  • The objective of this study was to discover the effects of the short-term home visiting physical therapy program involving patients with some chronic brain disorders at Gimhae City, Kyongnam. Recovery of activities of daily living (ADL) is a very important factor of rehabilitative procedures, and Functional Independence Measure (FIM) is a useful standard of evaluation for it. The FIM is widely used in brain disorder research because it measures real functional activities of daily living. We applied the physical therapy exercise program twice per week (10~15 times repeatedly); a warm-up and cool down exercise performed every ten minutes by active & active-assistive ROM and stretching exercises. Main exercises were composed of getting up & laying down in bed, standing training, walking exercise in the room, and window or wall sliding exercise using affected upper limbs for a total duration of 30 minutes. We collected the data from 20 patients with chronic brain disorders at his/her home and analyzed by means of SPSS/PC+ program (Ver. 10.0). After the six week long physical therapy exercise program, the average was $56.10{\pm}22.59$ point compared with initial $50.55{\pm}19.12$ point by FIM, improved functional ADL ability about 5.55 point, and these changed scores were statistically significant (p=.000). We also studied another factor regarding patient's satisfaction. The majority of subjects (10 people) rated the program with the maximum score of ten points (50.0%), and three people rated it a seven point program (15.0%), the other two subjects gave a rating of nine and eight points (10.0%). Because the program was effective at improving the physical ADL ability and satisfaction of each subject, we suggest continual development and implementation of a home visiting physical therapy program. Further study should involve a longer period of observation with a larger population that is involved in an individually designed home physical therapy program.

  • Purpose: This study identified the effects of dual-task gait training on balance, gait function, and activity of daily living in patients with Parkinson's disease. Methods: This study used a single-subject design. Two patients with Parkinson's disease participated in this study. Dual-task gait training was performed 1 hour per day 8 times during intervention phase. The subjects were measured 8 times in the baseline phase, 8 times in the intervention phase, and 8 times in the follow-up phase. The outcome measurements included a timed up and go test (TUG), a Berg balance scale (BBS), a 10 meter walk test (10MWT), a 6 minute walk test (6MWT), a dynamic gait index (DGI) and a Korean modified Barthel index (K-MBI). Results: When compared to the average of the baseline process, the data collected during the intervention period showed that the TUG and 10MWT results improved and the tendency line was above the baseline. In addition, BBS, 6MWT, DGI, and K-MBI values for both patients increased remarkably after the training. Conclusion: The results of this study revealed that dual-task gait training may be helpful to improve balance, walking function, and activity of daily living for patients with Parkinson's disease. Further studies need to confirm our findings.

  • Purpose : This study compared the cost-effectiveness ratio of physical therapy in health centers and home physical therapy, two physical therapy methods for home-bound stroke patients, and clarified the economic validity regarding the effect of home physical therapy. Methods : To measure and compare the cost and effectiveness of the two physical therapy methods for stroke patients, subjects were recruited based on in-hospital and home physical therapy. Among the entire data collected, 82 and 90 participants were selected for in-hospital and home physical therapy, respectively. To measure costs, regarding both in-hospital and home physical therapy, direct cost and indirect cost for patients, family, medical institutes, and the government were measured. In addition, activities of daily living were measured in both methods to measure their effectiveness. Through collected data, the cost-effectiveness and incremental cost-effectiveness ratios were analyzed. Results : Based on the analysis of cost-effectiveness, home physical therapy showed lower cost-effectiveness than in-hospital physical therapy. Furthermore, the incremental cost-effectiveness ratio also showed a difference, which implied home physical therapy could have high effectiveness compared to cost. Conclusion : Based on these results, home physical therapy could be considered as an alternativeto other methods of physical therapy, for home-bound stroke patients. In addition, the result of thisstudy contribute by providing evidence that home physical therapy offers economic benefits and canbe more effective in treating home-bound patients when policy decisions are made to establish a home physical therapy system.

  • In this study, we investigated the effects of five EMS lumbar back pressure belts produced on an anatomical basis on lumbar spine stabilization. Five core muscles were selected, including the urinal, vertebral column, endotracheal, external abdominal, and large back muscles, and patterns were designed using a conductive fabric considering the appropriate muscle shape and pain-causing points. We experimented with four motions to examine the effects of different EMS abdominal compression belts on lumbar spine stabilization. Five healthy men in their 20s were selected. The selection conditions include no back pain history for the past three months, no restricted movements through pre-inspection, and the muscular strength of the body should belong to the normal grade. Using SLR, the sequence of experimental actions was chosen from the following but not limited to left-hand, body-hand, and back-line forces. Resting between movements lasted for 2 min, and the experiments were conducted after wearing the EMS abdominal pressure belt. Electrical stimulation was applied for 10 min to increase blood flow and muscle activation. The statistics of the experimental results were analyzed for specific differences by conducting the Wilcoxon and Friedman tests with nonparametric tests. The ranking results of each pattern were successfully assessed in the order of 5, 4, 3, 1, 2 for the five patterns, and we could identify slightly more significant results for experimental behavior associated with each muscle movement. Patterns produced based on anatomy showed differentiated effects when electric stimulation was applied to each muscle in different shapes, which could improve the stabilization of the lumbar spine in everyday life or training to the public. Based on these results, subsequent research would focus on developing smart healthcare clothing that is practical in daily life by employing different anatomical mechanisms, depending on the back pain, to utilize trunk-type tights.
