  • Currently, medical data is basically confidential and difficult to access because it is protected under the Medical Protection Act. For the practical education of the students who are in the process of education, the researcher or the faculty members create and upload simulated data (chart of the question bank) close to the actual data. In this paper, the maintenance and repair easier on the basis of node.js and Ajax, mysql, jquery to a web-based research and enables users to easily approach the problem of the chart and the easy to the difficult access to patient contact respectively.

  • 본 연구는 서울 열린데이터 광장의 공공 데이터를 활용하여 의료 복지 서비스 지원 방안을 조사한다. '올해 의료비로 인한 가계 부담 정도' 데이터를 분석하여, 의료비 부담이 일반 가구에 미치는 영향을 깊이 있게 조사하였다. 이 데이터는 응답자들이 느끼는 의료비 부담의 다양한 정도(전혀 부담 안 됨, 부담 안 됨, 보통, 약간 부담됨, 매우 부담됨)를 포함하고 있으며, 이를 통해 지역별, 성별, 연령별, 소득별 의료비 부담 차이를 분석하였다. 연구 결과는 각 그룹별로 상이한 의료비 부담감을 보여주며, 이는 향후 의료 서비스 접근성 개선과 정책적 지원을 위한 근거 자료로 활용될 수 있다. 본 논문은 의료비 부담을 완화하기 위한 다양한 정책 제안을 포함하며, 공공 데이터를 활용한 의료 자원의 최적화를 목표로 한다. 이러한 분석은 보다 효율적인 의료복지 서비스 제공에 기여할 것으로 기대된다.

A Study of Patient's Privacy Protection in U-Healthcare (유헬스케어에서 환자의 프라이버시 보호 방안 연구)

  • On the strength of the rapid development and propagation of U-healthcare service, the service technologies are full of important changes. However, U-healthcare service has security problem that patient's biometric information can be easily exposed to the third party without service users' consent. This paper proposes a distributed model according authority and access level of hospital officials in order to safely access patients' private information in u-Healthcare Environment. Proposed model can both limit the access to patients' biometric information and keep safe system from DoS attack using time stamp. Also, it can prevent patients' data spill and privacy intrusion because the main server simultaneously controls hospital officials and the access by the access range of officials from each hospital.

  • 정보 통신과 의료 기술의 성장에 의해 의사는 이동 중 무선망을 통하여 인터넷 서비스에 접근이 가능해졌고 필요한 정보를 제공받을 수 있게 되었다. 또한 컴퓨터 그래픽의 기술은 통신 기술과 접목하여 무선네트워크를 이용한 의료 영상 전송이 가능해졌다. 그러나 기존 시스템들은 의사가 필요로 하는 의료정보를 저장하는 수준인 데이터 (의료 영상 정보) 중심의 의료 영상 전송 시스템 (Picture Archiving Communication System) 달이다. 유비쿼터스 환경에서의 의료 영상 전송 시스템은 의사의 현재 상황을 기반으로 한 서비스 전송을 요구 한다. 따라서 본 논문에서는 변화된 의사의 상황에 적합한 지능형 서비스를 제공하기위해 의사 중심의 의료 영상 정보 전송 미들웨어 (Doctor Centric PACS Middleware) 모델을 제안하다.

  • This paper suggested that medical image database construction technique that generated and recognized from variable medical device and professional medical experts for the formalization and pattern extraction from informal medical images. And then we transformed informal image characteristics to digital data, and generated the meaningful pattern matching informations. Through this experienced works, so many related researchers can easily access the medical images database and use this formalized image informations on the variable fields.

  • The characteristics of today's health care industry, based on the state of the art IT can be represented as a paradigm of human-oriented ubiquitous and accessible as possible by U-Health care. In addition, the healthcare industry is information and communication technologies (ICT) developments regarding the many advances and applications based on the research being carried out actively. Medical information system has been developed toward combining information systems of medical IT and it sets its sights on the fusion of developed IT and u-healthcare system. So changing distributed medical information systems into a safe PHR integrated system based on IaaS cloud computing is suggested in order to forge u-healthcare system with the times in this paper. Our experimental results show that our proposed system increased the data access time by about 24% and reduces the waiting time for processing service by about 4.3% over the web-based PHR.

Selective Encryption Scheme Based on Region of Interest for Medical Images (의료 영상을 위한 관심영역 기반 선택적 암호 기법)

  • For the patients' privacy, secure access control of medical images is essentially necessary. In this paper, two types of Region of Interest (ROI)-based selective encryption schemes are proposed, which concentrate on the security of crucial parts in medical images. The first scheme randomly inverts the most significant bits of ROI coefficients in several high frequency subbands in the transform domain, which only incurs little loss on compression efficiency. The second scheme employs a symmetric key encryption to encrypt selectively the ROI data in the final code-stream, which provides sufficient confidentiality. Both of two schemes are backward compatible so as to ensure a standard bitstream compliant decoder so the encrypted images can be reconstructed without any crash.

Prevalence and Risk Factors of Unmet Healthcare Needs among Korean Adults with Hypertension (한국인 고혈압 환자의 의료접근성 및 미 충족 의료실태와 위험요인 분석)

  • Purpose: The purposes of this secondary analysis study was to examine prevalence, risk factors and unmet healthcare needs among adults with hypertension. Methods: A sample of 3,386 adults over the age of 40 with hypertension were drawn from the Korea Health Panel Study (2013). Using SPSS 22.0 version, descriptive statistics including frequency, percentage, chi-square and logistic regression were performed. Results: Results showed that 18.9% of the sample reported unmet healthcare needs with the most frequently cited one was financial burdens (43.2%). The reported experiences of unmet healthcare needs differed by gender, marital status, vision or hearing impairment, memory problem, impaired mobility, subjective health status, total family income, depressive episode and the difficulty in making decisions. The sample participants were more likely to report unmet healthcare if they had vision impairment, low income and perception that their health status as moderate to poor. Those without vision impairment were less likely to report unmet healthcare needs. Conclusion: The identified risk factors of unmet healthcare needs should be addressed which would enhance access both to health care and to resolution of unmet healthcare needs. Since visual ability seems to impact perception of unmet healthcare needs, it may be useful to find ways to address this factor.

Service Quality of Dental Institutes Measured by SERVQUAL Model and Intention to Re-visit (SERVQUAL 모형으로 측정한 일부 치과의원의 서비스 질과 재이용의도)

  • The main purpose of this study was to identify the service quality of some dental clinics located in Gwangju and find an association between service quality and patient's intention to re-visit the same dental clinic. The search objects were 197 patients from five dental clinics, and data collected by self-administered questionnaire which was composed general characteristics, characteristics relating to visit of clinics, quality of service, patients satisfaction and intentions to re-visit. The used statistical analysis to find the association between quality of service and intention to re-visit was multiple regression analysis. This study looked at relations between quality of dental service, service satisfaction and intentions to visit again, and found that quality of service including six service areas had a statistically significant positive correlations with satisfaction and intentions of re-visit. When this study conducted a multiple regression analysis to identify variables that influence intentions of re-visit, it found that patients revisited the dental clinic only when quality of service was included. In conclusion, quality of service evaluated by patients influenced on their intention of re-visit and especially. Therefore, this study concludes that improvement in satisfaction and intentions of re-visit relies on Accessibility, that is, providing of medical information, consultant telephone, how delivery the dental medical system is. Interoperability Framework between GRID and PACS based on Web Services (웹서비스 기반의 Grid-PACS 상호운용성 프레임워크)

  • Recently the increasing collaborative research requires the remote medical and clinical data sharing and access of external institutions. In this paper, an interoperability framework between Grid and PACS using Web services is proposed and implemented in order to provide flexible and efficient medical data management. The Digital Imaging and Communications in Medicine(DICOM) standard defines medical image data exchange and transfer between PACSs and image databases. However, medical data exchange between hospitals is limited within the trusted and static environments. Moreover, DICOM does not provide medical data management and the Grid middleware does not include standard toolkit to access DICOM data. To address this issue, a Web services-based Grid Service Mediator (WGSM) which provides PACS integration and medical image data management is developed. The WGSM consists of several service mediators such as compress mediator, GridFTP mediator, RFT mediator, MyProxy mediator, MDS mediator, and RLS mediator and others. The proposed Web services-based framework provides user authentication and secure data access between PACSs in collaborative environments. In particular, the WGSM allows ordinary users to access remote PACS data in a simple and efficient manner without any the knowledge about underlying Grid middleware.
