일 대학병원 응급실에 내원한 암 환자 실태 (A Survey of Cancer Patients Who Visited Emergency Room)

  • 목적: 본 연구는 응급실을 내원하는 암 환자의 실태를 파악하기 위한 후향적 조사연구이다. 방법: 2006년 1월부터 2006년 12월까지 C 대학교병원 응급실에 내원한 종양내과 암 환자 564명의 응급실 의료정보지를 분석하였다. 실태 조사서는 암 병동 근무경력 6년 이상의 간호사 4인과 종양내과 교수 1인 및 간호대학 교수 1인에게 내용 타당도를 검증받아 사용하였으며, 수집된 자료는 SAS (ver 9.1)를 이용하여 빈도와 백분율로 분석하였다. 결과: 대상자의 질병관련 특성은 위장관계 암이 28.9%로 가장 많았고, 병기는 4기가 66.9%로 가장 많았다. 응급실 내원 전에 최근 받은 치료는 항암화학요법이 51.6%이었고, 항암화학요법 치료 횟수는 1~5회가 82.5%로 가장 많았으며, 최근 치료받은 장소는 입원 병동이 52.4%로 가장 많았다. 대상자의 응급실 내원관련 특성은 항암화학요법 치료 후 내원까지 2주 이하인 경우가 62.9%로 가장 많았고, 증상 발현에서 내원까지의 기간이 1일 이내가 34.9%, 응급실 내원 후 입원까지 1일이내가 71.8%로 가장 많았다. 내원요일은 평일이 73.2%로 가장 많았고, 응급실 내원 후 입원한 경우가 77.2%로 가장 많았다. 응급실을 내원 시 주요증상은 통증이 34.3%로 가장 많았고, 위 장관계 증상이 30.0%, 호흡기계 증상이 21.5%, 고열이 16.3% 순이었다. 응급실에 내원한 주요증상은 위장관계 암과 췌장 간 담도암은 통증, 림프종은 고열, 폐암은 호흡기계 증상, 두경부암이나 육종, 유방암을 포함한 기타 암은 위장관계 증상이 가장 많았다. 결론: 이상의 결과를 볼 때 암 환자들이 응급실을 내원하는 주요증상은 질환에 따라 차이가 있기 때문에 대상자의 특성에 따라 맞춤형 프로그램을 개발하는 것이 필요하다. 특히 3기 이상의 진행암 환자들이 응급실을 내원하지 않고 총체적인 의료서비스를 받을 수 있는 제도를 마련하는 것이 시급하다. 또한 암 환자들이 병원과 가정에서 적극적으로 통증 관리를 연계해서 받을 수있는 체계를 마련하고, 위장관계 암과 폐암 환자들이 퇴원 후 응급증상을 효율적으로 관리할 수 있는 맞춤형 교육자료를 개발해야 할 필요가 있다고 생각한다.

융복합 시대의 간호사의 감정노동, 직무스트레스 및 소진이 신체화 증상에 미치는 영향 (Effects of emotional labor, job stress and burnout on somatization in nurses : In convergence era)

  • 본 연구는 융복합 시대를 살아가는 간호사의 감정노동, 직무스트레스 및 소진이 신체화 증상에 영향을 미치는 요인을 규명하기 위해 실시되었다. 연구대상자는 D대학병원에 근무하는 간호사 총 387명을 대상으로 실시되었고, 연구기간은 2014년 4월 1일부터 4월 30일까지이었다. 간호사의 감정노동은 3.25점, 직무스트레스 3.60점, 소진 3.10점으로 중등도 이상의 점수를 보여주었고, 신체화 증상은 2.18점으로 나타났다. 신체화 증상에 영향을 미치는 일반적인 특성으로는 연령이 낮고, 배우자가 없는 경우, 교육수준이 낮을수록, 근무부서가 외과계와 응급실에 근무하는 경우, 총 임상경력이 낮을수록, 현 부서경력이 낮을수록, 일반간호사인 경우, 월 급여가 작을수록, 3교대 근무인 경우, 간호직에 대한 만족도가 낮을수록 신체화 증상 정도가 높았다. 또한 신체화 증상은 감정노동, 직무스트레스, 소진과 정적인 상관관계를 보여주었다. 신체화 증상을 설명하는 주요 변수는 소진으로 나타났으며, 신체화 증상을 29.6% 설명하였다. 따라서 간호사의 신체화 증상을 예방하고 감소하기 위해서는 소진을 최소화시킬 수 있는 방안이 마련, 적용되어야 할 것이다.

응급실 간호사의 죽음에 대한 인식, 영적안녕이 삶의 질에 미치는 영향 (Effects of Perception of Death and Spiritual Well-Being on Quality of Life among Nurses in Emergency Room)

  • Purpose: The purpose of this study was to identify factors determining the quality of life in nurses in emergency rooms. Methods: Subjects were 131 nurses working in emergency rooms in B metropolitan city, Y and J city. Data were collected from June 5 to July 10, 2015 using structured self-report questionnaires. Data were analyzed using t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, and stepwise multiple regression with SPSS/WIN 21.0 program. Results: Existential well-being (${\beta}$=.59) of nurses in emergency rooms and gender (${\beta}$=.19) were found to be significant predictors of quality of life in nurses in emergency rooms. Conclusion: The results suggest that it is needed to improve existential well-being of nurses in emergency rooms. Ultimately, holistic spiritual program for nurses should be developed the quality of life in nurses in emergency rooms.

군집분석을 이용한 응급실 간호사-환자 상호작용 유형 및 환자의 상호작용 만족도 (Nurse-Patient Interaction Patterns and Patient Satisfaction in the Emergency Department)

  • Purpose: The purpose of this study was to identify effective nurse interaction patterns with patients in the emergency department. Methods: For this study, video technology was used to record complete conversations between the nurse and patient. The participants were 28 nurses and 63 patients in the emergency department at one university hospital located in Seoul. The data were collected from November, 2002 to April, 2003. The video recordings were observed for 4 hr for each case and coded using an adapted version of Roter's Interaction Analysis System (RIAS). The data were analyzed using cluster analysis to identify the patterns of nurse-patient interaction. Results: Cluster analysis revealed 4 distinct nurse interaction patterns; 1) "closed" characterized by orientation and negative talk, 2) "positive" characterized by positive affective talk, 3) "informative and directing" characterized by task-focused behavior including data gathering, and giving information about medical condition and treatment, 4) "facilitative" characterized by balance of psychosocial and biomedical topics. Patient satisfaction was highest in the facilitative interaction pattern. Conclusion: The patient centered interaction pattern, balancing information exchange and psychosocial exchange are the most effective interactions in the emergency department, suggesting that effective interaction skill is a core clinical nursing intervention in acute care.

응급실 간호사의 역할갈등, 자기효능감, 회복탄력성이 간호업무 수행능력에 미치는 영향 (Impact of Role Conflict, Self-efficacy, and Resilience on Nursing Task Performance of Emergency Department Nurses)

  • Purpose: This study aimed to identify the effect of role conflict, self-efficacy, and resilience on the nursing task performance of emergency department (ED) nurses. Methods: Data were collected from 140 ED nurses working in 6 general hospitals, using self-reporting questionnaires. Data were analyzed using SPSS 23.0, and the analyses included descriptive statistics, a t-test, an ANOVA, the Pearson's correlation coefficient test, and a stepwise multiple regression. Results: Nursing task performance differed significantly in terms of the following general characteristics of the participants: age, marital status, total clinical experience, clinical experience in the ED, and position at the hospital. Nursing task performance was positively correlated with role conflict, self-efficacy, and resilience. Self-efficacy, role conflict, clinical experience in the ED, and resilience were significant predictors of nursing performance, and they accounted for 36.9% of the variance. Conclusion: Self-efficacy was identified as the most significant factor affecting the nursing task performance of ED nurses. Therefore, it is necessary to develop programs to improve the self-efficacy of ED nurses.

응급실 간호사의 언어폭력경험과 공감피로 및 직무열의의 관계 (The Relationship between Experience of Verbal Abuse, Compassion Fatigue, and Work Engagement in Emergency Nurses)

  • Purpose: This study is a descriptive research study to identify the relationship between the experience of verbal abuse, compassion fatigue, and work engagement among nurses in the emergency unit. Methods: Study subjects were 107 nurses working in a university hospital in Gyeonggi-Do. The instruments used were a verbal abuse checklist, a subscale of the Professional Quality of Life Scale used to measure compassion fatigue, and the Utrecht Work Engagement Scale. The data were analyzed using the IBM SPSS version 22.0 program for independent t-test, one-way ANOVA, and Pearson's correlation coefficients. Results: Emergency nurses experienced more verbal abuse and compassion fatigue than nurses in other departments, and the level of work engagement was low. There was a positive correlation between experience of verbal abuse and compassion fatigue and a negative correlation between both of these variables and work engagement. Particularly, verbal abuse appeared in the order of nurses, patients & caregivers, and doctors. This means there were serious problems with communication with nurses. Conclusion: In order to reduce verbal abuse, it is necessary to establish a communication culture of mutual respect between nurses, medical staff, patients & caregivers. Moreover, a compassion fatigue management program is needed to reduce emotional fatigue.

응급실 간호사의 환자안전 위험요인에 대한 위험성 인식과 안전 간호활동 (Perception of the Patient Safety Risk Factors and Safety Management by Nurses in Emergency Service, Hospitals)

  • Purpose: This was a descriptive research study to examine the patient safety risk factors and the level of safety management of nurses in emergency service, hospitals and to analyze the relationship between the two factors. Method: Data for analysis were collected from 232 nurses in emergency service, hospitals in Busan and Gyeongnam from July 30 to September 7, 2013. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, and Pearson correlation coefficients. Results: Therapeutic agents showed the highest risk level. The prevention of transfusion errors showed the highest performance. As the nurses were working in regional emergency medical centers and received education more than 7 sessions on patient safety, they readily recognized the riskiness of the safety risk factors. In addition, as the nurses were older than 40, married, having more education about safety and understood the incident report registration system well, they performed safety management better. There were significant correlations between perception of the patient safety risk factors and performance for safety management. Conclusion: Nurses in emergency service, hospitals should try to improve safety management to reduce the risk factors shown to be higher based on the results and ensure the patient safety.

응급실 간호사의 자아탄력성, 셀프리더십, 스트레스 대처방식이 직무만족에 미치는 영향 (Impact of Ego-resilience, Self-leadership and Stress Coping on Job Satisfaction in Emergency Department (ED) Nurses)

  • Purpose: The purpose of this study was to identify ego-resilience, self-leadership, stress coping which might influence job satisfaction in ED nurses. Methods: Data were collected by self-reporting questionnaires from 123 nurses in 10 EDs. Data analysis was done with SPSS/WIN 18.0 program. Results: Job satisfaction showed statistically significant differences depending on age, academic background, religion, clinical career, clinical career in emergency department, position, sleeping hour and reason for working. Job satisfaction showed significantly positive correlations with ego-resilience (r=.70, p<.001), self-leadership (r=.46, p<.001) and stress coping (r=.57, p<.001). Ego-resilience (${\beta}$=.54), sleeping hours over 8 hours (${\beta}$=.17) and reason for working (${\beta}$=.17) were significant predictors and accounted for 56.6%. Conclusion: Based on these results, it is needed to develop a program in order to improve ego-resilience of ED nurses.

응급실 간호사의 법의간호학적 역할 수행에 미치는 영향요인 (Influencing Factors on Forensic Nursing Role Performance of Emergency Department Nurses)

  • Purpose: Purpose of this study was to identify the factors influencing forensic nursing performance role of Emergency Department(ED) nurses. Methods: The study was conducted with 120 ED nurses from Busan. Data were collected during July, 2013 and analyzed using SPSS Win 19.0 program. Results: There were positive correlations between awareness of the forensic nursing role and forensic interest (r=.29, p=.001), between awareness of the forensic nursing role and forensic nursing performance role (r=.54, p<.001), and between forensic experience and forensic nursing performance role (r=.26, p=.004). Awareness of forensic nursing role (${\beta}=.50$, p<.001) and experience in forensics (${\beta}=.24$, p<.001) were significant predictors of forensic nursing role performance and explained 35.9% of the variance. Conclusion: To improve forensic experience, medical institutions should provide education and training for ED nurses to increase their indirect experience. Periodic assessment should also be done. Nurses with training assigned to the ED will enhance the forensic nursing role performance.

응급실 간호사의 업무환경, 직무만족도 및 이직의도 간의 관계 (Relationships among Work Environment, Job Satisfaction and Turnover Intention of Nurses in an Emergency Department)

  • Purpose: The purpose of this study is to identify relationships among work environment, job satisfaction and turnover intention of emergency department (ED) nurses. Methods: The survey was given to 220 ED nurses which are located in G metropolitan from May 1st to July 31st of 2012. The results were analyzed by descriptive statistics, t-test, ANOVA, pearson correlation coefficients and stepwise multiple regression using SPSS WIN 19.0 program. Results: The turnover intension according to the general characteristics of the subjects has significant difference between emergency room experience, current duty satisfaction, and turnover plan in 1 year. In the correlation between work environment, job satisfaction and turnover intention of the subjects, work environment and job satisfaction have positive correlation, job satisfaction and turnover intention either job satisfaction and turnover intention have a negative correlation. Factors influencing turnover intention are job satisfaction, career and current duty satisfaction, also the full explanatory power of the model was 24.4%. Conclusion: The results indicate factors that influence turnover intention are job satisfaction, career, and current job satisfaction. We hope these result help to provide effective intervention strategies for reducing ED nurses' turnover.
