A Case Report of the Traditional Korean Treatment of Female Patients with Both Functional Dyspepsia and Chronic Atrophic Gastritis (기능성소화불량과 만성 위축성 위염이 병발된 여성 환자에 대해 한방치료 경험 1례)

  • Objective: The aim of this study was to introduce gastric dysmotility as a common cause in patients with concurrent functional dyspepsia and chronic atrophic gastritis. Method: Dyspeptic symptoms, the Rydoraku score, gastric motility (electrogastrography, bowel sound analysis), gastric mucosa (gastroendoscopy), and blood and blood chemistry were all evaluated. For the treatment method, Pyengwi-san (solution) and Banwhasashim-tang (extract) were used as herbal drugs. Both ST36 electrical stimulation and simple immersion stimulation of CV11, 12, and 13 in the abdomen were applied. Results: Dyspeptic symptoms including indigestion, headache, and insomnia were all relieved. Gastric myoelectrical activity and gastric pyloric function were additionally improved. The condition of the gastric mucosa was changed from atrophic to erosive. Other side-effects of the treatment were not noted. Conclusion: The traditional Korean treatment showed effectiveness in the relief of dyspeptic symptoms and mucosal improvement of chronic atrophic gastritis. Gastric dysmotility is a common cause of the condition being concurrent with both functional dyspepsia and chronic atrophic gastritis without Helicobacter pylori infection.

Anti-oxidant Effects of Portulaca oleracea L. on HCl-ethanol Induced Gastritis in Rats (염산-에탄올에 의해 유발된 흰쥐 위염에 대한 마치현의 항산화 작용)

  • Objectives : The purpose of this study is to evaluate whether or not a pretreatment with Portulaca oleracea has an antioxidant effect in HCl-ethanol induced gastric mucosal damage. Methods : We elucidated the level of reactive oxygen species (ROS), lipid peroxidation, and two important constituents of antioxidant defense such as superoxide dismutase (SOD), glutathione (GSH) in these effects. Results : The oral administration of crude extract from P. oleracea attenuated the gastritic lesion area, submucosal edema and hemorrhage, and mucosal necrosis induced by HCl-ethanol. The MDA levels of control group were higher than those in the rats given the P. oleracea pretreatment. While the GSH levels of control were decreased, the GSH activity on the gastric mucosal layer maintain normal level in rats given the Portulaca oleracea pretreatment before HCl-ethanol induced gastritis significantly increased. However, the SOD activites were not altered by P. oleracea. Conclusions : The administration of Portulaca oleracea have a protective antioxidant effect against the gastric lesion induced by HCl-ethanol and may therefore be a promising drug for gastritis and gastric ulcer.

The Inhibitory Effects of Portulaca oleracea L. on HCl-ethanol Induced Gastritis in Rats (염산-에탄올에 의해 유발된 흰쥐 위염에 대한 마치현의 억제 작용)

  • Objectives : The objective of this study was to examine the effects of P. oleracea into the HCl-ethanol induced gastritis in rats, and to isolate and determine the chemical compounds from P. oleracea. Methods : The rats were orally administered with crude extract or fractions or isolated compounds of P. oleracea 30 mins before the induction of gastric lesion by oral administration of HCl-ethanol. The gastric lesional area was measured using pixel counting software. Then the chemical compounds from P. oleracea was isolated and determined by LC-MS and NMR. Results : The inhibition effect of oral administration of crude extract of P. oleracea at a dose of 500 mg/kg in HCl-ethanol induced gastritis was similar to cimetidine. Then, aqueous fraction at a dose of 240 mg/kg exhibited the effects similar to cimetidine. Then, the aqueous fraction was further separated by MPLC and yielded four sub fractions. Among those sub fractions, agent II at a dose of 40 mg/kg possessed the strongest effect in the HCl-ethanol induced gastritis. The water fraction yielded-Uridine, Adenosine, Guanosine, which were characterized by Mass, 1H-NMR, 13C-NMR. Conclusions : This study suggest that a P. oleracea and its compounds showed potent efficacy on the development of HCl-ethanol induced gastritis. Thus, P. olaracea can be a potential natural resource for the management of gastritis although the mechanism of action involved in the treatment remains to be explored.

Helicobacter pylori Infection in Children with Recurrent Abdominal Pain (소아에서 만성 반복성 복통(Recurrent Abdominal Pain)과 Helicobacter pylori 감염과의 연관성에 대한 연구)

  • Purpose: The aim of this study was to evaluate the relationship between H. pylori infection and recurrent abdominal pain (RAP) in children and to evaluate the effects of eradication therapy on RAP. Methods: From January 1998 to January 2005, 166 children with RAP (61 male, 105 female) aged $10.0{\pm}3.3$ years were included. Upper gastrointestinal endoscopies were performed for all the patients. All H. pylori infected children (n=70) received the eradication therapy and were divided into two groups: Group Ia (n=52); eradicated, Group Ib (n=18); non-eradicated. H. pylori-negative children (n=96) were divided into three groups according to the medication: Group IIa (n=67); no medication, Group IIb (n=13); acid-suppressant, Group IIc (n=16); both acid-suppressant and antibiotics. Questionnaire for symptoms were asked at the first, 6th, 12th, 24th, and 36th months following the treatment (grade 0; completely resolved, grade 1; definitely improved, but there are occasional episodes of mild abdominal pain, grade 2; no change in the frequency and intensity of abdominal pain). Results: In about 90% of H. pylori positive children, RAP improved in the both H. pylori-eradicated and non-eradicated children in a follow-up survey. In about 75% of H. pylori-negative children, RAP also improved among in the three groups of patients regardless of medication. Conclusion: These results suggest that there was no correlations between improvement of RAP and eradication of H. pylori, and between improvement of RAP and medication. Consequently the reassurance that the children with RAP have no serious organic cause was important to improvement of RAP.

  • Fitz-Hugh-Curtis 증후군은 골반 내 염증성 질환을 가진 환자의 직접적 간 실질 침범이 없는 간피막 염증에 의한 간주위염으로, 1930년대에 Thomas Fitz-Hugh와 Arthur Curtis에 의해 보고되었다. Neisseria gonorrhea나 Chlamydia trachomatis에 의해 발병하며, 항생제로 치료되는 양성 성교전파질환이다. 대부분 가임기의 젊은 여성에서 발견되지만, 15세의 청소년에서 진단된 증례가 있어 보고하는 바이다. 15세 여자 환자가 1개월 전부터 발생한 간헐적인 우상복부와 하복부의 통증을 주소로 내원하였다. 환자는 한 달 전 남자친구와 첫 성교를 한 이후, 5일 전까지 10여 차례정도 성교를 하였다. 사회력 상 고등학교 1학년 생이고, 월경 주기는 28-30일로 규칙적이었다. 내원 당일 시행한 임신 반응 검사는 음성이었다. 시행한 복부전산화단층촬영에서 우측 간엽의 가쪽 부분이 동맥기 조영증강을 보이며 골반내감염을 동반하고 있었다. 부인과 검진 상 질경부 면봉 검사에서 Chlamydia trachomatis 양성소견을 보였으며, 소변배양검사에서 Neisseria gonorrhea가 동정되었다. Fitz-Hugh-Curtis 증후군 진단 하에 4주간 doxycycline과 metronidazole의 경구용 항생제 복용 후 호전되었다.

Purification of the Recombinant Helicobacter pyrori Urease by Affinity Chromatography (Affinity Chromatography를 이용한 재조합 Helicobacter pylori urease의 분리 정제)

  • Helicobacter pylori is the etiologic agent of human gastritis and peptic ulceration and produces urease as the major protein component on its surface. H. pylori urease is known to serve as a major virulence factor and a potent immunogen. Recombinant H. pylori urease expressed in E. coli was purified by simple purification procedures utilizing (CNBr-activated Sepharose-anti-urease IgG immunoaffinity chromatography or epoxy- activated Sepharose-urea affinity chromatography. Urease was apparently bound so tightly to the anti-urease IgG resin that it could not be eluted at various elution conditions except at certain extreme pH 1, including 100 mM carbonate (pH 10.5) buffer solution, which was shown to elute slightly inactivated but relatively pure enzyme. Urease eluted from the epoxy-activated Sepharose-urea affinity column showed higher activity, but the smaller UreA subunit of the enzyme appeared as a Fainter band of diminished intensity when subjected to SDS-polyamide gel electrophoresis.

Antigastritic and Antiulcer action of Effective Compounds from Propolis Extract (Propolis 유효성분의 항위염 및 항위궤양 작용)

  • Propolis, a natural resinous compound collected from honey bees, contains many biochemical constituents(wax, flavonoids, phenolic compounds, etc.) and has been used in traditional medicines as early as 300 B.C. It was been demonstrated that ethanol, acetylsalicylic acid, ischemia reperfusion, non-steroidal antiin-flammatory drugs and stress induce gastric lesions by promoting the generation of reactive oxygen metabolites. Therefore, some drugs that are capable of scavenging or inhibiting the generation of reactive oxygen radicals might be expected to prevent the gastric mucosal injury. The aim of this study was 1) to examine the antiulcer effect of propolis, 2) to investigate the mechanism of action by determining gastric acid secretion, lipid per-oxidation, mucus content and proton pump ($H^+$/$K^+$-ATPase) activity on gastric mucus in varios experimental models, and finally, 3) to isolate and identify the pure compounds that exert antiulcer activity. Step 2-1 and 2-3 sub-sub fraction shoed a significant reduction of severity of gastirc damage at the dose of 25 mg/kg in various experimental models. We isolated 4 sub-sub-sub fractions by flash column chromatography of Step 2-1 sub-sub fraction and one sub-sub-sub fraction by recrystalization of Step 2-3 sub-sub fraction. The protective effects of propolis sub-sub-sub fraction manifested sifnificant effects in HCl-ethanol induced gastric erosion model and aspirin induced gastric ulcer model. These results showed that the gastric mucosal protective effect of propolis might result from the increase of mucus secretion, free radical scavenging effect as well as the reduction of acid secretion in accordance with the reduction of $H^+$/$K^+$-ATPase activitv. Three compounds were isolated and identified from sub-sub fraction of propolis which showed antiulcer effects. Subsequently, these compounds were identified as a flavonoid, namely, 2-acetoxy-5,7,-dihydroxy-flavanone, galangin and chrysin.

Antigastritic and Anti Helicobacter pylori of Trifolirhizin from Sophora Radix (고삼 Trifolirhizin의 항위염 및 항 H. pylori 균 효과)

  • Sophorae Radix, the dried roots of Sophora flavescens Aiton (Leguminosae), has been used in oriental traditional medicine for treatment of skin and mucosal ulcers, sores, gastrointestinal hemorrhage, diarrhea, inflammation and arrhythmia. Present study was carried out for the gastroprotective effect of trifolirhizin from Sophora flavescens. This reports were evaluated antibacterial activity against Helicobacter pylori and HCl ethanol-induced gastric lesion in rats and showed the significant effectiveness. In pylorus ligated rats, the treatments of trifolirhizin showed decrease in the volume of gastric secretion and acid output. Also we evaluated the antibacterial activity against H. pylori treated with methanol extract and trifolirhizin from Sophora flavescens had a equivalent antibacterial activity with ampicillin against H. pylori at the dose of $100{\mu}g/mL$. It may be regarded that the antigastritic effects and antibacterial activity of trifolirhizin from Sophora flavescens are originated from reduction of total acid output identified by gastric secretion reduce, free radical scavenging effects and the antibacterial activity against H. pylori.

Antigastritic Effect of Carbenoxolone Disodium from Glycyrrhizae Radix (감초성분 Carbenoxolone Disodium의 항위염 효과)

  • Glycyrrhizae Radix, the dried roots of Glycyrrhiza glabra or Glycyrrhiza uralensis Fischer(Legumino-sae), has been used as a medicine for treatment of imflammation, arthritis, respiratory ailment, skin diseases and liver problems. The purpose of this study was to examine the effect of 70% ethanol extract, 18-${\beta}$-glycyrrhetinic acid, glycyrol and carbenoxolone disodium from Glycyrrhizae Radix on gastritis and gastric cancer. Using these materials, we tested antibacterial activity against Helicobacter pylori, antigastritic activity for HCI-ethanol-induced gastric lesion and the pylorus ligated gastric secretion with rats, and cell viability in gastric cancer cell. 18-${\beta}$-glycyrrhetinic acid and carbenoxolone disodium decreased the volume of gastric secretion and acid output in pylorus ligated rats. Also, carbenoxolone disodium had a strong effect of antibacterial activity on H. pylori. In addition 18-${\beta}$-glycyrrhetinic acid and glycyrol reduced cell viability in human gastric cancer cells(AGS and SNU638 cell) in dose-dependent manner. The reduction of total acid output and gastric secretion as well as the anti-bacterial activity against H. pylori might account for the antigastritic effects of carbenoxolone disodium.

Detection of Helicobacter pylori in Saliva of Patient with Oral Lichen Planus (구강 편평태선 환자의 타액에서 Helicobacter pylori의 검출)

  • Lichen planus is a common, chronic inflammatory disease of the skin and mucous membrane for which no precise causes have been confirmed. But it is often connected with infections. Helicobacter pylori(H. pylori) among various bacteria has been associated with the cause of gastritis, peptic ulcer and gastric cancer. Considering the similarities of histological features between gastric ulcer and oral ulcers, it is resonable to assume that H. pylori might also be involved in the development oral mucosal ulceration. So we employed this study to investigate the possible involvement of H. pylori in the aetiology of erosive oral lichen planus. We analyzed detection rate of H. pylori in saliva of patients with erosive oral lichen planus by nested PCR. As a result, it revealed a significant difference statistically by showing positivity in 16 to 21(76.2%) saliva samples of patients group and in 11 of 44(25%) saliva samples of control group(P>0.001). We were able to suppose that H. pylori in saliva can be related to cause of erosive oral lichen planus.
