The purpose of this study is to provide ways to utilize and create valuable medical information utilizing Medical Big Data created by field in hospital information system. The results of this study first creates new medical information of Medical Information system through medical big data analysis and integration of created data of PACS linked with many kinds of testing equipment and medical image equipment along with medical treatment information. Medical information created in this way produces various health information for treatment and prevention of disease and infectious disease. Second, it creates profit statistics information in various ways by analyzing medical big data accumulated through integration of billings and receipt, admission breakdown of patients. Profit statistics information created in this way produces various administration information to be utilized in profit anaysis and operation of medical institution. Likewise, data integration of personal health history, medical information of public institutions, medical information created in hospital information system produces valuable medical health information utilizing medical data.

The purpose of this study was to examine the relationship between the awareness of health care workers on the protection of patient health information and their practice of it in an attempt to provide some information on the policy setting of health care institutions about medical information protection. As a result, the awareness of the health care workers on the protection of patient health information and their practice of it were both the best in the communication area, followed by the patient health information management area and the area of direct contact with health information. As for the variables linked to their awareness and practice of patient health information protection, the type of the health care institutions, job satisfaction, religion and the departments in which they worked were significantly related. To determine what factors affected the patient health information management area, a multiple regression analysis was carried out by selecting the area of direct contact with patient health information and the communication area as independent variables and by selecting the patient health information management area as a dependent variable. And it's found that the patient health information management area became better when the area of direct contact with the information and the communication area were better.

기존 의료정보 시스템은 본질적으로 성격이 다른 텍스트 기반 정보와 의료 영상 정보를 HL7과 DICOM이라는 각기 다른 두 표준을 기반으로 다루어 왔다. 따라서 상이한 두 표준 상호간의 정보 교환의 필요성과 최근들어 인터넷을 통한 개방 시스템으로의 전환의 필요성이 증가되면서 이를 위한 적절한 대안이 요구되고 있다. 본 논문에서는 이질적인 두 표준간의 데이터 교환과 통합을 위하여 XML을 활용하는 방안을 제시하였다. 또한 두 표준의 통합 DTD를 기반으로 XML 문서를 생성하고 통합 의료정보를 웹 상에서 검색, 저장할 수 있는 통합 의료정보 시스템을 구현하였다.

최근 의료데이터 표준화에 대한 중요성이 보건의료 빅데이터 구축과 맞물려 보건의료데이터 표준화와 마이데이터 생태계 조성을 추진하고 있다. 그리고 개인들의 휴대용 기기 이용증가와 모바일 환경으로 전반적인 디지털헬스의 패러다임 변화에 따라 HL7 FHIR의 사용이 점차 확대될 것으로 예측된다. 본 논문에서는 의료정보 표준인 HL7 FHIR와 의료영상 표준인 DICOM으로 환자 정보를 전달하기 위한 다중 의료 정보 중재 플랫폼에 대해서 기술한다. 이를 구현하기 위해 HL7 FHIR의 Patient, Observation, DiagnosticReport, Bundle 리소스를 활용하여 환자 정보와 임상 리포트 정보를 전달하여 StudyList에서 보여줄 수 있도록 구현하였다. 현재 구현된 내용은 FHIR 기반의 임상데이터로 의료영상을 포함한 표준화된 정보로 제공하여 마이데이터 실증 플랫폼으로 활용될 것으로 기대된다.

의료정보 표준화는 의료행위를 나타내는 용어의 표준화에서부터 진료기록의 형식 및 서식, 컴퓨터를 통하여 의로 정보들을 교환하는 방법, 이에 필요한 기자재 등을 약속된 형태로 표현하는 것을 말한다. 이와 같은 의료정보 표준화가 필요한 이유는 용어의 표현 및 그 사용범주를 모두 공통된 개념으로 받아들여야만 진료행위 및 이에 관련된 모든 업무에서 정확하고 유용한 정보의 교환이 가능할 수 있는 것이기 때문이다.

With a fast progress of medical information technology, there are many ongoing studies on utilization and integration of medical information in medical industries. But for now, the integration of medical information is a heavy burden to medical facilities since they have to change their systems in general. So the studies tend to introduce EMR system in a cloud base. The centralized system can have a lot of problems that exposure to health information in a cloud environment. This paper proposes a method for integration and authentication of medical information using broker agent in multi-cloud environment.

Ubiquitous health care system, which is one of the developing solution technologies of IT, BT and NT, could give us new medical environments in future. Implementing health information systems can be complex, expensive and frustrating. Healthcare professionals seeking to acquire or upgrade systems do not have convenient, reliable way of specifying a level of adherence to communication standards sufficient to achieve truly efficient interoperability. Great progress has been made in establishing such standards-DICOM, IHE and HL7, notably, are now highly advanced. In this paper, there are suggestions that provide a test bed, demonstration and specification of how standards such a IHE, HL7, ASTM can be used to provide an integrated environment.

국가 보건의료체계(health care system)가 올바르게 기능하기 위해서는 의료체계를 구성하는 각 요소들간의 유기적인 상호 작용이 대단히 중요하다. 정보체계는 이러한 유기적인 상호 작용을 위해 보건의료체계의 각 영역에서 꼭 필요한 시스템이다. 보건의료 서비스를 효율적으로 생산하기 위해서 투입하는 자원의 개발과 배치 체계는 물론 생산된 보건의료 서비스를 소비자가 효과적으로 이용할 수 있도록 하는 서비스 전달체계(delivery system)의 전과정에서 보건의료정보체계는 중요한 인프라가 되고 있다. 정보체계는 궁극적으로 보건의료체계가 지향하는 목적을 달성할 수 있도록 짜여져야 한다. 다시 말해, 의료체계의 각 하부영역에서 의료이용의 접근성(accessibility), 형평성(equity), 서비스의 의과학적 질(scientific technical quality), 포괄성(comprehensiveness) 등 보건의료체계가 추구하는 가치를 정보체계에서도 그대로 담아 낼 수 있어야 한다. 또한 정보체계를 구축하기 위한 요건으로 각 사업 영역별 표준화, 사업 영역간의 상호관계, 공공과 민간의 역할 분담, 정보 관련 시선과 인력, 정보기술 등의 인프라, 각 사업 영역별 정보화에서 담아야 할 정보 내용(contents, DB) 등에 대한 사전 기획이 수립되어야 한다. 그 동안 보건의료분야의 정보화사업은 1996년부터 보건소기관의 행정, 진료, 보건 업무지원을 위한 보건소정보화사업과, 1995년부터 초고속공공응용서비스 사업의 일환으로 추진해 온 전염병, 혈액유통, 장기이식, 외래진료예약, 원격진료, 응급의료, 원격치매진료 시스템 구축사업 등이 있으며, 응급의료, 혈액관리, 장기이식, 전염병정보, 외래진료예약시스템을 통합한 보건의료정보통합서비스시스템(1998년) 구축사업 등이 있었다. 이러한 사업들에 대해 현재 평가되고 있는 문제점들로는 정보화 사업에 대한 전략과 비젼이 부재한 상태에서 지나치게 정보기술 중심으로 추진되다 보니 보건의료 정보의 공유나 활용이 전혀 이루어지지 않거나 매우 제한적이었고, 시범사업의 경우는 초기 사업을 추진하기 위한 주체가 명확하지 않았고, 관련 인력의 훈련과 양성이 안 되었으며, 사업 확대를 위한 법적 기반의 취약, 정보화 사업이 부실한 데 대한 평가 기준과 대안 제시 등이 매우 취약한 것이 문제점으로 나타났다. 특히, 정보 내용과 DB 구축을 위한 표준화 등에 대한 사전 설계 부재는 개발된 정보시스템의 운영을 결정적으로 어렵게 만든 요인이 되고 있다. 전반적으로 기존의 정보화사업은 사업 추진을 위해서 꼭 필요한 비젼 수립, 비젼에 따른 목표 설정, 전략 개발, 관리체계 구축, 운영방안 수립 등 단계적인 정보화 계획이 부재한 것으로 평가되고 있어 보건의료정보체계 구축을 위한 종합 비젼 기획(ISP)이 시급한 실정이다.

의료부분 정보보호의 중요성이 높아감에 따라 의료정보보호 관리와 관련된 체계적인 방안이 필요하게 되었다. 이러한 체계적인 방안을 일반기업의 정보보호 관리체계(ISMS)와 역량성숙도모델통합(CMMI)을 통해 의료정보보호관리 기준, 의료정보보호관리 프로세스, 의료정보보호관리 프로세스 성숙수준으로 구성된 의료정보보호 관리체계를 제시하였다.
