A Study on Utilization of non-residential areal hospitals in Inpatient (입원의료의 타 지역 이용에 관한 연구)

  • Our study was carried out to suggest the way of improving the accessibility of medical service through identifying the factors that make patients be hospitalized in non-residential area not in their residential area. The subjects were 523,782 inpatients of the 2005 Patient Survey data. The 2004 Hospital Evaluation data, the 2005 census data which were obtained from the Korean National Statistics Office, and the 2006 Survey on National Healthcare Resources data were used. The data were analyzed using descriptive statistics, chi-square test and logistic regression in a SAS program. The most important factor was quality level of care of local hospitals. In the possibility of being hospitalized in non-residential areas, the region with the score of more than 9.5 per 100,000 people in the hospital evaluation was 8.3 times more than the region with the score of less than 9.5. However patients is hospitalized in the area with the hospitals with above 910 beds per 100,000 people more than in the area with the hospitals with less than 910 beds by 2.0 times. For the accessibility of medical services, government policy should focus on improving the quality of care in local hospitals not on increasing and distributing health care resources.

Evaluation on validity of health literacy measurement scale (의료정보이해능력 측정도구 척도의 타당성 평가)

  • As evaluating the health literacy is getting important, various measures for evaluation are being developed. Nevertheless, discussions about developing proper measures in Korean are still inactive. Therefore in this paper, we proposed Korean REALM (rapid estimate of adult literacy in medicine) measure that is composed of five point scale and investigated about its validity. As a result, we could find that Korean REALM measure which is composed of five point scale has high reliability, and that it formed one dimension as a result of factor analysis. Positive responses were lower than two point scale and correlation coefficient with NVS (the newest vital sign) appeared statistically significant. Therefore, we could conclude that Korean REALM measure that is composed of five point scale is a valid measurement. Furthermore, there were statistically significant differences between general students and department of nursing students about health literacy.

The Association between Patient Characteristics of Chungnam-do and External Medical Service Use Using Health Insurance Cohort DB 2.0 (건강보험 코호트 자료를 활용한 충청남도 지역 환자의 특성에 따른 관외 의료이용과의 연관성)

  • Background: The purpose of this study was to investigate the association between external medical service use and the characteristics of Chungcheongnam-do patients. We aimed to provide evidence of external medical service use enhance the healthcare delivery system in Chungcheongnam-do. Methods: We used the Health Insurance Cohort DB 2.0 of 2016-2019, and 2,570,439 patients were included in the study. Multivariate logistic regression and multinomial logistic regression were used to identify the association between external medical service use and each patient characteristic. Generalized linear model was used to identify the association between medical costs and external medical service use area. Results: During the study period, 32.2% of inpatients and 12.5% of outpatients had external medical service use in Chungcheongnam-do. In comparison to patients living in Cheonan and Asan, the odds ratio (OR) for external medical services use was higher across all regions. Specifically, hospitalized patients from Gyeryong, Nonsan, and Geumsan (OR, 116.817) and Gongju, Buyeo, and Cheongyang (OR, 72.931) demonstrated extremely high likelihood of external medical service use in the Daejeon area. Furthermore, compared to medical expenses incurred within Chungcheongnam-do, patients with external medical service use in the capitol area (outpatient=17.01%, inpatients=22.11%) and Daejeon area (outpatient=16.63%, inpatients=15.41%) spent more on healthcare services. Conclusion: This study found the evidence of external medical service use among Chungcheongnam-do patients. Further study should be conducted taking into account variables including satisfaction of local medical services, different types of patient diseases, and others. The study's findings may serve as a foundation for policy proposals aimed at ensuring the financial stability of our health insurance system, ensuring the efficient delivery of medical care, and localization of medical care.

The Effect of Sleep Duration on Obesity in Korean Adults (한국 성인남녀의 비만과 수면시간과의 연관성)

  • This study aims to evaluate the effect of sleep duration on obesity in Korean adult using data from the 8th Korea National Health And Nutrition Examination Survey in 2019. The study subjects were 5,213, and we performed frequency analysis, descriptive statistics, and multivariate regression analysis using the SPSS Win 27.0 program. The average sleep duration of the subjects was 6.8 hours per day, and the average body mass index (BMI) was 23.9 kg/m2. As a result of bivariate analysis of factors affecting sleep duration, age, education level, stress, depression, diabetes, hypertension, and arthritis were significantly associated with sleep duration. The body mass index (BMI) of the group with less than 6 hours of sleep duration was 24.4, which was higher than that of other groups, and there was a significant negative correlation between sleep duration and BMI (B=-0.247; 95% CI=-0.334~ -0.160). In conclusion, in order to prevent obesity in adults, various efforts from the country and local communities are needed to increase sleep duration.

A Study on the Outlook of Dental Hygiene Students on the Possible Countermeasure of Domestic Hospitals for the Opening of the Medical Market (의료시장 개방에 따른 국내병원 대응에 대한 치위생과 학생들의 견해에 관한 연구)

  • The purpose of this study was to examine the views of dental hygiene students about the possible countermeasure of domestic hospitals for the opening of the medical market. The subjects in this study were 269 dental hygiene sophomores, juniors and seniors at two different colleges in the region of Busan. The findings of the study were as follows: The opening of the medical market and views of possible countermove, whether they agreed to that or not made a statistically significant difference to their opinions on the necessity of customer-oriented marketing strategy(p=0.023), analysis of foreign medical markets/attempt to make inroads into the markets(p<0.000) and the improvement of the quality of medical services/the diversification of the services(p=0.025). As to an intention of going to a foreign hospital, they had a statistically significantly different intention about whether to go to a foreign hospital regardless of medical bills(p<0.000), whether to consult a doctor in a foreign hospital after going to a domestic hospital first (p<0.000), whether to consider the distance between their houses and a foreign hospital(p=0.05) and whether to take considerations on the assistance of an interpreter(p=0.023). In regard to preference for foreign hospitals, American hospitals ranked first(41.9), followed by Australian hospitals(19.9) and Canadian ones(14.2).

The Selection and Supplementation of Core Data for Injury Surveillance (손상감시를 위한 핵심데이터 선정과 보완)

  • The burden of injury is widely considered to be very severe in our society. Nonetheless, we don't have enough data for injury surveillance. The objective of this research is to select and supplement CORE DATA for injury surveillance. For this purpose, this study had analyzed the literature such as the Quality Assessment Report about 'Causes of Death Statistics', 'Health Insurance Statistics' and 'Hospital Discharge Injury Surveillance' according to the six dimension of Statistics Quality. The analysis result is that 'Cause of Death Statistics' and 'Health Insurance Statistics' have the usefulness as the CORE DATA for injury surveillance. But there is a significant shortcoming in the Health Insurance Statistics, which is that there is a lack of the data about the external causes of injury. For supplementing the defect, this study proposes the system that the medical institutions should obligatorily report the external causes of injury when claim National Health Insurance Medical Care Expenses. As the results of this system, we can expect 'Establishing of Injury pyramid', 'Data Connecting with the National Pension' and 'Improving the Promptness of Injury Data'. And we expect the follow-up study for the realization of this system.

A Nonparametric Stratified Test Based on the Jonckheere-Terpstra Trend Statistic (Jonckheere-Terpstra 추세 검정통계량에 근거한 비모수적 층화분석법)

  • Clinical trials are often carried out as multi-center studies because the patients enrolled for a trial study are very limited in one particular hospital. In these circumstances, the use of an ordinary Jonckheere (1954) and Terpstra (1952) test for testing trend among several independent treatment groups is invalid. We propose a the stratified Jonckheere-Terpstra test based on van Elteren (1960)'s stratified test of Wilcoxon (1945) statistics and an application of our method is demonstrated through example data. A simulation study compares the efficiency of stratified and unstratified Jonckheere-Terpstra trend tests.

A Convergence Study in the Severity-adjusted Mortality Ratio on inpatients with multiple chronic conditions (복합만성질환 입원환자의 중증도 보정 사망비에 대한 융복합 연구)

  • This study was to develop the predictive model for severity-adjusted mortality of inpatients with multiple chronic conditions and analyse the factors on the variation of hospital standardized mortality ratio(HSMR) to propose the plan to reduce the variation. We collect the data "Korean National Hospital Discharge In-depth Injury Survey" from 2008 to 2010 and select the final 110,700 objects of study who have chronic diseases for principal diagnosis and who are over the age of 30 with more than 2 chronic diseases including principal diagnosis. We designed a severity-adjusted mortality predictive model with using data-mining methods (logistic regression analysis, decision tree and neural network method). In this study, we used the predictive model for severity-adjusted mortality ratio by the decision tree using Elixhauser comorbidity index. As the result of the hospital standardized mortality ratio(HSMR) of inpatients with multiple chronic conditions, there were statistically significant differences in HSMR by the insurance type, bed number of hospital, and the location of hospital. We should find the method based on the result of this study to manage mortality ratio of inpatients with multiple chronic conditions efficiently as the national level. So we should make an effort to increase the quality of medical treatment for inpatients with multiple chronic diseases and to reduce growing medical expenses.

The Study on Factors Affecting Educational Need to Convergence Major of Dental Hygiene Students at K University (K대학교 치위생학과 학생의 융합전공 교육 요구도에 영향을 미치는 요인에 관한 연구)

  • Background: This study investigatedthe factors influencing the degree of education in the Department of Dental Hygiene at K University on the degree of education required for a convergence major related to the medical device industry. Methods: For 5 days from December 5, 2023, 257 students of the Department of Dental Hygiene at a university in Wonju Citywere surveyed, and an online survey was conducted. General characteristics, school satisfaction, satisfaction with majors, perceptions of multi-majors in universities, and the degree of demand for a convergence major related to the medical device industry were examined. Since normality was not satisfied, non-parametric analysis was performed. To confirm the differences in convergence major related to medical device industry education needs according to general characteristics, the Mann-Whitney U test and Kruskal-Wallis test were performed. Post-hoc analysis was performed, and the median, minimum, and maximum values were presented. Multiple regression analysis was performed to confirm the effect on the demand for a convergence major related to the medical device industry, and the statistical significance probability was set at 0.05. Results: As a result of examining the factors affecting the demand for a convergence major related to the medical device industry, it was found that the more experience one has in completing a double major in college (p<0.05) and the higher the degree of satisfaction with the major (p<0.05), the statistically significant need for an interdisciplinary major was higher(p<0.05).

The Effects of the Healthcare Accreditation on Hospital Employees' Satisfaction Level and Hospital Management Performance (의료기관 인증제도가 구성원 만족도와 병원운영 효과에 미치는 영향 : 비 인증병원과 인증병원 비교)

  • The purpose of this study was to grasp which effect the healthcare accreditation has on hospital employees' satisfaction level and hospital management performance by comparatively analyzing between accreditation hospital and non-accreditation hospital. As for difference in hospital employees' satisfaction level according to accreditation system, the incentive payment and pride of own task at the accreditation hospital and the hospital management effect were indicated to have positive(+) influence with statistically significant difference upon the item such as accuracy of task performance owing to the business standardization. It was indicated that hospital employees' satisfaction is high at the accreditation hospital and that the higher satisfaction level leads to having influence upon the effect of hospital management. What a country differentiates medical bill or supports medical equipment for medical institution that acquired accreditation is considered to be a plan for activation that can allow non-accreditation hospitals to be able to participate positively in the voluntary accreditation system.
