Analysis of Bony Structures by F.E.M. (생체역학의 유한요소법 응용)

  • 본 논문에서는 생체역학 중 주로 뼈 구조에 유한요소법이 어떻게 사용되고 있는지에 대해 주로 정형외과에서 중요시되는 대퇴부, 요추 등의 예제를 통해 어떻게 모델링되고 있으냐와 유한요소법의 응용분야에 대해 간략히 설명하였다. 물론, 생체역학이라고 하면 허파, 심장 등을 포함하는 순환기, 혈관, 치아 등등 여러 분야를 일컫는 용어로 그 연구분야에 구조공학자의 관심이 필요로 함을 강조하고 싶다. 아울러 유한요소해석 결과에 대해서는 본 논문에서는 언급하지 않았지만 비교적 실험치에 비해 근사함을 여러 문헌을 통해 알 수 있다. 국내에서는 아직도 이 분야에 대한 연구가 매우 미진한 상태이므로 향후 이에 대한 연구가 수행되어야 할 것으로 사료된다.

지속성 복막 투석 환자의 골밀도와 이와 관련된 영양소 섭취, 식행동, 우울정도에 관한 연구

  • 지속성복막투석환자의 경우 투석에 의해 단백질 대사물질의 배설은 효과적으로 이루어지나 오랫동안의 만성신부전으로 인한 인의 배설 저하에 따라 골밀도가 대부분 저하되어 있으며 신성골이영양증을 수반하는 경우가 많다. 본 연구에서는 종합병원 신장내과 클리닉에 정기적 checkup을 받고 있는 지속성복막투석환자를 대상으로 골밀도를 측정하였으며 요추(L2-L4)와 대퇴부(femoral neck, trochanter)의 평균 골밀도의 T값이 모두 -1 미만인 군을 골감소증군으로 하였고(N=86) 1군데라도 T값이 -1 이상인 군은 정상군으로(N=32) 분류하여 영양소섭취량, 식행동, 우울정도에 따른 차이를 보고자 하였다.(중략)

The Study of Technical Error Analysis on BMD Using DEXA (이중 에너지 X선 흡수 계측법을 이용한 BMD 검사 시 발생할 수 있는 기술적인 오류 분석)

  • Purpose: This study was conducted to search for the type of technical error in DEXA(dual-energy X-ray absorptiometry) and the effect of error to measurement of BMD. Materials and Methods: The changes of BMD($g/cm^2$, T-score) by patients information(Age, Weight, Height, Manopause age) input error and Confirming ROI error were investigated. Using spine phantom, we canned 10 times by age(5, 10), weight(10, 20 kg), height(5, 10 cm), manopause age(5, 10) increase & decrease respectively. Scanning region(L-spine, femur, Forearm) of 10 patients was calculated by changing ROI respectively. Analysis of difference for mean(precision 1%) were carried out. Results: The error of patient information(Age, Weight, Height, Manopause age) was not changed differently. In confirming ROI, the BMD and T-score of L-spine involving T-12 was decreased to $0.063\;g/cm^2$, 0.3 and involving L-5 increased to $0.077\;g/cm^2$, 0.5. In narrowing 1 cm of vertical line of ROI, the BMD and T-score decreased to $0.006\;g/cm^2$, 0.1 and in 2 cm, $0.021\;g/cm^2$, 0.15, each. In hip ROI, Upper and left shift(0.5 cm) of line was not influenced BMD and T-score. In 0.5 cm lower shift(lesser trochanter below), the BMD and T-score increased $0.031\;g/cm^2$, 0.3 and in 1 cm $0.094\;g/cm^2$, 0.65, each. In forearm ROI, the BMD and T-score decreased $0.042\;g/cm^2$, 0.9 involving 1 cm lower wrist. And expanding 1 cm of vertical line, the BMD and T-score decreased $0.008\;g/cm^2$, 0.1 and in 2 cm, $0.021\;g/cm^2$, 0.3, each. The L-spine, hip, forearm ROI error was changed differently. Conclusion: There are so many kinds of technical error in BMD processing. Errors according to age, weight, height, manopause age did not influent to $BMD(g/cm^2)$ and T-score. There are mean differences BMD and T-score in confirming ROI. For the precision exam, in L-spine processing, L1-4 have to confirmed without shift of ROI vertical line. In hip processing, the ROI have to included greater trochanter, femur head and lesser trochanter. In forearm processing, the ROI have to included wrist, radius and ulnar.

Effect of Dietary Factors on Bone Mineral Density in Korean College Women (한국 여대생의 골밀도에 영향을 미치는 식이 요인 분석)

  • This study was conducted to investigate dietary and other factors affecting bone mineral density in young Korean collage women. Thirty subjects were recruited; the mean age was 22.1 years and the mean age at menarche was 12.6 years. Bone mineral density (BMD) was measured in the spine (LS), femoral neck (FN), ward's triangle (WT), and femoral trochanter (FT) by Dual Energy X-ray Absorptiometry (DEXA), and physical activity was assessed by a questionnaire. Dietary intake data were obtained 8 times by using a 24 hour recall method to evaluate the subjects' usual diet. Bone mineral densities of lumbar spine, femoral neck, ward's triangle and femoral trochanter were 1.149 g/$\textrm{cm}^2$, 0.980 g/$\textrm{cm}^2$, 0.936 g/$\textrm{cm}^2$ and 0.759 g/$\textrm{cm}^2$ respectively. The met energy intake of subjects was 1,790 kcal, and calcium and iron intakes were found to be inadequate compared to the Korean RDA. The BMD of the lumbar spine was significantly and positively related to the BMI and intake of vitamin C, carbohydrate, potatoes, fruits and plant food. The BMD of the femur (FN, WT, FT) was positively related to the BMI, to carbohydrate and fruit intake, and to the percentage of plant food in total food intake. Multiple recession analysis showed that fruit intake was significantly related to the BMD of the LS. The subjects'ages and fruit intakes were significantly related to the BMD of the femur (FN, WT, FT). These results indicate that higher fruit intakes may have a beneficial effect on bone mineral density.

Bone mineral Density of lumbar Spine in Children With Spastic Quadriplegia and Spastic Diplegia (경직성 사지마비와 양하지마비 아동의 요추부 골밀도)

  • In children with cerebral palsy, bone density is decreased by disturbance of bone remodelling due to lack of normal weight bearing and muscle contraction through physical activity. Loss of bone density cause fracture, delays treatment with immobilization, and leads to functional limitation. The purpose of this study was to investigate bone mineral density of lumbar spine in children with spastic quadriplegia and diplegia. Six spastic quadriplegia and 14 spastic diplegia were evaluated in this study. QDR 4500 X-ray densitometer was used to measure bone density at lumbar spine (L1~L4). Children with cerebral palsy showed lower bone density than that of normal children. Bone density in children with spastic quadriplegia and diplegia was $-1.812{\pm}.962$, $-1.519{\pm}.935$, respectively. However, there was no significant differences in bone density between children with spastic quadriplegia and diplegia. There was no significant difference in bone density relation to motor development level, height, and weight. Further study is needed to find the appropriate interventions for preventing loss of bone density in children with cerebral palsy.

The Effects of Lumbar Stabilization Exercise for Spinal Function in Patients with Low Back Pain (요부 안정화운동이 요통환자의 요추부 기능개선에 미치는 영향)

  • The purpose of this study was to investigate the effects of lumbar stabilization exercise therapy on low back pain patients' lumbar spinal function. Identify the effect of stabilization exercise therapy, this study attempted to determine lumbar spinal functions, using spinoscopy, for 20 patients with low back pain This study applied lumbar stabilization exercise to 20 low back pain patients without a control group for 8 weeks and 4 times a week, and examined their spinal functions before and after the application. Data collected from the test were analyzed using Wilcoxon signed ranked test, a nonparametric test. Absolute index, functionality and performance increased significantly compared to them before treatment. FE loads and velocity control while conducting exercise tasks increased significantly compared to them before treatment. Test item ROL and ROM, which indicate the change of angle, both showed significant differences. Of stiff spine, stiff pelvic score and sprain score, which indicate the effects of the conduct of exercise tasks on the movement of the spine, stiff spine score and sprain score showed significant differences. According to the results as presented above, lumbar stabilization exercise may be greatly helpful in improving low back pain patients' lumbar spinal functions.

  • Ewing's sarcoma is an uncommon malignant neoplasm of the long bone and it has a poor prognosis due to its early metastasis and aggressive local spread. It is mostly found before the age of 30 and it is rare in extraskeletal sites. Extraskeletal Ewing's sarcoma has been reported to occur in various sites including the larynx, scalp, nasal fossa, neck, chest wall, lung, pelvis, perineum, arm, finger, leg and toe, but it is extremely rare as a primary epidural tumor of the spine. We experienced a case of extraosseous epidural Ewing's sarcoma arising in the lumbar spinal canal at L3-L5 level in a 9-year-old boy. Following total laminectomy from L3 to L5 with a lumbar vertebrae and mass excision, he received chemotherapy with complete remission.

A Clinical Pilot Study Comparing Sweet Bee Venom parallel treatment with only Acupuncture Treatment in patient diagnosed with lumbar spine sprain (요추부 염좌로 진단받은 환자에 대한 Sweet Bee Venom 병행 치료와 단독 침 치료의 효과 비교)

  • Objectives : This study was carried out to compare the Sweet Bee Venom (referred to as Sweet BV hereafter) acupuncture parallel treatment to treatment with acupuncture only for the patient diagnosed with lumbar spine sprain and find a better treatment. Methods : The subjects were patients diagnosed with lumbar spine sprain and hospitalized at Suncheon oriental medical hospital, which was randomly divided into sweet BV parallel treatment group and acupuncture-only group, and other treatment conditions were maintained the same. Then, VAS (Visual Analogue Scale) was used to compare the difference in the treatment period between the two groups from VAS 10 to VAS 0, from VAS 10 to VAS 5, and from VAS 5 to VAS 0. Result & Conclusion : Sweet BV parallel treatment group and acupuncture-only treatment group were compared regarding the respective treatment period, and as the result, the treatment period from VAS 10 to VAS 5 was significantly reduced in sweet BV parallel treatment group compared to the acupuncture-only treatment group, but the treatment period from VAS 5 to VAS 0 did not show a significant difference. Therefore, it can be said that sweet BV parallel treatment is effective in shortening the treatment period and controlling early pain compared to acupuncture-only treatment.

Effects of Manual Therapy Approach for Lumbar Spondylolisthesis (요추부 전방전위증 환자의 도수치료 효과 연구)

  • Purpose : The purpose of this study is to find out the evidence concerning the effectiveness of manual therapy intervention in the treatment of low back pain related to spondylolisthesis Methods : 12men with lumbar pain from L5~S1 spondylolisthesis of 2 or 3 grade were treated during 12-week period. The manual therapy applied twice per week. The results of back and radiating pain were measured by VAS, and abdominal muscle strength was measured by Power track II. The measurement of degree of slip in spondylolisthesis was confirmed by the lateral view of X-ray and took a measurements of the anterior displacement of a vertebral body in relation to the vertebral below. Results : 1. There was significant decrease in the back and radiating pain.(p<0.05) 2. There was significant increase in the abdominal muscle strength.(p<0.05). 3. There was significant decrease in the degree of the anterior displacement. Conclusion: The intervention of manual therapy for lumbar spondylolisthesis is effective in back pain, radiating pain, abdominal muscle strength and degree of the anterior displacement. So, nonsurgical treatment should be attempted before surgical treatment.
