Relationship of oral health behavior to subjective oral health status and the DMFT index in Korean adults (한국 성인의 구강보건행태와 주관적인 구강건강인지 및 우식경험영구치지수와의 관련성)

  • Objectives : The purpose of this study was to provide some information on down-to-earth oral health policy to improve the systemic health involving oral health and the quality of life in Korean adults. Methods : The third-year data of the 4th National Health and Nutrition Survey in 2009 were analyzed, and $x^2$(Chi-square) test was carried out to see whether there would be any gaps in subjective oral health status according to demographic characteristics, systemic health state, frequency of eating between meals and oral health behavior. In terms of the DMFT index, one-way ANOVA was utilized, and then Scheffe post-hoc analysis was conducted. Besides, multiple regression analysis was made to grasp the relationship of oral health behavior to subjective oral health state and the DMFT index. Results : The demographic characteristics, systemic health status and oral health behavior had a significant relationship to both of subjective oral health status and the DMFT index. As a result of analyzing the relationship of oral health behavior to subjective oral health state and the DMFT index, the subfactors of oral health behavior exerted a significant independent influence on subjective oral health status and the DMFT index. Conclusions : The findings of the study suggest that in order to promote the oral health of adults, preventive measures should be taken, and systematic oral health education should be provided. As there is an increase in the elderly population in Korea, the successful implementation of senior oral health plans and the development of oral health programs geared toward adults are both required.

The effects of xylitol and sorbitol mouth rinse on caries activity (자일리톨과 소르비톨 양치가 우식활성도에 미치는 영향)

  • Objectives: The purpose of this study was to identify the effects of xylitol and sorbitol mouth rinse on the salivary caries activity levels. Methods: The study subjects were 38 female college students in Gyeongju, Korea. The subjects visited the institution once a week for 4 weeks and the saliva samples were measured for the amount of salivary caries activity levels. The saliva was collected 5 times and incubated in Mitis Salivarius Agar for 48 hours(VS-1203P3L, Vision, Korea) and measured. Results: The Streptococcus mutans CFU decreased by statistically significant amounts as compared to before the experiment within the xylitol group and the sorbitol group(p<0.01). After three weeks of mouth rinse application, Streptococcus mutans CFU of the xylitol group and the sorbitol group showed statistically significant differences(p<0.05). The salivary flow rates within the xylitol group and sorbitol group increased by statistically significant amounts(p<0.01) than before the test. The change in the salivary buffering capacity decreased by a statistically significant amount as compared to before experiment within the xylitol group(p<0.01). Conclusions: The salivary caries activity levels decreased after using xylitol and sorbitol mouth rinse in CFU and the flow rate. The buffering activity levels increased within the xylitol group. Further follow-up studies would be necessary to identify the various effects of xylitol.

Remineralization effect of bamboo salt on incipient subsurface caries enamel (초기우식법랑질 표면 하방에 미치는 죽염의 재광화 효과)

  • Objectives: The aim of this study was to investigate the effects of bamboo salts on remineralization effects on subsurface of artificial carious enamel. Methods: Incipient carious enamel was formed in permanent bovine incisors and then specimens were divided into three groups randomly: 3% bamboo salt (BS), 2% (NaF) and the solution of mixed 3% BS and 2% NaF. For remineralization, specimens of each of the three groups were treated for 24 hrs at $37^{\circ}C$ incubator. After treatment, specimens were analyzed using SEM and VHN. Statistical analysis used was one-way ANOVA. Results: In SEM observation, the BS group showed narrower distances between enamel rods than the cases of incipient subsurface caries enamel. The NaF and BS+NaF groups showed that the enamel rods near the surface were destructed, and innumerable round small particles were deposited near the surface of enamel. The BS+NaF group showed more minerals attachment between enamel rods than the cases of other groups. The differences in subsurface microhardness (${\Delta}VHN$) increased all of three groups in total by $80{\mu}m$. The ${\Delta}VHN$ of the BS+NaF group increased significantly more than NaF and BS groups in depth of $50{\mu}m$, $80{\mu}m$. Conclusions: The 3% bamboo salt with 2% NaF solution was found to increase subsurface hardness of incipient caries enamel. Thus, bamboo salt will be used to contribute to prevention on dental caries.

Preoperative Factors of Immature First Permanent Molars Treated with Vital Pulp Therapy (생활치수치료를 시행한 미성숙 제1대구치의 술전 상태)

  • This study aimed to analyze the preoperative factors of immature first molars treated with vital pulp therapy and to find out their correlation in pediatric patients. From May 2014 to January 2020, 523 patients and 1,242 immature first molars were investigated. Factors including age, sex, tooth location, Molar-incisor hypomineralization (MIH), caries cavity location, and history of previous restoration were evaluated. As a result of the study, the vital pulp therapy group had 5.56 times more MIH, 3.39 times more mesial cavities, and 8.73 times more distal cavities. In order to avoid vital pulp therapy in immature first molar, early diagnosis and active management of MIH and preventive treatment of mesial and distal caries are necessary after its immediate eruption.

Changes in adult and middle-age Missing Teeth Index in Korea during the last 10 years: Data from 2007 to 2019 of the National Health and Nutrition Examination Survey (최근 10년간 우리나라 청·중장년기 우식경험상실치아수의 변화: 국민건강영양조사 2007년~2019년 자료)

  • Objectives: This study aimed to determine the Missing Teeth (MT; mean number of missing permanent teeth) index experienced by year for adults between the ages of 19 and 64. Methods: This study was conducted by analyzing the raw data of participants in the 4th to 8th periods of the National Health and Nutrition Examination Survey (NHANES). For the statistical program, IBM SPSS / WIN 26 was used and an independent sample t-test was performed. Results: The average number of MT by year showed a decreasing trend. In 2007, 2008, 2010 and 2011, the average number of MT by sex was higher among females, and this was statistically significant (p<0.05). However, in 2019, males had a higher average number of MT than females, which was statistically significant (p<0.05). Conclusions: Males had a higher average number of MT than females in Korea, so it is considered that oral health programs or oral hygiene management according to sex are necessary.

Antibacterial and phagocytosis control of natural extracts on S. mutans (S. mutans에 대한 천연추출물의 항균 및 탐식작용조절)

  • In this study, the antibacterial and phagocytosis regulation effects of Hordeum vulgare extract and pine needle extract on S. mutans, the causative bacteria of dental caries, were investigated. Ethanol extracts of domestic Hordeum vulgare powder and pine needle powder were used, and the antibacterial and phagocytic ability against S. mutans was confirmed according to the concentration of the extracts. As a result, S. mutans colony formation did not show a significant difference in the Hordeum vulgare extract but was significantly decreased in the pine needle extract. As a result of confirming the phagocytic ability of THP-1 cells for S. mutans, there was no significant difference in the Hordeum vulgare extract, but the phagocytic ability of immune cells was improved in the pine needle extract. Therefore, it suggests that pine needle extract can be used as a material for preventing dental caries.

Oral Health Behaviors and Subjective Oral Health Perception, Microorganism, and Relation between Oral Health Status

  • This study aims to analyze the relationship with oral health status through oral examinations, microorganism tests, and surveys of college students and present basic data necessary for the development of oral health education programs to prevent. Results of students who did not receive brushing education showed high motile activities of microorganism, and there was a significant relationship in which students with a large amount of microorganism subjectively felt more tooth pain. Symptoms of halitosis were greater in the DT, there were differences in the type of bacteria and the amount of bacteria in the MT, and oral health was worse if oral aids were not used in the DMFT index. Therefore, it was confirmed that oral health behavior, subjective oral health awareness, and the amount of microorganism had a significant relationship with oral health status. It will be necessary to develop and share and spread customized oral health education media for each life cycle.

Effect of fluoride releasing orthodontic sealants on enamel demineralization in vitro (불소가 유리되는 교정용 전색제가 법랑질 탈회에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study was to evaluate the effects of fluoride releasing orthodontic sealants, light-cured (Group L1&L2) and self-cured (Group S1&S2) $FluoroBond^{\circledR}$, on enamel microhardness under artificial carious solution in vitro.112 extracted human premolar teeth were collected for experiments and divided into seven groups. A Tukon microhardness tester equipped with a Knoop diamond indenter was employed to determine microhardness. Tukon 23 program converted the number of microhardness into KHN (Knoop hardness number). The results were as follows: 1. The microhardness of enamel depth of all groups were the least at the depth of $50{\mu}m$ and that of all groups except L2 group, the greatest at the depth of $200{\mu}m$, were the greatest at the depth of $300{\mu}m$. And as the enamel depth of all groups except L2 and S2 group increased, the microhardness value also increased. 2. There was a little preventive effect in enamel decalcification both light-and self-cured orthodontic sealant groups, but had no statistical significance between the groups(p>0.05). 3. Light-cured orthodontic sealant groups had a progressive inhibiting effect in enamel decalcification at the depth of $100{\mu}m,150{\mu}m,\;and\;200{\mu}m$ (p<0.05). 4. Self-cured orthodontic sealant groups had a progressive inhibiting effect in enamel decalcification at the depth of $150{\mu}m$ (p<0.05). 5. There was no difference of the anti-enamel demineralization effect between light- and self-cured orthodontic sealant groups (p>0.05).

DENTAL CARIES PATTERNS IN THE PRIMARY DENTITION: A CLUSTER ANALYSIS AND A MULTIDIMENSIONAL SCALING ANALYSIS (군집분석과 다차원척도법으로 본 유치열의 우식패턴)

  • The purpose of this study was to identify patterns of caries in the dentition of preschool children using cluster analysis and multidimensional scaling. 815 preschool children aged 36-71 months old were examined for caries experience. The first four clusters in the cluster analysis were (1) molars, (2) upper incisors, (3) lower 1st molars, and (4) upper 1st molars in teeth, (1) occlusal surfaces of lower molars and upper 2nd molars, (2) mesial surfaces of upper central incisors, (3) occlusal surfaces of lower 1st molars, and (4) separation between occlusal surfaces of upper and lower 2nd molars in tooth surfaces, and (1) proximal surfaces of upper anterior teeth, (2) occlusal surfaces of lower molars, (3) buccolingual surfaces of upper anterior teeth and (4) buccolingual, proximal, and occlusal surfaces of upper molars and buccolingual and proximal surfaces of lower molars in tooth surfaces groups. In the multidimensional scaling analysis, teeth could be divided into lower 1st molars, upper 1st molars, upper central incisors, upper lateral incisors, lower 2nd molars, upper 2nd molars, and the rest(canines, lower incisors), tooth surfaces could be divided into occlusal surfaces of lower molars, mesial surfaces of upper central incisors, occlusal surfaces of upper molars, and the rest, and tooth surface groups could be divided into proximal surfaces of upper anterior teeth, buccolingual surfaces of upper anteiror teeth, occlusal surfaces of lower molars, and the rest in the order of the distance from others. In the cluster analysis and the multidimensional scaling analysis, caries patterns were different according to the age.

The Association of Oral Impacts on Daily Performances for Children (C-OIDP), Oral Health Condition and Oral Health-Related Behaviors (어린이 일상생활구강영향지수(C-OIDP)와 구강관리 및 구강건강행태와의 관련성)

  • The purposes of this study were to investigate the factors affection the Oral Impacts on Daily Performances for Children (C-OIDP) in elementary and middle school students, and identify the association between oral health-related behaviors, oral health condition and C-OIDP. A cross-sectional study was conducted in three schools in Incheon, Asan, Korea. A total of 175 selected children were interviewed by a trained examiner using a questionnaire. Oral Health Related Quality of Life was assessed by the Korean version of C-OIDP. Socio-economic characteristics, oral health-related behaviors, oral health condition and C-OIDP were verified using the questionnaire. ANOVA analysis was performed to determine the oral health and C-OIDP, and multiple regression analysis was performed to determine the factors affecting the C-OIDP. The activities with the greatest effect were eating (28.0%), cleaning teeth (22.9%), and smiling (18.9%). In the logistic regression model, the high item score of C-OIDP was associated with experiencing dental caries and gum pain in the past month. The more the C-OIDP prevalence item, the more the fillng deciduous tooth surface (fs) (p=0.024), caries experienced deciduous tooth surface (dfs) (p=0.049), total caries tooth surface (ds+DS) (p=0.021), and total caries experienced tooth surface (dfs+DMFS) (p=0.047). It can be concluded that the factors affecting C-OIDP are fs, dfs, dfs+DMFS, and gingival pain. Based on these results, we can improve C-OIDP to advance preventive practice.
