Medicolegal Problems in Pediatric Area (소아과 영역에서 의료분쟁)

  • Purpose : Medicolegal problems start when the patient asserts the mistake of doctor and doctor does not accept it. The purpose of this study is to assess the actual condition of medicolegal problems and to provide solutions of medicolegal problems in the pediatric field. Methods : There is not official statistical data about medicolegal problems in our country. We gathered data of legal insurance program of Korean Medical Association(KMA) and court cases and other fragmentary data. Results : Between 1981 and 1995, of total 2,338 cases reported to legal problem insurance program of KMA, most common ones were 748 cases of obstetrics and gynecology. Pediatric case was ranked at the 5th, 74 cases(3.1%). According to analysis of 41 medicolegal cases' after 1990, maltreatment of patient had the highest incidence of 14 cases, injection and medication were related to 12 cases, misdiagnosis was 9 cases, patient management were related to 4 cases, and others were 2 cases. The trial result of the medicolegal cases was that 31 cases were compensated, and 8 cases were defeated, and 2 cases were still in the process. Conclusion : The aspect of medical legal problem has the tendency of radicalism and systematization. This brings an economic destitution in the patient and gives damage to a doctor. In order to reduce medicolegal problem, doctor should offer a duty of explanation and efforts to his best to satisfy patient and endeavor to make an intimate doctor-patient relationship.

Factors Associated with Unmet Needs for Medical Care among Island Inhabitants in Korea (일개 섬 지역 주민의 미충족 의료와 관련 요인)

  • Objective:Korea has 3,170 islands with about 188,000 inhabitants. These inhabitants' needs for health services might go unmet because of geographic isolation, slimmer availability of health services, and higher proportion of the elderly compared with the mainland population. Unmet health service needs might result in serious health problems for these island residents. Therefore, the purpose of this study was to investigate their unmet health service needs and related factors from population of Nowha island. Methods: The survey was conducted from July 30 to August 1 by trained interviewers. Target population were residents in Nohwa island and the sampling method was incomplete quota sampling. General characteristics, socioeconomic status, utilization of health service, and unmet medical service needs were investigated. First univariate and then multivariate logistic analyses were done for the statistical analysis. Results: 324 residents were surveyed and the proportion of unmet health services needs was 26.5%. People living alone and of female sex had increased health services needs based on univariable analysis. From the multivariate analysis, those living alone increased the health services needs. Self-determined low socioeconomic status and medical aid increased the proportion of unmet medical needs, but these were not statistically significant. Conclusion:In Nowha island, those who are elder, and of lower socioeconomic status had much higher unmet medical needs. The results suggest the need for more social support by qualified health services to solve this unmet medical needs problem.

A Study on the Pattern of Medical Utilization and Factors Affecting Medical Utilization of Island Residents in Chungnam Province (일부 도서지역주민의 의료이용 행태와 이에 영향을 미치는 요인)

  • Objectives: This study was conducted to investigate the patterns of medical utilization and factors affecting medical utilization of population living in islands of province of ChungCheongNam-Do. Methods: Demographic and socioeconomic state, medical utilization of 447 persons were surveyed from Sept. 9th to Sept. 18th, 2002. Results: In elder age group and lower educational group, the rate of medical utilization was high. As subjective health states get lower, the group used medical institution more(p<0.05). The group having more number of chronic diseases showed more medical utilization. And the group who needs 2 to 3 hours to reach to the medical institution showed high medical utilization(p<0.05). The factors having negative effect on using medical institution were economic concern and lack of proper clinic in the neighborhood. Conclusions: The lower economic level and the insufficiency of medical facilities on islands affect the utilization rate negatively. More accessible and effective medical systems for these population are expected.

  • This study aims to provide an important base resource for the effective medical supply system construction by figuring out the characteristics and changes of admission medical use of gastritis in 160 middle treatment areas classified by 2008 patient examination resources, and by figuring out factors affecting on changes. As a result, in terms of admission rate according to sex and age standardization per small area, there were differences as EQ was 15.1 and CV was 0.4. Top 10 variations were mainly in rural areas. It showed significant positive relations between admission and the number of bed, doctor and special medical equipment per 10,000 population. It is recommended that the government set a priority in the management of regional bed supply and the distribution of efficient medical resources.

의사의 자질과 태도가 의료서비스 만족도에 미치는 영향

  • 본 연구는 소비자의 의료서비스 만족도에 영향을 미치는 중요한 요인들을 규명해 보고 그 관계를 살펴보고자 하였다. 특히 의료소비자가 인지하는 의료서비스의 만족도에 영향을 미치는 구성요소 가운데 의사, 간호사 등 의료인력 요소와 시설, 대기시간, 행정절차 등 비의료적 구성요소를 중심으로 만족도에 영향을 미치는 관련요인을 규명해 보았다. 본 연구에서 사용된 실증적 자료는 서울에 거주하는 사람으로 조사시점인 2007년 10월을 기점으로 지난 3개월 내에 의료서비스를 이용한 경험이 있는 사람을 대상으로 설문조사에 의해 수집되었고, 최종 543사례가 분석에 활용되었다. 의료서비스에 대한 만족도와 요인들간의 관련성을 파악하기 위해 상관관계 분석과 다중회귀분석을 실시하였고 특히 의료서비스의 질과 만족도에 직접적인 영향을 미치는 의료 인력의 자질과 태도와의 연관성에 초점을 두어 분석하였다. 분석결과, 소비자의 의료서비스 만족도에 영향을 미치는 핵심적인 관련 요인으로는 의료인력 구성요소로서 통계적으로 유의미한 영향을 미치는 것으로 나타났다. 특히 "의사실력", "의사의 자상한 설명과 친절", "의사의 믿음직함", 그리고 "의사가 환자의 얘기를 귀담아 들어줌"이라는 의사의 자질과 태도가 가장 중요한 요인이라고 할 수 있다. 이와 같은 의료서비스의 질과 만족도연구는 소비자의 의료서비스 만족도 정보로 활용하므로 의료서비스 공급자와 의료기관 종사자들의 책임의식 고취와 고객중심적 의료서비스 문화 정착에 도움이 되고 소비자들에게 좀 더 나은 의료서비스를 제공할 수 있도록 의료공급자들에게 동기를 부여해 준다는 점에서도 의의가 있을 것이다.

The Effect of Dentist's Attitudes on Patient Satisfaction (치과의사의 태도가 환자 만족에 미치는 영향)

  • This study amis to investigate the effects of dentist's attitudes on patient satisfaction. To achieve research purpose, we used 2020 Medical Service Experience Survey (MSES) carried out by Korea Institute for Health and Social Affairs(KIHASA). The data of 560 patients was analyzed using SPSS Ver. 26.0. The main results are as follows. According to the regression analysis, 'politeness', 'empathize with anxiety', 'enough conversation', and 'easy-to-understand explanation' are influential factors to patient satisfaction. This study is meaningful in that it provided basic data for improving the satisfaction of patients who visited the dentist and the service quality of dental medical institutions.

Effects of the Role Conflict, Ambiguity, and Job Satisfaction of the Health Professionals to Their Customer Orientation (의료종사자의 역할갈등, 모호성, 직무만족이 고객지향성에 미치는 영향)

  • Recently in the health service the globalization and opening their market to foreigners are realized in Korea. So, it becomes necessary for the health service institutes to execute their management to focus on service quality to satisfy their customers. We examined the customer orientation of the individual health professional rather than the market orientation concept based on the health institute. For the market orientation of the health institute should require the individual health professional worked there. The hypotheses for the effect of the role conflict, ambiguity, and job satisfaction of the health professionals to their customer orientation were established based on the literature review and previous researches and tested to the 250 health professionals such as health technician, nurse, radiologist, and management clerk in D hospital located in Busan, in summer 2008. First, the hypothesis 'The role conflict between the individual health professionals might effect to the customer orientation' was rejected. Second, the hypothesis 'The role ambiguity of the individual health professional might effect to the customer orientation' was accepted. Third, the hypothesis 'The satisfaction of the individual health professional might effect to the customer orientation' was rejected. and the multiple regression analysis was taken with the dependent variables as each construct of customer orientation-responsiveness, empathy, tangibility, and reliability, and independent variables as role conflict, role ambiguity, and job satisfaction. With these analysis, we found also the role ambiguity effected to the customer orientation.

Dental Utilization Associated Factors among Elderly (노인의 치과의료서비스 이용에 영향을 미치는 요인)

  • The purpose of this study was to examine the relationship of predisposing, enabling, need factors and oral health care factors to the dental utilization in 4,521 senior citizens based on the 5th National Health and Nutrition Examination Survey data. As for the relationship of predisposing, enabling and need factors to the necessity of denture, higher academic credential and higher income level that were respectively one of predisposing and need factors led to a higher necessity of denture, and the relationship of these factors was statistically significant. As to influential factors for their dental utilization in Model 1, there were significant differences according to gender, marital status and whether to subscribe to private health insurance or not. In Model 2, the need factors of Model 1 were adjusted. As a result, the respondents who didn't receive any unsatisfactory dental treatment made 1.35-fold more dental utilization, and the respondents who complained about mastication difficulty made 1.34-fold more dental utilization. There were significant differences according to gender, age, marital status, academic credential, whether to subscribe to private health insurance, unsatisfactory dental treatment experience and mastication difficulty. Age, unsatisfactory dental treatment experience and whether to complain about mastication difficulty or not made statistically significant differences to the dental utilization in Model 3 that involved oral health status. The above-mentioned findings illustrated that the predisposing factors, the enabling factors and the need factors exerted an influence on the elderly dental utilization. As there are a variety of factors to affect elderly dental utilization, its required to make an effort to boost the accessibility of the elderly to dental service in order to improve their oral health of the elderly.
