Relationship between Body Mass Index and Insulin Resistance and Beta Cell Function by Obesity Type in Obese Middle-Aged Men (비만 중년 남성의 비만 유형별 체질량지수와 인슐린저항성 및 베타세포 기능간의 관련성)

  • This study wanted to confirm the relevance between body mass index(BMI) and insulin resistance and beta-cell function based on abdominal obesity in obese middle-aged men. This study targeted 797 obese middle-aged men who had undergone health checkups at general hospitals in Gyeonggi-do from January 2018 to June 2020. There were 327 in the group with abdominal obesity and 470 in the group without abdominal obesity. Glucose(p<0.001), HbA1c(p=0.003), insulin(p<0.001), HOMA-IR(p<0.001) was different between groups. BMI was a factor affecting insulin resistance and beta cell function regardless of the with or without of abdominal obesity. BMI was associated with the onset of disease of insulin resistance and beta cell functional degradation regardless of the with or without of abdominal obesity. Therefore, it is considered necessary to manage the indicators of the BMI through exercise programs and regular checkups for health management of middle-aged obese men.

  • The purpose of this study was to examine the effect of Health exercise program on insulin resistant and blood lipid factor of elderly women for 12 weeks. The two groups were classified into one group(exercise group: EG) with aerobics exercise and band exercise both, the other group(control group: CG) controled. The group of EG was applied to doing aerobics exercise and band exercise 4 times for 60 minutes a week. The intensity of the exercise was ACSM. Each measurement variable was measured before and after 12 weeks to investigate the effect. During this study the result came out with this step. First, EG have shown interaction with blood lipid(TC, HDL, LDL, TG) and blood pressure(SBP, DBP)factor. Second, EG have shown interaction with insulin resistant(insulin, glucose). Therefore, this study gives us positive result to effect of health exercise program on blood lipid factor and insulin resistant factor of elderly women for 12 weeks.

Relationship between Visceral Adiposity Index, Insulin Resistance and Pancreatic Beta Cell Function According to the Prevalence of Metabolic Syndrome in Korean Obese Adults (한국 비만성인의 대사증후군 유병에 따른 내장지방지수와 인슐린저항성, 췌장 베타세포기능과의 관련성)

  • The purpose of this study was to investigate the relationship between VAI, insulin resistance, and pancreatic beta cell function according to the prevalence of metabolic syndrome in obese adults. From 2017 to 2019, 1,797 obese adults who received medical checkups at a general hospital in Bundang. Diagnosis of metabolic syndrome is NCEP-ATP III. HOMA index was used for insulin resistance and pancreatic beta cell function. VAI was higher in the metabolic syndrome than in the control(p<.001). As the number of risk factors for metabolic syndrome increased, the VAI value was higher(p<.001). The prevalence of metabolic syndrome increased as the VAI quartile increased(p<.001). VAI was also shown to be related to HOMA-IR and HOMA-β in the control, but not in the metabolic syndrome.

  • 인슐린 저항성 증후군 환자나 당뇨병 발병 고위험군, 당뇨병 초기 환자에서는 인슐린 저항성 및 고인슐린혈증은 일반적으로 함께 나타나는 상황이다. 그러나 당뇨병이 이미 발병하여 매우 높은 혈당 상태에 있거나 당뇨병이 오래 진행되어 인슐린 분비능력이 매우 하강한 환자에서는 인슐린저항성이 심한데도 불구하고 고인슐린혈증은 나타나지 않을 수 있다.

  • This study analyzed the motivational interviewing of type 2 diabetes patients in order to understand the difficulties and motivations associated with starting insulin treatment in psychosomatic insulin-resistant patients. The method used the consistent comparative analysis. The results of study were as follows: building relationships, focusing (Self-explore of problems with diabetes self-management, Recognizing the limitations of diabetes management, Concerns about complications, Imagine the future), inducing (Imagine the expected benefits and disadvantages of insulin administration, Discovering the benefits of insulin, Changes in thinking about starting insulin therapy), planning (Show specific curiosity about change, Planning change), maintaining change behavior (Keeping change confident), and evaluating. This study will contribute to understanding patients with type 2 diabetes with psychological insulin resistance. It may also provide implications for professionals helping these subjects.

Assessing a Body Shape Index and Waist to Height Ratio as a Risk Predictor for Insulin Resistance and Metabolic Syndrome among Korean Adults (한국 성인의 인슐린저항성 및 대사증후군 위험 예측인자로서 체형지수와 허리둘레/신장 비율의 효용성)

  • The WHtR (waist to height ratio) and ABSI (a body shape index) are indicators that reflect abdominal obesity. This study examined the insulin resistance and metabolic syndrome prediction ability of ABSI and WHtR. In this study, 4,395 people aged 20 years or older, who underwent physical examinations at a General Hospital in Gyeonggi-do from January 2017 to September 2017 were assessed on a cross section survey. Metabolic syndrome was defined according to the criteria of the AHA/NHLBI. Insulin resistance was judged to be insulin resistance when the HOMA-IR value was 3.0 or more. Both men and women showed a stronger correlation between WHtR and the metabolic risk factors than ABSI. The AUC value of WHtR and ABSI was 0.849 and 0.676, respectively (p<0.001). The AUC value of WHtR and ABSI for predicting insulin resistance was 0.818 and 0.641, respectively (p<0.001). In conclusion, the ABSI has low predictive power of insulin resistance and metabolic syndrome whereas the WHtR has good predictive power for metabolic syndrome and insulin resistance.

Relationship Between Mitochondrial DNA Copy Number, Metabolic Abnormalities and Hepatic Steatosis (지방간 및 대사 인자들과 말초혈액 백혈구의 사립체 DNA copy 수와의 연관성)

  • Insulin resistance plays a central role in fatty liver, a part of the metabolic syndrome. This study examined the relationship between fatty liver, metabolic abnormalities and mitochondrial DNA [mtDNA] copy number in peripheral blood that is correlated with diabetes or metabolic markers. Fatty liver was assessed by questionnaire on alcohol consumption and abdominal ultrasonography. MtDNA copy number in peripheral leukocytes was measured by a real-time quantitative polymerase chain reaction [PCR]. Among 445 subjects, 148 subjects had hepatic steatosis and 297 were controls. mtDNA copy number was significantly lower in fatty liver group in comparison with that of normal finding group. This result is similar in both groups, alcoholic or non-alcoholic fatty liver group. MtDNA copy number was inversely correlated with alanine aminotransferase [ALT], aspartate aminotransferase [AST], gamma-glutamyltransferase [$\gamma$-GTP], body mass index [BMI], waist circumference, diastolic blood pressure, and free fatty acid. MtDNA copy number in peripheral leukocytes was associated with fatty liver and insulin resistance related factors.
