The Effect of Mental Practice with Task Activities on Upper Limbs Movement Speed and Accuracy of Ideomotor Apraxia Patients : Single Subject Experimental Research (상상연습과 과제활동 병행 치료가 관념운동 실행증 환자의 상지 움직임 속도와 정확성에 미치는 효과: 단일사례 실험연구)

  • Objective : The purpose of this study is to investigate the effect of mental practice with task activities through upper limbs movement speed and accuracy in ideomotor apraxia patient. Methods : As a single subject research design with multiple baseline across individuals, the patients were two ideomotor apraxia patients, employed in this study. The employed program included receiving mental practice and task activities, measuring change of the total performance times, time to put down the last cups and amount of water in cups for 4 weeks, including baseline and intervention periods. The subject were measured by Ideomotor apraxia test and MFT were analyzed. The analyses were performed using visually and two-standard deviation band method. Results : All subjects were a reduction in symptoms of apraxia, but noticeable change in upper limbs function was not there. Total time of performance from 376.67 to 355.24 seconds, 391.33 seconds to 434.60 seconds, reduced. Put down the cup of performance time from 15.67 to 13.30 seconds, 20.80 seconds to 10.73 seconds, reduced. The amount of water transferred from 52.38ml to 49.70ml, 50.89ml to 50.09ml, all results were closer to 50ml. Conclusion : As a result of this study, mental practice with task activities can be an effective method to ideomotor apraxia patients. This study would serve a fundamental data on ideomotor apraxia patients to the further studies.

Prevalence and Treatment Pattern of Korean Patients with Temporomandibular Disorders (한국인 턱관절장애 환자의 유병률과 진료 양태)

  • While previous epidemiological studies on temporomandibular disorders (TMD) have been based on a given health center or population sample, no study has been performed on general population of Korea, especially concerning about treatment pattern such as clinician’s specialty involved in TMD treatment, types and amount of prescription medication and cost. This study aimed to investigate magnitude of health visits and treatment patterns for Korean patients with TMD through the computerized database of Health Insurance Review and Assessment Service (HIRAS). Inclusion criteria were all patients registered on the HIRAS database over 3 years' period from 2003 to 2005 and the medical records of patients with TMD as a main diagnosis were extracted. Information collected was as follows; distribution related to gender, age and region and type of hospital the patients visited, treatment duration, clinicians' specialty involved in treatment, cost, types of prescription medication and surgical treatment. The results of this study indicated that 0.15% of the population yearly sought TMD treatment, presenting with increase of incidence over the three years. Most of TMD patients were women (99.8%) and the biggest age group was second and third decades and decreased with age. Seoul and Kyeonggi province presented with higher incidence of TMD compared to the other regions of Korea, which seems to be related with magnitude of population. 56% of TMD patients visited primary care sector and the numbers of treatment visits was the highest in dental clinic (38.4%), followed by orthopedics (28%) and ENT (13.6%) clinics in order. Duration of prescription medication was the longest for anti-inflammatory analgesics, followed by antipsychotic drugs and muscle relaxants. Inpatient care related to TMD was primarily performed in dental hospital compared to medical hospital. Medical database of HIRAS provided comprehensive and vast information on epidemiologic characteristics and treatment patterns for patients seeking TMD treatment, which can be more reliable data to expect medical demand for TMD in condition that accurate diagnosis and standardized treatment is delivered in clinical settings.

Analysis of Medical Charge for Inpatients with Stroke in Tertiary Hospital (종합전문요양기관에 입원한 뇌졸중환자의 진료비 분석)

  • This study was conducted to present strategies for efficient use of older people's medical expense and efficient management of hospital beds 삭제by analyzing factors which influenced medical charge of inpatients with stroke and medical charge of each medical treatment. The subjects was 1,070 inpatients with stroke in Academic hospital. The result of this study can be summarized as follows. In the case of cerebral hemorrhage, the rate of female was higher than that of male. In the case of cerebral infarction, the rate of male was higher than that of female. With increase of age, patients with cerebral hemorrhage decreased and patients with cerebral infarction increased. Medical charge for cerebral hemorrhage was 12,600,000 won, while that for cerebral infarction was 572,000 won. The medical charge with surgery was four times of that with non-surgery. The total medical charge for inpatients with stroke was 6,860,000 won. The patient payed 2,240,000 won(32.6%) and National Health Insurance Corporation payed 4,620,000 won(67.3%). Among charges of specific medical treatments, operation and treatment charge was highest(27.7%) in the case of cerebral hemorrhage, while examination charge was highest(32.2%)in the case of cerebral infarction. This study will provide basic information for efficient use of Medical Charge for Inpatients with Stroke.

Analysis of the Trend and the Factors Influencing the Usage Before and After the Application of the National Health Insurance for the Magnetic Resonance Imaging (자기공명영상의 건강보험 적용 전·후 이용량 동향 및 요인 분석)

  • This study was carried out to determine the factors affecting the trend and usage of Magnetic Resonance Imaging before and after the implementation of National Health Insurance. The research materials were the MRI Execution Data, Health Insurance Review Data, and Medical Expenses in the Health Insurance Data from a General Hospital located in Dae-jeon Metropolitan City from 2004 to 2013. The subjects of analysis were 3,754 people in 2004 (prior to the implementation of Health Insurance), and 4,107-8,603 people from 2005-2013(after the implementation of Health Insurance). In terms of analysis, a comparison of the use of MRI before and after implementation of the Health Insurance of User Characteristics and Provider Characteristics were taken as $X^2$, while factor analysis for the elements that affect MRI usage was carried out by Hierarchical Multiple Regression Analysis. According to the results of this study, the level of use decreased temporarily in the initial application stages of the national health insurance, but it soon increased. In particular, the inspection rates for women, the head and neck, those not subject to the application of the national health insurance, internal medicine, and the inpatients increased. The application of the national health insurance for MRI influenced the increasing inspection rates (P<.0001). As the inspection rate for the MRI increased, it is important to expand the application of the national health insurance to reduce the national health expenditure.

The Effects of Active Exercise Program using Sling on the Pain and Balance Following Total Knee Replacement (슬링을 이용한 능동 운동프로그램이 무릎 관절 전치환술 환자의 통증과 균형에 미치는 영향)

  • This study was conducted to investigate the effect of the active exercise program using sling on the pain and balance of total knee replacement patients. Subjects were 20 patients who received total knee replacement and are hospitalized, 10 patients in each group were randomly assigned to a group (CPM group) that applied only CPM (Continuous passive motion) and a group (CSG) that combined CPM with a active exercise program using sling. CG was performed CPM 5 days a week, CSG performed CPM 2 days a week and a active exercise program using sling 3 days a week, and each intervention was performed for 40 minutes a day for a total of 4 weeks. Pain was evaluated using VAS (Visual analog scale), and balance was measured using BT4 (Balance training 4) to measure C90 area, trace length, and Sway average velocity with eyes open and closed. As a result, there was a significant decrease in pain in both groups, and there was also a significant difference in the amount of change between groups. In balance, all variables except C90 of CG showed significant changes after intervention, and there was a significant difference between C90 and Vel with eyes closed in the amount of change between groups. Therefore, we believe that CPM and active exercise program using sling are effective interventions to reduce pain and improve balance in total knee replacement patients.

Capstone Design Trail in Nursing Education and Its Outcome (간호학에 적용한 캡스톤 디자인의 적용사례 및 결과)

  • This study is a methodological study analyzing the results of applying capstone design to nursing education. Among the nursing college students who completed the fundamental nursing subject, the students who agreed to participate in the research were composed of the project team and proceeded as team activities. It was conducted through knowledge - team composition - topic selection / analysis - design / development - simulation - evaluation / feedback process starting from knowledge learned through fundamental nursing class. The research period was from January 1, 2015 to October 30, 2015 for 6 months. BBS (Bed Sore Socks) was developed for the prevention of pressure ulcers, and BBS was applied for seven days to randomly selected patients in the long term care facility. The incidence and pressure ulcer risk scores were assessed. The results of the pilot study showed that the intervention group had an effect on the incidence of pressure ulcer ($x^2=.40$, p = .500) and the pressure ulcer risk score (z = -.45, p = .690), but it was not statistically significant. Capstone design was trailed in the field of nursing education and produced the bed sore socks. In the nursing education, not many capstone design trails appeared yet, this study might be the first trail in the field of nursing, and it could be challenging for expanding of nursing education.
