Ethical Considerations in Hospice and Palliative Care Research (호스피스 완화의료 연구에서의 윤리적 고려사항)

  • Along with the advances in medical technology and the economic development, more terminally ill patients are receiving hospice and palliative care services. Moreover, hospice and palliative care clinicians have been showing considerable interest in studies that aim to improve the quality of said care for patients and their families. Meanwhile, after the government has strengthened its policy to protect research participants, the institutional review boards (IRBs) are more closely examining various ethical issues related to patients' vulnerability when reviewing protocols for hospice and palliative care research. However, terminally ill patients should be provided with guaranteed qualities of hospice and palliative care to improve and maintain their quality of life. To that end, support should be provided for efforts to conduct ethical and safe studies with hospice and palliative care patients. Thus, this review paper proposes ethical guidelines for hospice and palliative care research. The guidelines could be appropriately used as a reference for researchers who should prepare for ethically safe and scientifically valued research protocols and the IRBs that will review the protocols.

Consciouess of Biomedical Ethics in Dental Hygiene Students and Non-Dental Hygiene Students (치위생계열 학생과 비 치위생계열 학생의 생명의료윤리 의식)

  • The purpose of this study was to provide basic data necessary for educating dental hygiene students by establishing a desirable sense of ethics values. In this descriptive research, 97 dental hygiene students and 100 non-dental hygiene students (in other health related fields) from Daegu Gyeongbuk City were selected. Tools used for data analysis were descriptive statistics, t-test, one-way ANOVA, Scheffe test and correlation analysis with SPSS/WIN 17.0 version. Average scores for consciousness of biomedical ethics in dental hygiene students was 2.87, and in non-dental hygiene students, 2.82. When differences in the consciousness of biomedical ethics of dental hygiene and non-dental hygiene students were compared, dental hygiene students had significantly higher scores for prenatal diagnosis of fetus, right to life of newborn, right to life of fetus, brain death, artificial abortion, but significantly lower for organ transplantation. Variables which influenced consciousness of biomedical ethics were etihcal values in the dental hygiene students and intent to attend a class in biomedical ethics, quantity and quality of experience of hearing for biomedical ethics and religion in the non-dental hygiene students. Continuing educational programs need to be considered and attention given to the significant variables that can promote consciousness of biomedical ethics in dental hygiene students.

Effects of Biomedical Ethics Education on Moral sensitivity, Critical thinking and Biomedical ethics Awareness of Nursing Students (생명의료 윤리교육이 간호학생의 도덕적 민감성, 비판적 사고성향, 생명의료 윤리의식에 미치는 효과)

  • The purpose of this study was to find the effect of systemic biomedical ethics education program on moral sensitivity, critical thinking disposition and consciousness of biomedical ethics nursing student. This study is a quasi-experimental research design and repeated measured ANOVA was used to analyzed the collected data statistically on this study. Data was collected from 1st September to 31 of December 2014. Nursing students and nurses in A and S cities participated in this study as subjects. 32 nursing students were assigned to an experimental group, and 33 nursing students were assigned to control group A, 26 nurses were assigned control group B, total subjects were 91. There are significant different in consciousness of biomedical ethics(F=3.432, p=.037). In the category of consciousness of biomedical ethics, there are significant different in 'right to life of fetus'(F=4.476, p=.014) and 'prenatal diagnosis of fetus'(F=6.207, p=.003), and 'right to life of newborn(F=6.207, p=.003) between experimental group and two control groups. The biomedical ethics education influenced the positive effect on moral sensitivity, critical thinking disposition and consciousness of biomedical ethics of nursing students, and shown the significant effect to enhance the consciousness of biomedical ethics for nursing students.

A Study on the Relationship between Nursing Ethics Education on Ethical Values, Nursing Professionalism, and Consciousness of Biomedical Ethics of Nursing Students (간호윤리 교육이 간호대학생의 윤리적 가치관, 간호전문직관 및 생명의료윤리의식에 미치는 관련성 연구)

  • The purpose of this study is to understand the effects of nursing ethics education on ethical values, nursing professional intuition and biomedical ethics of nursing students. Data collection period was from December 2, 2017 to December 31, 2017 and the data were collected from 186 nursing college students at 2 universities. Data were analyzed by t-tests, one-way ANOVA, Pearson's correlation coefficient analysis using the SPSS 21.0 program. The average score for the ethics values was 3.08±0.33 points. the average score of nursing professionalism was 3.22±0.84. The average score of consciousness of biomedical ethics was 2.86±0.22. It is necessary to develop case-centered textbooks in order to raise the competence to apply ethics management about biomedical issues in the course program of nursing ethics to improve the awareness of biomedical ethics for nursing college students.

The difference of biomedical ethics consciousness, related knowledge, awareness, and attitude of nursing college students according to the completion of biomedical ethics education (생명의료윤리 교육 이수에 따른 간호대학생의 생명의료윤리 의식, 관련 지식, 인식 및 태도의 차이)

  • This study is a descriptive evaluation of the differences in biomedical ethics consciousness, related knowledge, awareness, and attitude of nursing college students according to the completion of biomedical ethics education. The data were collected from 112 nursing students using self-report questionnaires and analyzed by t-test, Chi-Square test, Fisher's exact probability test and Pearson's correlation coefficient using SPSS 21.0. There were significant differences between the two groups in the sub-domains of biomedical ethics consciousness, artificial insemination, euthanasia and organ transplantation. The biomedical ethics consciousness was positively correlated with attitudes toward human tissue donation and transplantation, awareness of death, and awareness of hospice palliative care. The results of this study showed that after participating in the biomedical ethics education, nursing college students had statistically significant differences in the consciousness of artificial insemination, euthanasia, and organ transplantation. Moreover, the consciousness of biomedical ethics was related to attitude toward human tissue donation and transplantation and awareness of hospice palliative care. Based on the results of this study, we would like to suggest the need for development of various curricula related to death, hospice palliative care, human tissue donation and transplantation in the biomedical ethics curriculum. In addition, as the study was conducted in a single college, it is difficult to generalize the results; therefore, additional studies at different facilities are recommended.

The Influence of Biomedical Ethics and Brain-Death Organ Donation Education on Biomedical Ethics Awareness and Brain-Death Organ Donation Knowledge of Nursing students (생명의료윤리와 뇌사자 장기기증 교육이 간호대학생의 생명의료윤리 의식과 장기기증 지식에 미치는 영향)

  • This study is the quasi-experimental research of nonequivalent groups and is designed to measure and compare the awareness and knowledge of 2 groups of undergraduate nursing students - one group who received the education of biomedical ethics and brain-death organ donation and the other group who did not receive the education - regarding biomedical ethics and brain-death organ donation. We conducted this research for the 76 undergraduate nursing students of a certain university located in "C" city. Collected data were analyzed by Fischer's exact test and t-test, ANCOVA using SPSS 23.0 program. As result of the research, it was found that there was significant difference in the knowledge of brain-death organ donation between the above 2 groups (t=35.21, p=.000). In this regard, the education of biomedical ethics should be included in the curriculum of undergraduate nursing students in consideration of factors influencing the awareness of biomedical ethics.

Develop and Evaluate the Short Form Biomedical Ethics Tool for Medical Workers In Convergence Era (융합 시대의 의료종사자를 위한 단축형 생명의료윤리 도구 개발 및 평가)

  • The purpose of this study was to develop a short biomedical ethics tool for healthcare workers, compare it with pre-existing tool, and increase reliability. Data were collected from 211 healthcare workers working in G-do. Exploratory factor analysis was carried out using Varimax rotation extraction method in IBM SPSS WIN/21.0. Convergent validity of the tool was verified by regression and correlation analysis with original tool score. Reliability was verified by calculating intraclass correlation coefficient and internal consistency coefficient. The short, reduced 21 questions tool reflected 84% of pre-existing tool's biomedical ethics. Its reliability was higher than the 29 question tool for nursing students, but there were differences in the components of subdomains and reliability coefficient. Additional development of questions through qualitative research and interviews are needed to increase reliability of the subdomains. Measurement of biomedical ethics dilemma with the tool that has validity and reliability is needed, followed by replication studies.

The Influence of Biomedical Ethics and Critical Thinking Disposition on Nursing Professionalism in Nursing College Students (간호대학생의 생명의료윤리의식과 비판적 사고성향이 간호전문직관에 미치는 영향)

  • This study was attempted to find out the effect of biomedical ethics and critical thinking disposition of nursing college students on professionalism. For this study, from May to June 2021, 144 students were participated from S University in A City, using self reported questionnaire. Descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation analysis, and multiple regression analysis were used for data analysis. As results of this study, it was revealed that there was a significant positive correlation between the professionalism of nursing students on biomedical ethics and critical thinking disposition(r=.400, p<.001, r=.431, p<.001). Also, the factors affecting the professionalism of nursing students were grade(𝛽=0.303, p<.001), biomedical ethics(𝛽=0.287, p<.001) and critical thinking disposition(𝛽=0.294, p<.001) with a total explanatory power of 31.2%. As the results of this study, in order to increase the professionalism of nursing students, it is necessary to develop their biomedical ethics and critical thinking disposition. The specific strategies are to make and apply special programs by their grades.

Survey on Ethical Issues, Ethical Dilemma, and Needs for Ethics Education in Healthcare Providers (의료인의 윤리적 이슈, 윤리적 딜레마와 윤리교육 요구도 조사)

  • The purpose of this study is to provide basic data for developing future ethics education programs by surveying healthcare providers working at institutions of various sizes, using Ethical Issues in Clinical Practice tool. Data were collected from 149 healthcare providers working at medical institutions in G-do. Analysis was done using t-test, ANOVA, Pearson correlation coefficient, and Multiple regression with IBM SPSS WIN/21.0. Needs for ethics education was inversely correlated to end-of-life treatment issues (r=-.22, p=.007) and patient care issues (r=-.28, p<.001). The most influential factor on the subjects' needs for ethics education was patient care issues (β=-.246, p=.035), followed by Buddhism (β=.208, p=.010). The explanatory power was 13.4% (F=5.596, p<.001). Therefore, when organizing the contents of ethics education program, patient care issues should be included to meet the needs of the subjects. Also, a customized program regarding the subjects' religion is necessary.
