The rapidly aging society entails the increases of medical service utilizations among the elderly. Medical services utilized by the elderly influence their life satisfaction. However, little is known about the effects of medical service utilizations on life satisfaction. Much less is known about whether the effects of medical service utilizations on life satisfaction tend to vary by the presence of chronic health conditions. Including 3,944 individuals aged 65 and over who participated in the 3rd wave of Korean Welfare Panel Study, the current study aims to examine (1) the relationships between medical service utilizations and life satisfaction and (2) whether the effects of medical service utilizations on life satisfaction vary by the presence of chronic health conditions. Data were processed through structural equation modeling(SEM) and multi-group SEM. Results indicate that (1) levels of both outpatient and inpatient service utilizations are related to life satisfaction, whereas levels of health monitoring service utilizations are not and (2) the effects of medical service utilizations varied by the chronic health condition status of the elderly. These results indicate that, in order to enhance levels of life satisfaction, medical services should be strategically utilized by the elderly depending on their chronic health condition status. Based on the findings, we discussed implications for practice and policy, suggesting future research directions based on the limitations of the current study.

The purpose of this study is to analyse the country image, the country attitude, the medical service attitude and the use intention about international medical service market in china and how these factors are related to one another. Data were collected from 609 chinese consumers who have known international medical service. We found the country image about international medical service institute has direct relation to the country attitude, the medical service attitude and use intention. And the country attitude influences the medical service attitude and use intention. However the medical service attitude of international medical service is not related to use intention. For a successful international medical service business, marketer should recognize that it is important to formulate the policy using the country image as well as the corporation image.

뉴욕시 정부의 이민자들에 대한 다각적인 접근과 시도에도 불구하고 의료 서비스 제공자들과 대상자간에 언어적 장벽과 문화적 차이로 인한 상호협조의 어려움은 계속되고있다. 본 연구는 뉴욕시 보건국 소속 다민족 다문화 계층의 의료수준 향상을 위한 노력의 일환으로 실시된 프로젝트 중 일부이다. 뉴욕시의 한국 이민자들이 뉴욕시에서 증가하는 추세를 감안할 때, 이들의 의료시설 이용 및 서비스 수혜에 대한 기초 조사를 실시하여 한국 이민자들의 문화적 배경과 그로 기인한 건강 신념을 알고자하는 시 정부 차원의 요구가 이 연구의 배경이다. 즉, 한국이민 사회의 독특한 특성을 뉴욕시 정부 관련 의료서비스 제공자들에게 그 이해를 높여서 한국 이민자들의 미국에서의 의료서비스 수혜의 기회를 높이고자 한 것이다. 기초 관련 연구가 전무한 한국 이민자들의 건강관련 이슈를 다루기 위해 미국 센서스와 병원 입퇴원 기록 현황에 나타난 한국인 혹은 아시안의 의료보장 실태 등을 고찰하였고, 한국이민사회의 주요 기관의 대표들과의 면담과 이민자들을 대상으로 한 직접 설문조사로 자료를 수집하여 분석한 결과는 다음과 같다. 한국 이민자들은 한국어를 주요 언어$(83\%)$로 사용하는 것으로 나타나 문화의 동화율이 낮았고, 낮은 문화 동화율은 의료 서비스 접근도를 낮추는 요인으로 나타났다. 또한, 의료 보험이 없는 것$(55\%)$이 가장 큰 요인으로 지적되었는데, 이는 미국 직장에 적응하기 어려운 이민생활로 자영업을 주로 하는 생활 양태가 비싼 사 보험(private health insurance)을 사지 못하는 요인과 관련되어있다고 본다. 따라서, 주로 이용하는 의료 서비스의 종류에도 많은 제한점이 있어서, $60\%$가 한국인 의사를 선호하며, $31\%$가 한국인 약사에게 건강관리를 의존하는 것으로 나타나 미국사회의 의료 서비스 접근도는 극히 취약한 것으로 드러났다. 의료서비스 접근을 막는 주요 장벽으로는 비싼 의료비 $(53\%)$ , 의사소통장애$(37\%)$로 나타났다. 보건의료 서비스를 위해 주로 이용하며 생활의 정보를 얻는 통로로는 한국어 신문$(69\%)$과 한국어 TV$(61\%)$, 한국어 라디오 $(57\%)$로 밝혀졌다. 결론적으로 한국 이민자들에게 좀더 나은 의료 서비스 수혜를 위해서는 문화 친밀도가 높은 의료환경 조성 및 의료 서비스 제공자들의 이해를 높이는 일 등과 함께 한국 이민자들이 의료보험을 살수 있도록 한국어로 된 의료 서비스 정보를 제공하는 등의 노력이 필요할 것으로 생각한다.

2014년 2월 정보통신 진흥 및 융합 활성화 등에 관한 특별법(ICT 특별법)이 시행되었고, 2014년 6월 의료법개정에 따른 원격의료서비스 시범사업이 가능해졌다. 본 연구에서는 기존의 병원을 방문하여 진료 후에 약 처방을 받던 일반적인 내시경 의료서비스와 병원 방문 없이 전화를 이용한 진료 및 처방과 함께 약을 배송해 주는 원격의료서비스를 의료서비스 제공자들을 대상으로 FGI(Focus Group Interview)를 통하여 비교하였다. 본 연구는 향후 보다 나은 내시경환자를 위한 원격의료서비스 설계를 위한 기초자료로 활용될 것으로 기대한다.

This study analyze long debate issues by the analysis of existing studies and the effect of private health insurance in the satisfaction of health service utilization. Then make developmental role of private health insurance. The analysis results of literatures, high-income earners are more subscriptions and the poor people in health status are excluded. Thus, enable private health insurance has the potential to lead the polarization of people. The medical use of private health insurance subscriber is more than non-subscriber and is likely to result in additional expenditure spending of public insurance. The contribution of private health insurance on improvement of the health option is clear. However, is not clear the contribution on health care quality improvement and health service customer satisfaction. The contribution on the national health care system efficiency of private health insurance is not clear. Private health insurance in the satisfaction of health service utilization is on effect. In short, supplementary private health insurance is desirable in our country.

The purpose of this study, is to identify the factors affecting reuse intention, determine the accurate demand of suppliers and consumers for qualitative dental care service, and identify the cyclic factors positively affecting one another. The data were analyzed by SPSSWIN 18.0, t-test and one way ANOVA, Pearson correlation and Stepwise multiple regression. Human service quality was significantly positively correlated with service value (r=0.609) and reuse intention (r=0.627) and service value was significantly positively correlated with reuse intention (r=0.645). The higher service value, the higher human service quality, the lower medical service discrimination, the higher level of reuse intention, which accounted for 56.9%. Therefore, it is necessary to make optimal maintenance of the system from the broad viewpoint of dental care in general and apply it in practice by developing good methods steadily.

본 연구는 소비자의 의료서비스 만족도에 영향을 미치는 중요한 요인들을 규명해 보고 그 관계를 살펴보고자 하였다. 특히 의료소비자가 인지하는 의료서비스의 만족도에 영향을 미치는 구성요소 가운데 의사, 간호사 등 의료인력 요소와 시설, 대기시간, 행정절차 등 비의료적 구성요소를 중심으로 만족도에 영향을 미치는 관련요인을 규명해 보았다. 본 연구에서 사용된 실증적 자료는 서울에 거주하는 사람으로 조사시점인 2007년 10월을 기점으로 지난 3개월 내에 의료서비스를 이용한 경험이 있는 사람을 대상으로 설문조사에 의해 수집되었고, 최종 543사례가 분석에 활용되었다. 의료서비스에 대한 만족도와 요인들간의 관련성을 파악하기 위해 상관관계 분석과 다중회귀분석을 실시하였고 특히 의료서비스의 질과 만족도에 직접적인 영향을 미치는 의료 인력의 자질과 태도와의 연관성에 초점을 두어 분석하였다. 분석결과, 소비자의 의료서비스 만족도에 영향을 미치는 핵심적인 관련 요인으로는 의료인력 구성요소로서 통계적으로 유의미한 영향을 미치는 것으로 나타났다. 특히 "의사실력", "의사의 자상한 설명과 친절", "의사의 믿음직함", 그리고 "의사가 환자의 얘기를 귀담아 들어줌"이라는 의사의 자질과 태도가 가장 중요한 요인이라고 할 수 있다. 이와 같은 의료서비스의 질과 만족도연구는 소비자의 의료서비스 만족도 정보로 활용하므로 의료서비스 공급자와 의료기관 종사자들의 책임의식 고취와 고객중심적 의료서비스 문화 정착에 도움이 되고 소비자들에게 좀 더 나은 의료서비스를 제공할 수 있도록 의료공급자들에게 동기를 부여해 준다는 점에서도 의의가 있을 것이다.

Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.

This study was conducted to provide with baseline data with the purpose of increasing the values of medical services. Self-administered questionnaire survey was conducted on 236 patients at a dental clinic follow-up visit in dentist clinic Gyeongnam area from June 2013 to August 2013. All statistical analyses were performed using SPSS. The motivation visiting the dental clinic is that the first one is introduction from the family and friends, the second is accessibility, the third is conspicuity and the last one is awareness of the dentist. The main variables in the process of treatment are service system, kindness, satisfaction of service, efficient of re-call system. The relief of discomfort at revisit show the highest score in the process of implant treatment and intention of revisit hereafter do in the prostheses process. In the correlation between main variables, service system and relief of discomfort at revisit (r=0.440, p<0.001), kindness and satisfaction of medical service (r=0.675, p<0.001), revisit hereafter and satisfaction of service (r=0.387, p<0.001) and efficiency of re-call system and revisit showed the highest correlation. The influence on satisfaction of dental service show meaningful level in kindness (p<0.001) and efficiency of re-call system (p<0.05). The intention of revisit is affected meaningfully by relief of uncomfort (p<0.05), service system (p<0.05), kindness (p<0.01) and efficiency of re-call system (p<0.01). In summary, the personal network of patients is most important variable at intention for revisit of dental clinic. As satisfaction of kindness and efficiency of re-call system is higher, satisfaction of medical service and intention for revisit are shown higher. Therefore further research for improvement of satisfaction for medical service and of intention of revisit at the dental clinic should be carried out.

This study was to find the factors influencing satisfaction of Korean medical tour services of Chinese college students. Structured questionnaire was developed to collect data. The data were collected from the 175 students between May 1 and May 15 in 2015 in an international college in China. The expectations and the evaluations on the Korean medical tour services were compare by t-test. To find the factors influencing satisfaction on Korean medical tour services, diverse linear regression models were estimated. According to the best fit regression model, technologies, quality of medical tour services, and health care cost significantly and positively influenced satisfaction on the Korean medical tour services. The results of the study suggested that we need to prepare marketing strategies to improve understandings on Korean medical tour services for the Chinese college students. Improvement of technology, improvement of quality of health service, and setting of reasonable price are important to attract more Chinese patients to Korea.
