Effects of Spinal Stabilization Training Program on Muscle Function and Gait Ability for Private Security Guard (민간경호원의 규칙적인 척추안정화운동 프로그램 참여가 근기능 및 보행능력에 미치는 영향)

  • The purpose of this study was to analyze the variation of muscle function and gait ability according to regular spinal stabilization exercise program for private security guards and the conclusion as follows. First, it increased lumbar extension strength and lumbar flexion strength both exercise and non exercise group according to the variation of muscle function with the spinal stabilization exercise program. There was significant difference between exercise and non exercise group after the program but not before the program. Second, it appeared the functional vanishment of lumbar extensor before the program both exercise and non exercise group according to the variation of muscle function with the spinal stabilization exercise program for lumbar flexor extensor ratio. Third, it analyzed for the spinal stabilization with 8 ways according to the variation of muscle function with the spinal stabilization exercise program for the change of the spinal stabilization. The spinal stabilizaton increased each angle for the execise group but decreased or not changed for non exercise group. Fourth, it used 10m gait test for the speed and step length according to the variation of muscle function with the spinal stabilization exercise program. It appeared the increasement of gait ability for exercise group and there was significant difference between exercise and non exercise group. In conclusion, there was a positive effect for lumbar extension strength, lumbar flexion strength, lumbar flexor extensor ratio, spinal stabilization, and gait ability with regular spinal stabilization exercise program. This means that spinal stabilization exercise program give positive effect to the decrease of back pain and the development of muscular strength for private security guard, also will help to recover and return to work more faster. Also, it seems that the exercise prescription program may be applicable to prevent or to improve the function for private security guard.

The Effects of Lumbar Stabilization Exercise on Pain-related Functions of Industrial Workers with Chronic Low Back Pain and Lumbar Instability (요부안정화운동이 요부불안정성을 가진 산업체 만성요통환자의 통증관련 기능에 미치는 효과)

  • The purpose of this study is to investigate the effects of lumbar stabilization exercise on pain-related function of industrial workers with chronic low back pain and lumbar instability. 20 industrial workers with chronic low back pain were divided into two groups, control group(n=10) and experimental group(n=10). Back muscle strength, flexibility, and balance ability were measured to assess physical functions, and visual analog scale(VAS) was used to evaluate pain levels. In both groups, back muscle strength and balance ability increased significantly, and pain levels decreased significantly. In comparison between the groups, the experimental group compared to the control group showed significant improvement in balance ability and significant diminishment in pain levels. In conclusion, the lumbar stabilization exercise has positive effects on industrial workers with lumbar instability and chronic low back pain by improving balance ability and reducing pain.

Effect of Lumbar Stabilizing Exercise SEBT Training on Isometric Lumbar Strength, Dynamic Balance Ability and Range of Motion in Open Laser Lumbar Microdisectomy Patients (미세 현미경 레이저 요추 디스크 절제술 환자의 요부안정화운동과 SEBT 트레이닝이 등척성 요부근력과 동적균형능력, 관절가동범위에 미치는 영향)

  • This study examines the effects on isometric lumbar extension strength, dynamic balance ability, and range of motion, after administering 8 weeks SEBT exercise (dynamic balance exercise) and Lumbar Stabilizing exercise, to open laser lumbar microdiscectomy patients. Totally, 14 patients who underwent preservation treatment for 6 weeks after undergoing open laser lumbar microdiscectomy, were enrolled for the study. Patients were randomly assigned to the Lumbar Stabilizing Exercise Group (LSG, n=7) and SEBT Exercise + Lumbar Stabilizing Exercise Group (SGLS, n=7). Results indicate that isometric lumbar extension strength ratio significantly decreases after 8 weeks rehabilitation exercise in both the LSG (p=0.007) as well as SLSG (p=0.024) groups. Normalized reach distance of the three directions in the Y-balance test to examine the dynamic balance capability showed a significant increase in both groups. The dynamic balance capability showed significant increase to the left (LSG, p=0.010; SLSG, p=0.002) and right (LSG, p=0.002; SLSG, p=0.002). Moreover, significant increase was also obtained in the range of joint operation, in both LSG (p=0.006) and SLSG (p=0.017) groups. These results indicate that both groups of rehabilitation exercise achieve positive outcomes on the isometric lumbar extension strength, range of motion, and dynamic balance ability. However, some results suggest that the SEBT program is likely to have a better efficacy.

Effects of Lumbar Stabilization Exercise using PNF Techniques on Thickness of Lumbar Deep Muscle and Functional Activity in Chronic Low Back Pain Patients (만성요통환자에서 PNF 기법을 이용한 요부안정화 운동이 요부 심부근 두께 및 기능적 활동에 미치는 효과)

  • The purpose of this study was to examine the effects of lumbar stabilization exercise using the PNF techniques on the lumbar deep muscles thickness and functional activity in chronic low back pain patient. Group I(n=10): general physical therapy group; Group II(n=10): general lumbar stabilization exercise group; Group III(n=10): lumbar stabilization exercise using PNF techniques(stabilizing reversal, rhythmic stabilization, combination of isotonic) group. Change of pain was measured with visual analog scale(VAS). To observe muscle thickness changes, we measured transverse abdominis(TrA), external oblique(EO), multifidus with real time ultrasound scanning. The functional activity were measured with Oswestry Disability Questionnaire(ODQ) and Roland & Morris Disability Questionnaire(RMDQ). In VAS test, group III had more significantly decreased than before exercise. In muscle thickness test, group III had more significantly increased than before exercise in right/left TrA, EO, multifidus. In ODQ & RMDQ test, group III had more significantly decreased than before exercise. This study show that the PNF techniques is effective in improving the lumbar stability and functional activity in chronic low back pain patients.

Effects of lumbar stabilization exercise on multifidus muscle cross-sectional area, static posture control and pain in patient's with chronic low back pain (요부 안정화운동이 만성요통환자의 다열근 단면적, 정적자세조절, 통증에 미치는 영향)

  • This study analyzed the effects of an exercise program that combined lumbar stabilization exercise and muscle strengthening exercise and an exercise program that applied only lumbar muscle strengthening exercise on multifidus muscle cross-sectional area, static posture control ability, and low back pain in patients with CLBP. For the study method, 28 patients with CLBP were divided into two exercise propram groups and exercised three times a week for 8-weeks. The data analysis of this study used the t-test to compare before and after group exercise program application, and between each group (p<.05). As a result of the study, significant results were found before and after application of the exercise program within each group, such as multifidus muscle cross-sectional area, static posture control ability, and low back pain (p<.05). Between each group, the exercise program that combined lumbar stabilization exercise and muscle strengthening exercise was found to be more effective (p<.05). In conclusion, it was found that the exercise program with additional lumbar stabilization exercise improved the structure and function of various lumbar vertebrae and reduced lumbar pain.

Changes in Balance Ability and Muscle Thickness of the Transverse Abdominis and Multifidus of Elderly Females after Eight Weeks of the Lumbar Stabilization Exercise (8주간의 요부안정화운동 시 여성노인의 균형능력 및 복횡.다열근 두께의 변화)

  • This study was conducted to examine the effects of the lumbar stabilization exercise (LSE) on fall prevention in elderly females (n = 21) by measuring changes in the isometric strength, multifidus, transverse abdominis muscle thickness, and posture stability index. The strength of isometric contraction was measured by the 3-D Pegasus system ; the thickness of the multifidus and transverse abdominis were measured by using Sonoace 6000C, and the posture stability index was measured using a computerized Biodex Balance System SD apparatus analysis. These results lead us to the conclusion that flexion and extension, left rotation on strength of isometric contraction, and thickness of the transverse abdominis (when released) statically increased. Overall, the anterior/posterior and medial/lateral elements in the posture stability index statically decreased, Consequently, the LSE should the prevent falls in elderly females.

Extraction of Muscle Areas form Ultrasonographic Images using Subcutaneous Fat Areas and Thoracic Vertebra (피하지방층과 등뼈 영역을 이용한 초음파 영상에서의 근육 영역 추출)

  • In this paper, we propose a novel method to extract muscle area from lumbar ultrasonographic image. The muscle area resided in lumbar area can be defined as the area between thoracic vertebra and subcutaneous fat area. A modified 4-directional contour tracing algorithm is designed to detect the boundaries and candidate areas are extracted and verified by the morphological characteristics of lumbar area. The experiment using 392 lumbar images verifies that the proposed method is sufficiently effective by showing over 94% accuracy in extraction.

Quantization of Lumbar Muscle using FCM Algorithm (FCM 알고리즘을 이용한 요부 근육 양자화)

  • In this paper, we propose a new quantization method using fuzzy C-means clustering(FCM) for lumbar ultrasound image recognition. Unlike usual histogram based quantization, our method first classifies regions into 10 clusters and sorts them by the central value of each cluster. Those clusters are represented with different colors. This method is efficient to handle lumbar ultrasound image since in this part of human body, the brightness values are distributed to doubly skewed histogram in general thus the usual histogram based quantization is not strong to extract different areas. Experiment conducted with 15 real lumbar images verified the efficacy of proposed method.

  • 본 논문에서는 초음파 영상에서 퍼지 C-Means 클러스터링을 이용한 양자화 기법을 제안한다. 제안된 방법은 초음파 영상에서 나타난 명암도를 이용하여 n개의 그룹으로 클러스터링한다. 그리고 각 클러스터의 중심 값을 기준으로 정렬한 뒤, 각 그룹에 지정된 색상을 요부 초음파 영상에서 나타낸다. 본 논문에서 제안하는 기법을 적용한 요부 초음파 영상과 일반적으로 자주 이용되는 히스토그램 기반 양자화 기법을 적용한 요부 초음파 영상을 비교하였을 때, 본 논문에서 제안하는 퍼지 C-Means 클러스터링을 이용한 양자화를 적용한 영상이 근육 내의 지방을 분석하는데 효과적인 것을 확인할 수 있었다.

Mediastinoscopic Lumbar Sympathectomy (종경동경을 이용한 요부 교감신경 절제술)

  • Lumbar Sympathectomy is a surgery for plantar hyperhidrosis, vascular and other reflex sympathetic diseases and has a various indications and physiologic effects. However it is not performed actively compared to thoracic sympathectomy because of its invasiveness. Therefore, we tried to perform lumbar sympathectomy using mediastinoscopy with small incision and introduce this new surgical technique. Material and Method: From July 2003 to December 2004, 18 patients undewent lumbar sympathectomy with mediastinoscopy at Inje University Sanggye Paik Hospital. There were 12 males and 6 females whose mean age was 24.3$\pm$8.2 years ranging from 18 to 67 30 cases of lumbar sympathectomy was performed with mediastinoscopy of which 24 cases were for plantar hyperhidrosis and 6 cases for other diseases. Result: Mean operation time was 37.2$\pm$12.5 minutes and mean post operation hospital stay was 3.1$\pm$2.2 days. There was one post sympathetic neuralgia and one peritoneal opening. Conclusion: Lumbar sympathectomy using mediastinoscopy is a simple and effective technique and has the advantage of cosmetics, post operative pain and hospital stay. However, further studies with large number of cases should be carried out for better outcome.
