Multiple extrarenal manifestations in hemolytic uremic syndrome: A case report (다발성 신외 증상이 동반된 용혈요독증후군 1례)

  • Extrarenal manifestations of hemolytic uremic syndrome (HUS) have increasingly been recognized and may be major determinants of mortality and morbidity. Although microthrombi are often found in the pulmonary and coronary circulation, apparent lung and cardiac involvement are clinically infrequent. We describe here a 10-month-old boy with HUS who developed pulmonary hemorrhage, acute respiratory distress syndrome and dilated cardiomyopathy. Complete renal as well as clinical recovery from these very uncommon complications was achieved by optimum supportive care.

  • 당뇨병성 신증은 투석을 필요로 하는 말기신부전의 주요한 원인이 되며 신장이 제 기능을 못하는 신증 후기단계로 진행하면 노폐물이 몸에 쌓이거나 부종, 전해질 이상, 빈혈 등의 증상이 나타날 수 있다. 이때 신장합병증의 진행의 위험을 줄이고 그 진행속도를 지연시키며 요독증과 부종, 전해질 이상 등의 합병증을 예방하고, 좋은 영양 상태를 유지하는데 적절한 식사관리는 꼭 필요하다고 할 수 있다.

  • The purpose of this case report was to present the treatment of continuous renal replacement therapy (CRRT) in dogs with end-stage CKD with uncontrolled uremia. Hemodialysis were carried out 6 patients who failed to improve clinical status with conventional management for CKD. Four dogs with urea reduction ratio (URR) range of 57-72% and 1 dog with URR of 37.3% showed good outcome with decreasing tendency of pre-dialysis Therefore, we suggest that CRRT could be recommended for use in CKD dogs with uncontrolled azotemia or uremia and should be monitor carefully throughout the CRRT.

  • A 7 month old male shepherd was presented with anorexia, diarrhea, and salivation, and euthanized due to no clinical improvement. Grossly, ulcers were seen on ventral surface of tongue and stomach. The mineralization was obvious in the intercostal space, lung and inner surface of aorta. Kidney was pale, firm and irregular. Histopathology confirmed uremic pneumonitis, end stage kidney and hyperplasia of parathyroid. This case was the typical uremic condition of juvenile dog, which was characterized the diffuse soft tissue mineralization due to hyperparathyroidism secondary to chronic renal failure.

  • 가축을 사육하다보면 가축이 질병에 걸려 폐사하는 경우가 가끔 있는데 이것처럼 양축농가의 아품과 경제적 손실이 큰 것은 없다. 특히 일반비육을 하다가 고급육을 생산하려고 거세하여 사육하다 어느정도 자란 큰 소가 뇨석증에 걸려 식욕부진이 나타나며 불안감, 배뇨시 산통증상이 나타나며 심하면 요도나 방광이 파열되어 요독증세와 더불어 폐사하는 경우를 가끔 볼 수 있는데 고급육 생산시 필수 조건인 거세를 실시하며 이런 질병에 대하여 그 원인과 치료법에 대하여 알고자 인터넷에 올라있는 자료를 본지에 실었으니 고급육 생산에 도움이 되었으면 한다.

  • A 13-year-old castrated male mixed breed dog and a 12-year-old castrated male mixed breed cat were referred to the hospital for the treatment of pulmonary edema and azotemia at the same time. To resolve the pulmonary edema and azotemia, intermittent hemodialysis (HD) was performed using ultrafiltration (UF), and the pulmonary edema, azotemia, other electrolyte and acid-base imbalances were improved. This case study demonstrated that when we encountered pulmonary edema patients with diuretic resistance, severe electrolyte imbalance, and impaired renal function complicated by decongestive therapy using diuretics, UF therapy can be considered a life-saving intervention.

  • A 47-year-old male who had hypertension and diabetes mellitus for 7 years suddenly developed bradykinesia, generalized limb muscular rigidity, dysarthria and dysphagia. Uremia developed 5 months prior to this and he had been on hemodialysis. A T2-weighted brain MRI showed extensive hyperintensity over the bilateral basal ganglia, extending to the adjacent periventricular white matter. In T1-weighted images the lesions were hypointense. Supportive treatments were given and his symptoms improved. Exacerbation of glucose utilization failure or vasogenic edema is suggested as the etiology of basal ganglia lesions, but the exact underlying pathophysiology is unknown.
