Closure of secundum atrial septal defect: comparison between percutaneous and surgical occlusion (이차공 심방 중격 결손의 폐쇄: 경피적 방법과 수술적 방법의 비교)

  • Purpose : This study was performed to compare the safety, efficacy and clinical results of the Amplatzer septal occluder (ASO) for closure of secundum atrial septal defect (ASD) with surgery. Methods : One hundred fifteen patients diagnosed as isolated secundum ASD in Gil Medical Center, Gachon University of Medicine from January 2000 to July 2006 were included. Seventy patients underwent surgical repair of ostium secundum ASD. Forty-five consecutive patients were treated with percutaneous closure using ASO. We compared the mortality, morbidity, hospital stay, and efficacy between two groups. Results : Male to female ratio was 1:2.4. The mean age and the size of defects were not statistically different. No mortality occurred in either group. The success rate was 97.8% in the device group and 100% in the surgical group. The overall rate of complications was higher in the surgical group than in the device group (64.0 vs. 15.6%, P<0.05). Hospital stay was shorter in the device group than in the surgical group ($4.2{\pm}1.2$ vs. $12.4{\pm}4.7days$, P<0.0001). Residual shunt rates were more frequent in the device group (8.9%) than in the surgical group (4.3%) at discharge. All residual shunts disappeared at 3 months follow-up. Conclusion : Percutaneous closure of ASD using ASO is a safe and effective alternative to surgical repair. The indications of percutaneous ASD closure with ASO would be expanded by accumulation of experiences and evolutions of device.

$C_a/C_c$ for Marine Clay at Southern Part of Korea by Laboratory Consolidation Tests (실내압밀시험에 의한 남해안 해성점토의 $C_a/C_c$)

  • Consolidation settlements on soft clay are often greatly and potentially damaging to structures. Currently, large-scale projects are in planning or progressing in Korea. These structures will be constructed on both thick and soft clay layers, and so the accurate evaluation of magnitude of settlement is required at every step in design and construction. Especially, secondary compression may play an important role in consolidation settlements of soft clay. Generally, the magnitudes of secondary compression are evaluated by laboratory and in-situ consolidation tests. The empirical $C_a/C_c$ may be economical, fast and powerful tool in estimating secondary consolidation settlement. However, the databases of the $C_a/C_c$ at construction site in Korea are insufficient. The purpose of this study is to investigate the relationship of $C_a/C_c$ on marine clay near the southern sea in Korea. A series of incremental loading consolidation tests (measuring base pore water pressure) is peformed. It was found that the $C_a/C_c$ on undisturbed marine clay is 0.0397.

Predicting Factors on Performance Confidence of Cardiopulmonary Resuscitation in Community Members (지역사회 주민의 심폐소생술 수행 자신감 예측요인)

  • This study is a descriptive investigatory study that secondarily analyzes the community health survey in order to identify the characteristics of confidence regarding the execution of cardiopulmonary resuscitation among community members of Korea. Study subjects included 357,176 people who were aware of cardiopulmonary resuscitation based on 2014 and 2016 community health survey. The collected data were analyzed for composite sample frequency and decision-making tree using SPSS WIN 25.0 program. According to the results of this study, a confidence regarding execution of cardiopulmonary resuscitation of the community members was higher if the subject has experienced cardiopulmonary resuscitation education, trained on mannequin within the past 2 years, received cardiopulmonary resuscitation education within the past 2 years, is of male sex, and is 41.5 years of age or younger.

  • In this paper, Ive investigated algebraic concepts which are contained in Euclid's Elements. In the Books II, V, and VII∼X of Elements, there are concepts of quadratic equation, ratio, irrational numbers, and so on. We also analyzed them for mathematical meaning with modem symbols and terms. From this, we can find the essence of the genesis of algebra, and the implications for students' mathematization through the experience of the situation where mathematics was made at first.

  • The development of different entities of soft tissue sarcoma in one patient is rare. It usually affects head and neck or abdominal region, whereas those affecting the extremities are much rarer. We describe a patient with double primary presentation of liposarcoma and Ewing's sarcoma in extremity. This case implies that sarcoma patients are at increased risk of a second malignancy, and this implies a need to search for occult tumors during follow up.

  • Fifty-four marginal rotation procedures for cicatrical entropion and marginal entropion were performed in twenty-eight patients(8 males, 20 females). The age ranged from 4 to 74 years old(mean 51.1 years old). After the follow up periods of 6 to 48 months, excellent correction of the eyelid margin and good cosmetic appearance were noticed in most cases, but nine cases combined with severe trichiasis showed recurred trichiasis requiring reoperation.

  • Loss of appetite is an important factor in the quality of life for advanced cancer patients. Megestrol acetate is used to stimulate appetite, but it can cause suppression of the pituitary adrenal axis due to the affinity of the glucocorticoid receptor. Adrenal insufficiency is a life threatening disorder if left, untreated, but the initial clinical symptoms of the patients are vague. Awareness of the glucocorticoid-like activity of megestrol acetate and its side effects are important for the diagnosis of adrenal insufficiency. We present a case of secondary adrenal insufficiency associated with megestrol acetate in a patient with lung cancer.

  • Somnambulism is classified as a parasomnia and has been reported with the use of antidepressants and other psychotropics. However, to our knowledge, there have been no reports of somnambulism associated with the use of mirtazapine (a noradrenergic and specific serotonergic antidepressant;NaSSA). We experienced a case of the mirtazapine (30 mg/day)- induced somnambulism in a 65 year-old woman with major depressive disorder who has never been diagnosed as panic disorder, somnambulism, other parasomnias, neurological disorders (including epilepsy), and other medical diseases. The sleepwalking symptoms disappeared after the antidepressant was replaced with paroxetine (20 mg/day).

  • Mesenteric venous thrombophlebitis secondary to inflammatory processes such as diverticulitis and appendicitis is a rare disease; however, it can nonetheless cause bowel ischemia and infarctions. Radiologic diagnosis is vital for mesenteric venous thrombophlebitis complicated with diverticulitis due to its non-specific clinical presentation and very low incidence. We report a case of a 61-year-old woman with superior mesenteric vein thrombosis and ileocecal diverticulitis on CT, which was resolved after treatment with a combination of antibiotic therapy and right hemicolectomy.

Silica Enrichment in Mantle Xenoliths Trapped in Basalt, Jeju Island: Modal Metasomatic Evidences (제주도 맨틀포획암내의 실리카 부화작용: 모달 교대작용의 증거)

  • Mantle-derived xenoliths, corresponding to spinel harzburgite and lherzolite in alkali basalts from Jeju Island, are metasomatized to various extents. They contain distinctive secondary orthopyroxene, forming corona or poikilitic textures. It clearly indicate that this secondary orthopyroxene has been produced at the expense of olivine along the grain boundaries and margins, suggesting silica-enriched metasomatic melt infiltrated through grain boundaries. Based on the geotectonic characteristics of Jeju Island and textural characteristics and major elements composition of mantle xenoliths, it is suggested that the silica-enriched melt/fluid could have derived from the ancient subducted slab, possibly in the mantle wedge, implying that the high $SiO_2$ activity in the lithospheric upper mantle beneath Jeju Island at that time.
