritis was evaluated by calculating the Disease Activity Score 28. Disability in activities of daily living (ADL) was assessed with the Korean Health Assessment Questionnaire, and depression with The Center for Epidemiologic Studies Depression Scale. HRQoL was evaluated using The Short Form 36 Health Survey. Data were analyzed using descriptive statistics, correlation, and hierarchical multiple regression. Results: Pain, disability in ADL, disease activity, and depression correlated negatively with physical and mental dimensions of HRQoL. But hierachical multiple regression analysis revealed that disability in ADL and depression were the only variables negatively influencing physical and mental QoL after adjustment for influences of sociodemographic variables. Conclusion: Results of this study suggest that disability in ADL and depression, rather than disease activity and pain have profound effects on HRQoL in patients with RA. Further studies are needed to assess the predictive ability of disease activity and pain on HRQoL in this population.

A Study on the Characteristics and Satisfaction of the Recipients of Home Care (재가복지대상자의 특성과 서비스 만족도)

  • The objectives of this research are (1) to identify characteristics and needs of the recipients of home care, and type of services (2) to examine the factors that predicts the degrees of recipients' satisfaction. This study uses the data based on the responses of 490 recipients selected randomly from 22 home care centers. The results show that recipients have higher frequencies in age of 60+, disabled status, difficulty with ADL, IADL items, no family, and lowest class. And the type of services offered by home care centers have highest frequencies in home maker services. Recipients' attitude toward home care services is "helpful," but they are not satisfied with the frequency of visit. The study found that disabled status, difficulty with ADL, IADL, health services, and home maker services are negatively related with degrees of recipients' satisfaction. Based on the findings, the study would conclude that home care policy should be recipients' need-based, developing systematic: need assessment tool. Expanded home health caring must be required for the disabled elderly.

The Correlation Among the Activity of Daily Living, the Manual Ability and the Gross Motor Function (뇌성마비 아동의 일상생활활동 수행능력과 사물조작기능, 대동작기능의 상관관계 연구)

  • Objective : The purpose of this study is intended to provide useful information about the disability evaluation indicators, the Modified Barthel Index (MBI) as a measurement tool for assessing the ability to perform activities of daily living through the correlation between the Modified Barthel Index (MBI) and the Manual Ability Classification System (MACS), and Gross Motor Function Classification System (GMFCS). Methods : The subject was 82 children with cerebral palsy. The validity was aexamined by calculation of correlation between the Modified Barthel Index (MBI), the Manual Ability Classification System (MACS) and Gross Motor Function Classification System (GMFCS). Results : There were a good significant correlation between the MBI and MACS (r = -.765, p <0.001), the MBI and the GMFCS (r = -.851, p < 0.001) and the MACS and the GMFCS (r = .615, p <0.001). Conclusion : The close correlation between the MBI that is used as the basis of grading cerebral palsy and high reliability and validity of the MACS, GMFCS suggest that disability evaluation indicators, the MBI could be useful for children with disabilities.

A Study on the Relationship between Activity of Daily Living and Job Status of the Disabled from the Functional Limitation Perspectives (기능제한(Functional Limitation) 관점에서 본 뇌성마비인의 일상생활수행능력과 취업여부의 관계에 대한 연구)

  • This study is intended to investigate if the standard of vocational rehabilitation service could be solely determined by medical standard for disability definement and to find out what alternatives are available to resolve practical problems for those who are not identified as the severely disabled by the disability definement but do experience severe handicaps at work. Through the literature review, the researcher argued that the concept of 'functional limitation' would be a criteria, and thereby applied Modified Barthel Index to measure the degree of functional limitation. In view of researching the relationship among physical impairment, functional limitation, and job status based on 381 cerebral palsied, variables such as the type of cerebral palsy and the part of paralysis, have neither direct nor indirect influence to the job status; however, the pain and the mobility have indirect effects, and the ability of self-management has direct effect on the job status. The researcher concluded that future research about disabilities needs to provide a serious thought on what it is to be defined as 'disabilities', as well as to focus on further research conceptualizing the degree of physical 'functional limitation'.

  • This study was to evaluate the effects how to have an influence on activities daily living and quality of life in day hospital program for the stroke patient. 41 experimental groups and 41 control groups in National rehabilitation hospital selected and examined K-ADL, K-IADL and quality of life. The result of this study indicated that experimental group was higher than control group in activity daily living performance and instrumental activity daily living performance. also experimental group was higher than control group all of total items in quality of life. age and economic state of general characteristic had an effect on K-ADL, K-IADL and quality of life and economic state, scholarship, marriage, job, helper had effect on quality of life. pain, social function, mental health, physical role in item of quality of life had effect on K-ADL and limit of physical role had best effect on K-IADL. Consequently we found that it needs to continuous rehabilitation to maintain body function, to prevent secondary disability, to improve social integration and quality of life for stroke patient actually.

The Correlation between Motor Capacity, Capability, and Performance in Children with Cerebral Palsy (뇌성마비 아동의 능력, 수행능력, 수행 간 상관관계)

  • The aim of this study was to distinguish 3 concepts(capacity, capability, and performance) for the motor activities of children with cerebral palsy(CP) and examinate relation between capacity(can do in a standardized environment), capability(can do in daily environment), and performance(does do in daily environment). Cross-sectional analysis with a assessment record of children with CP (n=40; 19males, 21 females; mean age 6y 6mo, SD 3y 8mo) was performed. Levels of severity according to the Gross Motor Function Classification System(GMFCS) included level1(13%), level2(10%), level3(43%), level 4(33%), and level 5(3%). Motor activities capacity was assessed by the Gross Motor Function Measure (GMFM-66). Capability and performance were assessed using 2 scales(functional skill, caregiver assistance) of the Pediatric Evaluation of Disability Inventory(PEDI). Correlations between capacity and capability was 0.811(p < .05), and between capability and performance were high(r=0.711, p < .05). And the correlation between capacity and performance is the lowest(r=0.711, p < .05). Motor performance levels are only partly reflected by the motor capacity and motor capability levels in children with CP. Because performance is influenced by Contextual factors (particularly, social factors such as family function). This study suggests that it is necessary to distinguish and evaluate the capacity, capability, and performance in children with cerebral palsy.

A Study on Activities of Daily Living and Perceived Health Status of Disabled Persons at Home in Rural Areas (농촌 지역 재가 장애인의 일상생활 수행능력과 지각된 건강 상태)

  • Purpose: This study was attempted to provide basic data required to develop community-base rehabilitation program for disabled persons at home by investigating their characteristics. ADL, and perceived health status. Method: The subjects were 146 disabled persons at home in rural areas. Data were collected from January 22 to January 31, 2004. Data were analyzed using frequencies, percentages, means, standard deviations, minimums, maximums, $\chi^2$-test, Fisher's exact test, t-test, one-way ANOVA, Scheffe test, Contingency coefficient, Pearson's correlation, Stepwise multiple regression. Results: The average score of ADL was $5.51{\pm}1.92$: sphincter control $(5.85{\pm}2.10)$, communication $(5.66{\pm}2.02)$, transfers $(5.67{\pm}2.21)$, self care $(5.44{\pm}2.05)$, social cognition $(5.33{\pm}2.08)$, and locomotion $(4.85{\pm}2.32)$. This means that the respondents were in the state of dependence. The main items requiring others' help were 'bathing' (57.7%) and 'going up and down stairs' (51.1%) ADL was significantly different according to age, sender, occupation, medical fee payer, type, severity and duration of disability, BMI, and alcohol drinking. Among the respondents, 82.8% perceived that their health status was bad. Main factors influencing the perceived health status of disability were age and ADL$(R^2=.343)$. Conclusion: Perception of health status among the disabled is related to their characteristics and ADL, so it is necessary to develop community-base rehabilitation programs in order to improve ADL and the perception of health status.

The Impacts of Cognitive Function, Disease Severity, and Disability on Ability to Perform Activities of Daily Living after Stroke (뇌졸중 환자의 인지기능, 질병의 심각도 및 장애 정도가 일상생활수행능력에 미치는 영향)

  • Purpose: This study aimed to examine influence of cognitive function, disease severity and disability on ability to perform activity of daily living (ADL) after ischemic stroke. Methods: A total of 88 patients with ischemic stroke were recruited from January 1, 2008 to December 31, 2012 and assessed with the standardized cognitive test battery and self-reports about disease severity, disability, and ADL. To analyze the data, ANOVA, Pearson correlation coefficients and multiple regression were conducted using SPSS/WIN program. Results: Significant correlations were found between ADL and visuospatial function, visual memory, executive function, and disability (r=.29~.38). Executive function and disability explained 17.3% of total variability in ADL performance after ischemic stroke. Conclusion: Executive function may be a promising target for cognitive rehabilitation after ischemic stroke. Thus, effective therapeutic interventions such as cognitive training for stroke patients should be considered to improve their ability to perform ADL.

Factors Associated with Cognitive Impairment of the Elderly in Nursing Homes (노인요양시설 입소노인의 인지장애 관련요인)

  • The purpose of this study was to identify cognitive impairment and it's related factors of the elderly in long term care facilities. The participants of this study were 229 residents of five nursing homes. Data were analyzed using two sample t-test, ${\chi}^2$-test, and logistic regression. The results revealed eight factors related to cognitive impairment: gender(OR=2.41, p=.035), age(75-84: OR=3.26, p=.002, Over 85: OR=4.46, p<.001), living with family before admission(OR=2.17, p=.015), hearing(OR=8.88, p=.004), family history of dementia(OR=4.39, p=.009), ADL(OR=0.82, p<.001), hypertension(OR=4.07, p<.001), and diabetes(OR=3.42, p=.001). Based on the findings of this study, continuous nursing intervention focused on adjustable factors would be helpful in order to improve nursing home elderly people's quality of life.

Estimation of Elderly Needing Long-term Care in S-city Gyeonggi-do according to ADL and IADL (경기도 S시 노인의 신체적 및 도구적 일상생활수행능력에 근거한 장기요양시설 보호대상자 추계)

  • Objectives: The purpose of this study is to estimate the needs of long-term care in S-city Gyeonggi-do according to ADL and IADL. Methods: We surveyed to investigate the needs of 397 study samples which had been selected by stratified randomized sampling, 2.7% in each Dong of S-city. We estimated the disability of elderly by using standards based on ADL and IADL of OECD and Korea Institute for Health and Social Affairs. Results and Conclusions: According to standards of OECD, the elderly with severe disability were 4,712(31.2%). According to standards of Korea Institute for Health and Social Affairs, the severe disabled elderly with helper were 3,776(24.9%) and the severe disabled elderly without helper 2,130(14.1%) So We concluded that the long-term care beds estimated by need were from 2,130 to 4,712. To estimate the long-term care beds, we have to consider together physical, mental, nutrition, and cognition status, existence of helper, helper characteristics, and emotional relationship between elderly and helper. But we considered only disability grades of physical states and existence of helper in this study. So it will be useful to conduct study to consider these other factors in future. At the same time this study is of value to estimate objectively long-term care needs based on physical needs in one region.
