Geographical Characteristics and Patients' Determinants of Online Referrals : A Case Study of Choongbook, Korea (온라인 협진에 대한 지리적 특성과 환자의 결정에 관한 연구 : 충청북도 사례를 중심으로)

  • This study employs qualitative approaches to examining geographical characteristics and patients' determinants of online referrals in terms of regionalization. In this light, I conducted interviews with 20 patients receiving online referrals in Choongbook, Korea, and investigated their behaviors regarding these referrals between July and August 2009. I found that many patients who suffered from various levels of illness preferred tertiary care centers outside of Choongbook and did not enjoy their experience with the local medical institutions as the online referral service sites. This result might be because patients choose online referrals for psychological considerations such as quality and level of health care services, personal stakes in online referral service sites, acceptability and credibility of good tertiary care centers, and easy access to and use of medical institutions. Meanwhile, immediate benefits with regard to the technological value of online referrals, such as convenience, utility, and original purpose associated with regionalization, did not influence patients' decision-making. Therefore, the social and public networks affiliated with online referrals plus the effect of Korean medical laws play hostage to private decisions made by citizens, who prefer high-level medical institutions. Accordingly, the technological contribution of online referrals does not halt the outflow of patients from local, tertiary care centers. Especially, the existing health care system and patients' behaviors are deeply related to referrals in the online system. To protect regionalization, the improvement of health care services from the present state of affairs is required.

  • Recently consumers pursue optimized health condition and high leveled quality of life through active promotion and prevention activity of health as the importance of health during daily life is emerged to meet the demand of aging society. Thus healthcare service is evolved into smart healthcare stage using various type of smart devices depending upon the needs of consumers. However the connectivity and interoperability among individual medical devices and smart devices are insufficient, thus data accumulation through individual medical devices is also insufficient. This thesis suggest the methodology to establish the connection among Bluetooth HDP and individual medical devices based on smart TV in addition to the monitoring of biometrics.

  • 의료 분야의 정보화 움직임으로 인해 단순한 정보저장과 검색시스템에서 벗어나 지능화된 서비스를 제공해주는 시멘틱 웹 기반의 의료 시스템이 요구되고 있다. 이에 한의학 분야도 한의사의 진단을 보조할 수 있는 지식기반시스템에 대한 요구가 증대되고 있다. 온톨로지는 시멘틱 웹의 핵심적인 지식체계로 지식의 처리와 추론이 가능하므로 질의 및 논리 추론을 통하여 진단을 내리는 한의학 지식 데이터베이스 구축에 적합하다. 따라서 본 연구에서는 한의학 진단의 기반이 되는 환자의 질병, 증상, 원인, 처방 등의 관계를 온톨로지로 구축하여 한의학 분야의 지식을 표현하기 위한 기본 지식 체계를 제시하고 구축된 한의학 온톨로지를 기반으로 한의학 진단보조시스템을 개발한다. 구축 된 시스템은 한의사 진단을 보조하여 줄뿐만 아니라 한의학의 진단 결과를 객관적으로 점검할 수 있게 해준다. 또한, 진단보조시스템과 온톨로지 관리시스템을 결합함으로써 컴퓨터 비전문가에 의한 한의학 온톨로지의 확장 및 수정이 가능하도록 한다.

An Exploratory Study on Quality of Care-Giving Service with Use of SERVQUAL -Targeting Inpatients of Long-Term Care Hospitals in Daejeon Region- (SERVQUAL을 이용한 간병서비스 질에 대한 탐색적 연구 - 대전지역 요양병원 입원환자를 대상으로 -)

  • The purpose of this study is ultimately to contribute to enhancing quality of patients' life, by grasping quality level of care-giving service that is recognized by inpatients of long-term care hospital, and by pursuing a plan for improving quality of care-giving service based on this. For this, targeting 500 patients who entered 40 long-term care hospitals in Daejeon Metropolitan City, the quality of service was multi-dimensionally grasped by using SERVQUAL scale. As a result of research, the quality of care-giving service, which is recognized by patients as survey subjects, tended to be relatively high with about 4 points as a whole out of 5-point perfection. By item, reliability was indicated to be the highest with 4.56 points. The next was surveyed to be in order of tangibility, empathy, responsiveness, and assurance.

  • 의료 분야의 정보화 움직임으로 인해 단순한 정보 저장과 검색 시스템에서 벗어나 지능화된 서비스를 제공 해주는 시멘틱 웹 기반의 의료 시스템이 요구되고 있다. 이에 한의학 분야도 한의사의 진단을 보조할 수 있는 지식 기반 시스템에 대한 요구가 증대되고 있으며 관련 시스템들이 개발되어지고 있다. 온톨로지는 시멘틱 웹의 핵심적인 지식 체계로 지식의 처리와 추론이 가능하므로 질의 및 논리 추론을 통하여 진단을 내리는 한의학 지식 데이터베이스 구축에 적합하다. 본 연구에서는 한의학 분야의 지식을 보다 의미적이고 체계적으로 표현하고 온톨로지를 이용한 검색 결과의 이점을 보여주기 위해 추론 기술을 접목시켜 한의학 온톨로지를 개발하였다.

  • Recently hospitals have divided into many divisions, specialized the medical service, and shown organic cooperation, all to provide patients with various and high quality medical service. They have also showed improvement in information protection by introducing an information protection system to regulate the access to patients' medical and personal information. The purpose of this paper is to present a case study to implement of a limited hospital medical information system that can regulate the access to medical information. For this, a router-based virtual network applying an ACL(: Access Control List) to regulate access to information was made using a packet tracer.

Research on Enhancing Reliability of IT Convergence Technology Applied Emergency Management Information System (IT 융합 기술을 적용한 응급관리 정보 시스템의 신뢰성 향상 방안에 관한 연구)

  • In a situation of crisis, an ambulance is dispatched according to the given emergency management information system(EMIS). However, the problem of past EMIS is that they did not concentrate on the reliable communication between the ambulance and medical specialists, thereby causing some needless casualties. This paper presents HEMIS, a highly-reliable emergency management information system, to solve such flaws. The effects showed that HEMIS was superior in data packet size, data safety and quality of service(QoS) to that of typical EMIS. Therefore, HEMIS is expected to increase the quality of medical service in ambulances, thereby decreasing casualties of patients carried in ambulances.

Factors Affecting Nursing Service Quality of Nurses at Local Medical Centers for COVID-19 Patients (COVID-19 환자를 간호한 지방의료원 간호사의 간호서비스 질 영향요인)

  • Purpose: This study aimed to analyze the effects of fatigue, resilience, and self-leadership on nursing service quality of local medical center nurses who nursed COVID-19 patients. Methods: The participants were 135 nurses who worked at regional public hospitals located in H-gun, G, and C-city in province C. The collected data were analyzed using descriptive statistics, t-test, analysis of variance (ANOVA), Pearson's correlation coefficients, and stepwise multiple regression using IBM SPSS Statistics version 25. Results: The participants' nursing service quality showed significant positive correlation with resilience (r=.53, p<.001), and self-leadership (r=.60, p<.001). The factors affecting participants' nursing service quality were commitment to self-leadership (β=.57, p<.001) and work position (chief nursing officer) (β=.26, p<.001), which explained 42% of the participants' nursing service quality. Conclusion: During a crisis such as the COVID-19 pandemic, it is necessary to help nurses enhance their self-leadership skills and build their career continuously by developing relevant policies, systems, and nursing intervention programs. Future studies could expand the knowledge base by including more participants to explore other ways to improve nursing service quality during the COVID-19 pandemic.

A Study on the Development of Satisfaction Index in the Medical Tourism of Foreign Tourist (외국인관광객의 의료관광 만족지표와 척도개발 연구)

  • The purpose of this study was a methodological research to develop an instrument to assess the medical tourism satisfaction of foreign tourist. A primary instrument with 63 questions was developed based on literature review. A group of experts revised individual questions and removed 9 irrelevant questions. This secondary instrument, then, was distriuted from Jury 1, 2012 to October 20, 2012. It was tested with 589 members of foreign tourist. After factor analysis on the final 42 questions, 7 factors were identified; quality of medical service, correspondance, tourism connection, sympathy, convenience, believability, and economical efficiency. The total variance explained was 60.9%. This instrument was statistically reliable and valid to measure medical tourism satisfaction of foreign tourist.
