Oral Health Status and Dental Treatment Need of Liver Transplant Candidates (간 이식 예정 환자의 구강건강상태 및 치과치료 요구도에 관한 연구)

  • Liver transplantation is definitive treatment for the patients suffering from hepatitis, severe liver cirrhosis and liver cancer. In these patients, systemic infections under immunosuppression may occur easily. Therefore, primary object of dental treatments before liver transplantation is absolute removal of oral infection source. In addition, comprehensive dental management plan is essential for success of liver transplantation. The present study has been performed to investigate decayed, missing and filled permanent teeth index(DMFT index), degree of oral hygiene, past medical history, need of dental treatment, completion of dental treatment need and time interval between dental visit and operation date of liver transplantation in liver transplant candidates. Obtained results were as follows; 1. Decayed teeth of the patients were 2.68, missing teeth were 4.02 and filled teeth were 3.42. DMFT index was 10.12. 2. Twenty percents of patients showed moderate to severe food impactions, 42.2% of patients had moderate to heavy calculus and 37.8% of patients displayed gingival inflammation with swelling. 3. Patients needed periodontal treatments more than any other dental treatments. Periodontal treatments were needed for 88.9% of patients, operative & endodontic treatments were 46.7% of patients and 33.3% of patients needed for oral & maxillofacial surgical treatments. 4. Among 90 patients, time interval between scheduled operation date of liver transplantation and dental visit was within 2 weeks for 32.2% of patients, within 1 week for 20.0% of patients. In conclusion, most liver transplant candidates needed dental treatments for removal of potential infection sources. However because of insufficient interval between dental visit and operation date, they had taken liver transplantation procedures without comprehensive dental management. Development of preventive and comprehensive dental management program is mandatory for these patients. Cooperative interdisciplinary management will play a positive role for successful liver transplantation.

Partial Left Ventriculectomy as a Bridge to Cardiac Transplantation in a Patient of End-Stage Heart Failure -Case Report- (말기 심부전 환자에서 심장이식의 교량으로서 좌심실 축소술 - 중례보고 -)

  • A 40-years-old male with dilated cardiomyopathy(DCMP) and end-stage heart failure had undergone partial left ventriculectomy(PLV) in July 1997 and then underwent cardiac transplantation in January 1999. Three months later he showed increased ejection fraction (EF) from 26% to 42.6%, decreased left ventricular end diastolic diameter(LVEDD) from 71mm to 45mm, cardiac output(CO) 3.95 L/min and cardiac index(CI) 2.28 L/min/m$^2$echocardiographically. Eight months later, left ventriclular end diastolic and systolic diameters increased to 56 and 51 mm respectively and EF decreased to 17% in echocardiographic follow-up. He had been on maximum medication until he underwent cardiac transplantation 18 months after the PLV. Consecutive myocardial biopsies (1, 3 and 6 month later) showed ISHLT (international society of heart and lung transplantation) class la and the treatment for rejection was not needed until now on. We report a partial left ventriculectomy as a successful bridge to cardiac transplantation in a patient with DCMP and end-stage heart failure.

  • The patient was an eight-year-old female. She was diagnosed as dilated cardiomyopathy. She was supported with bi-ventricular assist because of heart failure for 15 days. After 7 days, she was suffered from prerenal type ARF and support with continuous veno-veno hemodyalisis(CVVHD). And then heart transplantation was performed, heart donor's blood type was A. Immune suppressants were used after due consideration for renal toxicity. ARF was resolved on post operative $14^{th}$ day. She was discharged on post operative $52^{nd}$ day without any specific post operative complication. She has been followed up without any immune rejection reaction upto 14 months.

  • Anomalous origin of left coronary artery from pulmonary artery(ALCAPA) is a rare fatal congenital anormaly that needs early surgical intervention. Many reports say that the choice of operative procedure is reimplantation of the left coronary artery into the ascending aorta. We experienced the surgical management of a case of the ALCAPA. The patient was 44 days old and 3.45 kg weighed female baby who had a symptom of congest ve heart failure. She underwent implantation of coronary artery on the aorta with cardiopulmonary bypass and recovered without any complications.

  • The first human meniscal allograft transplantations (MATs) were performed 30 years ago. In the early era, candidates were limited to patients who have favorable joint conditions. MAT is currently indicated for patients with post-meniscectomy symptoms, such as compartmental pain or effusion after a subtotal or total meniscectomy. The current indication for MAT is being expanded to other patients who were not indicated previously. The present article reviews how the indications of MAT have changed over the years.

  • A visual concurrent programming environment, called ESTGVP is designed and implemented, which is easy to understand, highly portable, and can represent parallel behaviors. For our purpose, a conventional state transition graph is extended so as to enable both of synchronous and asynchronous parallel operations. We call it extended state transition graph (ESTG). ESTGVP uses the ESTG and texts for programming, and makes it easy programming sequential and parallel behaviors. Also, it is easy to understand the control structure of a program because ESTGVP is a visual programming environment based on the graph. ESTGVP is written in Tel language and thus it is highly portable on various operating systems. It consists of three major components; edition, transformation and execution. If necessary, ESTG can be transformed into C or Tel language, and its execution is based on Tel.

연골 결손의 치료

  • 연골 결손은 자체 치유 능력이 적어 그 크기가 1 cm 이상인 경우 골관절염이 발생할 수 있다. 최근에는 3-tesla MRI 등의 발전으로 인해 수술 전에 미리 연골 손상을 정확히 진단할 수 있어 연골 손상의 치료를 더욱 적극적으로 할 수 있다. 이제까지 연골 손상에 대한 수술적 치료는 세척술(lavage) 및 변연 절제술(debridement), 미세 천공술(microfracture), 자가 골연골 이식술(autologous osteochondral graft), 자가 연골 세포 이식술(autologous chondrocyte implantation, ACI), 동종 골연골 이식술(osteochondral allograft)이 있다.

Effect of Timing of Vaccination on the Pregnancy Rate of Recipients and the Viability of Calves Derived from In Vitro Produced Hanwoo Embryos (한우 체외 수정란이 이식된 수란우에 백신 투여 시기가 임신과 송아지의 생존에 미치는 영향)

  • The present study was conducted to investigate the effects of time of vaccination in recipients on the pregnancy rate and the viability of calves derived from embryos produced in vitro. In experiment 1, control group was non-vaccinated, group 1-1 received vaccine (Pfizer, Exton, PA, USA) during 0$\sim$4 weeks and group 1-2 received vaccine during 4$\sim$8 weeks before embryo transfer. The pregnancy rates in the control (42.6%) and group 1-2 (45.3%) were significantly higher (p<0.05) than that in the group 1-1 (32.6%). However, the abortion rates were similar among groups (4.9 to 13.5%). In experiment 2, the recipients received embryos produced in vitro were non-vaccinated (control) or vaccinated. Vaccine was injected during 0$\sim$4 weeks (group 2-1) and 4$\sim$8 weeks (group 2-2) before parturition. The incidence of a disease in calves was significantly higher in the control (22.4%) than in other vaccinated groups (2.2% and 3.1%, p<0.05). The mortality of calves in the control is 27.6%, which was significantly higher than that of group 2-1 and group 2-2 (11.1% and 7.8%).

  • We report on two children with a high risk of recurrent focal segmental glomerulosclerosis (FSGS) after renal transplantation that could be effectively prevented by prophylactic administration of cyclosporine combined with preemptive plasmapheresis prior to renal transplantation.

  • Sarcoidosis is a systemic inflammatory disease with granulomatous lesions, and cardiac involvement occurs in 20~60% of patients. Isolated cardiac sarcoidosis is extremely rare, and heart transplantation can be performed, if indicated, contingent upon the absence of systemic manifestations of the disease. We present a case of isolated cardiac sarcoidosis with progressive heart failure, which was successfully managed by heart transplantation.
