Incidence and Procedure-Related Risk Factors of Delirium in Patients Admitted to an Intensive Care Unit (중환자실 입원 환자의 섬망 발생과 처치 관련 위험인자)

  • Objectives : Although delirium is a common complication among patients hospitalized in intensive care units(ICUs), little is known about the roles that diagnostic and therapeutic procedures play in its development. This study investigates the procedure-related risk factors of delirium in ICU patients. Methods : All the consecutive patients admitted to the ICU between June 2016 and May 2017 were routinely evaluated for delirium by psychiatrists. In total, 1156 patients met the inclusion criteria and were retrospectively analyzed. A multiple logistic regression analysis was conducted to investigate independent risk factors of delirium development while adjusting for other characteristics. Results : The age, Acute Physiology and Chronic Health Evaluation (APACHE II) score, proportion of patients who had undergone an operation, and proportion of patients who were foley catheterized, mechanically ventilated, and physically restrained were higher in the delirium group. The multiple logistic regression analysis confirmed that the use of restraint was an independent risk factor of delirium (odds ratio : 10.006 ; 95% confidence interval : 6.120-16.360 ; p<0.001). The patient factors independently associated with delirium were an advanced age and a higher APACHE II score. The incidence of delirium was 15.3%. Conclusions : There is a high prevalence of delirium influenced by potentially harmful procedures in patients in ICU settings. The use of physical restraint had the strongest association with the development of delirium. These findings advocate the need to target procedure-related risk factors such as the use of restraints as preventive intervention measures for ICU delirium.

Research on Factors Affecting Smartphone App Market Selection: App Market Platform Provider's Perspective (스마트폰 앱 마켓 선택에 영향을 미치는 요인에 관한 연구: 앱 마켓 플랫폼 사업자 관점으로)

  • This paper empirically investigates the factors that influence the consumer choice of an app market based on the rational choice theory. The app market is the only channel where a consumer can buy smartphone apps, which give various functional convenience and are considered to be a major contributor to the proliferation of smartphones. Analyses of 281 questionnaires show that usability and structural guarantees as benefit factors significantly influence the app market choice. From the cost perspectives, both monetary and non-monetary conversion costs are found to significantly influence the app market choice. On the other hand, customer trust, information quality, and market image were found to have no significant effect on app market selection. In particular, Korean app market platform providers (KT, LG U +) seem to be superior in terms of structural guarantees, such as customer center operation and damage compensation regulations, compared to overseas app market platform operators (Google). However, in the case of the Google App Market, it is pre-installed on all Android phones, so it is not inconvenient to install additional apps to use other app market. This is disadvantageous to domestic app market platform operators, and it is necessary to establish a policy solution point. In terms of operator costs, both monetary and non-monetary conversion costs have a significant impact on app market choice. In particular, non-monetary conversion costs have a negative impact on Korean app market platform operators. It can be explained that the service expectation level of the domestic app market is low and it is recognized that the time cost factor such as membership is large for new users to use. It seems to be necessary to improve the domestic app market business. Meanwhile, extant research on smartphone apps focuses on the purchase of apps themselves, but not on the selection of the app market itself. In order to fill in this gap, this study focuses on the determinants of app market selection, including the characteristics of an app market and the switching costs.

Sexual Trauma Survivors and Their Mental Health: Assessing Based on Types of the Traumatic Event (성적 트라우마 경험자의 정신건강: 트라우마 사건유형에 따른 비교 분석)

  • Background: The mental health issues caused by trauma can manifest differently depending on the characteristics of the traumatic event. Particularly, individuals who have experienced sexual trauma are known to have more negative mental health outcomes compared to those who have experienced non-sexual trauma. The mental health issues of individuals who have experienced sexual trauma are severe, and new forms of threats, such as digital sexual crimes, are emerging. This study aimed to investigate whether the type of traumatic event, particularly focusing on sexual trauma events, contributes to differences in mental health outcomes and to identify factors influencing suicidal ideation and potential post-traumatic stress disorder (PTSD) risk. Methods: Based on an online survey conducted nationwide among adults aged 20 to 50, participants were categorized based on the type of trauma they experienced (sexual trauma events and non-sexual trauma events). The study conducted propensity score matching (PSM) using demographic factors (sex, age group, subjective economic status, and marital status) and resilience protective factors (cognition of recoverability, social support, and protection experiences in childhood) as control variables, excluding the experience of sexual trauma events, to investigate their potential impact on mental health (suicidal ideation and potential PTSD risk). Subsequently, binary logistic regression analysis was conducted to identify factors influencing mental health. Results: Even after PSM, individuals who experienced sexual trauma exhibited more negative outcomes in terms of suicidal ideation and potential PTSD risk compared to those who experienced non-sexual trauma. The results of binary logistic regression analysis showed that sexual trauma survivors were 1.9 times more likely to have suicidal thoughts (odds ratio [OR], 1.911) and 2.5 times more likely to have a potential PTSD risk (OR, 2.472). Furthermore, as resilience protective factors became more negative, the likelihood of suicidal ideation and potential PTSD risk increased. Conclusion: This study emphasizes the importance of understanding and supporting individuals who have experienced sexual trauma, highlighting the necessity for strategies aimed at mitigating suicidal ideation and potential PTSD risk among sexual trauma survivors, while also facilitating recovery through the promotion of resilience protective factors.

Correlations between Neurologic and Psychiatric Symptoms in Acute Stroke Patients (급성기 뇌졸중 환자에서 신경 및 정신 증상간 상호관계)

  • Objectives : This study aimed to investigate the correlations between neurological and psychiatric symptoms at two weeks after stroke. Methods : For 412 stroke patients, stroke severity was evaluated by the National Institutes of Health Stroke Scale(NIHSS), disability by the Barthel Index(BI) and modified Rankin Scale(mRS), cognitive function by the Korean Mini-Mental State Examination(K-MMSE), and muscle power by grip strength. Psychiatric symptoms were assessed by Symptom check list-90-Revision(SCL-90-R), consisted of nine symptom domains : Somatization, Obsessive-compulsive, Interpersonal sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid ideation, Psychoticism, and Additional items. The correlations between the neurological and psychiatric symptoms were investigated at the time of admission and before discharge(i.e. before and after treatment). Results : At the time of admission, NIHSS score was associated with scores on Phobic anxiety and Additional items ; and scores on BI and mRS were associated with Depression, Phobic anxiety and Additional items. At the time of discharge, NIHSS score was associated with scores on Somatization, Depression, Phobic anxiety, and Additional items ; scores on BI and mRS were associated with scores on Depression, Phobic anxiety and Additional items ; MMSE score was associated with Obsessive-compulsive, Depression, Phobic anxiety, and Additional items ; and grip strength was associated with Somatization, Depression, Anxiety and Additional items. Conclusions : More severe neurological symptoms were associated with higher psychiatric morbidity particularly in depression, phobic anxiety, sleep and appetite disturbance at acute stage of stroke. More intensive psychiatric care and intervention are needed for the high risk group.

Analysis of the Importance of Planning Elements for Healing Gardens (치유정원 조성을 위한 계획요소의 중요도 분석)

  • The purpose of this study is to establish a system of planning elements necessary for effective healing garden. We suggested new planning parts and elements for healing garden through analyzing prior literature and evaluated the importance of planning elements through expert surveys. The results of the study are stated as follows. First, we suggested five planning parts and 19 planning elements. $\LARGE{\cdot}$ Planning parts: Physical environment, social environment, cognitive environment, functional environment, psychological environment. $\LARGE{\cdot}$ Planning elements: Plant planting, garden view, hydroponic space, shade space, work space, recreation space, community space, exercise space, landmark element, sign system, safe facilities and materials, disaster safety facility, universal design, accessible path, economical maintenance, sight openness, privacy guaranteed, variety of resting space, artistic element. Second, as a result of analyzing the importance of planning elements, the first place was the planting, the second place was the safety facilities for the disaster, and the third was diversity of rest space. In addition, the artistic elements and the landmark elements were ranked 18th and 19th, respectively. Third, we evaluated the level of importance of planning elements and classified it into three groups. The most important A group included two elements(plant planting, disaster safety facility), the next important B group included eleven elements(variety of resting space, community space, hydroponic space, universal design, safe facilities and materials, economical maintenance, accessible path, privacy guaranteed, garden view, work space, sight openness) and the less important C group included six elements(shade space, exercise space, recreation space, sign system, artistic element, landmark element). We suggest that, in creating a healing garden, the two elements in A group are essential elements and the eleven elements in B group should be importantly applied. In addition, the six elements in C group are recognized as arbitrarily selectable elements due to the relatively low importance levels.

  • It's essential to meet the expectations of drivers through reasonable road design, which makes the drivers to recognize the conditions of road sufficiently. In this case, we could say that because the roads are consistently designed, drivers can make a safe and comfortable drive. There are so many studies about the alignment design consistency methods which are previously mentioned. In this study, Firstly we categorize the studies which are concerned with the estimation of the alignment design consistency methods and then apply the method which fits for the actual condition into the practice. And we'll make a realistic method by using the data which aye collected from the National Road. The developed model in this study is the predicting model including speed($V_t$) as the variable on the upper stream 100m of the curve. This model was developed which divided to two cases in the all directions of two lanes; One case is the $R{\leq}200m$ and another case is the R>200m. In the section of the $R{\leq}200m$, this case was influenced on the road alignment(R) and($V_t$) and another section was found that is affected to the speed($V_t$) on the upper stream loom rather than the effect of road alignment(R). In the case of all directions of four lanes, however, this study is predicted divided to two sections on the 400m of R, these section have more influenced to($V_t$) than the road alignment factors. This result of the four lanes was represented to different result with the two lanes. This study will further need development of the predicting model with the higher confidence through collecting data with more the exact data, the various road alignment data and speed of the several sections on the upper stream on the curve.

Usability of Urinary Reagent Strips in Diagnosis of Meningitis (뇌수막염의 진단에서 뇨검사 시험지를 이용한 뇌척수액 검사의 유용성)

  • Purpose: Rapid diagnosis and prompt treatment remain the cornerstone of management of patients with meningitis. Examination of cerebrospinal fluid(CSF) for glucose, protein, cells, and organisms is necessary for accurate diagnosis, but in many parts of the world facilities do not exist to do so. We tested CSF samples from 69 children with suspected meningitis with urinary reagent strips and analyzed the results to know the usability of urinary reagent strips in diagnosis of meningitis. Methods: 69 CSF samples obtained from children with suspected meningitis were analysed. Each sample was divided into two; one was sent to the laboratory for routine diagnostic evaluation, and the other was tested with the reagent strip(Combur10 Test M). Laboratory values of CSF glucose, protein, and leucocytes for each CSF sample were plotted against the corresponding reagent-strip category. Results: There was good agreement between the dipstick and laboratory values for CSF protein and cell count but not for glucose. Conclusion: Rapid diagnosis of meningitis could be made with three component of the Combur10 test strip. It is easy to do and would be of particular value to those working in parts of the world where no laboratory facilities exist.

A Study of the Images of General Supers and a Department Store in a Local City (지방도시에 입점하고 있는 종합슈퍼와 백화점에 대한 점포이미지 비교 분석)

  • Suncheon is a city comprising a rural and urban area, where there are four types of large stores. Studies have shown that there are too many large stores serving the local population of just 300,000. However, geographically, Suncheon is located at a transportation hub that borders the cities of Gwangyang and Yeosu as well as the local counties of Boseong and Gurae. Residents of these areas can reach these shopping stores within an hour's drive. Thus, the managers of these four stores regard residents in these areas as their valued customers and endeavor to create a differentiated image among them. In this study, 13 different images were used to determine the public's opinions and feelings towards these stores and the differences were analyzed. The store images measured overall store impression, diversity of the product, the quality of products displayed at the store, accessibility, the atmosphere, service to the customers, and so on. These images are evaluated subjectively by each customer and are major factors in them deciding to revisit the stores. The 13 images are classified into five main categories and further classified into 13 sub-categories. Three kinds of factor images were extracted from the store images in the five main categories by factor analysis using SPSS Ver. 19. The first factor image was extracted from the images of convenience, atmosphere, and service in the main categories and is called a sub-service factor for the store in this study. Accessibility to the store was classified as a convenience image in the main category and was extracted as a common factor along with diversity and the price of goods. These differences are expected according to the store location, that is, the difference between stores located in a large city and those in a small local city, and depending on the nature of survey respondents. The result shows that there is a significant difference between the stores' images with regard to accessibility, the price of products, brand image, and lighting/sound image. This study has the following limitations. First, the survey sample was restricted to residents of a small local city that includes rural and urban populations. The differences between the store images regarding traffic and accessibility are factored by store location, whether they are located within a large or a small city as well as the economic situation of these cities. Second, only the customers of large-scale stores were included in the survey as respondents. Relatively large traditional markets are held every five days in local cities and there is competition between large-scale stores and traditional markets with regard to diversity and the price of goods. It could be expected that customers in large-scale stores and customers in traditional markets would hold different store images. In future studies, images of stores in large cities should be compared with the images of stores located in small local cities. In addition, customer behavior when buying goods in large-scale stores should be compared with their behavior when buying goods in traditional markets.

Study on the Educational Plan to Enhance Intercultural Abilities Using the Oral Folktales of Immigrants who Mov ed to Korea (이주민 구술 설화를 활용한 상호문화능력 신장의 교육 방안연구)

  • As a way of enhancing the intercultural ability needed for diverse cultural eras, this study focuses on the "narration" of the Italian education scholar Maddalena De Carlo in order to determine the "diverse values" created by the "symbolic representation" based on the folktales narrated by immigrants living in Korea. Through this, it specifically presents educational elements and contents that can raise relative sensitivity. The authors of this paper have connected, empathized, and communicated with people of various cultures in order to go beyond Carlo's discussion. The paper discusses the expansion of cultural sensitivity as an element of education through narrative topics using the folktales of immigrant narrators in Korea. It also recognizes the limitations of a desire for a homogeneous union within an intercultural society and thus formulates educational contents for creating a relationship with heterogeneous ideas through the elimination of communication barriers through heterogeneity and a consideration of the surface and the back. This is systemized in six steps. Step 1: Listening to oral folktales of immigrants, Step 2: Finding heterogeneous motifs imprinted in the immigrants' memories, Step 3: Understanding the meaning of the opposing qualities symbolized by heterogeneous motifs, Step 4: Creating narrative topics containing the key motifs, Step 5: Generating the value of symbolic representation as a narrative topic, and Step 6: Expanding the value of life into a cultural symbol. In Chapter 3, this study focuses on educational contents using immigrants' folktales by applying these six steps. The class contents include the recognition of the limitations of desire for a homogeneous union within an intercultural society and the consideration of how to create a relationship with heterogeneous ideas through the elimination of communication barriers through heterogeneity and consideration of the surface and the back. This paper then compares the Indonesian folktale, The Inverted Ship Mountain and the Mom's Mountain, with the world-famous Oedipus myth, to determine what the symbolic representation of these heterogeneous motifs is. In Step 6, when the symbolic system is culturally extended, the incestuous desire that appears in the "inverted ship" is interpreted as a fixation that was created when the character sought to unite with homogenous idea. The Cambodian folktale, The Girl and the Tiger, is a story that is reminiscent of the Korean folktale, The Old Man with a Lump. Through the motif in "Tiger," this paper generates a narrative topic that will enhance the students' intercultural abilities by culturally expanding their skills in how to relate with a heterogeneous being that is usually represented as an animal. The Vietnamese folktale, The Coconut Bowl, similar to the Korean folktale, GureongDeongDeong SinSeonBi, is a story that draws a variety of considerations about the surface and theback, and it shows readers how to build a relationship with a heterogeneous idea and how to develop and grow with such a relationship. Thus, if a narrative topic is generated and readers are able to empathize using an opposing feature formed by the core motif of the folktale, it becomes possible, through immigrant folklore, to construct a possibility of a new life through the formation of a relationship with an unfamiliar and heterogeneous culture.

Proposal of Localization Policy Based on the Status of Chinese's Research Facilities and Equipment Construction in Korean Basic and Analytical Science Field (국내 기초·분석과학 분야 내 중국산 연구시설·장비 구축 현황에 따른 국산화 정책 제언)

  • The aim of this study was to examine the scale and market share of Chinese's research facility & equipment in the domestic research equipment market of basic and analytical science field for analyzing the difference of the number and amount of construction by year of acquisition, national research facility equipment standard classification code, and type of institution based on the information of the research equipment invested by the Korean government for the past 14 years. In addition, we analyzed the correlation among the year of acquisition, equipment standard classification code, and type of institution variables. As of January 1 2019, from 2005 to 2018, 50 Chinese's research facilities & equipments (main equipment with a construction cost of 30 million won or more) built in the basic and analytical science fields were selected for this study and their number of construction, amount of construction, year of acquisition, type of institution, and standard classification code were analyzed. Differences of the number and amount of construction with-in and by year of acquisition, standard classification code, and type of institution were tested using a single sample Chi-square test, Mann-Whitney U test, and Kruskal-wallis test. The correlation among the three variables was analyzed by using the Chi-square test of cross-tabulation analysis. And there was a statistically significant correlation among the year of acquisition, standard classification code, and type of institution (p<.05). Compared to the 2000s, in the 2010s, high-priced Optical Electronics/Video Equipment was installed at private universities, private enterprises, and government-affiliated research institute. Therefore, the domestic construction status of Chinese's research facility & equipment in the basic science and analytical science field is less than that of the domestic ones, but the number and the amount of construction are increasing statistically. So it is necessary for the government to be able to recognize the possibility that the Chinese's research facility and equipment can encroach on the domestic research industry market and to prepare related provision.
