  • 본 연구는 화학사고 시 피해에 대한 응급의료정보를 신속, 적정히 제공, 관리하기 위하여 데이터베이스를 설계하며, 그 내용으로서 화학사고 응급의료 정보 및 대응 시나리오를 포함한 컨텐트를 제작하여 화학사고 응급 콜센터를 운영하게 될 때 사용 가능하게 하며, 직접 화학사고 응급 콜센터를 응급의료진이 직접 참여하고 관련 기관이 연계되어 시범 운영함으로써 그 결과를 환류, 수정 반영하여 응급의료 정보가 적절히 제공되는 지속적인 화학사고 응급 콜센터가 운영될 수 있는 기반을 마련하는 내용이며 이를 위하여 학사고 시 응급의료정보 제공이 필요한 화학물질의 도출의 적절한 방법에 대한 분석과 제안을 통하여 화학사고 응급 의료정보 제공시스템 개발 및 서비스 운영 기반 확보하는 기초 구조를 마련하고자 한다.

Policy suggestions to educate and produce outstanding paramedics in Korea (우수한 1급 응급구조사 인력 양성 및 배출을 위한 정책 제안)

  • Purpose: With the purpose of educating and producing outstanding paramedics by enhancing their competencies, this study aimed to make policy suggestions to re-establish the education system and improve the national examination and the certification scheme. Methods: This study used focus group interviews and questionnaires to collect data. Totally, there were 277 subjects, including experts from the education and field. Data were collected from September 9 to 20, 2016, and analyzed using SPSS 22.0. Results: To strengthen the curriculum of paramedics, this study suggested 27 courses with 94 credits as the standardized curriculum and derived 9 core competencies of paramedics. For the national examination, this study suggested consolidating written test subjects, adding scenario questions to practical tests, and applying critical criteria to simple practical tests that performs a procedure, grading these tests on a pass/fail basis. In addition, this study suggested converting certification into license, reflecting paramedics' healthcare job characteristics. Conclusion: The quality of emergency medical services in Korea will improve when those with core competencies that originated from the standardized curriculum based on the results of this study acquire their certification through the national test scheme, and the certification management system creates a virtuous cycle to further enhance paramedics' professionalism.

Perceptions about the Professional Ethics of EMT (응급구조사 직업윤리에 대한 인식조사)

  • Complex ethical issues of Emergency Medical Techinician (EMT) out-of hospital emergency medical scene and the ER (Emergency Room) behaviors were studied. The survey was conducted by 500 EMT group members working in the field of ambulance work and general hospital and it was about their work ethics, discussions and solutions about the transferred patients, and ethics regarding Do Not Attempt Resuscitate (DNAR). The survey includes work ethics, awareness about the target job, a discussion on the transfer of patients, measures, and deathbed. Discussions about the patient's condition and diagnosis results were majorly absent during patient transportation at the emergency care scene. More than 90% of emergency care transfer were inappropriate. Sometimes, EMT working in the field facing morally unethical problems beyond their responsibility. When EMT, who can not make death diagnosis, received deathbed related DNAR issues, they gone through severe ethical conflicts. The institutional support and therapy for EMT was weak. In Korea, especially in the accident site, ethical issues education is more needed than DNAR prevalence of education and guidance. If ethics training and guidance are given to EMT, a lot of moral errors in the field can be resolved.

Analysis of major indicators of departments of emergency medical technology in college through the university information disclosure system (대학정보공시를 통한 3년제 응급구조과의 주요 지표 분석)

  • Purpose: This study looked into the reality of the emergency medical technology department by analyzing the major indicators of university information disclosure systems and finding the competitiveness of the department of emergency medical technology by reviewing various evaluation indices. Methods: This study is a survey of 24 colleges with emergency medical technology and emergency medical technology departments across the country. Quantitative index data from 2017 to 2019 were collected and analyzed through the university information disclosure center web site. Results: Departments of emergency medical technology are generally higher than the target colleges in quantitative indicators, but the indices are somewhat insufficient in terms of "rate of faculty in full service" and "research performance of per one faculty in full service." Conclusion: Based on the results of this study, we recommend increasing the low indicators to enhance the competitiveness of the departments of emergency medical technology.

  • 환자의 긍정적인 예후에 있어 골든 타임을 최대한 확보하는 것은 중요한 요소로 손꼽힌다. 현재 응급의료 체계에서는 사고가 일어난 직후부터 응급 차량이 도착하기 전까지 응급구조요원이 환자의 상태를 파악 할 수 있는 방법은 제보자가 전달한 정보로 한정된다. 따라서 환자 상태의 수치적 측정의 부재 및 부족한 환자 정보는 골든 타임 확보에 있어 중요한 응급 처치를 결정하는데 큰 어려움을 야기한다. 이 같은 문제를 해결하기 위해 본 논문은 사고 직후부터 응급 차량이 도착하기 전까지 응급 구조 요원이 도착하기 전까지 환자의 생체 정후를 파악할 수 있도록 하여 적절한 응급 처치를 예측할 수 있도록 돕는다 또한 환자 주변에 전문지식을 보유한 사람이 없는 경우에도 환자의 상태에 따른 최소한의 조치를 취할 수 있도록 도움을 줌으로써 응급 차량이 도착하기 전까지 환자가 최상의 상태를 유지할 수 있도록 한다.

Convergent responses and impacts of verbal abuse during clinical training practice of Paramedic students (병원(임상) 실습 시 응급구조학과 학생이 경험하는 언어폭력에 대한 융합적 반응 및 영향)

  • This study was conducted to investigate experiences, convergent responses and expected negative impacts of verbal abuse of 253 paramedic students when they were doing clinical training practice. The results were as follows. First, 34%(86 people) experienced verbal abuse, the most frequently verbal abuse was done by Patients and Patient's family. Second, the most frequently contents of verbal abuse by Patient and Patient's family, Paramedic, Doctor and Nurse was 'Talking down or rude talk to me'. Third, the most frequently emotional responses of the students was 'Angry' and coping was 'Tolerate'. Fourth, the most frequently expected negative impacts was 'Practice productivity will be compromised'. Fifth, when experiencing verbal abuse by Paramedic was correlated with emotional responses(r=.458 p=.000), emotional responses was correlated with stress and expected negative impacts(r=.553 p=.000, r=.540 p=.000). In conclusion, it will be necessary to provide preventive education on verbal abuse before clinical training practice and to take active measures that have been protocolized.

Measuring rescuer's fatigue by evaluating varying sized groups of rescuers performing chest compressions on a manikin study for suspected COVID-19 patients (COVID-19 대응 심정지에서 가슴압박 교대 인원에 따른 구조자의 피로도 -마네킹 연구-)

  • Purpose: The aim of the study is to measure the quality of cardiopulmonary resuscitation (CPR) and the fatigue of rescuers wearing PPE (Level D) during a CPR session and to ultimately provide suggestions of safety standards for rescuers. Methods: 36 subjects were enrolled in the study. The subjects were divided randomly into three groups of two-members, three-members, and four-members. Each group performed CPR for 30 minutes. Blood lactate concentration, heart rate, rating of perceived exertion, chest compression depth and rate were measured before experiment and after each cycle. Results: There was a difference in the blood lactate concentration during CPR cycle by member of shifts (p=.014). The blood lactate concentration increased during CPR (p=.000). Subjective fatigue was a significant difference of chest compression in cycles 3, 4, and 5 for the member of shifts during CPR (p=.049, p=.009, p=.015). Depth and rate of chest compression were not different for the member of shifts during CPR. Conclusion: It is necessary to establish standards for the member of shifts during CPR, to reduce the fatigue of rescuers.

A comparison of ventilation effects using bag-valve-mask assist device (백-밸브-마스크 보조장치를 이용한 환기효과 비교)

  • Purpose: TThe experiment was designed to compare the efficiency of ventilation between conventional BVM ventilation and a newly devised A-BVM ventilation method with Tidal volume, total ventilation rate, average Ventilation speed, and average Ventilation volume. Methods: 40 Paramedical students who agreed to participate in the study were analyzed. Values were measured using IMB PASS after 2 minutes of Brayden Pro manikin with BVM and A-BVM ventilation. The difference in general characteristics was assessed by t-test and ANOVA and the difference in ventilation methods was analyzed by IBM SPSS. Results: A significant difference was found between the two ventilation methods in terms of tidal volume (t=-11.203, p<.001), ventilation time (t=-3.834, p<.001), and optimum ventilation probability (t=10.770, p<.001). A-BVM ventilation method, rather than BVM ventilation method, showed a value close to the appropriate amount recommended by Korean Advanced Life Support (500~600mL) in tidal volume, and higher in optimum ventilation probability. Conclusion: We could identify the a better mode of ventilation. Further studies on the efficacy of existing BVM ventilation methods as compared to device augmented BVM ventilation methods should be carried out to ensure that adequate ventilation is available to patients in clinical practice.

  • This study was conducted to find out the relationship between knowledge and attitudes about COVID-19 and infection control performance among college students in paramedicine. The subjects of the study were 154 university students attending a 3-4 year paramedicine located in Gwangju Metropolitan City, Jeollabuk-do Province, and the collected data were frequency, percentage, sum, average and It was analyzed by standard deviation, t-test, ANOVA(Scheffe), Pearson's Correlation Coefficient, and Multiple Linear Regression. The order of attitude and knowledge about COVID-19 affected the infection control performance of college students in the paramedicine. Therefore, it is necessary to develop and apply regular or non-regular curriculum and multidisciplinary curriculum that can improve knowledge and attitudes about COVID-19 in order to improve the infection control performance of paramedic students.

A study on university students' preparedness for disaster, disaster response competency, and willingness to participate in disaster response (대학생들의 재난준비도, 재난대응역량 및 재난대응 참여의지에 대한 연구)

  • Purpose: We aimed to identify disaster preparedness, disaster response competency, and willingness to participate in disaster response among university students. Methods: Data were collected from the paramedic students after consent to this study through online. The period of data collection was from June 13 to August 3, 2023 and 207 questionnaires were analyzed using SPSS 27.0 program. Results: The subject's disaster preparedness was 7.67±3.10 out of 15, disaster response competency was 2.79±0.69 out of 5, and willingness to participate in disaster response was 6.06±4.59 out of 15. There was a significant difference in disaster preparedness depending on the disaster education experience (p<.001), in disaster response competency depending on the number of courses completed (p<.05), in willingness to participate in disaster response in cases of disaster experience (p<.05) and number of times disaster education was completed (F=3.146, p=.047). In particular, if the number of disaster training courses completed was three or more, disaster response competency and willingness to participate in disaster response were significantly high. There was a significant positive correlation between the subjects' disaster preparedness and disaster response competency (r=.655, p<.001). Conclusion: Three or more sessions of continuous disaster education is important, it will be necessary to develop educational content that matches the characteristics of the target subjects or college.
