  • 기존의 병리 영상을 판독하고 저장, 관리 하는 시스템이 수작업으로 이루어져 발생하는 문제점들을 보완하는 방안으로 유방종양 영상을 사용하여 세포영상 내용기반 검색 시스템을 구축 하고자 한다. 유방암 세포를 사용하여 효율적인 내용기반 영상 검색 시스템을 구축하기 위해서는 유방암 영상에서 검색에 가장 적합한 영상의 질감, 칼라, 형태학적 특징값의 조합이 필요하다. 따라서 본 논문에서는 세포영상의 분류에 많이 사용되는 질감 특징과 칼라값을 사용하여 내용기반 검색 시스템을 구축 하였으며, 칼라값과 질감특징값을 사용하여 검색했을 때의 효율성을 비교하였다. 향후 이런한 실험들을 통하여 세포영상검색에서 가장 최적의 파라미터들을 조합한 내용기반 검색 시스템을 구축하고자 한다.

  • In Korea, the number of institutions providing breast MRI, as well as the number of breast MRIs, has recently increased. However, MRI-guided procedures, including biopsy and needle localization, are rarely performed compared to ultrasound-guided or stereotactic biopsy. As breast MRI has high sensitivity but limited specificity, lesions detected only on MRI require pathologic confirmation through MRI-guided biopsy or surgical excision with MRI-guided needle localization. Thus, we aimed to review MRI-guided procedures, including their indications, techniques, procedural considerations, and limitations.

Incidental Breast Lesions on Chest CT: Clinical Significance and Differential Features Requiring Referral (흉부 전산화단층촬영에서 우연히 발견된 유방 병변: 임상적 중요성 및 진료 의뢰가 필요한 특징적 영상 소견)

  • Purpose: To evaluate the CT features of incidental breast lesions on chest CT and to suggest useful criteria for referral to a specialized breast unit. Materials and Methods: Between May 2009 and April 2014, enhanced chest CT examination reports containing the key word 'breast' were reviewed retrospectively. Patients who had incidental breast lesion and were referred to a specialized breast unit and then underwent pathological confirmation or follow-up over a 1-year period were included. Finally, 86 patients (all female, mean age, $48.9{\pm}12.6years$) were enrolled. Two radiologists evaluated lesion characteristics, including size, shape, margins, and enhancement. The correlations between the CT features and pathologies were evaluated, and the diagnostic accuracy of CT features in various combinations was assessed. Results: Among the CT features, irregular shape, non-circumscribed margin, and high contrast enhancement were different between malignant and benign lesions (p < 0.05). The combination of non-circumscribed margin and high contrast enhancement had the highest accuracy (97.7%). Conclusion: Reliable CT features for incidental malignant breast masses are irregular shape, non-circumscribed margin, and high contrast enhancement. The combination of non-circumscribed margin and high contrast enhancement could help distinguish incidental malignant breast lesions and indicate referral to a specialized breast unit.

Applications of Artificial Intelligence in Mammography from a Development and Validation Perspective (유방촬영술에서 인공지능의 적용: 알고리즘 개발 및 평가 관점)

  • Mammography is the primary imaging modality for breast cancer detection; however, a high level of expertise is needed for its interpretation. To overcome this difficulty, artificial intelligence (AI) algorithms for breast cancer detection have recently been investigated. In this review, we describe the characteristics of AI algorithms compared to conventional computer-aided diagnosis software and share our thoughts on the best methods to develop and validate the algorithms. Additionally, several AI algorithms have introduced for triaging screening mammograms, breast density assessment, and prediction of breast cancer risk have been introduced. Finally, we emphasize the need for interest and guidance from radiologists regarding AI research in mammography, considering the possibility that AI will be introduced shortly into clinical practice.

  • Angiosarcoma of the breast is a rare malignant sarcoma of endovascular origin that presents with various radiological findings. We encountered a case of angiosarcoma of the breast presenting as an irregular indistinct hyperechoic mass on ultrasonography, with a large single angiosarcoma of the spleen in a previously healthy 36-year-old female. Herein, we report the imaging and pathologic findings and review the literature.

Usefulness of Three-Dimensional Maximal Intensity Projection (MIP) Reconstruction Image in Breast MRI (유방자기공명영상에서 3 차원 최대 강도 투사 재건 영상의 유용성)

  • Purpose : To evaluate the usefulness of three-dimensional (3D) maximal intensity projection (MIP) reconstruction method in breast MRI. Materials and Methods : Total 54 breasts of consecutive 27 patients were examined by breast MRI. Breast MRI was performed using GE Signa Excite Twin speed (GE medical system, Wisconsin, USA) 1.5T. We obtained routine breast MR images including axial T2WI, T1WI, sagittal T1FS, dynamic contrast-enhanced T1FS, and subtraction images. 3D MIP reconstruction images were obtained as follows; subtraction images were obtained using TIPS and early stage of contrast-enhanced TIPS images. And then 3D MIP images were obtained using the subtraction images through advantage workstation (GE Medical system). We detected and analyzed the lesions in the 3D MIP and routine MRI images according to ACR $BIRADS^{(R)}$ MRI lexicon. And then we compared the findings of 3D MIP and those of routine breast MR images and evaluated whether 3D MIP had additional information comparing to routine MR images. Results : 3D MIP images detect the 43 of 56 masses found on routine MR images (76.8%). In non-mass like enhancement, 3D MIP detected 17 of 20 lesions (85 %). And there were one hundred sixty nine foci at 3D MIP images and one hundred nine foci at routine MR images. 3D MIP images detected 14 of 23 category 3 lesions (60.9%), 11 of 16 category 4 lesions (68.87%), 28 of 28 Category 5 lesions (100%). In analyzing the enhancing lesions at 3D MIP images, assessment categories of the lesions were correlated as the results at routine MR images (p-value < 0.0001). 3D MIP detected additional two daughter nodules that were descriped foci at routine MR images and additional one nodule that was not detected at routine MR images. Conclusion : 3D MIP image has some limitations but is useful as additional image of routine breast MR Images.

Imaging Findings of Breast Metastasis from Malignant Peripheral Nerve Sheath Tumor of Mediastinum: A Case Report (종격동 악성 말초 신경 초종으로부터 유방 전이의 영상 소견: 증례 보고)

  • Malignant peripheral nerve sheath tumor (MPNST), a rare soft-tissue sarcoma, is most commonly located in the trunk, extremities, and head and neck, but rare in the breast. We report a metastatic breast MPNST in a 27-year-old woman with neurofibromatosis type 1 (NF-1). Chest computed tomography revealed a well-defined, oval, mildly enhancing nodule in the right breast. US revealed a circumscribed, oval, heterogeneous echoic mass with vascularity and intermediate elasticity in the right upper outer breast. The breast mass was excised and diagnosed as MPNST on histopathology evaluation. Although rare, it should be included in the differential diagnosis of breast mass in NF-1 patient.

Breast Imaging Reporting and Data System (BI-RADS): Advantages and Limitations (유방영상 판독과 자료체계: 장점과 한계)

  • Breast Imaging Reporting and Data System (BI-RADS) is a communication and data tracking system that standardizes and controls the quality of reporting by presenting lexicon descriptors, assessment categories, and recommendations for managing breast lesions. Using standardized terminology recommended by BI-RADS, radiologists can concisely and reproducibly communicate breast imaging results to clinicians. They can also provide the estimated malignant probability of the lesions found and guide management for them by determining the final assessment category. The limitations of BI-RADS 5th edition currently in use are that there are some areas for which standardized terminologies still need to be established, and that the diagnostic criteria of MRI assessment categories 3 and 4 are ambiguous compared to those for mammography or ultrasound. The next revision of BI-RADS is expected to include solutions for overcoming current limitations.

  • When the X-ray energy is high, the X-ray penetrates the object and decrease the contrast of imaging, and when the X-ray energy is low, the X-ray increases the contrast of imaging but it is to be absorbed into the object, which in the long run increases patient's radiation exposure level. Therefore, appropriate X-ray energy is an essential element affecting the imaging quality and radiation exposure level. This study simulated the energy spectrums according to the target materials of mammography, and compared qualities of phantom imaging for the management of radiolographic quality and patient's radiation exposure level with the introduction of the mammography that employs diversified radiation quality by using new anode materials.

Myxoid Liposarcoma of the Breast Mimicking Phyllodes Tumor: A Case Report (엽상종양과 유사한 유방의 점액성 지방육종: 증례 보고)

  • Myxoid liposarcoma is an extremely rare malignant breast tumor. We report the case of a 44-year-old woman who had myxoid liposarcoma of the breast with a history of phyllodes tumor and describe the imaging findings on US, mammography, and MRI. Before surgery, the mass was considered to be a recurrent phyllodes tumor. However, using US, we retrospectively identified some differences between myxoid liposarcomas and phyllodes tumors.
