Analysis of Differences in Family Problem Solving, Family Relationship Satisfaction, and Family Life Satisfaction According to Demographic Characteristics of the Disabled (장애인의 인구사회학적 특성에 따른 가족문제해결, 가족관계만족도, 가족생활만족도의 차이 분석)

  • 본 연구는 한국복지패널(KOWEPS) 15차 연도(2020년) 자료를 활용하여 장애인의 인구사회학적 특성인 성별, 연령, 교육수준, 혼인상태에 따라 가족문제해결, 가족관계만족도, 가족생활만족도에 대하여 어떠한 차이가 있는가를 분석하고, 이를 장애인 복지 정책 기초자료로 활용하는데 목적을 두었다. 연구대상은 한국복지패널 15차 년도 전체패널 27,447명에서 2021년 현재 장애인으로 등록된 1,281명중 80세(1942년생) 이상 422명을 제외한 859명으로 하였다. 연구결과 성별, 연령, 교육수준에 따른 가족문제해결, 가족관계만족도, 가족생활만족도의 평균값이 대부분 유의미한 차이가 없는 양상으로 나타났으나 가족문제해결, 가족관계만족도, 가족생활만족도의 3개변인 모두에서 결혼 상태에 따른 평균의 차이는 각각 유의미한 차이가 있는 것으로 나타났다. 유배우자의 경우는 그렇지 않은 결혼 상태에 비해 가족관계가 원만하게 이루어지고 있는 것으로 나타나 장애인들의 경우 가족 간의 순기능적 의사소통과 사회적 지지가 반드시 필요한 것으로 판단된다.

  • A salivary stone on mouth floor was removed in a 10-year-old girl with cerebral palsy and epilepsy. If left untreated, sialolithiasis can lead to inflammation of whole salivary gland with severe pain and inflammation that spread to other side of salivary gland causing damaging of teeth, abscess, and nerve disorders. Special needs patients have difficulty to express their pain or discomfort. To overcome communication problem and offer appropriate treatment in proper time, a careful and comprehensive clinical examination including soft tissue and tooth should be prioritized in special needs patients visiting the dental clinic.

  • 현재 4차 산업혁명으로 인해 많은 이들의 삶의 질이 이전보다 개선되었음에도 불구하고, 소외된 계층을 위한 개발은 타 분야에 비해서 더뎌지고 있는 실정이다. 현대의 청각 장애인과 언어 장애인들은 시각 언어인 수화를 이용하여 의사소통을 한다. 그러나 수화는 진입 장벽이 높기 때문에, 이를 사용하지 않는 사람들은 청각 장애인 및 언어 장애인과 의사소통을 하는데 어려움을 겪는다. 본 논문은 이러한 불편함을 줄이기 위해 서포트 벡터 머신(Support Vector Machine, SVM) 기반의 HOG(Histogram of Oriented Gradients) 특징 벡터를 이용하여 수화의 기본인 숫자를 분류할 수 있는 시스템을 구현하여 수화를 번역할 수 있는 가능성을 제안한다.

  • 사람 간 의사소통에서 표정, 몸짓과 같은 비언어적 시각 요소들은 감정 표현의 중요한 요소이나, 시각장애인들은 이러한 감정 정보들을 받아들이는데 제한적이다. 본 논문은 시각장애인에게 이러한 비언어적 시각 요소 기반의 감정 정보를 인식하여 온각으로 전달하기 위한 방법론을 제안한다. 상대방의 표정은 안경 착용형 카메라로 인식되어 감정으로 분류된다. 인식된 표정이 웃는 얼굴과 같이 호감인 경우, 이 상태는 온각으로 변환되어 안경에 착용된 온도전달 장치에서 시각장애인에게 호감을 전달한다. 이러한 온각기반 감정전달 장치는 시각장애인의 의사소통 향상을 위한 장치의 개발에 응용될 수 있다.

ICU Nurses' Perceptions of Communication Difficulties, Importance, Satisfaction and Communication Barrier with Patient Families (중환자실 간호사의 의사소통 난이도, 중요도 및 만족도에 관한 인식과 환자 가족과의 의사소통 장애에 대한 조사연구)

  • Purpose: This study was conducted to investigate ICU nurses' perceptions of communication difficulties, the importance of and satisfaction with communication with doctors, other nurses, patients, and family, as well as to explore communication barrier with patient families. Methods: Investigators developed a 15-item communication perception questionnaire and 58-item communication barrier questionnaire. Communication barrier included 4 domains: nurses, family, environment, and patient condition. A total of 151 ICU nurses with a minimum of one year of ICU experience participated. Results: ICU patients ($3.38{\pm}0.73$) were the most difficult group to communicate with, followed by family ($3.32{\pm}0.72$), senior nurses ($3.25{\pm}0.74$), doctors ($3.21{\pm}0.68$), and nurse colleagues ($2.64{\pm}0.73$). Doctors ($4.61{\pm}0.53$) were the most important group to communicate with, followed by nurse colleagues ($4.52{\pm}0.54$), patients ($4.49{\pm}0.58$), senior nurses ($4.44{\pm}0.55$), and family ($4.43{\pm}0.61$). Satisfaction with communication was the highest with colleague nurses ($3.60{\pm}0.68$), then senior nurses ($3.37{\pm}0.74$), family ($3.18{\pm}0.71$), patients ($3.09{\pm}0.75$), and doctors ($3.06{\pm}0.83$).The total score of the communication barrier was $2.83{\pm}0.52$, where each domain was scored as follows: patient condition $3.13{\pm}0.74$, nurses $2.83{\pm}0.60$, environment $2.81{\pm}0.66$, and family $2.76{\pm}0.57$. The ICU nurses reported that communication was difficult due to 'sudden deterioration in the patient's condition', 'being too busy', 'a noisy environment', and 'information not being shared between family members.' Significant differences were noted by age, clinical experience, and marital status of nurse respondents. Conclusion: The findings indicated that development of a protocol on communication between nurses and doctors as well as development of an educational program on communication skills are necessary.

Expectations of the First Visit to Orofacial Pain Clinic for the Patients with Temporomandibular Disorders (구강안면통증클리닉에 처음 내원한 턱관절장애 환자들의 내원 목표)

  • Understanding patients' expectation for health visits and providing appropriate care may increase patients' satisfaction with health care, leading to more positive treatment outcome. The study aimed to investigate expectations of the patients with temporomandibular disorders (TMD) at their first visit to university-based orofacial pain clinic and to evaluate any relation with duration of pain, presence of previous treatment, pain severity and pain interference. Inclusion criterion was patients aged over and 18 years old and diagnosed as TMD during 3 months' period from Aug to Nov 2010. They were asked to complete the questionnaires for patients' expectation and the Brief Pain Inventory (BPI) at waiting room prior to consultation. 322 TMD patients participated in the study(M:F=1:1.5, mean age=36 years old). The study indicated that the most important top 3 expectations were 'cure of pain', 'understanding their problem' and 'doctor-patient communication' in order. This finding was not affected by gender, duration and previous treatment history but affected by sub-category of TMD and BPI pain severity and pain interference. 'Pain relief' and 'understanding their problems' were relatively highlighted in the patients with muscle disorders and combination (joint-muscle) disorders of TMD than those with joint disorders who wanted communication and further investigation relatively more (p=0.000). While expectation for pain relief was expected more with increase of pain severity and interference, patients with mild level of pain severity and interference expected communication and further investigation relatively more (p=0.000, 0.017, respectively). Based on the results of the study, though pain relief was the primary concern for TMD patients suffering from pain, their satisfaction with care may be increased by explanation for etiology and mechanism of TMD to make them understand their problems better and doctor-patient communication and collaborative decision-making for treatment. Importance of patient-centered consultations and availability of written material or web sites for patient information should be stressed out.

Pain Assessment in Nonverbal Older Adults with Dementia (언어적 의사소통이 어려운 치매환자에서의 통증 사정)

  • This study was performed to evaluate the existing pain assessment methods including the tools developed for use with nonverbal older adults with dementia, and to suggest recommendations to clinicians based on the evaluations. Computerized literature searches published after year 2000 using databases - Google scholar, RISS, KoreaMed, Medline, ScienceDirect, CINAHL - were done. Searching keywords were 'pain', 'pain assessment', and 'cognitive impairment/dementia'. The pain assessments for non-communicative dementia patients who are unable to self-report their pains are often made using the assessment tools relying on the observation of behavioral indicators or alternatively the strategy of surrogate reporting. While several tools in English version and only one in Korean are suggested for the pain assessments based on the observation of behavioral indicators, none are commonly used. In this review, we selectively evaluated those tools known to show relatively higher degree of validity and reliability for nonverbal older adults with dementia, namely, CNPI, DOLOPLUS 2, PACSLAC, PAINAD, and DS-DAT. It is hoped that the present review of selected tools for assessing pain in those vulnerable population and the general recommendations given be useful for clinicians in their palliative care practice. And future studies should focus on enriching the validation of the useful tools used to observe the nonverbal patient's behavioral indicators for pain in Korean.

Development of Character Input System using Facial Muscle Signal and Minimum List Keyboard (안면근 신호를 이용한 최소 자판 문자 입력 시스템의 개발)

  • A person does communication between each other using language. But In the case of disabled person can not communication own idea to use writing and gesture. Therefore, In this paper, we embodied communication system using the facial muscle signals so that disabled person can do communication. Especially, After feature extraction of the EEG included facial muscle, it is converted the facial muscle into control signal, and then select character and communicate using a minimum list keyboard.

Analyses of Priorities in Contexts for the Script-based Speech Language Intervention by Age for 3~6 years Old Children in SLPs and Caregivers (3~6세 연령대별 언어장애아동의 언어중재 스크립트 상황에 대한 양육자와 언어재활사의 우선순위 분석)

  • The current study aimed to examine how the caregivers and speech language pathologists (SLPs) would perceive the important communication contexts for 3~6 year-old children in the script-based intervention. Analytic Hierarchy Process analyses were conducted for the obtained survey data from SLPs and caregivers. Results showed that caregivers perceived 'educational and social life' and SLPs perceived 'family life' as the most important contexts from among three high-level contexts. Within each high-level context, 'meal time' in 'family life', 'playing with friends' in 'educational and social life', and 'playground' in 'culture and leisure life' were the most important communication contexts. Also, from among 35 global priority rank, caregivers perceived 'playing with friends' while SLPs perceived 'meal time' as the most important contexts, and 'family life' was high priority among first five ranked context both in caregivers and SLPs. In caregiver groups by children's age, 3 year- and 6 year-caregiver groups perceived 'family life' and 4 year- and 5 year-caregiver groups perceived 'educational and social life' as the most important contexts. There were also slight differences between caregiver groups by age in the rank order of communication contexts across the high level contexts. These results suggest that SLPs may need to consider not only child's age and developmental level but also caregiver's demands in selecting the appropriate communication contexts when delivering the script-based speech language intervention.

Inclusive educational effectiveness through Metaverse for the disabled students and policy suggestions (장애학생 메타버스 교육의 포용적 공공소통적 효과성과 정책적 제언)

  • In the midst of going through a non-face-to-face society, most of human activities narrowed down to the platform, restrictions on external activities are bringing the internal scalability of digital technology. Metaverse is virtually shifting reality and increasing the possibility of utilization in various areas. However, researches linked to the educational effects of metaverse, especially students with disabilities, are still an unknown area that lacks exploration. This paper focuses on the fact that metaverse-education is widening educational fields that meets the various needs of disabled students to realize social good and inclusive education, and communication effects such as resolving barriers to interaction are prominent. As a research method, examining literature research papers linked to AR/VR, metaverse with communication skills, interviews, articles, and columns by experts, and policy suggestions and implications for the special education was conducted. Although the limitations of research are confirmed, significant results are found on inclusive education, which provides educational maximizing effects and realizing human rights through direct immersive experience reflecting the Cone of Experience Theory. Hopefully follow-up studies on meta-edu for disabled students will be carried out in the future, and various interdisciplinary discussions are needed to carefully observe inclusive policies and benefits so that the socially vulnerable are not excluded from technologies in ICT society.
