Development of Closed-loop Control Type FES System for Restoration of Gait in Patients with Foot Drop (족하수 환자의 보행보조를 위한 피드백 제어형 전기자극기 개발)

  • The purpose of this study was to develop a portable and convenient closed-loop contrel type electrical stimulator for patients with foot drop. This system restores walking movement as well as prevents from atrophy or necrosis of lower limb muscles and increases blood circulation in hemiplegic patients caused by traffic accident, industrial disaster or stoke. This system detects the changes of the ankle joint angle during walking, and then controls the stimulus intensity automatically to maintain the programmed level of the ankle joint angle. Also, this automatic system controls the stimulus intensity which is affected by increased electrode impedance resulting from long time use. The system detects the joint angle by an optical sensor and includes modified PID control which adjusts the stimulus intensity if the joint angle deviates from the preset value. Stimulus parameters are 30~80 volt, 40 Hz, and 0.2 ms. The system was applied to five hemiplegic patients for 42 days. Duration of stimulation was 15 min/day for the first week and then the duration was gradually increased to 30, 60, 90 and 120 min/day. The muscle force was increased up to 29.7%, muscle fatigue was decreased compared with the level before stimulation and the pattern of locomotion was improved. These results suggest that the electrical stimulator with closed-loop control type is more convenient and effective in restoration of locomotion of patients with foot drop than open-loop system.

A Fundamental Study of VIV Fatigue Analysis Procedure for Dynamic Power Cables Subjected to Severely Sheared Currents (강한 전단 해류 환경에서 동적 전력케이블의 VIV 피로해석 절차에 관한 기초 연구)

  • The subsea power cables are increasingly important for harvesting renewable energies as we develop offshore wind farms located at a long distance from shore. Particularly, the continuous flexural motion of inter-array dynamic power cable of floating offshore wind turbine causes tremendous fatigue damages on the cable. As the subsea power cable consists of the helical structures with various components unlike a mooring line and a steel pipe riser, the fatigue analysis of the cables should be performed using special procedures that consider stick/slip phenomenon. This phenomenon occurs between inner helically wound components when they are tensioned or compressed by environmental loads and the floater motions. In particular, Vortex-induced vibration (VIV) can be generated by currents and have significant impacts on the fatigue life of the cable. In this study, the procedure for VIV fatigue analysis of the dynamic power cable has been established. Additionally, the respective roles of programs employed and required inputs and outputs are explained in detail. Demonstrations of case studies are provided under severely sheared currents to investigate the influences on amplitude variations of dynamic power cables caused by the excitation of high mode numbers. Finally, sensitivity studies have been performed to compare dynamic cable design parameters, specifically, structural damping ratio, higher order harmonics, and lift coefficients tables. In the future, one of the fundamental assumptions to assess the VIV response will be examined in detail, namely a narrow-banded Gaussian process derived from the VIV amplitudes. Although this approach is consistent with current industry standards, the level of consistency and the potential errors between the Gaussian process and the fatigue damage generated from deterministic time-domain results are to be confirmed to verify VIV fatigue analysis procedure for slender marine structures.

Comparison of Rotational Strength in Shoulders with Anterior Instability and Normal Shoulders Using Isokinetic Testing (등속성 검사를 통한 견관절 전방 불안정 환자와 정상인의 회전력 비교)

  • Objective: It has been expected that patient with posttraumatic recurrent anterior shoulder dislocation might have limited daily life activity because of pain and apprehension of dislocation. But there have been only a small number of investigations regarding the rotator strength in this patient. The aim of this study is to find the characteristics about rotator strength of patient with posttraumatic recurrent anterior shoulder dislocation using an isokinetic testing. Method: We enrolled thirteen patients with posttraumatic recurrent anterior shoulder dislocation and fifteen sex, age-matched healthy nonathletic subjects in this controlled study. All participants were male and there were no significant differences between the two groups in age, height, weight, BMI. Isokinetic internal rotator and external rotator strength was evaluated with a Biodex Isokinetic Testing machine (Biodex Medical Systems, Shirley, NY, USA), tests were performed at 60 deg/sec and 180 deg/sec for both sides. Peak torque normalized to body weight, external rotator to internal rotator ratio, total work and fatigue were calculated for each angular velocity. The association between internal rotator and external rotator strength and shoulder instability was analyzed by comparisons with a control group. Results: Any notable differences could not be found between the two groups given all data from no symptomatic left shoulder. There were no significant differences between the two groups statistically in internal rotation strength of right shoulder. However, there has been a tendency that at all angular velocities, external rotator peak torque to body weight, total work and external rotator to internal rotator ratio were significantly lower in the anterior instability group than the control group at all angular velocities. There was no substantial difference between those groups with respect to the fatigue of external rotator and internal rotator in our study. Conclusion: The prominent characteristics of posttraumatic recurrent anterior shoulder dislocation are external rotator weakness and loss of balance with external rotator and internal rotator. Therefore selective training using this information rotator might be helpful in conservative treatment and rehabilitation.

Narcolepsy Variant Presented with Difficult Waking (각성장애로 발현한 기면증의 변종)

  • Objectives Summary: A 20-year-old man was presented with a history of difficult waking for 10 years. He suffered from morning headache, chronic fatigue and mild daytime sleepiness but had no history of irresistible sleep attack, cataplexy, hypnagogic hallucination or sleep paralysis. Methods: Night polysomnography (PSG), multiple sleep latency test (MSLT) and HLA-typing were carried out. Results: The PSG showed short sleep latency (4.0 min) and REM latency (2.5 min), increased arousal index (15.7/hour), periodic limb movements during sleep (PLMS index=8.1/hr) with movement arousal index 2.1/hr and normal sleep efficiency (97.5%). The MSLT revealed normal sleep latency (15 min 21 sec) and 4 times sleep-onset REM (SOREM). HLA-typing showed DQ6- positive, that corresponded at the genomic level to the subregion DQB1*0601, which was different from the usual locus in narcolepsy patients (DQB1*0602 and DQA1*0102). Conclusion: Differential diagnosis should be made with circadian rhythm disorder and other causes of primary waking disorder. The possibility of a variant type of narcolepsy could be suggested with an unusual clinical manifestation and a new genetic marker.

Effects of Landing Height and Knee Joint Muscle Fatigue on Movement of the Lower Extremity during Cutting After Landing (착지 높이와 무릎관절 근육 피로가 착지 후 방향 전환 동작 시 하지관절의 움직임에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of landing height and knee joint muscle fatigue on the movement of the lower extremity during cutting after landing. Method : Subjects included 29 adults (age: $20.83{\pm}1.56years$, height: $172.42{\pm}9.51cm$, weight: $65.07{\pm}10.18kg$). The subjects were asked to stand on their dominant lower limb on jump stands that were 30 and 40 cm in height and jump from each stand to land with the dominant lower limb on a force plate making a side step cutting move at a $45^{\circ}$ angle with the non-dominant lower limb. The fatigue level at 30% of the knee extension peak torque using an isokinetic dynamometer. Results : The results showed that the difference of landing height increased maximum range of motion and angular velocity of hip, knee, and ankle joints in the sagittal plane, and in the angular velocity of motion of the hip joint in the sagittal plane. The maximum range of motion of the knee joint in the sagittal plane and the frontal plane decreased on landing from both heights after the fatigue exercise. The angular velocity of the hip joint in the sagittal plane, and the maximum range of motion of the hip joint in the transverse plane decreased for both landing heights after the fatigue exercise. The angular velocity of the hip joint in the frontal plane decreased for the 30 cm landing height after the fatigue exercise. On the other hand, the angular velocity and maximum range of motion of the ankle joint in the sagittal plane for both landing heights, and the angular velocity and maximum range of motion of the ankle joint in the frontal plane increased on landing from the 40 cm height after the fatigue exercise. Conclusion : Different landing heights of 30 and 40 cm and 30% fatigue of peak torque of knee extensor found a forefoot and stiff landing strategy, when cutting after landing. These results might be due to decline in the shock absorption capability of the knee joint and the movement capability related to cutting while increasing the contribution of the ankle joint, which may cause increased ankle joint injuries.

Health Behaviors of Nurses at a University Hospital according to Type of Work (일개 대학병원 간호사들의 근무형태에 따른 건강행태)

  • Background : The goal of this study was to examine the health behaviors of nurses and provide basic information on health promotion for nurses. Materials and Methods : A self-administered questionnaire survey was completed by 340 nurses working at a university hospital in Daegu, Korea from March 6 to March 30, 2006. The questionnaire included demographic characteristics, working environment, health behaviors, self-assessed health status, and the practice of health promotion activities according to the work group(shift group vs. non-shift group). Results : There were significant differences in 'regular diet', 'drinking', 'ordinary health management', 'regular exercise', 'leisure activities', 'rest', and 'feeling of fatigue' between the two groups. The score for the practice of health promotion activities of the non-shift group was 121.9, which was higher than the 115.4 for the shift group. By field of practice of health promotion activities, there were significant differences in 'self-realization' and 'nutrition' between the two groups. Conclusion : The shift group did not appear to have enough time to take care of their health. It is therefore important to provide a working environment in which nurses are encouraged to increase their interest and efforts to maintain healthy behaviors. In addition, programs for education and training should be developed to help nurses adopt healthy life styles and enhance their self-esteem.

  • 신체를 구성하는 여러 가지 요소 중에서 인간이 일상생활을 하는 데에는 신진대사를 일정하게 유지하기 위한 인체내의 여러 가지 전달물질들이 그 구성에 맞게 역할을 수행하여야 한다. 그러나, 만약 그 구성 비율이 바뀔 때에는 인체 내에 이상이 발생하게 된다. 그 대표적인 예로는 운동을 심하게 하였을 경우 갈증과 근육의 피로감이 몰려오게 되는데 이는 세포내의 이온농도의 변화에 의한 것이며 이때 물, 스포츠 이온 음료 등을 섭취한다. 순간적으로 그 증상이 회복되지는 않지만 신경계 내에서 회복하는데 에는 많은 시간이 소요된다. 생리학, 임상적으로 나트륨과 물은 신진대사에 밀접한 연관이 있다. 인체의 나트륨은 음식물의 섭취량과 나트륨의 생리작용사이의 균형에 좌우된다. 건강한 상태에서 나트륨의 생리적인 손실은 무시할 만 하며 나트륨 생리작용은 음식물의 섭취량과 밀접한 관련이 있다. 칼륨은 단지 전체 신경계 용질의 일부이기 때문에 이것은 세포의 체적과 세포 용질의 삼투현상을 결정하는 주요한 것이다. 더욱더 칼륨은 신진대사 과정의 많은 중요한 변수가 되며, 세포밖의 칼륨은 전체중에 매우 작은 일부이긴 하지만 신경근육 작용에 매우 큰 영향을 준다. 세포안과 밖의 칼륨의 농도 비율은 흥분성 조직내의 막 전위의 주요한 결정요소가 된다. 본연구에서는 세포막 설계 후 시뮬레이션을 통한 정상상태의 세포막 전위의 활동전위 전달 특성과 Na, K의 이온 농도를 변화시킬 경우의 세포막 전위의 활동저위 전달 특성곡선을 비교 관찰하여 운동 생리학적인 신경세포내의 전달특성 메카니즘을 이해, 분석하였다.서의 속도를 최소한으로 줄이면서 스케줄링에 의해 시간 문제를 해결하는 방법을 제시하도록 한다. 이는 기초과학 수준이 높은 북방권 국가들의 과학자들이 주로 활용되고 있다는 점에서도 잘 알 수 있으며 우리의 과학기술 약점을 보완하는 원천으로써 외국인 연구 인력이 대안이 되고 있음을 시사한다. 본 연구에서는 한국 연구 조직에서 일하는 외국인 연구자들의 동기 및 성과에 영향을 미치는 많은 요인들을 확인할 수 있었다. 상관관계, 분산분석, 회귀분석 등을 통해 활용 성과에 미치는 영향 요인들을 도출하였다. 설문 분석을 통하여 동기 및 성과 사이에는 강한 상관관계가 존재하는 것을 확인할 수 있었으며 이는 전통적인 동기 이론들과 부합한다. 대부분의 변수가 동기 및 성과에 동시에 영향을 미치는 것으로 조사되었으며 그중에서도 조직 협력 문화, 외국인 연구자의 의사소통 및 협력성, 외국인 연구자의 연구 능력 관련 변수들 및 연구 프로젝트의 기술수명주기, 외국인 연구자의 기존 기술지식의 흡수 등이 가장 중요한 변수로 나타났다. 이는 우리가 주로 중국 및 러시아 과학자들을 활용하여 상업화하는 외국인 연구인력 활용 패턴과도 일치하는 결과이다. 즉 우호적인 조직문화를 가지고 있는 연구 조직에서, 이미 과학기술 지식을 많이 가지고 있고 연구 능력도 높은 외국인 과학기술자를, 한국에서 기술이 태동 또는 성장하고 있는 연구 분야에서 활용하는 것이 가장 성과가 좋다는 사실을 확인시켜 주고 있다. 국내에서 최초로 수행된 본 연구는 외국인 연구 인력의 활용 성과가 매우 높으며, 우리의 과학기술혁신시스템을 보완하는

Effect of Heat Stress of Extreme Heat Lever on Muscle functionand Muscle Injury Markers in Elderly Women (열 스트레스가 여성노인들의 근기능 및 근손상에 미치는 영향)

  • The purpose of this study was to examine the influence of extreme heat on muscle function and muscle injury marker in elderly women. The subjects of this study were eight post-menopausal elderly women without any metabolic disease. All eight subjects were asked to perform the knee joint isokinetic exercise using isokinetic equipment (cybex) in the laboratory and experimental temperature within laboratory was adjusted to two conditions: extreme heat temperature(33±0.5℃) and normal temperature(20±0.5℃) maintained in 50±3% humidity conditions. Each experimental exercise was monitored and analyzed the change of HSP70, LDH and CK. Muscular functions (peak torque, total work, percentage of peak torque body weight, fatigue index, average power and total work) were significant differences at exercise between temperatural conditions (p<.05). In extreme heat temperature, muscular injury markers (HSP70, LDH and CK) were increased, threfore resulted in significantly higher than normal temperature(p<.05). These results show that extreme heat temperature can decrease muscle function in elderly women.

The Effects of Reinforced Walking Exercise on Dyspnea-fatigue Symptoms, Daily Activities, Walking Ability, and Health related Quality of Life in Heart Failure Patients (강화된 걷기운동 중재가 심부전 환자의 호흡곤란과 피로증상, 일상생활 기능상태, 보행능력 및 건강 관련 삶의 질에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of reinforced walking exercise on dyspnea-fatigue symptoms, daily activities, walking ability and health related quality of life (HRQoL) in heart failure patients. Methods: This study used a randomized controlled trial design. The participants (experimental group=16, control group=25) were recruited from a university hospital in Kyeong-nam area. Data were collected from March to September, 2015. The reinforced walking exercise included goal setting and feedback (telephone and text message) provided for 12 weeks. Dyspnea-Fatigue Index, Korean Activity Scale/Index (KASI), six-minute walking distance (6MWD) and HRQoL were measured. Data were analyzed using descriptive statistics, t-test, Fisher's exact test, $x^2$ test, and Kolmogrove-Smirnov test. Results: Prior to the intervention there were no differences in the research variables between two groups. The exercise compliance in the experimental group was 100% (walking for 50 minutes per day, 5 times per week). The experimental group had improved dyspnea-fatigue symptoms (t=8.63, p<.001), daily activities (t=-4.92, p<.001), longer 6MWD (t=-5.66, p<.001), and increased HRQoL (t=-9.05, p<.001) compared to the control group. Conclusion: The reinforced walking exercise could be a cost-effective intervention in heart failure patient, which could enhance patients' outcomes, such as improving dyspnea-fatigue symptoms, daily activities, walking ability, and quality of life.

Effects of Yeast Fermented Ganoderma Lucidum Extract on the Recovery of Exercise Fatigue in Forced Swimming Rats (효모를 이용한 발효영지 추출물이 흰쥐의 유영 운동 피로에 미치는 영향)

  • Objectives : Ganoderma Lucidum has been used in traditional oriental medicine to treat various conditions including chronic fatigue and cough. This experimental study was designed to investigate the effects of yeast-fermented Ganoderma Lucidum extract on the fatigue of rats elicited by forced swimming. Materials and methods : The rat subjects were divided into 4 groups: normal group, control group administered with 0.9% normal saline, experimental I(Exp I) group administered with yeast-fermented Ganoderma lucidum extract and experimental II(Exp II) group administered with Ganoderma lucidum extract for 4 weeks. All the serum samples collected by heart puncture were used to measure biochemical factors such as glucose, lactate, lactate dehydrogenase(LDH), creatine kinase(CK), inorganic phosphorus, ammonia, total cholesterol(TC), triglycerides(TG), high-density lipoprotein(HDL), low-density lipoprotein(LDL), aspartate transaminase(AST), alanine transaminase(ALT). Results : 1. Weight change and swimming time were not significantly different in the experimental groups as compared with control group. 2. Lactate was significantly decreased in the experimental groups as compared with control group. Inorganic phosphorus was also significantly decreased in the Exp I group as compared with control group(p<0.05). 3. Serum lipid concentration was not significantly different in the experimental groups as compared with control group. 4. AST and ALT were significantly decrease in the experimental groups as compared with control group. Conclusions : Yeast-fermented Ganoderma lucidum in forced swimming rats would appear to reduce fatigue-related factors, AST, ALT of serum and promote anti-fatigue effect.
