A Study of Tube Angle and Patient's Rotation Angle in Scapular Y View (어깨뼈 후전사방향촬영에서 Tube 각도와 환자의 회전각도 관한 연구)

  • Shoulder bone scan everyday life, sports activities at the side of the shoulder joint, or applying strong force in the lateral occurs. Mainly on upper arm abduction, temple, other rotational dislocation of the shoulder joint gahaejyeo as useful for observing the presence or absence of lesions is used. Shoulder PA oblique projection prevent distortion of the image due to the angle and the most useful diagnostic radiological investigate shooting angle. Shoulder blade body and the acromion and coracoid process Y-view is formed characters. $10^{\circ}$ angle between the shoulder blades and the acromion is the obstruction. $20^{\circ}{\sim}25^{\circ}$ to the Y-view of the humerus head looks superimposed, the position of the shoulder joint and seemed appropriate. $35^{\circ}$ of the humerus head superimposed on the Y-view, but above the humerus head and shoulder joints were distorted. When $45^{\circ}$ elevation observed on the side of the best has come Y-view also acromion and coracoid process is finished.

A Study on Shoulder Joint Motions in the Caudal Gliding of Kaltenborn-Evjenth Concept (칼텐본-에비엔즈컨셉의 어깨관절 아래쪽미끄러뜨림 적용시 관절의 이동성 연구)

  • This study aimed at identifying changes in the acromiohumeral distance (AHD) and arm when the caudal gliding gradeII and III of Kaltenborn-Evjenth Concept$^{(R)}$ are applied to the right glenohumeral joint. The humeral head moved down about 5mm from the initial position when the gliding gradeII was applied, and about 8mm from the initial position when the gliding gradeIII was applied. Although men showed a higher acromiohumeral distance per grade than women in comparisons by gender for the acromiohumeral distance, there was no significant difference in statistics. The Abduction angle improved about $10^{\circ}$ from the initial angle when the gliding gradeII was applied, and about $12^{\circ}$ from the initial angle when the gliding gradeIII was applied. Although women showed the abduction angle greater than men for every grade in comparisons by gender for the abduction angle, there was no significant difference in statistics. Based on the aforesaid findings, the extent of kinematic changes in the humeral head could be identified when the gliding grades were applied. Accordingly, it is considered that more scientific evidence based treatments could be expected if influences on the surrounding structures by these changes could be learned through more studies in the future.

A Convergence Study on the 5-axis Machining Technology using the DICOM Image of the Humerus Bone (위팔뼈 의료용 디지털 영상 및 통신 표준 영상을 이용한 5축 가공기술의 융합적 연구)

  • The present study aimed to obtain basic knowledge of a customized artificial joint based on the convergence research of Digital Imaging and Communications in Medicine(DICOM) and 5-axis machining technology. In the case of the research method, three-dimensional modeling was generated based on the medical image of the humerus bone, and the shape was machined using a chemical wood material. Then, the anatomical characteristics and the modeling machining computation times were compared. The results showed that the Stereolithography (STL) modeling required twice more time for semi-finishing and 10 times more time for finishing compared to the Initial Graphics Exchange Specification(IGES) modeling. For the 5-axis machining humerus bone, the anatomical structures of the anatomic neck, greater tubercle, lesser tubercle, and intertubercular groove were similar to those in the three-dimensional medical image. In the future, the convergence machining technology, where 5-axis machining of various structures(e.g., the surgical neck undercut of the humerus bone) is performed as described above, can be efficiently applied to the manufacture of a customized joint that pursues the precise model of a human body.

The Effect of Novel Stretching on the Range of Motion and Acromio-Humeral Distance in Patients with Glenohumeral Internal Rotation Deficits with Posterior Shoulder Tightness (노벨 스트레칭과 관절가동술이 어깨 후방 뻣뻣함으로 인한 어깨관절 안쪽돌림 결핍 환자의 관절가동범위와 봉우리-위팔뼈 거리에 미치는 영향)

  • Background: The cross-body and the sleeper stretches have been used to improve posterior shoulder tightness (PST). However, such stretching techniques may not always help achieve scapular stability and may even aggravate patient's symptoms. Therefore, a new stretching technique (passive glenohumeral internal rotation with bridging) was developed as a more effective method that may allow for greater scapular stability without aggravating the symptoms. Thus, this study aimed to examine and compare this novel stretching technique to determine its effect on the range of motion (ROM) and acromio-humeral distance (AHD) in patients with glenohumeral internal rotation deficits (GIRD) with posterior shoulder tightness. Methods: A total of 30 symptomatic patients with asymmetrical GIRD were randomly assigned to two groups: the novel stretching group (n=15) and the joint mobilization group (n=15). The intervention was conducted twice a week for a total of four weeks. The ROM of shoulder internal rotation was measured by a goniometer and the AHD was evaluated by an X-ray before and after the intervention. Results: Both the treatments improved ROM and AHD in patients with GIRD (p<.05). However, the improvements in internal rotation ROM and AHD in the novel stretching group were significantly greater than that of the subjects in the mobilization group. There was a significant difference between the two groups (p<.05). Conclusion: These results show that both novel stretching and joint mobilization improved ROM and AHD in patients with GIRD with PST. However, novel stretching was more effective than joint mobilization.

The Effect of Mobilization Combined with Shoulder Active Contraction of Depth between the Coracoid Process and Humeral Head (어깨의 능동 수축을 동반한 관절가동술이 부리돌기와 위팔뼈 머리 사이의 거리에 미치는 영향)

  • Purpose: This study examined the distance between the coracoid process and the humeral head using an ultrasonography device when shoulder active contraction were applied according to the guided direction in the end range of shoulder mobilization. This study aims to provide essential data on treating shoulder disease patients. Methods: The subjects of this study were 20 adults with healthy shoulder joints. ultrasonography (US) equipment was used to examine shoulder joint mobilization under two conditions: (1) anteroposterior (AP) joint mobilization and (2) superoinferior (SI) joint mobilization. Shoulder active contraction was assessed in the end range. The distance between the coracoid process and the humeral head was measured. A linear probe was used for US; the frequency was set to 7.5MHz, and the US image display method was set to B-mode. The US measurement values were measured in (1) the starting position, (2) the end range position, and (3) the end range position of the shoulder active contraction, and the moving distance was drawn in a straight line through the US image. The distance was determined as the measurement value, and the average values were compared. Reults: The results were as follows: (1) the measured AP Joint mobilization increased by an average of .52cm from the end range of the joint mobilization with shoulder active contraction; (2) the measured SI Joint mobilization increased by an average of .49cm from the end range of the joint. Conclusion: When shoulder mobilization is applied, the distance between the coracoid process and the humeral head increases when muscle contraction occurs through shoulder active contraction in the end range, according to the therapist's guidance. Therefore, shoulder mobilization combined with shoulder active contraction is an effective treatment method for patients with shoulder injuries.

Image Analysis of Angle Changes in the Forearm during Elbow Joint Lateral General Radiography: Evaluation of Humerus Epicondyle and Elbow Joint (팔꿉관절 측방향 일반촬영에서 아래팔뼈 각도 변화에 따른 영상 분석 : 위팔뼈 위관절융기와 팔꿉관절 평가)

  • Clear overlapping of the bilateral epicondyle and proper separation of the elbow joint are crucial for obtaining accurate lateral general radiographs of the elbow. However, due to the complex anatomical structure of the elbow, achieving optimal positioning is challenging, leading to the need for repeated x-ray examinations. Therefore, the purpose of this study was to investigate the angle of the forearm in patients where accurate lateral images of the elbow joint can't be obtained after vertical incidence using a styrofoam device during elbow joint lateral x-ray imaging. Twenty patients were enrolled in our study following the established protocol. First, a vertical x-ray at an angle of 0° between the forearm and the table was taken (control group). Here, if the lateral image of the elbow joint was deemed inadequate, the forearm angle was adjusted using custom-made styrofoam supports with 5° and 10° inclinations (experimental groups). For the evaluation method, two assessors utilized a 5-point Likert scale to assess the images. The reliability of the assessments was analyzed using Cronbach's alpha coefficient. As a result, patients with inadequate overlap of the bilateral epicondyle and separation of the elbow joint in the initial examination (control group) were able to obtain the best images when setting a 10° angle between the forearm and the table. The subjective evaluation was 1.6 ± 0.8 points at 0°, 2.7 ± 0.8 points at 5°, and 4.4 ± 1.3 points at 10°, respectively. The reliability analysis for the angles of 0°, 5°, and 10° yielded Cronbach's alpha values of 0.867, 0.697, and 0.922, respectively. In conclusion, when it is not possible to obtain accurate images using the conventional position and X-ray beam direction, it is considered that by initially acquiring images with an angle of 10° between the forearm and the table, and gradually decreasing the angle while obtaining images, it would be possible to achieve the optimal image while reducing the number of repeat examinations.

The Effect of Scapular Stabilization Exercise and Thoracic Joint Mobilization on the Scapular Function in Adults with Scapular Dysfunction (어깨뼈 안정화운동과 등뼈관절가동술이 어깨뼈 기능이상이 있는 성인의 어깨 기능에 미치는 영향)

  • The purpose of this study was to investigate the effect of scapular stabilization exercise and thoracic joint mobilization on scapular position, scapular motion and shoulder pain and disability index(SPADI) in adults with scapular dysfunction. A total 42 subjects were divided into a scapular stabilization exercise group(A, n=14), a thoracic joint mobilization group(B, n=14) and scapular stabilization exercise with thoracic joint mobilization group(C, n=14) and performed exercise and joint mobilization three times per week for four weeks, total 12 times. As for changes in scapular position among the groups, group C showed statistically significant increase(p<.05). In changes of scapular motion among the groups, group C showed statically significant improvement in shoulder abduction 45° and 90° both(p<.05), In changes of SPADI among the groups, group C were showed statistically significant improvement(p<.05). In conclusion, scapular stabilization exercise and thoracic joint mobilization were effective on improving scapular position, movement and SPADI score in adults with scapular dysfunction.

Development of Elbow Joint X-ray Examination Aid for Medical Imaging Diagnosis (의료영상 진단을 위한 팔꿉관절 X-선 검사 보조기구 개발)

  • The elbow joint is made up of three different bones. X-rays or other radiological exams are commonly used to diagnose elbow injuries or disorders caused by physical activity and external forces. Previous research on the elbow joint reported a new examination method that meets the imaging evaluation criteria in the tilt position by Z-axis elevation of the forearm. Therefore, this study aims to design an optimized instrument and develop an aid applicable to other upper extremity exams. After completing the 2D drawing and 3D modeling design, the final design divided into four parts was fabricated with a 3D printer using ABS plastic and assembled. The developed examination aid consists of a four-stage Z-axis elevation tilt angle function (0°, 5°, 10°, and 15°) and can rotate and fixate 360° in 1-degree increments. It was designed to withstand a maximum equivalent stress of 56.107 Pa and a displacement of 1.6548e-5 mm through structural analysis to address loading issues caused by cumulative frequency of use and physical utilization. In addition to X-ray exams of the elbow joint, the developed aid can be used for shoulder function tests by rotating the humerus and also be applied to MRI and CT exams as it is made of non-metallic materials. It will contribute to the accuracy and efficiency of medical imaging diagnosis through clinical applications of various devices and medical imaging exams in the future.
