Assessment of Influenza Vaccine Immunogenicity in Immunocompromized Host During 2009 Influenza Season: A Single Institution Experience (면역저하환자에서 인플루엔자백신의 면역원성 평가)

  • Purpose : Although influenza is regarded as one of the major causes of morbidity and mortality in children with cancer, the actual vaccine coverage remains poor. We conducted evaluation of immunogenicity and safety of influenza vaccine in children with cancer. Methods : In this study, 25 children with cancer who received influenza vaccine (SK influenza IX vaccine$^{(R)}$) at the Korea Cancer Center Hospital between October and December 2009 were analyzed. Blood samples of patients were collected twice (at the beginning of this study and at 30th day after vaccination) and their antibody titers were measured using the hemagglutination-inhibition (HI) assay. Immunogenicity of the influenza vaccine was assessed by seroprotection rate on days 0 and 30, seroconversion rate on day 30, and mean fold increase (MFI) of geometric mean titer (GMT) of HI between days 0 and 30. Results : Any of the subjects in our study did not experienced serious adverse events after influenza vaccination. Seroprotection rates were 68% for H1N1, 40% for H3N2, and 36% for B. Seroconversion rates were 12% for H1N1, 16% for H3N2, and 20% for B. MFIs were 0.9 for H1N1, 1.2 for H3N2, and 1.8 for B. Conclusion : In the study, we found a limited protective immune response to influenza vaccine, among subjects with cancer. However, some subjects showed seroconversion, and there were no severe adverse events among all subjects, supporting the recommendation of annual influenza vaccination in children with cancer.

Post-marketing Surveillance Study of an Inactivated Split-Virion Influenza Vaccine in Korea (불활화 분할 인플루엔자 백신의 국내 시판 후 조사 연구)

  • Purpose : This post-marketing surveillance study (NCT00750360) assessed the safety and reactogenicity of an inactivated, trivalent split-virion influenza vaccine licensed for use in the Korea since 2002. Methods : Eight hundred and eighty three subjects aged 6 months received a single dose of the ${\geq}$ vaccine; an additional dose was administered to those aged <9 years and unprimed with an influenza vaccine. Four hundred and eleven subjects used diary cards to record safety information; this report presents data from these subjects. Incidence of solicited local, general and unsolicited adverse events (4-days and 21-days post-vaccination follow-up periods, respectively) were recorded. Serious adverse events (SAEs) were recorded throughout the study period. Results : Injection site pain (subjects aged <6 years: 12.6% of subjects, ${\geq}$6 years: 34.7%), fever (<6 years: 1.3%) and myalgia (${\geq}$6 years: 13.9%) were the most frequently recorded solicited local and general adverse events. Grade 3 solicited adverse events were reported by ${\leq}$4.0% subjects. No vaccine-related SAEs were recorded (KFDA criteria). Conclusion : Considering the vaccine's well-established immunogenicity and its favourable safety and reactogenicity profile across all age groups and its high coverage rate in Korea, it may be recommended as a candidate to facilitate annual seasonal influenza vaccination for all ages as part of the Korean National Immunization Program.

Non-Mandatory Influenza Vaccination Rates among Healthcare Workers during the 2017-2018 Influenza Season: a Multicenter Study in Korea

  • Purpose: Annual influenza vaccination is the best strategy to prevent healthcare-associated influenza transmission. Influenza vaccination rates among healthcare workers (HCWs) vary by country, region, and year. We investigated the influenza vaccination rates for HCWs during the 2017-2018 influenza season in South Korea, where a non-mandatory vaccination campaign was conducted. Methods: We retrospectively investigated factors affecting the influenza vaccination rate among HCWs during the 2017-2018 influenza season in three tertiary hospitals in Goyang City, where the non-mandatory influenza vaccination program is conducted. Results: Consequently, 6,994 of 7,180 HCWs (97%) were included, and the overall vaccination rate was 85%. Nurses had the highest rate with 92%, followed by health technicians (88%), physicians (84%), and non-medical HCWs (79%, P<0.001). Vaccination rates differed, depending on the frequency of contact with patients in the non-medical HCWs (frequent contact vs. less-frequent contact; 90% vs. 73%, P<0.001). Conclusions: The influenza vaccination rate among HCWs during the 2017-2018 influenza season in Korea was 85%, which is among the highest rates compared with previously reported non-mandatory vaccination rates in other countries. The vaccination rate may vary depending on the HCW's occupational characteristics, including the extent of contact with the patient. Therefore, a multifaceted strategy is needed to increase the vaccination rate of HCWs.

Clinical Comparison of Influenza A and B Virus Infection in Hospitalized Children (소아 입원환자에서의 A형 및 B형 인플루엔자 임상 비교)

  • Purpose: The objective of this study was to compare the clinical characteristics of influenza A and B infections and analyze the effect of oseltamivir in hospitalized children. Methods: We investigated children under the age of 15, who were diagnosed with influenza A/H1N1, A/H3N2, or B from January to April 2014. The subjects were admitted to the Changwon Fatima Hospital and diagnosed using a rapid antigen test from nasopharyngeal swabs. The medical records of the patients were retrospectively reviewed. Results: A total of 302 pediatric patients with influenza were enrolled. Influenza B infection was the most common type (n=187, 61.9%), followed by A/H3N2 (n=100, 33.1%) and A/H1N1 (n=15, 5.0%). Compared to patients diagnosed with influenza A, patients diagnosed with influenza B were older (P=0.005), and the duration of fever was significantly longer (P=0.001). A total of 161 patients (53.3%) had been vaccinated against influenza during the season, before admission. Among the patients infected with A/H3N2 and B, the duration of fever was shorter in oseltamivir recipients compared to oseltamivir non-recipients (P=0.026 and P=0.004, respectively). Conclusions: There were significant differences between influenza A and B groups in terms of age, demographics, and clinical course. Although the effectiveness of oseltamivir on influenza differs according to the type of influenza, our data provides evidence that oseltamivir is beneficial for both A and B infections.

호흡기,소화기계 감염환자로부터 전염성 바이러스 분리 및 특성

  • 호흡기계 및 소화기계에 감염된 전염성 바이러스에 대한 역학적 기초자료로 이용하고자 1999년 1월부터 12월까지 부산지역에서 분리된 전염성 바이러스의 특징과 계절적 발생추이, 환자의 성별, 연령별 발생에 대해 조사한 결과는 다음과 같다. 1. 1999년도 바이러스 가검물 2261건에서 분리한 호흡기계감염 바이러스 279건과 소화기계 감염 바이러스 83주를 분리하였으며, 이중 인플루엔자 바이러스 A형이 96주(29.6%), B형이 107주(33.0%)로 대부분을 차지하였다. 2. 1999년의 바이러스 분리의 성별 분포는 총 360명의 환자 중 179명(49.7%)의 남성 및 181명(50.3%)의 여성으로 비슷한 양상을 나타내었다. 이중 호흡기계의 경우 279명의 감염환자 중 남성이 130명(46.6%), 여성이 149명(53.4%)으로 여성의 감염율이 비교적 높았으나, 소화기계의 경우 83명의 감염환자 중 남성이 51명(61.4%), 여성이 32명(38.5%)으로 남성의 감염율이 거의 2배정도 높게 나타났다. 3. 1999년의 연령별 분포는 10세 이하의 어린이가 194명(59.9%)으로 대부분을 차지하였으며, 이중 인플루엔자 바이러스가 99명(30.6%)으로 가장 높은 감염을을 나타내었다 유행성이하선염 바이러스의 감염어린이 중에 $l1{\sim}15$세의 연령층이 15명으로(53.3%)로 가장 높게 나타났다. 4. 1999년 월별 감염율은 호흡기계 감염증 바이러스의 경우 1월부터 4월까지, 그리고 12월에 증가 추세를 보이면서 4월에 가장 높은 감염율을 나타내었다 소화기계 감염증 바이러스의 경우 9, 10, 11월을 제외한 모든 월별에 관찰되었으며, echo와 coxsackie 바이러스는 무균성 수막염 환자에서 하절기에 집중적으로 발생하였다. 동절기에 유행하는 설사 바이러스는 12월에 비교적 높은 양상을 나타내었다. 5. 인플루엔자 바이러스는 MDCK 세포에서, 아데노 바이러스와 유행성 이하선염 바이러스는 HEp-2 세포에서, 파라인플루엔자 바이러스는 Vero 세포에서, 그리고 echo, coxsackie B 바이러스와 장내 바이러스는 HEp-2, Vero, BGM 세포에서 뚜렷한 세포병변 효과를 나타내었다. 6. 분리한 바이러스는 전자현미경으로 관찰한 결과 인플루엔자 바이러스 A 형(HINI, H3N2)은 95nm, B 형은 70nm크기의 구형을 나타내었으며, 바이러스표면의 지질 이중층이 뚜렷하게 관찰되었다. 아데노 바이러스는 외피가 관찰되지 않았으며, nucleocapsid는 symmetry이고 크기는 71nm로서 바이러스 입자 표면에 icosaheral capsomer의 배열이 명확하게 관찰되었고, 파라인플루엔자 바이러스와 유행성 이하선염 바이러스는 외피가 있는 구형의 큰 viron으로180, 170nm 크기이었다. 7. Echo와 coxsackie B group 바이러스는 모두 외피가 없는 isometric 형으로 크기는 $30{\sim}45nm$ 이었고, enteric adeno 바이러스는 84nm 크기로서 외피가 없고, 입자 표면에 capsomer의 배열이 명확하게 관찰되었고, rotavirus는 크기가 70nm이며 외층 capsid 단백질과 내층 capsid 단백질이 두층으로 되어 있는 전형적인 수레바퀴 모양을 나타내었다. 이상의 결과로 보아 호흡기계 및 소화기계에 감염되는 전염성 바이러스는 연중 지속적으로 분리되고 있으며 전염성이 강하여 집단 발생은 일으키는 경우도 많고 최근 들어 유행성 이하선염과 흥역 바이러스의 발생률이 높은 추이를 나타내고 있지만 아직은 특이한 바이러스 치료제가 개발되어 있지 않았으므로 지속적인 대책과 아울러 장기적인 발생 가능에 대한 예방책을 흥보하여야 할 것으로 보이며 계속적인 바이러스성 전염병 유행예측조사 및 역학조사가 적극적으로 이루어져야 할 것으로 사료된다.

Fusion Analytical Sensitivity of Rapid Influenza Antigen Limit of Detection Tests for Human Influenza virus (인플루엔자 바이러스에 대한 신속 항원 검출 검사 검출한계의 융합적 분석)

  • In this study, to analyze the detection of limit for sensitivity of the influenza rapid antigen test kit, the positive detection of limits were analyzed by serial dilution of influenza virus A and B type for five influenza rapid antigen test kits in Korea. As a result of analysis, visual measurement of type A were up to 1:8192 for the Wellsbio product and up to 1:4096 for the II product, up to 1:512 for the I and III products, and only 1:128 for the IV product, and type B were positive for up to 1:8192 for the Wellsbio product, up to 1: 4096 for the II product and up to 1:1024 for the I, III and IV products. For instrument readings with the same specimen, both A and B types were found to be positive for up to 1:8192 for the Wellsbio product, up to 1: 4096 for the II product, and up to 1:2048 for the I product. The sensitivity of the rapid antigen test for influenza differs greatly depending on the sampling area of the patient, infection period, specimen volume, etc. Therefore, it is necessary to observe exactly the collection timing and method of the specimen. And it is necessary further study to improve the sensitivity for influenza rapid antigen test.

Characterization and Outbreak Pattern of Isolated Influenzavirus in Busan, 2000-2002 Years. (최근 3년 부산지역에서 분리한 인플루엔자바이러스의 특성 및 발생 양상)

  • Influenzavirus were isolated from patients with acute respiratory infections in Busan during 2000-2002 and characterized their antigenic properties. In 2000, 39 viruses were isolated and they were identified as Influenzavirus. Among the isolated influenzavirus, 23 were A type influenzavirus and 16 were B type influenzavirus. As a result of antigenic characterization, the influenzavirus were determined to A/Sydney/05/97 (H3N2)-like, A/Beijing/262/95 (H1N1)-like, and B/Harbin/07/94-like virus. In 2001, 56 viruses were isolated and all of the viruses were identified as influenzavirus. They were A/Panama/2007/99 (H3N2)-like and A/Newcaledonia/20/99 (H1N1)-like virus In 2002, 114 Influenzaviruses were isolated. Among the isolated influenzavirus, 83 were A type influenzaviruses, and 31 were B type influenzaviruses. They were A/Panama/2007/99 (H3N2)-like, A/Newcaledonia/20/99 (H1N1)-like and B/Beijing/243/97, B/Honkong/22/2001, B/Sichuam/379/99-like virus when determined their antigenic properties. These results may be useful for the establishment of reliable epidemic data.

Antiviral Activity of Plant-derived Natural Products against Influenza Viruses (식물 유래 천연물의 인플루엔자에 대한 항바이러스 활성)

  • Influenza viruses are zoonotic respiratory pathogens, and influenza infections have caused a substantial burden on public health systems and the livestock industry. Although currently approved seasonal influenza vaccines have shown potent protection efficacy against antigenically well-matched strains, there are considerable unmet needs for the efficient control of viral infections. Enormous efforts have been made to develop broadly protective universal influenza vaccines to tackle the huge levels of genetic diversity and variability of influenza viruses. In addition, antiviral drugs have been considered important interventions for the treatment of viral infections. The viral neuraminidase inhibitor oseltamivir is the most widely used antiviral medication to treat influenza A and influenza B viruses. However, unsatisfactory clinical outcomes resulting from side effects and the emergence of resistant variants have led to greater attention being paid to plants as a natural resource for anti-influenza drugs. In particular, the recent COVID-19 pandemic has underpinned the need for safe and effective antiviral drugs with a broad spectrum of antiviral activity to prevent the rapid spread of viruses among humans. This review outlines the results of the antiviral activities of various natural products isolated from plants against influenza viruses. Special focus is paid to the virucidal effects and the immune-enhancing effects of antiviral natural products, since the products have broad applications as inactivating agents for the preparation of inactivated vaccines and vaccine adjuvants.

  • 양계산업에서 태풍의 핵인 고병원성조류인플루엔자가 또 다시 국내에서 상륙하며 양계산업에 긴장감이 감돌고 있다. 지금까지 HPAI가 전라도 익산·거제지역과 충남 아산에 총 4차례 발생하면서 추후 AI 확산에 대한 두려움이 사라지지 않고 있다. 현재까지 익산ㆍ김제 위험지역내 가금류 및 김제 경계지역 농장에서 총 1,121천마리(171농가)가 살처분되었다. 방역당국에서는 발생지역은 물론 집단 사육지역을 중심으로 방역활동을 강화해 나가고 있으며, 소비감소에 따른 가격하락으로 인해 전국적으로 소비홍보활동이 지속적으로 이루어지고 있다. 한편 21일에 충남 아산 종오리 농장에서 정밀검사결과 HPAI로 판명되면서 AI가 확산되는것이 아니냐는 불안감이 가중되고있어 그 추이를 지켜봐야 할 것으로 보인다
